3.125(a) FACILITIES, GENERAL. An eleven year old male Thomson's named "Fodder" was able to escape his primary enclosure. Fodder sustained injuries which included two abrasions to the gazelle's legs and a jaw injury. The zoo's exhibit design team inspected the enclosure thoroughly and found no obvious exhibit failures. The zoo determined that all gates were closed and electric wire was on at the time of escape. All primary enclosure fences should be constructed in order to protect the from injury and to contain the animals. Correct by October 20,2016

2.40(a)(1) ATTENDING VETERINARIAN AND ADEQUATE VETERINARY CARE (DEALERS AND EXHIBITORS). *The current written program of veterinary care (3/16) has been signed by the attending veterinarian but has not been signed by a representative of the facility. A signature represents an agreement to the formal arrangements providing for regular veterinary care for the animals. The 2015 written program of veterinary care was also signed by just the veterinarian. *The current written program of veterinary care (3/16) needs more details. The facility has added the new lemurs to the plan but has not addressed TB testing. The facility is currently administering Heartgard to the otters but the parasite control section does not describe the regular use of this product. Lack of sufficient detail may lead to confusion of care required by the attending veterinarian. The formal arrangements between a consultant attending veterinarian and the exhibitor shall include a complete written program of veterinary that includes all regular preventative care for all species at the facility and has been agreed to by both the veterinarian and the facility. Correct by Nov. 14, 2016.

2.40(a)(1) ATTENDING VETERINARIAN AND ADEQUATE VETERINARY CARE (DEALERS AND EXHIBITORS). The written program of veterinary care (PVC) form signed by the facility owner on 4/6/16 has not been completed by the attending veterinarian. The attending veterinarian did sign the addendum/signature page on 4/6/16 but did not identify the frequency of vet visits to the facility, sign the PVC or include any written instructions for veterinary care. A written program of veterinary care is essential to ensure that preventative health care and emergency care for the animals are agreed upon by the attending veterinarian and the facility owner. Formal arrangements between the attending veterinarian and the exhibitor shall include a written program of veterinary care and regularly scheduled visits to the premises. Correct by 4/30/17.

2.40(b)(2) ATTENDING VETERINARIAN AND ADEQUATE VETERINARY CARE (DEALERS AND EXHIBITORS). One female brown and white goat was noted to have a thin haircoat over her back, sides and the outside of her rear legs. The skin visible on her abdomen appeared pinker than normal. In addition she was noted frequently licking at her flanks and scratching at her ears/neck. Itching and hair loss can be symptoms of several conditions some of which may be contagious. The goats need to be examined by a veterinarian to ensure that a diagnosis is made regarding the itching and hairloss and that a treatment plan is instituted. Correct by 4/30/17.

3.55 REPEAT WATERING. The water receptacle for the rabbits is dirty and requires cleaning and sanitizing. There is a visible green layer of algae and growth along sides and bottom of the water pan. There is small amount of greenish debris floating on the top surface of the water. Drinking contaminated water can lead to disease and illness or may cause animals to stop drinking leading to dehydration. All water receptacles must be cleaned and sanitized when dirty and at least once every 2 weeks.
