It also serves as a general policy guide reference to answer questions you may have. This is not a complete list of Star Valley Health policies but presents what we feel is important for you to know. In addition to the policies and procedures you are expected to follow our code of ethics as well. We expect all employees and visitors to keep a high level of hygiene in order to minimize the spreading of germs. This mainly includes often and proper hand washing. We expect all employees and visitors (clinical or otherwise) to be sensitive and empathetic to patients and residents circumstances. This means acting/talking softly and professionally. Summarized Code of Ethics All employees and visitors are expected to follow our code of conduct/ethics. These are more or less summarized as: You will follow all employment laws and standards (ADA, FMLA, EEOC, OSHA, etc.) You commit to keeping sensitive information secure, following HIPAA regulations You will act in the best interest of the patient, giving them freedom to exercise their patient rights You will always act professionally, honestly, and respectfully Policies Parking As a courtesy to our customers, we ask that employees reserve the parking areas nearest the facilities for patients and their families. Remember: Employees park on outer perimeter of the campus Lines on pavement indicate who they are for: white for patient/visitor and yellow for employee Substance Abuse All employees, students, and other visitors are required to adhere to our Substance Abuse policy, it is summarized as: Using, possessing, buying, selling, manufacturing, or dispensing illegal drugs Coming under the influence of alcohol, and or illegal drugs is not tolerated, Don’t Do IT SV Health is a Tobacco & Nicotine Free Campus, (including parking lots) SV Health Drug Testing Policy o Pre-employment o Post- accident o Random o Reasonable suspicion Dress Policy All employees, students, and other business related visitors must adhere to our dress policy. This includes dressing in a professional manner. To reduce safety risks we require proper shoes to be worn and additional PPE as needed by circumstances. In our facilities remember: Tattoos, extreme hair colors, and body piercings will be left to the discretion of department directors, and employees may be asked to make adjustments to meet the standards of their department. Limit the use of perfume, scented lotions, or cologne to accommodate those who are hyper-sensitive to smells Clinical personnel-direct patient care Natural nails cannot exceed ¼ inches long. No artificial nails or nail jewelry Clerical Personnel Natural or artificial nails no longer than ¼ in long. Nail jewelry is not acceptable Emergency Codes Code Red – Fire Danger Code Yellow – Emergency Operations Plan Code Pink – Lost Baby Protocol Code Green – Campus Lockdown Protocol Code Blue – Life Threatening Code Silver – Wandering Resident Protocol Code Purple – HIPPA Violation Patient Rights Star Valley Health provides all patient rights afforded by state and federal laws. Patient Rights include: COMMUNICATION Become informed of their rights in advance Receive effective communication regarding their care in a language they can understand. This includes using interpreters and other methods as circumstances dictate Be informed about their healthcare requirements following his/her discharge from SV Health PARTICIPATE IN CARE DECISIONS Participate in the development and implementation of their care plan to the extent the law allows When a patient is unable or unwilling to make decisions about his/her care, SV Health involves a surrogate decision-maker in making these decisions The surrogate decision-maker is responsible for making care, treatment, and services decisions, and the hospital respects the surrogate’s decisions INFORMED CONSENT Receive all necessary information regarding their care and treatment and have the right to refuse care or treatment In emergencies, information shall include at minimum, a description of treatment, risks, and alternate courses of treatment RIGHT TO KNOW CARE PROVIDERS Have knowledge of the physician’s name who is providing care, be informed of other providers, and are able to opt for a different care giver Have their personal physician notified if treatment is being performed by someone else END OF LIFE ISSUES Formulate advance directives regarding their healthcare SVMC respects patients end of life decisions PRIVACY OF PERSONAL INFORMATION- HIPAA Confidentiality and HIPAA Health Insurance Portability and Accountability Act Personal information includes: Patient name Social security number Telephone number E-mail address Physical and mailing address Name employer Name of relatives Birthdate Any healthcare account number Insurance information Anything within a medical file Keeping Information safe • Use the Minimum Necessary Rule, only those who have a Need to know and are Authorized to know • Penalties can be individual and include fines/jail • Katheryne Mattoon- HIPAA Compliance Officer, investigates any breach of sensitive information • To discuss or use information any place other than directly with the patient, you must obtain written release of information from the patient. • Taking pictures inside SV Health is strictly prohibited Contact Information 901 Adams Street PO Box 579 Afton WY, 83110 Phone: 307-885-5800 Fax: 307-885-5989 starvalleyhealth.org .
