All articles, columns, news releases, book reviews, ethics opinions, judicial reprimands, proposed rule announcements, and final rule announcements with an identified author are also indexed in the author index. Kentucky Bench and Bar Index – 2014-2019 v How to Use This Index Arrangement of Index: General Arrangement: The Kentucky Bench & Bar Cumulative Index: 2014-2019 is divided into two sections: Subject Index and Author Index. Use of each section is described below. In both sections, headings appear in alphabetic order. Numeric entries are treated as if the numbers had been spelled out. Under each heading or sub-heading, entries appear in reverse chronological order (with the newest material appearing first). If there are multiple entries under a heading that have the same date, entries with higher page numbers appear first. Minutes of the Kentucky Bar Association Board of Governors meetings are included only under the topic of “Kentucky Bar Association Board of Governors – Minutes” and have not been indexed by subjects covered in the minutes. Subject Index: The subject index lists entries by subject, as well as by personal names and other proper names (for articles which discuss specific individuals or institutions). Subject headings and Sub-headings are continued from the Kentucky Bench & Bar Cumulative Index: 2014 Edition with new entries added where appropriate. An entry in the subject index consists of six elements: Title, Author, B & B – Bench & Bar volume, issue, page on which the article begins, and date of the issue. See the sample entry below. Volume Page Issue Date Title: Capitalization, punctuation, and italicization of titles have been changed, where necessary, for consistency in the formatting of this index. In many issues of the B & B – Bench & Bar, several articles have been collected under a running title such as “Immigration Law.” This running title has been included only if the title of the article is awkward or unclear without the running title. Otherwise this running title has been omitted. Regular authors often have a column which has a column name, such as Michael Losavio’s “Shop Talk” column or the “President’s Page.” If the article has a title, the name of the column is omitted. If the article does not have a title, then the column name is used to identify the article. The only exceptions to this rule are book reviews and Hot Topic articles. Book reviews always include the identifier “Book Review:” preceding the title. Hot topic articles will always include the identifier “Hot Topic:” preceding the title. Author(s): Immediately following the title, the author’s name is listed. Titles such as Professor or Judge are omitted. Otherwise, author’s names are listed exactly as they appeared in the original journal. If the article was written by two or more authors, the authors are listed in the order in which they appeared in the original journal. Kentucky Bar Association Financial Statements and Independent Auditors reports do not have either the Kentucky Bar Association or the independent auditor credited as authors in the subject index. Hilyerd,William A. vi Kentucky Bench and Bar Index – 2014-2019 Citation elements: The volume number of the B & B – Bench & Bar is listed first, followed by the issue number (in parenthesis). B & B – Bench & Bar is issued six times per year. Following the issue number is the page number on which the article begins. Occasionally, materials which were issued as a supplement to or a special insert in an issue of the B & B – Bench & Bar are included in the index. In this case, the page number has been replaced with the word “supplement.” Hot topic articles (available only on the Bench & Bar website), have the URL of the article instead of the page number. Finally, the date of the issue is included (in parenthesis). Many terms which were not used as subject headings are also included in the index. These terms are included (along with a “see” reference) to assist the user in locating information relating to those topics. Many subject terms also have a “see also” reference to refer the user to other subject headings where related material can be found. In the PDF version of this index, the “see” and “see also” references are hyperlinked for the convenience of the user. Examples: Copyright - See Intellectual Property. Housing (See also Real Property.) Author Index: The author index lists entries alphabetically by author’s name. Only articles which have one or more identified authors are included. If an article was written by two or more authors, it is indexed under each author. If an author has written under multiple variations of their name, the most complete version of their name is used as the Author Heading, with any shortened versions appearing in parenthesis after their name. Example: Chen, James Ming (Jim). Some authors have written under more than one last name (i.e.
