It can result from trauma during childbirth. There can be bruising, swelling, superficial grazes, minor lacerations or tears or episiotomies. Some tears and all episiotomies require suturing. The sutures used are dissolvable and will disappear gradually over a period of a few weeks. Tears can be classified as: First Degree Superficial tear extending through the vaginal tissue and/or perineal skin Second Degree Extending into the perineal muscles Third Degree Involving the external anal sphincter Fourth Degree Involving the anal sphincter and anal tissue (rectum – the lowermost part of the bowel) If you have had a third or fourth degree tear, you will have a 6-8 week follow-up appointment with the physiotherapist. 1 If you have had an episiotomy or tear, you should be How to do a pelvic floor contraction: completely healed by 4-6 weeks after the birth of your baby. Unifesp- Brasil RELATION BETWEEN VAGINAL HIATUS AND PERINEAL BODY LENGTHS WITH EPISIOTOMY IN VAGINAL DELIVERY Hypothesis / aims of study The aim of the study was to assess the relationship between vaginal hiatus and perineal body lengths with the occurrence of episiotomy during vaginal delivery. Study design, materials and methods It´s a cross-sectional observational study with a consecutive sample of 60 parturients, made from July 2009 to March 2010 in the Obstetric Center at University Hospital in São Paulo, Brazil. Inclusion criteria were parturients at term (37 to 42 weeks gestation) in the first stage of labour, with less than 9 cm dilatation, with a single fetus in cephalic presentation and good vitality confirmed by cardiotocography. Exclusion criteria were parturients submitted to cesarean section or forceps delivery. The patients were evaluated in the lithotomic position. The measurement was performed in the first stage of labour, by the same examiner using a metric measuring tape previously cleaned with alcohol 70% and discarded after each use. The vaginal hiatus length (distance between the external urethral meatus and the vulvar fourchette) and the perineal body (distance between the vulvar fourchette and the center of the anal orifice) were evaluated. For statistical analysis the SPSS (Statistical Package for Social Sciences) version 17® was used, applying Mann-Whitney Test and Spearman Rank Correlation Test to determine the importance of vaginal hiatus and perineal body length in the occurrence of episiotomy, with p<0.05. -

Advice following a 3rd or 4th degree perineal tear Information for patients This leaflet can be made available in other formats including large print, CD and Braille and in languages other than English, upon request. During the delivery of your baby you have had a 3rd or 4th degree tear to your perineum (the skin and muscles around the entrance to your vagina). This leaflet tells you how it has been repaired and gives advice about how you can help it to heal. What is a 3rd or 4th degree perineal tear? During childbirth it is common to get tears of the skin and muscles around the entrance to the vagina. Sometimes tears can reach from the vagina to the muscle around the anus (the opening to the back passage). This is known as a 3rd degree tear. Sometimes the rectal mucosa (lining of the lower bowel) may also be slightly torn. This is then known as a 4th degree tear. About 1 woman in every 100 who have a vaginal delivery can have 1 a 3rd or 4th degree tear. Why does a perineal tear happen? Anyone can get a perineal tear during a vaginal delivery. Some reasons which may increase the chances of a 3rd or 4th degree perineal tear happening include: when you have your first baby having a large baby the direction the baby is facing at delivery shoulder dystocia (one of your baby's shoulders becomes stuck behind your pubic bone) induction of labour having an epidural having a long labour or you are pushing for a long time 1 having a forceps or suction delivery. -

Maternity Information for Childbirth Services
Maternity information for childbirth services What you need to know 20905-3-17 New York State’s Maternity Information Law requires each hospital to provide the following information about its childbirth practices and procedures. This information will help you to better understand what to expect, learn more about your childbirth choices, and plan for your baby’s birth. Data shown are for 2014. Most of the information is given in percentages of all deliveries occurring in the hospital during a given year. For example, if 20 births out of 100 are by cesarean section, the cesarean section rate will be 20 percent. If external fetal monitoring is used in 50 out of 100 births, or one-half of all births, the rate will be 50 percent. This information alone doesn’t tell you that one hospital is better than another. If a hospital has fewer than 200 births per year, the use of special procedures in just a few births could change its rates. The types of births could affect the rates as well. Some hospitals offer specialized services to women who are expected to have complicated or high-risk births, or whose babies are not expected to develop normally. These hospitals can be expected to have higher rates of the special procedures than hospitals that do not offer these services. This information also does not tell you about your doctor’s or nurse-midwife’s practice. However, the information can be used when discussing your wishes with your doctor or nurse-midwife, and to find out if his or her use of special procedures is similar to or different from that of the hospital. -

Methods: All nulliparous women registering at the maternity health care in Region Örebro County, Sweden, in early pregnancy between 1 October 2014 and 1 October 2017 were invited to participate in a prospective cohort study. Data on maternal and obstetric characteristics were extracted from questionnaires completed in early and late pregnancy, from a study-specific delivery protocol, and from the obstetric record system. These data were analyzed using unadjusted and adjusted multinomial and logistic regression models. Results: A total of 644 women were included in the study sample. Fetal weight exceeding 4000 g and vacuum extraction were found to be independent risk factors for both second-degree perineal tears (aOR 2.22 (95% CI: 1.17, 4.22) and 2.41 (95% CI: 1.24, 4.68) respectively) and OASI (aOR 6.02 (95% CI: 2.32, 15.6) and 3.91 (95% CI: 1.32, 11.6) respectively). Post-term delivery significantly increased the risk for second-degree perineal tear (aOR 2.44 (95% CI: 1.03, 5.77), whereas, maternal birth positions with reduced sacrum flexibility significantly decreased the risk of second-degree perineal tear (aOR 0.53 (95% CI 0.32, 0.90)). -

Perineal Massage
Perineal massage Why massage the perineum? Around 85% of women who give birth vaginally sustain some degree of perineal trauma. Most heal without any problems or adverse effects, but for some women there may be longer term implications. It is thought that massaging the perineum during pregnancy may increase muscle and tissue elasticity and make it easier to avoid tearing during a vaginal birth. What is the evidence for perineal massage? In 2006, Beckmann and Garrett combined the results from four randomised, controlled trials that enrolled 2,497 pregnant women. Three out of the four studies included first time mothers only. All four of the studies were of good quality. The findings were that women who were randomly assigned to do perineal massage had a 10% decrease in the risk of tears that required stitches (‘perineal trauma’), and a 16% decrease in the risk of episiotomy (a surgical cut to the perineum as the baby is born). These findings were only true for first-time mothers. However, the mothers in the perineal massage group reported a 32% decrease in ongoing perineal pain at three months after having their baby. Surprisingly, Beckmann and Garrett found that the more frequently women used perineal massage the less likely they were to see any benefits. Women who massaged 1 - 2 times per week had a 17% reduced risk of perineal trauma. -
