Prescribing Trends of Antihistamines in the Outpatient Setting in Al-Kharj Nehad J. Ahmed1*, Menshawy A. Materials and Methods: This is a retrospective study that included the evaluation of antihistamines in the outpatient setting in a public hospital in Al-Kharj. The data were collected from the pharmacy-based computer system. Results: The total number of prescriptions that included antihistamines was 799. Most of the prescribed ORIGINAL ARTICLE ARTICLE ORIGINAL antihistamines were first-generation sedating antihistamines (chlorphenamine and diphenhydramine) (66.33%). About 63.20% of the prescribed antihistamines included chlorpheniramine followed by cetirizine (19.27%) and loratadine (14.39%). Conclusion: Antihistamines were prescribed commonly in the outpatient setting mainly first-generation sedating antihistamines. It is recommended to increase the awareness of health- care providers about the efficacy and the side effects of antihistamines and to encourage them to use these agents wisely. www.nature.com/scientificreports OPEN Dezocine exhibits antihypersensitivity activities in neuropathy through spinal Received: 09 November 2016 Accepted: 19 January 2017 μ-opioid receptor activation and Published: 23 February 2017 norepinephrine reuptake inhibition Yong-Xiang Wang1, Xiao-Fang Mao1, Teng-Fei Li1, Nian Gong1 & Ma-Zhong Zhang2 Dezocine is the number one opioid painkiller prescribed and sold in China, occupying 44% of the nation’s opioid analgesics market today and far ahead of the gold-standard morphine. We discovered the mechanisms underlying dezocine antihypersensitivity activity and assessed their implications to antihypersensitivity tolerance. Dezocine, given subcutaneously in spinal nerve-ligated neuropathic rats, time- and dose-dependently produced mechanical antiallodynia and thermal antihyperalgesia, significantly increased ipsilateral spinal norepinephrine and serotonin levels, and induced less antiallodynic tolerance than morphine. Its mechanical antiallodynia was partially (40% or 60%) and completely (100%) attenuated by spinal μ-opioid receptor (MOR) antagonism or norepinephrine depletion/α2-adrenoceptor antagonism and combined antagonism of MORs and α2-adenoceptors, respectively. In contrast, antagonism of spinal κ-opioid receptors (KORs) and δ-opioid receptors (DORs) or depletion of spinal serotonin did not significantly alter dezocine antiallodynia. In addition, dezocine- delayed antiallodynic tolerance was accelerated by spinal norepinephrine depletion/α2-adenoceptor antagonism. Thus dezocine produces antihypersensitivity activity through spinal MOR activation and norepinephrine reuptake inhibition (NRI), but apparently not through spinal KOR and DOR activation, serotonin reuptake inhibition or other mechanisms. Our findings reclassify dezocine as the first analgesic of the recently proposed MOR-NRI, and reveal its potential as an alternative to as well as concurrent use with morphine in treating pain. -

Quzyttir™ (cetirizine) – New drug approval • On October 4, 2019, the FDA approved TerSera Therapeutics’ Quzyttir (cetirizine) injection, for the treatment of acute urticaria in adults and children 6 months of age and older. — Quzyttir is not recommended in pediatric patients less than 6 years of age with impaired renal or hepatic function. • Quzyttir is the first FDA approved intravenous (IV) formulation of cetirizine. Oral formulations of cetirizine are available generically as prescription as well as well over-the-counter (OTC) (eg, Zyrtec®). — Prescription oral cetirizine is approved for perennial allergic rhinitis and chronic urticaria. — Zyrtec and other OTC products are approved for use to temporarily relieve symptoms due to hay fever or other upper respiratory allergies: runny nose, sneezing, itchy and watery eyes, and itching of the nose or throat. • The efficacy of Quzyttir was established in a randomized, active-controlled, double-blind, single-dose study in 262 adult patients with acute urticaria. Patients were treated with Quzyttir or diphenhydramine injection. The primary efficacy endpoint was the change from baseline in patient-rated pruritus score assessed 2 hours post treatment. The study was non-inferiority design with the pre-specified non- inferiority margin of 0.50 for the difference between treatment groups. — The effectiveness of Quzyttir was demonstrated to be non-inferior to the effectiveness of diphenhydramine. The mean change from baseline in patient-rated pruritus score was -1.61 and -1.50 for Quzyttir and diphenhydramine, respectively (adjusted difference: 0.06; 95% CI: - 0.28, 0.40). • The efficacy of Quzyttir for the treatment of acute urticaria down to 6 months of age is based on extrapolation of the efficacy of Quzyttir in adults with acute urticaria and supported by pharmacokinetic data with oral cetirizine hydrochloride in patients 6 months to 17 years of age. -

Four tests commonly used to assess sedation in rodents—the open field test, forced swim test, behavioral scale, and traction test—were conducted in 10 juveniles iguanas. The animals received midazolam (2 mg/kg IM) or 0.9% NaCl (0.4 mL/kg IM), and the tests were conducted between 0 and 300 min thereafter. To verify the effects of midazolam and flumazenil, the most informative tests from the evaluation stage and the limb withdrawal latency time (LWLT) were used. All 10 iguanas were tested under 4 conditions, as follows: MS, midazolam (2 mg/kg IM), followed 30 min later by 0.9% NaCl (0.4 mL/kg IM); FS, flumazenil (0.05 mg/kg IM), followed by 0.9% NaCl (0.4 mL/kg IM) 30 min later; MF, midazolam (2 mg/ kg IM), followed by flumazenil (0.05 mg/kg IM) 30 min later; and CON, 0.9% NaCl (0.4 mL/kg IM). The behavioral scale and the forced swim test showed the best detection of the onset, peak effect, and the differences between the sedated and con- trol iguanas, with testing done between 15 and 240 min after drug administration. -

PATIENT & CAREGIVER EDUCATION Tube Feeding Using the Bolus Method This information will help teach you how to use the bolus method to feed yourself and take your medications through your percutaneous endoscopic gastrostomy (PEG), gastrostomy tube (GT), or nasogastric tube (NGT). About Tube Feeding Tube feeding is when you get your nutrients through a feeding tube if you aren’t able to get enough through eating and drinking, or if you can’t swallow safely. Nutrients provide energy and help you heal. The bolus method is a type of feeding where a syringe is used to send formula through your feeding tube. The syringe you’ll use is called a catheter syringe. A catheter syringe doesn’t have a needle. It has a hole with a plunger in it. You draw up formula through the hole in the syringe then push the formula into your feeding tube with the plunger. A bolus refers to 1 “meal” of formula. You may have a feeding tube with a legacy connector or an ENFit connector. In this resource, we’ll show images of both types of connectors.
