A day in the life of PHMC 2005 annual report What a difference a day makes!

As I head home from work each evening, I find comfort in knowing that staff from PHMC and its affiliates have reached out to thousands of people since the day began.

We believe it’s important to reach people where they are, so every day, we’re in community centers, schools, churches, and people’s homes—wherever we need to be to serve the region’s most vulnerable.

In 2005, we were fortunate to bring some unique programs into our network of services:

· The PHMC Care Clinic, formerly known as the St. Joseph’s Care Clinic, brings vital medical care to people living with HIV and AIDS in North Philadelphia.

· The Bridge began managing the new Parkside E-3 Power Center in West Philadelphia, part of a Philadelphia Youth Network initiative that focuses on empowerment, education, and employment for out-of-school youth and youth returning from the juvenile justice system.

· The Joseph J. Peters Institute (JJPI) started an innovative screening and counseling program for Philadelphia grade school children who act in sexually inappropriate ways.

· The National Nursing Centers Consortium (NNCC) started its popular STUDENTS RUN Philly Style Program, which mentors Philadelphia students through running programs.

· The Health Promotion Council (HPC) expanded its nutrition education work. In addition to their work in the School District of Philadelphia, HPC started working with the North Penn., Wissahickon and Souderton school districts in Montgomery County to fight obesity through promoting healthy eating and healthy lifestyles.

· Project Salud, La Comunidad Hispana’s primary care center, offered patients state-of-the-art HIV Rapid Testing.

· The S. A. F. E. Home Visiting Program, part of Resources for Children’s Health (RCH), started helping young parents who come to South Philadelphia from all over the world, including Indonesia, Vietnam, , Mexico, Peru, El Salvador, Argentina, and Guatemala.

This is just a sample of the innovative programs and people with whom we have been fortunate to work over the past year.

ChildLink program Services to Assist Families Excel (SAFE) program

Since 1992, PHMC’s ChildLink programs have helped nearly 33,000 children with developmental delays or disabilities get a better start in life by connecting them to services to help them in their development.

Reaching children and families in their natural environments a key part of both ChildLink Philadelphia, which is funded by the Philadelphia Department of Behavioral Health and Mental Retardation Services, and ChildLink Delaware County, which is funded by the Delaware County Department of Human Services. Unlike the traditional medical system, in which patients go to a doctor, or the traditional educational system, in which students go to school, ChildLink staff go where families are. Each day, eighty ChildLink service coordinators work with families in their homes, family and friends’ homes, child care centers, community centers, early learning schools, recreation centers, and anywhere else they need to be to help families where they live, play, and work.

Staff from Resources for Children’s Health’s new Services to Assist Families Excel Program (S.A.F.E.), funded by the Philadelphia Department of Human Services, the Philadelphia Foundation, and the United Way of Southeastern Pennsylvania, also reach out to parents through home visits. The 149 South Philadelphia parents who participated in this program in 2005 reflect the diversity of their neighborhood, with parents who come from Argentina, China, El Salvador, Guatemala, Indonesia, and Vietnam to build a better life for their children. PHMC’s TArgeted Solutions

During more than thirty years of work in the health and social service sectors, PHMC has gained a wealth of experience applicable to government, foundation and community-based nonprofits. As a result, organizations turn to PHMC’s TArgeted Solutions for technical assistance on issues such as financial management, information technology, organizational development, marketing and communications, research and evaluation, and staffing.

In addition, PHMC’s Community Health Database Project (CHDB) has helped more than 300 organizations across the region use data from its Southeastern Pennsylvania Household Health Survey (SEPA HHS) and other data sets. The SEPA HHS is a telephone survey of 10,000 households in Bucks, Chester, Delaware, Montgomery and Philadelphia Counties. It has been conducted ten times since its inception in 1983. Community groups use SEPA HHS and other data to assess community needs, plan appropriate interventions, and evaluate program effectiveness.

STUDENTS RUN Philly Style

According to PHMC’s Household Health Survey, more than half of 12-17 year olds in some Philadelphia “STUDENTS RUN communities are at risk for obesity. Forensic Intensive Recovery (FIR) Forensic Intensive Recovery (FIR)

PHMC understands that people with alcohol and/or drug problems who commit a crime are more likely to do it again if they do not get help for their addiction. As a result, PHMC partners with the City of Philadelphia to diagnose substance abuse conditions in criminal offenders who participate part in Philadelphia’s Forensic Intensive Recovery (FIR) program. FIR is a national model that reduces recidivism by providing substance abuse treatment to eligible criminal offenders. Research shows this approach works: people who complete a minimum of six months of substance abuse treatment through FIR are 66% less likely to be convicted of a new crime.

When an offender becomes part of the FIR program, trained PHMC staff use validated clinical instruments to determine if a substance abuse problem exists. If it does, FIR staff work with staff from either Community Behavioral Health (CBH) or the Behavioral Health Special Initiative (BHSI) to identify what kind of treatment is most appropriate for the client and what substance abuse treatment programs are available. FIR case managers meet bi-weekly with the clients throughout their treatment and into recovery, to assist them with housing, employment, education and community reintegration.

Interim House West Facilities, Inc About Us Interim House West Facilities, Inc. is a nonprofit corporation that was created when Philadelphia Health Management Corporation built a new facility for Interim House West, a PHMC program that is one of the few residential substance abuse treatment programs in Philadelphia for women and their children. Public Health Management Services Corporation (PHMSC) Completed in 2002, Interim House West’s facility was the first commercial construction project in Philadelphia’s East Parkside neighborhood in over 80 years.
