Carson Street Carson City, NV 89701-4747 Re: Annual Patient Safety Report per NRS 439.877 Spring Valley Hospital Medical Center Dea r Director Erdoes: Pursuant to NRS 439.877 Spring Valley Hospital Medical Ce nter, is required to annually submit a summary of its Patient Safety Committee Activities to your office, including information relating to the development, revision and usage of patient safety checklists and pa tient safety policies and a summary of the annual review conducted for the prior 12 month period. The following information is being provided in conformity with this requirement. Establishment of Patient Safety Checkli sts: Pursuant to t he provisions of NRS 439.877 the Patient Safety Committee adopted Patient Safety Checklists. These checklists are reviewed and modified as necessary based upon outcome and performance data on a yearly basis. Currently, Spring Valley Hospital Medical Cent er utilizes and monitors compliance with checklist s covering numerous patient areas. These include Hand Hygiene, Patient Ident ifica tion, Code Blue Cart Inspection, Central Line Insertion, (etc.) Checklist. Adoption of Pol icies, Procedures and Protocols: In conjunction with the checklists, Spring Valley Hospital Medical Center has developed and implemented policies, procedures and protocols to ensure compliance with the letter and intent of t he checkl ists. Patient Safety Compliance: During the ca lendar year 2019- 2020 the hospital Patient Safety Officer in conjunction with the hospital Qua lity Management and Risk Management Departments and Nursing leadership conducted reviews of staff and physician compliance with estab lished patient safety po Ii c i es and checklists. The committee discusses primary safety related topics affecting or having the potential to affect the facility, using a 4-step methodology to identify the issue, best practice, recommendations and implementation of safety measures, as well as monitoring for success or need for the application of additional measures. Shogren, Director of Communications Nevada Hospital Association 775-827-0184 • [email protected] LAS VEGAS HOSPITALS RESPOND TO SHOOTING CRISIS Health care personnel and patient needs met October 6, 2017 – LAS VEGAS, NEV. – In the minutes, hours and days after the unspeakable shooting in Las Vegas, Clark County hospitals activated emergency protocols to provide exceptional care to patients. University Medical Center of Southern Nevada, Sunrise Health which includes Sunrise Hospital & Medical Center, Southern Hills Hospital & Medical Center, and MountainView Hospital, The Valley Health System which includes Desert Springs Hospital Medical Center, Spring Valley Hospital Medical Center, Henderson Hospital, Valley Hospital Medical Center, Summerlin Hospital and Medical Center and Centennial Hills Hospital Medical Center, and the Dignity Health-St. Rose Dominican System including Rose de Lima, San Martin and Siena campuses have collectively treated and transferred more than 500 patients. Several hospitals still have patients admitted, some critical. Due to the incredible response from within the community, health care professional resources were available to meet the needs of patients impacted by this event. At this time, the situation has stabilized and is now in recovery mode. However, the situation will be closely monitored to ensure that the needs of those impacted are met. “The compassionate care provided to patients, families and loved ones by the health care professionals in our Las Vegas hospitals is unsurpassed,” Bill M.     Centennial Hills Hospital Launches Partnership with Nevada State College Nursing Students Beginning in September, eight nursing students from Nevada State College are embarking upon a new model of learning as they care for patients on the sixth floor of Centennial Hills Hospital once a week for the next two years. Known as the Caring Science Unit, the students are mentored by Centennial Hills Hospital nurses, who serve as Caring Coaches. In their role, they teach both clinical skills and caring practices to students as they work with patients. Caring practices, which are based upon the work of Dr. Jean Watson, focus on “providing care with loving kindness, having helping and trusting relationships, creating caring environments, meeting basic needs, and valuing the patient’s personal beliefs and faith,” according to the Watson Caring Science Institute website (2015). John Coldsmith, RN, MSN, DNP (c), Chief Nursing Officer for Centennial Hills Hospital, is excited to be part of the program. “Our ongoing mission is to provide a caring clinical environment which helps our nursing students transition from college to a new graduate nurse. It will teach them important clinical skills, along with how to show caring, compassion and empathy. All staff need to be able to genuinely connect with our patients and families. This builds trust with our patients who aren’t feeling well or may be receiving bad news.” Dr. Neal Rosenburg, Dean of Nursing at Nevada State College, is a long-time advocate of ensuring that Caring is the anchor to the nursing profession. MEDICAL CENTER The Valley ' Health System' October 23,2018 Legislative Committee on Healthcare To Whom It May Concern: Pursuant to Chapter 449 of the Nevada Revised Statuses, Spring Valley Hospital Medical Center, a 358 bed licensed hospital in Clark County, Nevada is enclosed the required elements of the bill. The hospital established a Staffing Committee on August 27,2009 and prepared a policy outlining the establishment, activities and progress, and measurement of efficacy of the Committee. We added a certified nursing assistant to the committee on September, 2013. Enclosed is the Committee Report describing the activities and resolutions of issues brought forward which determines the efficacy of the Committee. The Committee consists of not less than one-half of its total members from the licensed nursing staff as well as a certified nursing assistant and another half of those appointed by Administration. Thank you for your review of the enclosed report. In 2018, we celebrated these notable milestones: the second anniversary of Henderson Hospital, the 10-year anniversary of Centennial Hills Hospital and the 15-year anniversary of Spring Valley Hospital. We also opened our fi rst freestanding emergency center, The ER at Green Valley Ranch, which is an extension of Henderson Hospital’s emergency department. We also broke ground on Spring Valley Hospital’s freestanding emergency center, the ER at Blue Diamond, which opens July 2019. Our hospitals are training the next generation of healthcare providers through our Graduate Medical Education program. Recently, The Valley Health System was approved for a General Surgery Residency program by the Accreditation Council for Graduate Medical Education. Along with a focus on primary care physicians, we’re adding residencies for various specialties to meet our community’s needs.
