Biol Blood Marrow Transplant. 2018; 24:1814-1822. https://www.ncbi.nlm.nih.gov/pubmed/29857196 Multicenter, retrospective, cohort study of patient receiving BEAM vs FEAM chemotherapy with autologous transplant between 2008-2015 The FEAM regimen used the standard BEAM backbone and substituted fotemustine 150 mg/m2 on days -7 and -6 in place for carmustine 1038 patients were included (607 treated with BEAM and 431 with FEAM) The most common diagnoses in both groups was aggressive NHL followed by MCL then indolent NHL and the BEAM group had significantly higher HCT-CI driven by rate of pulmonary comorbidities (15.3% vs 10.1%) FEAM patients experienced statistically significantly more GI toxicity (mucositis, diarrhea, nausea/vomiting), grade 4 severe infectious events, higher incidence of gram negative and fungal isolates, slightly longer platelet engraftment, and slightly longer hospital stays (21 days with BEAM vs 23 days with FEAM) Disease response, OS, PFS, NRM, and relapse were not different between groups. FEAM was associated with higher infection related mortality (HR: 1.99; p = 0.04) Although response rates were similar between groups, the increase in toxicity limits the usefulness of the FEAM regimen. [Epub ahead of print]. http://www.ncbi.nlm.nih.gov/pubmed/30212291 2 Cost effectiveness evaluation of tisagenlecleucel in relapsed or refractory pediatric B-cell ALL from a US health payer perspective over a lifetime horizon. The authors based their assessment and evaluation on three model scenarios: 0%, 20%, 40% 5-year relapse-free survival. Markov modeling was informed by recent multicenter, single-arm clinical trials in comparison with blinatumomab, clofarabine combination therapy (clofarabine, etoposide, and cyclophosphamide) and clofarabine monotherapy. Scenario and probabilistic sensitivity analyses were used to explore uncertainty. The main outcomes evaluated were life-years, discounted lifetime costs, QALYs, and incremental cost-effectiveness ratio (3% discount rate). Assuming a 40% 5-year RFS rate, tisagenlecleucel increased life expectancies by 12.1 years and cost $61,000/QALY gained. Assuming a 20% 5-year RFS rate, tisagenlecleucel increased life expectancies by 3.8 years and cost $151,000/QALY gained. Assuming a 0% 5-year RFS rate and use as a bridge to HCT, tisagenlecleucel increased life expectancies by 5.7 years and cost $184,000/QALY gained. Reduction of tisagenlecleucel price from $475,000 to $200,000 or $300,000 would allow it to meet a $100,000/QALY or $150,000/QALY willingness-to-pay threshold in all scenarios. Based on this study, the long-term effectiveness of tisagenlecleucel is a critical but is an uncertain determinant of its cost effectiveness. At current pricing, tisagenlecleucel represents reasonable value only if it can keep a substantial fraction of patients in remission without HCT; however, if all patients ultimately require HCT to remain in remission, it will not be cost effective at generally accepted thresholds. *Svoboda J, Rheingold SR, Gill SI, et al. Experimental Design: Previously untreated, metastatic melanoma patients (n ¼ 20) received bevaci- zumab (at 15 mg/kg every 3 weeks) and fotemustine (100 mg/m2 by intravenous administration on days 1, 8, and 15, repeated after 4 weeks) in a multicenter, single-arm, open-label, phase II study. Primary endpoint was the best overall response rate; other endpoints were toxicity, time to progression (TTP), and overall survival (OS). Serum cytokines, angiogenesis, and lymphangiogenesis factors were monitored by multiplex arrays and by in vitro angiogenesis assays. Effects of fotemustine on melanoma cells, in vitro, on vascular endothelial growth factor (VEGF)-C release and apoptosis were assessed by ELISA and flow cytometry, respectively. Results: One complete response, 2 partial responses (PR), and 10 patients with stable disease were observed. TTP and OS were 8.3 and 20.5 months, respectively. Fourteen patients experienced adverse events of toxicity grade 3–4. Serum VEGF-A levels in evaluated patients (n ¼ 15) and overall serum proangiogenic activity were significantly inhibited. A significant reduction in VEGF-C levels was found in several post-versus pretherapy serum samples.

An analysis of toxicity and response outcomes from dose-finding clinical trials in cancer Kristian Brock1* ,VictoriaHomer1, Gurjinder Soul2, Claire Potter1,CodyChiuzan3 and Shing Lee3 Abstract Background: The overwhelming majority of dose-escalation clinical trials use methods that seek a maximum tolerable dose, including rule-based methods like the 3+3, and model-based methods like CRM and EWOC. These methods assume that the incidences of efficacy and toxicity always increase as dose is increased. This assumption is widely accepted with cytotoxic therapies. In recent decades, however, the search for novel cancer treatments has broadened, increasingly focusing on inhibitors and antibodies. The rationale that higher doses are always associated with superior efficacy is less clear for these types of therapies. Methods: We extracted dose-level efficacy and toxicity outcomes from 115 manuscripts reporting dose-finding clinical trials in cancer between 2008 and 2014. We analysed the outcomes from each manuscript using flexible non-linear regression models to investigate the evidence supporting the monotonic efficacy and toxicity assumptions. Results: We found that the monotonic toxicity assumption was well-supported across most treatment classes and disease areas. In contrast, we found very little evidence supporting the monotonic efficacy assumption. Conclusions: Our conclusion is that dose-escalation trials routinely use methods whose assumptions are violated by the outcomes observed. As a consequence, dose-finding trials risk recommending unjustifiably high doses that may be harmful to patients. We recommend that trialists consider experimental designs that allow toxicity and efficacy outcomes to jointly determine the doses given to patients and recommended for further study.

    Anticancer agents The Alkylating agents The alkylating agents are the oldest group of cytostatic drugs. The first of them – mechlorethamine was introduced into therapy in 1949. The alkylating agents can contain one or two alkylating groups. In the body the alkylating agents may react with DNA, RNA and proteins, although their reaction with DNA is believed to be the most important. 1 • The primary target of DNA coross-linking agents is the actively dividing DNA molecule. • The DNA cross-linkers are all extremly reactive nucleophilic structures (+). • When encountered, the nucleophilic groups on various DNA bases (particularly, but not exclusively, the N7 of guaninę) readily attack the electrophilic drud, resulting in irreversible alkylation or complexation of the DNA base. • Some DNA alkylating agents, such as the nitrogen mustards and nitrosoureas, are bifunctional, meaning that one molecule of the drug can bind two distinct DNA bases. 2 • Most commonly, the alkylated bases are on different DNA molecules, and intrastrand DNA cross-linking through two guaninę N7 atoms results. • The DNA alkylating antineoplastics are not cel cycle specific, but they are more toxic to cells in the late G1 or S phases of the cycle. • This is a time when DNA unwinding and exposing its nucleotides, increasing the Chance that vulnerable DNA functional groups will encounter the nucleophilic antineoplastic drug and launch the nucleophilic attack that leads to its own destruction. • The DNA alkylators have a great capacity for inducing toth mutagenesis and carcinogenesis, they can promote caner in addition to treating it. 3 • Organometallic antineoplastics – Platinum coordination complexes, also cross-link DNA, and many do so by binding to adjacent guanine nucleotides, called disguanosine dinucleotides, on the single strand of DNA.

QUALITATIVE AND QUANTITATIVE COMPOSITION Each single-dose vial of Dacarbazine Lipomed 500 mg contains 500 mg dacarbazine (as dacarbazine citrate, formed in situ). Each single-dose vial of Dacarbazine Lipomed 1000 mg contains 1000 mg dacarbazine (as dacarbazine citrate, formed in situ). After reconstitution of Dacarbazine Lipomed 500 mg with 50 ml of water for injections, 1 ml of solution contains 10 mg dacarbazine (see section 6.6). CLINICAL PARTICULARS 4.1 Therapeutic indications Dacarbazine Lipomed is indicated for the treatment of patients with metastasized malignant melanoma. Further indications for dacarbazine as part of a combination chemotherapy are: Advanced Hodgkin’s disease. Advanced adult soft tissue sarcomas (except mesothelioma, Kaposi sarcoma). 4.2 Posology and method of administration Treatment with Dacarbazine Lipomed should only be carried out by physicians experienced in oncology or haematology respectively. During treatment with Dacarbazine Lipomed, frequent monitoring of blood counts as well as monitoring of hepatic and renal function are required. Since severe gastrointestinal reactions frequently occur, anti-emetic and supportive measures are advisable. Because severe gastrointestinal and haematological disturbances can occur, an extremely careful benefit-risk analysis has to be made before every course of therapy with dacarbazine.

E-mail: [email protected] ABSTRACT Glioblastoma multiforme is the most common type of primary central nervous system tumor and is noted for its short survival and poor response to chemotherapeutic agents. Unfortunately, the relapse rate is very high, and there is no reference drug for second-line treatment. In this study, a patient was treated with the Soffi etti regimen. The induction phase was fotemustine 75 mg/m2 at day 1 and day 8 and bevacizumab 10 mg/kg at day 1 and day 15. The maintenance phase was fotemustine 75 mg/m2 and bevacizumab 10 mg/kg every 3 weeks for two cycles. Follow-up magnetic resonance imaging showed post-surgical changes at the left occipital level, without contrast enhancement, and toxic left leuko-encephalopathy post-treatment without mass effect and with no evidence of tumor residue. The patient then was maintained with bevacizumab monotherapy until it was withdrawn when pulmonary thromboembolism occurred. Following tumor regrowth, fotemustine was started again as maintenance therapy. The patient achieved stabilization of his disease until his death due to thromboembolic and infectious complications. Through these evidence-based guidelines, a task force of EANO provides recommendations for the diagnosis, treatment and follow- up of adult patients with diffuse gliomas. The diagnostic component is based on the 2016 update of the WHO Classification of Tumors of the Central Nervous System and the subsequent recommendations of the Consortium to Inform Molecular and Practical Approaches to CNS Tumour Taxonomy — Not Officially WHO (cIMPACT- NOW). With regard to therapy, we formulated recommendations based on the results from the latest practice-changing clinical trials and also provide guidance for neuropathological and neuroradiological assessment. In these guidelines, we define the role of the major treatment modalities of surgery, radiotherapy and systemic pharmacotherapy, covering current advances and cognizant that unnecessary interventions and expenses should be avoided.
