Healthcare Entrepreneurship: Conception, Growth and Scaling

Concept note by Devendra Tayade and Prakash Satyavageeswaran

Introduction 80% of the curative healthcare in India is delivered through private players and roughly 70% of it is financed through out of pocket expenditure, which often has catastrophic effects, pushing families into debt traps. Government is making an attempt towards providing better physical and financial access to healthcare through its two pronged Ayushman Bharat scheme, wherein it plans to open another 1.5 lakh health and wellness centres and provide health insurance worth Rs. 5 lakhs per family for 40% of the people that is at the bottom of the economic pyramid through the National Health Protection Scheme. This still won’t be enough if India has to realise its dream of providing equitable healthcare access to everyone. In addition to Government’s efforts, the private players (covering the current 80% of healthcare provision) will have to consider delivering care that is equitable. Often these private facilities are started with an intent to serve a larger population and eliminate suffering. However, the current realities at these facilities may not reflect the original purpose. The question, therefore, is how can the private healthcare providers be supported, enabled, and nudged to provide more equitable healthcare? This large group of private healthcare providers, which is knowingly or unknowingly on an entrepreneurship journey, needs to be understood with regards to the challenges they face as well as the objectives they want to fulfil. The upcoming EHAC meeting will thus focus on the healthcare provider as an entrepreneur and explore how this group can be empowered and nudged to provide equitable healthcare.

Sub-Themes The meeting will start with the Government’s perspective on healthcare first. Following that, it will have following sub-themes to consider how the private sector can supplement the Government’s efforts through entrepreneurial efforts.

Dream Vs Reality: People often start healthcare enterprises with good intentions but get side-tracked along the way. Under this theme, the discussion will be among healthcare entrepreneurs about what had they envisioned when they had started their entrepreneurial journey and where they are now. Have their dreams/ objectives behind starting a hospital been fulfilled? If yes, then how and if not, then what were the challenges they faced that ultimately took them away from their objective?

Is Growth Necessary? At what cost? Once a typical hospital starts functioning, the next goal is to scale rapidly. The focus of the discussion relating to this theme is to explore why if all, is it imperative for a hospital to grow and at what cost? If the reasons for growth are valid, what options are available to the hospital’s management to finance the growth?

Ways of financing (Debt, Equity, patient capital) and their limitations: Under this theme, we will explore various instruments of financing. Each instrument has its pros and cons; a

8 detailed discussion about these options will help healthcare providers get a clearer understanding about how to finance the growth of the hospital.

Barriers to provision of equitable healthcare - Managers’ perspective: In recent times, professional managers have been blamed for introducing exploitative practices in healthcare and thus increasing the trust deficit between healthcare providers and patients. Under this theme, we will explore various forces that influence managerial decision making in the context of managing a hospital.

How does Technology change the economics of healthcare - It is widely accepted that research can lead to improved health outcomes. However, translating research into meaningful impact in peoples’ lives requires actions that stretch well beyond those traditionally associated with knowledge creation. Technological change brings benefits in addition to costs. Under this theme we will explore how data from health research is being interpreted and used to make clinical products.

Guest Speaker Profiles

Dr. Aravind Srinivasan Aravind Eye Hospital Dr. Aravind Srinivasan graduated in medicine from PSG Institute of Medical Sciences, , South India in 1992 and completed his residency in ophthalmology leading to Masters in Ophthalmology from Aravind Eye Institute and Postgraduate Institute of Ophthalmology, , South India in 1996. In 2000 he did his MBA with specialization in strategy from the University of Michigan, Ann Arbor, USA.

After his MBA, he took over as the Administrator of Aravind Eye Care System and was instrumental in bringing changes to the various facets of the organization. From 2011, he is serving the organization in the role of Director-Projects of Aravind Eye Care System. He is on the Board of Govel Trust that runs the Aravind Eye Hospitals. From September 2017, he is leading the Aravind Eye Hospital Chennai as Chief Medical Officer.

Dr. Aravind’s area of specialization concentrates on overall evaluation and interpretation of performance of each Division and to track progress of performance so as to facilitate benchmarking both internally and externally to achieve patient centred care across the system. Besides administration of the hospitals and grooming managers, Dr.Aravind also evaluates new projects and ensures replication of the “ARAVIND Way” - As a Trainer and Mentor at Management courses, he contributes in extending the Aravind Model of high volume, high quality, affordable cost eye care to developing nation

Dhruv Joshi Cloudphysician Dhruv Joshi is a Pulmonary and Critical Care specialist who trained at the Cleveland Clinic Foundation, USA. He graduated medical school from St John’s Medical College, Bengaluru, India, following which he completed his Internal Medicine training at the Good Samaritan Hospital, USA.

Dhruv has a keen interest in the improvement of quality of care delivery. His clinical focus was on advanced respiratory failure, mechanical ventilation and pulmonary hypertension. During his residency and fellowship, Dhruv was involved in numerous projects focused on quality improvement of healthcare delivery, improvement of patient outcomes and optimizing resource utilization. Several of his endeavors have resulted in peer reviewed publications.

Dileep Raman Cloudphysician Dileep Raman is a Pulmonary, Critical Care and Sleep specialist who trained at the Cleveland Clinic Foundation, USA. He graduated medical school from the Government Medical College Thrissur, India, with a distinguished gold medal in internal medicine for academic excellence. Following which he completed his residency in Internal Medicine at Texas Tech University, USA, where he served as Chief Medical Resident.

Dileep is interested in training and has numerous teaching awards for resident and fellow education including the Cleveland Clinic Foundation teaching excellence award. His academic and clinical interests center around mechanical ventilation nomenclature, ARDS, neuromuscular respiratory diseases, interstitial lung diseases, thoracic and procedural ultrasound and heart-lung interactions in sleep.

Dhruv and Dileep moved back to India from the US in 2015 and in 2017 started Cloudphysician Healthcare, a healthcare technology company that leverages technology to remotely provide ICU expertise to hospitals. Cloudphysician is based in Bangalore and hospitals across 7 states in India use their solution to manage their ICU patients.

Dushyantsinh Jadeja Google India Dushyant works as a Program Manager in the Google Health Team and is responsible for leading research and product development efforts in India.

Prior to Google he held similar roles at Intel & Microsoft.

In his free time, he learns a new language and visits a new place every year.

Jagdeep Gambhir Karma Healthcare Jagdeep is the CEO and Co-Founder of Karma Healthcare in Udaipur, a startup working in the area of nurse-assisted e-health.

Karma Healthcare aims to provide medical services to the under- served communities in India, thereby improving health outcomes by early detection and evidence-based treatment.

Prior to Karma Healthcare, Jagdeep was a Senior Program Manager at the Population Foundation of India. He also worked with Innovators in Health (a nonprofit working in rural Bihar on Tuberculosis) and with Goldman Sachs as a Technology Analyst. Jagdeep is an alumnus of Indian School of Business, Hyderabad and NITK, Surathkal.

Karuna Jain Enzia Ventures A Chartered Accountant by profession and an alunnus of the Indian School of Business, Hyderabad, Karuna Jain is the Founding Partner of Enzia Ventures.

She is an advisor to marquee businesses and startups in healthcare. She has worked for Acumen Fund as the Head, of India Investments and was their Global Health Lead.

Karuna's 12 + years of industry experience has been in the areas of private equity , M&A and investing portfolio management. Prior to Acumen she has also worked at Milestone Capital, Aditya Birla Group, Dr. Reddy's Laboratories to name a few.

Manoj Gopalakrishna Prozela Healthcare Manoj Gopalakrishna, a Wharton Alumnus , has over twenty eight years of leadership experience in Global as well as Indian healthcare organizations.

Manoj is the Co Founder and Chairman of Prozela Healthcare, a professional services firm enabling medical technology companies and healthcare providers design and execute transformative initiatives for accelerating their growth and impact. Manoj is active participant in the health-tech start-up ecosystem and was the Chairman of Biosense Technologies (currently acquired by Perkin Elmer) and Management Advisor to AINDRA, Life Voxel and Secmed - start-ups focused on developing Artificial Intelligence based solutions for healthcare delivery.

He has been a key contributor to the transformation of Hindustan Latex Limited a leading healthcare organization with diversified product, service and program portfolio. Post his career with Hindustan Latex Limited, he joined Becton Dickinson and Co (BD) as Managing Director leading its South Asia operations. He was a member of the worldwide leadership team of BD, a Fortune 500 medical technology company. Under his leadership BD grew to a company with manufacturing, R&D and marketing operations in India.

Manoj’s last career assignment was with HCL as CEO of its healthcare business. As CEO of HCL Healthcare he has pioneered development of primary ambulatory care centres in Delhi NCR in partnership with Johns Hopkins International.

Rahul Chatterjee iKure Rahul Chatterjee is a professional with over 25 years’ experience in enterprise & technology solutions for healthcare, management consulting and business unit management. He has significant competence in solutions for the healthcare provider segment and population health management.

Rahul is currently the Chief Growth Officer of iKure, a technology enabled primary healthcare start-up. He is responsible for driving strategy, growth, operations, partnerships and initiatives for fund raising.

Rahul has earlier worked for companies like Reliance Industries, Siemens IT Solutions & Atos. Has played pivotal roles in managing start- ups through the incubation stage during 2016-2018. He has specific expertise in delivery and program management, implementation of IT solutions, business unit management, management consulting, sales & pre-sales, business planning, investor engagement, fund raising and establishing systems & processes.

M.D. Sriram Aurolab

Mr. R.D. Sriram is associated with Aurolab since its inception. He is an experienced professional in all aspects of identifying opportunities for strategic business expansion. As a founding member of Aurolab, Sriram has been responsible for the company's high quality delivery excellence - model. He has accomplished a number of significant milestones for Aurolab.

He has served as a Production engineer in 1992 and thereafter held the post of Manager-Operations and is currently the Managing Director of Aurolab.

Mr.Sriram holds his B.E in Electronics and Instrumentation from Annamalai University, Chidambaram.

Brief profile of the Aravind built a sustainable eye care delivery system which actually gave Organization away over 60% of its services free or at a very steeply subsidized rate often taking its services to the doorstep of rural India and yet remaining financially very viable. This model grew an eleven-bed eye clinic in an old temple city of Madurai in South India into one of the largest and most productive eye care facilities in the world. Thulasiraj Ravilla Mr. Thulasiraj Ravilla has been working in the field of eye care since 1981 at Aravind Eye Care System. He is an active member of the leadership team that established a sustainable, scalable model of service delivery which serves all strata of the society. This has since been studied widely and replicated by eye care providers across the globe. In 1992, he shifted his focus to capacity building, training and consultancy by setting up the Lions Aravind Institute of Community Ophthalmology (LAICO) which by 2017, had worked with over 350 hospitals across the globe.

With his expertise in setting up culturally relevant solutions to the problem of avoidable blindness, he has served as an Advisor on several national and international bodies: he was the South East Asia’s Regional Chairman for the International Agency for the Prevention of Blindness (2000-2005) and till recently was the President of Vision 2020 The Right to Sight: India. He has been a Technical Advisor to India’s National Programme for the Control of Blindness and to the World Health Organization.

Brief profile of the Basic HealthCare Services (BHS) is a specialized not for profit Organization organization set up in the year 2012, driven with a vision of high quality and low-cost healthcare and nutrition services to the last mile communities, with dignity. BHS, in partnership with Aajeevika Bureau, has designed and set up a unique initiative of AMRIT Clinics that provide round the clock, high quality and affordable healthcare services, to a population of 80,000 living in rural, remote locations.

BHS also partners with government to manage a public Primary Health Center (PHC), providing healthcare to an additional 6000 families, most of who are tribal. Recognizing high levels of childhood malnutrition in these areas, BHS has also set-up day care centers (called AMRIT Phulwaris) within the clinic catchment areas to provide nutrition and childcare to children under five years of age.

Based on its experience, BHS has developed tools, norms and guidelines to help set up primary health care services in similar remote and rural underserved communities. Our help has been sought by some state governments and some NGOs for setting up Primary Health Care Clinics in similar areas. Brief profile of the The Tata Memorial Centre constitutes the national comprehensive Organization cancer centre for the prevention, treatment and research on Cancer and is comparable to any similar center in the world. This achievement has been possible due to the far-sighted and total support of the Department of Atomic Energy, responsible for managing this Institution since 1962.

Nearly 64,000 patients visit the clinics annually not only from all over India but also from neighboring countries. Nearly 60% of these cancer patients receive primary care at the Hospital of which over 70% are treated free of any charges. Over 1300 patients attend the OPD daily for medical advice, comprehensive care or for follow-up treatment. The Hospital has expanded from a 80 bed Hospital covering an area of 15,363 sq. meters to a 629-beds spread over 53,890 sq. meters. Our Annual Budget was Rs.5 lacs in 1941 - today it runs close to Rs.200 crores.

Joined Indian Railways in 1957 as a Special Class apprentice, then worked as Asst. Mechanical Engineer, moving up to Chief Motive Power Engineer.

He voluntarily retired in 1982 to move to Industry, working in different roles as General Manager Indian Oxygen, Joint MD Usha Telehoist, VP (Tech.& operations) Shalimar Paints till 1988. In 1988 he joined Royal Commonwealth Society for the Blind U. K. as their Regional Director for India where he served till Oct 1997.

On reaching 60 years he took retirement from employment, and started working as a volunteer in Eye care with L.V. P E. I, Aravind Eye Care, SEVA Foundation USA. He is also serving on the board of SEVA, So-Hum Foundation USA.

Dr. Rajesh Iyer

Dr. Iyer is a consultant neurologist at Vikram Hospital in Bangalore. Before his association with Vikram Hospital, he was associated with Narayana Hrudayalya Institute of Neurosciences where he established the Comprehensive Epilepsy Care Center. He has also served as a faculty for several of the regional and national Epilepsy Conferences and Workshops. He is also the president of Karnataka Chapter of Multiple Sclerosis Society of India.

Dr. Sanjeev Upadhyaya

Dr Sanjeev Upadhyaya is presently working as the Health Specialist, UNICEF, Hyderabad (India), where he is in-charge of UNICEF’s health activities in three large south Indian states - Andhra Pradesh, Telangana and Karnataka. He is instrumental in grounding Facility Based Newborn Care, Kangaroo Mother Care, Labour Room Standardization, and Midwifery in these states. Dr. Pooja Sanghvi

Dr Pooja Sanghvi is a dentist turned public health professional having worked in many roles in women’s health, child health and community eye care.

She is an alumnus of Nair Hospital Dental College, Mumbai and Tata Institute of Social Sciences, Mumbai from where she attained her Masters in Hospital Administration. During her stint at Sankara Eye Foundation – India, she found motivation and joy in working towards promoting accessibility of healthcare. She went to work at Avni Health Foundation, Mumbai to work extensively in reproductive and maternal health.

Currently, she is working at Collective Good Foundation, on the Home Based Newborn Care program, in collaboration with the Tata trusts and Government of Maharashtra.

Rajesh is a doctoral student (4th Year) in Marketing area at ISB, Hyderabad. His specialization is in Quantitative Marketing, including Marketing Analytics, Pricing and use of ML/AI in Marketing. Before joining his doctoral studies, he had worked in the IT Consulting for over 13 years and as a Research Assistant in IIM Bangalore for one year.

Rajesh believes that while the need for a good return on marketing initiatives is important in businesses, it is even more crucial in Nonprofit organizations. Hence he wants to work with NGOs and nonprofit organizations in the healthcare space on research engagements so that he can use his learning to help the organizations measure the impact of their initiatives, optimally run those initiatives, and help with getting actionable insights out of their data.

Dr. Sathya Bala

Dr. Sathya Bala is a fertility specialist. Worked in Fertility units from 1999. Trained in Reproductive Medicine from CMC Vellore. Involved in teaching and training Post doctoral Fellows in Reproductive Medicine since 2005. Been an Examiner for National Board of Examinations and MRCOG. Recently been accepted as FRCOG. Currently working as Clinical Director Cloudnine Fertility.

Also working in resource limited settings in VHS hospital Taramani, St Anthony hospital Puzhal and Julia hospital Uthukottai.

Keen on setting up a self sustaining IVF unit in a suburaban/rural setting.

Vasanthi Ranganathan

Vasanthi Ranganathan is the Resource head, Lake View Learning and Development center. She completed her PG and teaching degrees in English from India, management and Career Counseling and Guidance from the US. A Fulbrighter she introduced a number of outcome based learning in educational institutions and industries. East west center recognized her with Makana award in 1989 at Honolulu and at Seoul in 2018 for her work in the community.

Her work with healthcare include Aravind Eye Hospital and , Meenakshi Mission Hospital, Apollo hospitals, As President of Rotary Club of Ambattur, 13-14 expanded Ambattur Rotary hospital and conducted monthly general medical camps in construction sites. Established Flicker to Flare Consultants Pvt Ltd 1991, launched trainers throughout India, worked for corporate, private and public sectors. Introduced the 1st abacus program in India, 1998 creating educational entrepreneurs at all levels throughout the country. Sits in the POSH committee of companies.

This pilot collaboration is to help foster livelihood opportunities for these students. They are exposed to functioning of a Hospital. Need observed on the ground for Amrit Clinics are nurse-led primary health care clinics based in undertaking the pilot remote villages of Udaipur. Setting up quality assurance processes that are both practical and beneficial for a set-up of this kind was challenging. BHS was preparing for the NABH accreditation of these clinics since 2017.

Amrit Clinics has experience of ground realities in running such facilities, while LVPEI has the technical expertise in setting up processes, preparing the team and accreditation for a hospital.

This pilot gives both parties the opportunity to work on standards to assure the highest quality for primary healthcare centers in India often riddled with resource constraints. Desired outcome from the pilot Amrit Clinics finally certified with NABH

Activities completed (till date)  Prepared 9/13 SOPs  Regular convening of necessary Infection control and Quality assurance committee meetings, and minuted  Applied for all necessary statutory licenses and received acknowledgement- Fire NOC, pharmacy, Clinic establishment, Bio waste management, pollution control board.  Finalized infrastructural changes to be made to comply with standards.  Collecting and analyzing data for quality indicators on a monthly basis.  Started conducting training of clinic team in the SOPs.  Prepared drafts of new audit checklist to monitor infection control, facility safety and documentation Impact of the achieved outcomes Setting up of committees for infection control and quality so far assurance, as well as preparing SOPs has provided an opportunity to discuss lapses in quality on a regular basis and has made processes clear for the Amrit Clinic team.

Lactation Counselor Course

Need observed on the ground for Urbanization had led to several mothers moving away from undertaking the pilot breast feeding, to using various substitutes such as artificial milk/formula milk, animal milk and water, thereby, unknowingly depriving their infants. This deprivation makes infants 15 times more prone to death due to pneumonia and 11 times more prone to death due to diarrhoea. Spread awareness and sensitize women on the importance of lactation.

Desired outcome from the pilot Through this course, lactation consultants will be trained and equipped to encourage mothers to breastfeed, provide nutrition advice, provide counseling, along with physical and emotional support to mothers.

Activities completed (till date) Developed training modules and handed over to SAKSHAM

Impact of the achieved outcomes Trained counselor who provide new mothers access to correct so far information and help them develop a positive attitude towards breastfeeding.

Primary Healthcare Initiative

Need observed on the ground for The rural and tribal belt of Southern Rajasthan is distinctly undertaking the pilot underserved in terms of availability of and access to health services, low HDI (less than 0.5) and high disease burden (higher than national and state average). Different providers of health services co-exist which includes public health system, private (for- profit and not-for-profit) providers.

There is significant scope for primary healthcare initiatives to supplement the services of the public health system to contribute to universal coverage of primary healthcare in the country. Therefore, the search for comprehensive, equitable, self-sustained initiatives (with or without partnership with government for primary healthcare) and the endeavour to replicate these primary healthcare models in underserved populations, continues.

The Initiative involves designing, testing and evaluating a primary healthcare model at the block level. It aims to supplement the public health provision by leveraging other ethical providers in region and optimizing community contributions to good quality care.

Desired outcome from the pilot In the chosen block: 1. Increased proportion of rural populations have access to high- quality, responsive healthcare

2. Reduced morbidity and mortality in the families, especially maternal and infant

3. Reduced unmet needs for communicable (especially TB and diarrhea) and non-communicable (especially HT, DM and COPD)

4. Reduced out-of-pocket expenditure on healthcare

5. Improvement in positive health behaviours and appropriate care-seeking (especially for key MCH, communicable and non-

Desired outcome from the pilot The "Critical Care Assistant” certification program has been specially designed by PTH for SSES students and is for the duration of one year. Thereafter the students will be absorbed by PTH and given full time jobs should they wish to continue. This certification will also help them get better paying jobs with other hospitals.

Activities completed (till date)  8 students are in training  Training, Food and Stay arrangements sponsored free of cost to the students by PT Hospital  Stipend of Rs.8000/- per month is being given to the students Impact of the achieved outcomes Gives the students a whole year to study and learn on the job. so far Makes them market ready as they are now exposed to a real hospital and how it works.
