L. van Tilburg11,12,13 and Paul Enck6 ✉ Abstract | Paediatric functional abdominal pain disorders, currently referred to as disorders of gut–brain interaction, comprise irritable bowel syndrome, functional dyspepsia, abdominal migraine and functional abdominal pain not otherwise specified, as defined by the Rome IV diagnostic criteria. Functional abdominal pain disorders are common disorders with a prevalence of 3–16% depending on country, age and sex. A greater understanding of aetiopathogenesis and pathophysiology is emerging and includes intestinal components (inflammation, motility and the microbiota), central factors (psychological aspects, sensitization and/or differences in connectivity or activity of certain brain regions) as well as extrinsic factors (infections). In particular, the timing of disruption of the microbiota–gut–brain axis seems to be important. Diagnosis is challenging but is primarily based on clinical symptoms and exclusion of other organic causes, with an emphasis on avoiding unnecessary invasive diagnostic procedures. The available pharmacological interventions are limited in children and, therefore, management has focused on combined approaches, including mind- targeted interventions (hypnotherapy and cognitive behavioural therapy), diet (probiotics) and percutaneous electrical nerve field stimulation. The evidence for their clinical efficacy, although limited, is favourable, with positive impacts on symptoms and overall quality of life. The coming decades hold promise for improved understanding and management of these enigmatic disorders. ABSTRACT The main aim of this study was to optimize and evaluate the floating tablets of Drotaverine HCl that prolong the gastric residence time, increasing drug bioavailability and control pain for longer duration by oral administration. A floating drug delivery system(FDDS) was developed using gas forming agent like sodium bicarbonate, citric acid polymers like hydroxypropyl methyl cellulose(HPMC), Sod CMC, Carbopol-934P, PVP K-30. In 32 factorial design amount of HPMC(X1) and gas generating agents(X2) were selected as independent variable and % drug release for 30min,1h, 2h,4h,6h, 8h,12h, 16h , 24h and floating lag time (FLT) were taken as dependent variable. The floating tablet formulations were evaluated for Bulk density (gm/cm3), Tapped density(gm/cm3), Hausner ratio(HR), Carr index, Angle of repose, flow property, assay, in-vitro drug release, hardness, friability,weight variation. The results of in vitro release studies showed that the optimized formulation (F9) could sustain drug release (98.74%) for 24h and remain buoyant for more than 24h. The combination of hydrophilic (HPMC) and hydrophobic (carbopol-934P) polymer provides a better option for 24h release action, bioavailability, stability of tablets at 400C/75%RH, of optimized formulation was carried for one month and no significant change was observed. Key messages • Few systematic reviews provide a comprehensive rigorous assessment of all potential interventions, including nondrug interventions, to treat pain attributable to each acute pain condition. Acute pain conditions that may need a comprehensive systematic review or overview of systematic reviews include postoperative postdischarge pain, acute back pain, acute neck pain, renal colic, and acute migraine.

    National Institute for Health and Care Excellence Surveillance programme Surveillance proposal consultation document Irritable bowel syndrome NICE guideline CG61 – 8-year surveillance review Background information Guideline issue date: February 2008 3-year review (no update)* 6-year review (yes to update) *Although the 3-year review decision was no update, the findings were subsequently used to pilot the NICE’s rapid update process. Surveillance proposal for consultation We will not update the guideline at this time. Reason for the proposal New evidence We found 105 new studies in a search for randomised controlled trials (RCTs) and systematic reviews published between 01 September 2013 and 18 July 2016. We also considered studies identified by members of the guideline committee who originally worked on this guideline. Evidence identified in previous surveillance 3 years and 6 years after publication of the guideline was also considered. This included 52 studies identified by search. From all sources, 157 studies were considered to be relevant to the guideline. Surveillance proposal consultation document for Irritable bowel syndrome (2008) NICE guideline CG61 1 of 44 This included new evidence that is consistent with current recommendations on: diagnosis of irritable bowel syndrome (IBS) dietary interventions physical activity interventions drug treatments (antispasmodics, laxatives, anti-motility agents and antidepressants) psychotherapy hypnotherapy, biofeedback and relaxation therapy acupuncture and patient information. We also identified new evidence in the following areas that was inconsistent with, or not covered by, current recommendations, but the evidence was not considered to impact on the guideline: ondansetron vitamin D supplementation herbal medicines. We did not find any new evidence on reflexology, psychosocial interventions, or self-help and support groups.
