8:15 am - 9:15 am Depression in Pregnancy: Making Sense of Conflicting Literature about Risks of Treatment with Antidepressants

Vivien K. Burt, MD, PhD, is Professor Emeritus of Psychiatry at The David Geffen School of Medicine at UCLA in , California. She is Founder and Co-Director of The Women’s Life Center of the Resnick UCLA Neuropsychiatric Hospital. Dr. Burt has authored numerous articles and book chapters in the field of women’s psychiatry, has coauthored The Clinical Manual for Women’s Mental Health, published by the American Psychiatric Press in May 2008 and is the first author of the chapter on Women’s Mental Health in the American Psychiatric Press Textbook of Psychiatry, published in 2014. She has been honored with an Outstanding House Staff eachingT Award from the UCLA Neuropsychiatric Institute, both a President’s Award and an Outstanding Achievement Award from the Southern California Psychiatric Society, and the Distinguished Service Award of the UCLA Neuropsychiatric Institute and Hospital, the 2008 Andre Boivin Professorship of the Motherisk Program of The Hospital for Sick Children in Toronto, Canada . The focus of her activities includes projects in women’s mental health with a particular focus on psychiatric illnesses associated with reproductive transitions.

Sherry Payne, MSN, RN, CNE, IBCLC, presents nationally on topics related to perinatal health disparities, infant mortality, and lactation professions . She is a master’s prepared nurse educator and lactation consultant . Ms . Payne is the Executive Director of Uzazi Village, a nonprofit organization devoted to decreasing pregnancy related health disparities in the urban core of Kansas City . She is the creator of the Sister Doula ProgramTM and the Chocolate Milk CafeTM .

Aarti Sequeira, well-known for her victory on season six of The Next Star, Aarti is host of Taste in Translation on the Cooking Channel. She published a new cookbook, Aarti Paarti: An American Kitchen with an Indian Soul. Aarti won our hearts and the winner’s prize on : Superstar Sabotage. She has a food blog, www.aartipaarti.com and an online , youtube .com/aartipaarti . Born in Bombay and raised in Dubai, Aarti moved to Chicago to attend Northwestern University, and now lives in Los Angeles with her husband and their babies .

8:15 am - 9:15 am Adverse Childhood Experiences and Mental Health

Vincent J. Felitti, MD is a 1962 graduate of the Johns Hopkins Medical School. He is an internist who, subsequent to being Post Surgeon at the U.S. Army Pine Bluff Arsenal, completed his training at Johns Hopkins and the University of Maryland and started as an infectious disease physician in 1968 at Kaiser Permanente in San Diego and then in 1975 founded the Department of Preventive Medicine; he served as the Chief of Preventive Medicine until 2001. In that setting, Dr. Felitti became co-Principal Investigator, with Robert F. Anda MD at the Centers for Disease Control, of the Adverse Childhood Experiences (ACE) Study, ongoing collaborative research between the Kaiser Permanente Medical Care Program and the CDC. The ACE Study with its more than sixty publications explores prospectively and retrospectively in a 17,000-person cohort the profound relationship of ten categories of adverse life experiences in childhood to health, well being, disease, and death decades later.

Under Dr. Felitti’s leadership over the years, the Department of Preventive Medicine provided comprehensive, biopsychosocial medical evaluation to assess the health risks and disease burden of over one million individual adults. Major health-risk abatement programs were developed for obesity, smoking, and stress, as well as population-based screening for the genetic disease, Hemochromatosis . He is Clinical Professor of Medicine at the University of California and a Fellow of The American College of Physicians. He is Senior Editor of The Permanente Journal and on the International Editorial Board of the Swiss medical journal, Trauma und Gewalt. Dr. Felitti has served on advisory committees of the Institute of Medicine, the American Psychiatric Association, and on the Committee of the Secretary of Health and Human Services for Healthy People 2020. Presently, he is a member of the Advisory Committee on Women’s Services at SAMHSA .

Daniel Singley, PhD, appearing on the Father’s Perinatal Mental Health Panel, is a San Diego-based board certified psychologist and Director of The Center for Men’s Excellence . His research and practice focus on men’s mental health with a particular emphasis on reproductive psychology and the transition to fatherhood .

David Levine, MD, appearing on the Father’s Perinatal Mental Health Panel, is an adolescent clinic preceptor for the Atlantic Health System Internal Medicine Residency Program, and was featured in the New Jersey Family magazine’s “New Jersey’s Favorite Kids’ Docs”, as well as the Charlie Rose show, discussing his own experience with paternal postpartum mental health.

1A: Working with Native American Women with Postpartum Depression and PMADs - Hillcrest 1 Presenter: Pam Barnes-Palty, MEd, MA Providing appropriate multi-cultural care when working with Native American mothers. Who are “Native American” mothers? Seeking help for PPD and PMADs is complicated by historical trauma and current social and economic realities . The session will include lecture and a question-and-answer session. Additionally, story-telling, an important component of many Native American tribal cultures, will be incorporated to provide a multi-cultural experience for participants.

1B: Bridging the Gap: National Interdisciplinary Recommendations for Psychosocial Support of NICU Parents - North Park Presenters: Sage Saxton, PsyD and Pec Indman, EdD, MFT The new national support guidelines for perinatal health will be presented. These guidelines were created through the collaborative work of over 50 professionals and the National Perinatal Association (NPA) . These recommendations have been endorsed by multiple national and parent support organizations (including PSI) .

1C: A Skills-Based Approach to Helping Couples Navigate Their Postpartum Journey - Cortez Ballroom Presenters: Crystal Clancy, MA, LMFT and Lisa Cross, MA, LMFT If you work with expecting or postpartum couples, this workshop will give you some tools to help navigate their postpartum journey more smoothly, and feel more connected. You will learn several different approaches that can be integrated into counseling.

1D: Culture, Shame, and Motherhood Identity - Kensington 2 Presenter: Meggan Hartman, PhD Mother-blame/shame is pervasive in American culture. Come and discuss the ways culture shames and blames mothers and explore the ramifications of this phenomenon on the development of the Maternal Identity .

1E: The Breast is Best Paradigm & Maternal Mental Health: Best Practices in Psychotherapy and Lactation - South Park Presenters: Amy-Rose White MSW, LCSW and Elizabeth Smith, MPH, ICCE, IBCLE, RLC Expectations around breastfeeding have a unique & direct impact on women with postpartum mood & anxiety disorders, including birth trauma PTSD . This presentation explores this dynamic relationship, the tension between “optimal infant feeding” and maternal mental health, as well as concrete strategies for professionals in clinical practice. Appropriate for all birth professionals interested in addressing the balance between sound maternal mental health and optimal infant feeding practices .

1F: Bipolar Disorder and Postpartum Psychosis - Kensington 1 Presenter: Amanda Tinkelman, MD A review of the epidemiology, manifestation and management of postpartum psychosis, and its strong association with Bipolar Disorder.

1G: Postpartum Action Institute - University Heights Presenters: Shoshana Bennett, PhD and Jane Honikman, MS Mini Postpartum Action Institute (PAI) . Practice your skills and gain new insights for supporting new parents. Interact in a safe environment to overcome challenges and barriers as you empower new families.

2A: Maternal Mental Health Care and the Orthodox Jewish Family: Cultural Insights - Hillcrest 1 Presenter: Dvora Entin, LCSW The Orthodox Jewish Community struggles with stigma and fear of anyone knowing; however, the community offers a strong framework of supportive networks and opportunity for collaboration. Increasing cultural understanding can increase opportunity for care of an often vulnerable population of women.

2B: A Hybrid Integrated Care Model: A Novel Approach to Maternal Mental Health - South Park Presenters: Malina Spirito, PsyD, MEd and Megan O’Hara, MSW, LCSW The overall objective of this presentation is to describe the factors vital to implementing an integrated maternal mental health program in a large women’s and children’s hospital system.

2C: Increasing Access to Care Using a Private Practice Training Model - Kensington 2 Presenters: Sherry J. Duson, MA, LPC-S, LMFT-S and Kim Jones, MA, LMFT Overview of a practice model which includes using interns and associates under supervision, offering specialized training in perinatal mental health, and providing counseling, assessment, support and educational services to the community on a sliding scale. Workshop will include information about finances, supervision procedures and record keeping. Experiential data will be presented from presenters’ personal experience.

2D: Avoiding Crisis and the Loss of Life by Recognizing and Treating Postpartum Psychosis - Cortez Ballroom Presenters: Angela Burling RN, MSN and Chris Raines, RN, MSN, APRN-BC Postpartum Psychosis: A Medical Emergency . Why is postpartum psychosis considered a medical emergency? What are the signs and symptoms that require immediate action and the potential dangers of untreated PPP?

2E: Fathers' Perinatal Mental Health: Yes, It Exists and What You Should Do About It - Kensington 1 Presenter: Daniel B. Singley, PhD, ABPP This introductory workshop provides in-depth information regarding research, theory, and best practices in the psychology of new fatherhood throughout the perinatal period spanning from conception to one year postpartum .

2F: The New Family Care Clinic: Integration in Every Direction - North Park Presenters: Erica Shoemaker, MD, MPH; Gabrielle Kaufman, MA, LPCC, BC-DMT, NCC; Michael Regalado MD; Nyssa Adackapara, MD The New Family Care Clinic is an innovative model that integrates maternal mental health care in multiple ways: between obstetrics and pediatrics to cover the entire perinatal period, and between psychiatry, traditional pediatrics, and Promoting First Relationships, an attachment-based approach.

2G: Roundtable Discussion for Nonprofit Leadership – Part I - University Heights Presenters: Adrienne Griffen, MPP; Tonya Fulwider; Crystal Clancy, LMFT This discussion will aid executive directors and staff of nonprofit organizations addressing PMADs by providing a support group to be a sounding board for ideas, concerns, strategies, and growth. 3A: Mamás y Bebés Program - Replication of a Perinatal Preventive Intervention of Postpartum Depression for the Latina Population - Adapting and Supporting the use of the Promotora Model - Kensington 2 Presenter: Diana Griffis, MS, LMFT Utilizing the evidenced-based practice Mamás y Bebés Program (MyB) , the workshop will give attendees a brief overview, as well as identify risk and protective factors of Postpartum Depression (PDD). Review the nationally available postpartum depression prevention program Mamás y Bebés. Findings of the Washington DC implementation will be discussed. Replication and the adaptation of Mamás y Bebés program in Riverside County using the Promotora Model will be reviewed. Strategies of implementation and ongoing support for MyB providers will also be presented .

3B: California Maternal Mental Health Commission - Findings and Recommendations - Hillcrest 1 Presenters: Joy Burkhard, MBA and Bruce Spurlock, MD This presentation will showcase the findings and recommendations made by the California Commission on Maternal Mental Health Care after completing its year long collaborative work.

3C: Integrative Systems Model For Infertility: Patient Benefits and Provider Considerations for PMADs Prevention - Cortez Ballroom Presenters: Amber Rukaj, MA, LMFT, PP Doula; Chelsea Haakmeester, MA, IMF This workshop will focus on an interdisciplinary model for infertility treatment and provide recommendations for working with patients that have been treated for infertility and are transitioning from pregnancy to postpartum .

3D: Keeping Mothers and Babies Safe: Building Relationships with Child Protective Services - North Park Presenters: Gabrielle Kaufman, MA, LPCC, BC-DMT, NCC, Caron Post, PhD, and Patrisha Taylor, MFT, PhD (candidate) Maternal mental health concerns often lead to worries about child safety. This workshop will review how to form partnerships with local Child Protective Services and educate social workers to advocate not only for children, but for mothers too .

3E: Perinatal Mental Health Warmlines: Training Volunteers, Sustaining Services, and Dealing with Emergencies - South Park Presenters: Wendy Davis, PhD; Marisela Rosales, LCSW; Dalia Kenig, MA, MFT Learn from our experienced panel as they describe the guidelines, challenges, and effective methods of running a telephone support line for pregnant and postpartum families . Our panelists are the warmline coordinators and volunteer trainers for the PSI English and Spanish warmlines. Topics include the role of social support, assessing, training, and supporting volunteers, description of calls, challenges and solutions, providing culturally informed services, sharing resources and referrals, and emergency protocols .

3F: Amita: Developing A Perinatal Behavioral Health Intensive Outpatient Program - Kensington 1 Presenters: Kimberly McCue, MA, Lita Simanis, MSW, LCSW, and Melissa Gonski, MA, LCPC A detailed presentation of the development of the AMITA Perinatal IOP from idea to 1st day, as well as day-to-day operations, curriculum development, PMAD CLS, inclusion of labor and delivery nursing staff, and psychopharmacology management .

3G: Postpartum Psychosis Survivors – Making a Difference (Second Annual Postpartum Psychosis Survivors Support Group) - University Heights Presenters: Angela Burling RN, MSN and Sharon Gerdes, BS, CFS An opportunity to meet, share, and strategize with others who have lived the experience of postpartum psychosis (PPP) .

Bridging the Gap: Physicians and Clinicians Creating Proactive Care; (2) Collaboration for Community Change in Perinatal Mental Health - Hillcrest 1 Presenter: Allison Murphy, MFT The Mothers Care Model: Created by an Ob/Gyn and a MFT as a response to unmet perinatal mental health needs . This model partners mental health providers, health providers and funders to disrupt the current delivery of services. Mothers Care was designed to accurately assess and treat PMADs and to provide support from the start . Presenter: Emma Goforth, MPH; Lisa Jansen Thompson, MA; LPC Lauren Jassil, MSW, LCSW Collaboration for Community Change in Perinatal Mental Health: Hear how one high risk community in Colorado came together to break down barriers that exist for perinatal mental health, focusing on multiple levels: from women and their supports to referral organizations to systems .

4C: (1) The Spaces In Between: Facilitating Wellness in Perinatal Transitions (IPT); (2) Postpartum Eco-Map: A Tool for Identifying and Strengthening Support Networks in Pregnancy and Postpartum - North Park Presenter: Jane Hesser, MFA, MSW, LICSW Facilitating Wellness in Perinatal Transitions (IPT): Becoming a mother is dynamic and complex, involving changes in identity and role, as well as biological and concrete changes. This session will draw on the group’s collective experience and a general Interpersonal Psychotherapy (IPT) framework to identify challenges and organize thoughts around ways of facilitating wellness throughout the transition to motherhood . Presenter: Arianna Taboada, MSW, MSPH Postpartum Eco-Map: Learn how a social work tool can help pregnant and postpartum women assess their connections with family, community, and social networks, and develop a clear postpartum support plan.

4D: (1) Grief Work Following Termination for Anomalies; (2) A Model for Perinatal Palliative Care: Helping Mothers and Families Say “Goodbye” at Birth - Cortez Ballroom Presenter: Laurie Chameides, LCSW Grief Work Following Termination: This workshop will discuss the impact of decision making on emotional adjustment following the diagnosis of anomalies in a desired pregnancy. Cultural issues and treatment modalities will be addressed. Presenter: Joanna Cole, PhD Perinatal Palliative Care: This workshop will explore the evolution of perinatal palliative care and share a model that supports women and families who decide to carry their pregnancy to term knowing their baby’s life will be limited.

4E: Managing Difficult Situations in Support Groups - South Park Presenters: Birdie Gunyon Meyer, RN, MA and Annette Cycon, LICSW Hear two seasoned group leaders share lessons learned over 25 years. Participants will learn about managing challenging situations, personalities, crisis, and mandated reporting, as well as a problem-solving tool they can use in their own groups.

5D: Trauma Informed Understanding of Postpartum Psychosis - Kensington 1 Presenters: Diana Lynn Barnes, PhD Postpartum psychosis affects 1 to 3 in 1000 mothers and is currently understood to be a serious mental illness with significant rates of infanticide and suicide. Learn about the new research that is focusing on the causal relationship between childhood trauma and psychosis. Implications of a trauma-informed understanding of postpartum psychosis and the importance of decreasing stigma and increasing access to care for mothers will be discussed.

This session will briefly explore research on the potential impact of traumatic labor and delivery on both maternal mental health and maternal-infant bonding. Discussion will emphasize the importance of embracing dynamic complexity in both assessment and treatment of women suffering from PMADs, in the context of traumatic birth recovery. The discussion will be enriched and illustrated by case examples from the Department of Women’s Behavioral Health, at Women & Infants Hospital, in Providence, RI.

6E: Postpartum Depression in the Military - South Park Presenters: Alison Reminick, MD; Theresa Nguyen, PMHNP; Amber Rukaj, MFT The University of California, San Diego is at the center of a number of different military bases in Southern California and, as a result, the Women’s Mental Health Program cares for many military families suffering from postpartum depression. In the past three months, ew have treated over 40 military families who presented with antenatal or postpartum depression. The experiences of these military families are unique and we will discuss predictors, risk factors, barriers to care and impact of postpartum depression in these families. We will also discuss how frequent moves, deployments and military environment contributes to postpartum depression and how these factors also affect attachment and bonding with the newborn. e W will be presenting and discussing our own preliminary data obtained from the focus groups we will be conducting over the next few months with these families to understand how we can better serve them through our Women’s Mental Health Program .

6F: Integrating Group Medication Management into Obstetrical Care: A Multidisciplinary Treatment Model for PMADs - North Park Presenters: Shannon L. Clark, MD; Elizabeth A. DeFazio RN, MS; Nuria S.
