The Royal College – Our Professional Home An independent review on diversity and inclusion for the Royal College of Surgeons of England An exciting call for radical change Introduction ‘The President of the Royal College of Surgeons of England (RCS England) Professor Neil Mortensen, asked me to lead this Review soon after he was elected to the Presidency. His decision was triggered by vocal expressions of dissatisfaction from a significant part of the surgical profession, largely women and people of colour. The most recent elections to leadership positions had produced a line-up of senior White men – all very distinguished – who seemingly came from very similar social backgrounds. It was felt that the College was not demonstrating itself to be a diverse and inclusive institution, reflecting the society in which we live or the changing profession of surgery. Any examination of the statistics show that the complaints are well founded. The reputation of the College is affected by such negative feelings, as disaffection can cause long-term loss of confidence in an organisation. MEDICAL PRACTITIONERS 2 Hospital Staff The people who work in any type of workplace, including hospitals, are called the staff. The medical staff in a British hospital belong to one of four main groups: • A pre-registration house officer (PRHO), or house officer, is a newly graduated doctor in the first year of postgraduate training. After a year, he or she becomes a registered medical practitioner. In the current system of training, the Foundation Programme, the name for these junior doctors is Foundation Year 1 doctor (FYI). • A senior house officer (SHO) is in the second year of postgraduate training. The title is now Foundation Year 2 doctor (FY2), but the old terms senior house officer and SHO are still used. • A specialist registrar (SpR) is a doctor who has completed the Foundation Programme, and is training in one of the medical specialties. There are also some non-training registrars -- doctors who have completed their training but do not wish to specialize yet. • A consultant is a fully qualified specialist. There may also be some associate specialists – senior doctors who do not wish to become consultants. In addition, there is at least one medical (or clinical) director, who is responsible for all of the medical staff. Medical Teams Consultant physicians and surgeons are responsible for a specific number of patients in the hospital. Each consultant has a team of junior doctors to help care for those patients. In many hospitals, there are multidisciplinary teams which consist not only of doctors but also of physiotherapists and other allied health professionals. When patients enter- or are admitted to –hospital, they are usually seen first by one of the junior doctors on the ward where they will receive treatment and care. -

The Royal College program directors handbook: A practical guide for leading an exceptional program Preface Consider the following tale, which will be familiar to many new program directors (PDs) in Canada: When I became a PD in 2000, I thought I knew what I was getting into. I was expecting a rewarding, challenging leadership position in education that would allow me to set a direction and provide support for a future generation of doctors. It turned out to be all of this and more; I’m thankful that the rewards far exceeded my expectations and the challenges were mitigated by a strong support network and resident cohort. But I admit that my initial experiences in the job were largely a trial by fire. I had a brief handover from my predecessor and was given a new book on the “newest thing” in residency education — CanMEDS 2000 — but other than that I was set adrift to run the residency program and seek help when needed. One resource that proved very helpful was the Royal College of Physicians and Surgeons of Canada’s annual workshop for new PDs and accreditation surveyors. I was assigned to the surveyor workshop by mistake; it turned out to be very useful, as our accreditation visit was scheduled to take place in 8 short months’ time, and I had still received no instruction on the nuts and bolts of running a program. KUSSEROW INSPECTOR GENERAL o AI-07 -88-00460 APRIL 1989 EXECUTIVE SUMMARY OBJECTIVES This inspection focuses on issues involving optometrsts ' providing postoperative care to Medicare beneficiares following catat surgery. The overall objectives of the inspection were to detennne: the extent and frequency of postoperative care by optometrsts; the extent of referral arangements between ophthalmologists and optometrsts; the manner of billng by ophthalmologists and optometrsts for cataract surgery when an optometrst provides postoperative car; and whether the practice of optometrsts ' providing cataract surgery postoperative care could lead to abusive referral arrangements, and possible duplicate bilings. BACKGROUND This program inspection was requested by the Administrtor of the Health Care Financing Ad ministration (HCFA), who was concerned about the issue of optometrsts ' providing postopera tive care to Medicare beneficiares who had cataract surgery. This practice increased after Medicare coverage was expanded by the Omnibus Budget Reconciliation Act of 1986. This legislation penntted coverage of optometrsts as physicians for any services they ar legally authorized to perform in the State in which they practice. All 50 States permit optometrsts to use diagnostic drgs. Funher, 23 States have passed legislation allowing optometrsts to use and prescribe therapeutic drugs, greatly expanding their scope of practice and ability to treat patients during the postoperative period following cataract surgery. METHODOLOGY Preinspection work included meetings and contacts with the Americ n Academy of Ophthal mology, the American Optometrc Association, State licensing boards, Medicare carrers, and HCFA staf. -

The views presented in this paper do not represent the official views of the Department or Minister for Public Expenditure and Reform. Executive Summary Features of Consultant Grade of employment . In the Irish public health service, a Consultant is a specialist grade of doctor working in the acute hospital or community sectors. The purpose of this paper is to ascertain the total number of WTE Consultants employed by the HSE and the categories of consultant specialty employed. It also aims to provide a definite figure on the Consultants pay bill of in the context of over-all public expenditure, use this to undertake an international comparison and draw policy conclusions in light of the analysis. Key Trends . Almost 3% of the total WTE health service employment are Consultants. In the five years from 2012, the number of Consultants employed by the HSE has grown twice as fast as total HSE employment; 18% compared to 9% growth in overall HSE staff.
