Sealed Proposals shall consist of one (1) USB Flash Drive, one (1) Original bound copy of the proposal, and three (3) bound copies of the proposal containing all materials as listed in the RFP. FWHS reserves the right to reject any or all proposals. MBE/WBE firms are encouraged to submit proposals. INTRODUCTION FWHS is seeking sealed proposals from interested, qualified, Internet Service Providers (ISPs), to provide a high speed WIFI service to our new headquarters. By issuing this Request for Proposal (RFP), FWHS intends to screen and qualify ISPs desiring to provide this service to ascertain the technical capacity of the ISP’s networks to be deployed for use by FWHS Business, Located at, 1407 Texas Street, Fort Worth, TX 76102. FWHS is changing the face of affordable and attainable housing opportunities by providing mixed- income rental and homeownership possibilities that enable our clients to flourish in the community. FWHS accomplishes this by assisting our clients in elevating themselves to the next level of self- sufficiency and personal responsibility through life-changing programs. SCOPE OF SERVICES The purpose of this request is to formalize the legal relationship between the selected Internet Service Providers (ISPs) and FWHS, so the agency may conduct continuous business transactions. FWHS intends to have its new (eventual) headquarters be a ‘wireless’ building, utilizing several technical ‘access’ devices throughout. With this approach in mind, adequate throughput, consistent service availability, and superior technical support services are paramount. Details Beyond offering access and support, ISPs shall maintain systems at their sole costs and expense, except maintenance, repair and expense is caused by FWHS or its agents. Respondents Technical Requirements for ISP; 1. Shall be wholly responsible for installation and all/any other associated costs incurred while establishing a footprint within FWHS Headquarters’ location. 2. Shall provide two separate Service Lines of 1GBPS to connect. 3. FWHS is requesting separate line-item for a “Redundancy” (failsafe) Internet Service in case of the interruption of the primary Internet provision. 4. Shall provide the Price matrix for the levels of Service being offered along with associated benefits. 5. Shall state clearly the Costs for required equipment to effectuate the expected transmission of the ISP services. 4 | P a g e 6. Shall provide an industry standard minimum mean time to restore for electronics/equipment that will be installed to effectuate the operation of the Internet service. 7. Shall guarantee monthly average uptime to be greater than 99% in the Service Level Agreement (SLA) to FWHS 8. Shall provide both telephone and internet support access to report any interruption of service to the FWHS headquarter location. Pricing Requirements 1. All prices shall be firm and fixed for each Service Speed Identified 2. Pricing shall show per month cost per Service Lines (1 GBPS or higher). III. SUBMISSION PROCEDURES & REQUIREMENTS Proposal(s) must be submitted no later than Thursday, August 26, 2021 at 1:00 p.m. CST. Faxed proposals will not be considered. Any proposal received after the specified date and time will not be considered. Proposals must be in the specified office of FWHS on or before the above specified date and time. If you choose to mail your proposal, it must arrive by the specified date and time, regardless of the postmark date. Letter of Interest (TAB 1) a) Include contact name and address: name, title, email, telephone and fax number to be contacted for clarification or additional information regarding proposals (Cover letter). 5 | P a g e b) A brief statement summarizing the Respondent’s company and relevant experience and qualifications. 2. Past Performance on Similar Projects (TAB 2) Provide information about past clients for whom the Respondent provided the same or similar services. You may also provide references from related service providers or agency personnel. Include a brief description of Respondent’s business relationship with the reference. 3. Key Personnel/ Staff (TAB 3) Please list your firm’s organizational chart and list staff member(s) to be assigned for this project, resumes can be provided. 4. Approach (TAB 4) Describe your philosophy, approach(es) and preferred methods for meeting requirements as listed in the scope of services. 5. Proposed Fees (TAB 5) Please provide a cost breakdown the firm’s proposed fees. FWHS is not to be invoiced for the cost of support services and general overhead for operation of the successful consultant’s main office. All fees are all-exclusive of all related costs that a proposer will incur to provide the noted goods or services in compliance with this solicitation; including but not limited to: employee wages and benefits, clerical support, travel and lodging, overhead, profit, licensing, insurance, materials, supplies, tools, equipment, long distance telephone calls, document copying and motor vehicle fuel, all costs shall be fully burdened. 7. HUD Forms (TAB 6) Each Respondent must complete the forms as provided in Exhibit B. 8. FWHS Forms (TAB 7) Each Respondent must complete the applicable forms as provided in Exhibit C. All forms must contain a signature. When not applicable to your firm, please write “N/A,” sign and date. a. Business References: Provide three (3) references from related service providers or agency personnel. Include a brief description of Respondent’s business relationship with the reference. Minority and Women Business Participation: Proposals submitted in response to this solicitation MUST include an MBE/WBE participation plan which, at a minimum, demonstrates “Best Efforts” have been taken to achieve compliance with MBE/WBE goals. c.
