No targeted treatment for COVID-19 infection has been identified so far, thus supportive treatment, invasive and non-invasive oxygen support, and corticosteroids remain a common therapy. High-flow nasal cannula (HFNC), a non-invasive oxygen support method, has become a prominent treatment option for respiratory failure during the SARS-CoV-2 pandemic. Methods: The First Aliated Hospital of Xiamen University in January, 2019 was collected The clinical data of 229 children with severe adenovirus pneumonia from January to January 2020 were divided into obliterative bronchiolitis group (BO group) and non obstructive bronchiolitis group (non BO group) according to the follow-up clinical manifestations and imaging data. The clinical data, laboratory examination and imaging data of the children were retrospectively analyzed. Results: Among 229 children with severe adenovirus pneumonia, 46 cases were in BO group. The number of days of hospitalization, oxygen consumption time, LDH, IL-6, AST, D-dimer and hypoxemia in BO group were signicantly higher than those in non BO group; The difference was statistically signicant (P < 0.05). Univariate logistic regression analysis showed that there were signicant differences in the blood routine neutrophil ratio, platelet level, Oxygen supply time, hospitalization days, AST level, whether there was hypoxemia, timing of using hormone, more than two bacterial feelings were found in the two groups, levels of LDH, albumin and Scope of lung imaging (P < 0.05). -

We describe a Center case of intrapulmonary A-V shunt as the etiology of an acute ischemic event. A 63 year old male with a past history of (Dominik supraventricular tachycardia and recurrent deep vein thrombosis; who has been non-compliant on Rivaroxaban, presents with Zampino) pleuritic chest pain and was found to have a right lower lobe pulmonary embolus. The deep vein thrombosis and pulmonary embolus were not significant enough to warrant ultrasound-enhanced thrombolysis by Ekosonic EndoWave Infusion Catheter System, and the patient was subsequently restarted on Rivaroxaban and discharged. The patient presented five days later with left arm tightness and was found to have multiple areas of punctuate infarction of both cerebellar hemispheres, more confluent within the right frontal lobe. Of note he was compliant at this time with Rivaroxaban. The patient was started on unfractionated heparin drip and subsequently admitted. On admission, his vital signs showed a blood pressure of 138/93, heart rate 65 bpm, and respiratory rate 16. Cardiopulmonary examination revealed regular rate and rhythm, without murmurs, rubs or gallops and his lungs were clear to auscultation. Neurologic examination revealed intact cranial nerves, preserved strength in all extremities, mild dysmetria in the left upper extremity and an NIH score of 1. -

172 Acute Medicine 2012; 11(3): 172-182 Trainee Section 172 Problem based review: Pleuritic Chest Pain RW Lee, LE Hodgson, MB Jackson & N Adams Abstract Pleuritic pain, a sharp discomfort near the chest wall exacerbated by inspiration is associated with a number of pathologies. Pulmonary embolus and infection are two common causes but diagnosis can often be challenging, both for experienced physicians and trainees. The underlying anatomy and pathophysiology of such pain and the most common aetiologies are presented. Clinical symptoms and signs that may arise alongside pleuritic pain are then discussed, followed by an introduction to the diagnostic tools such as the Wells’ score and current guidelines that can help to select the most appropriate investigation(s). Management of pulmonary embolism and other common causes of pleuritic pain are also discussed and highlighted by a clinical vignette. In susceptible individuals, inhalation of Aspergillus spores can affect the respiratory tract in many ways. These spores get trapped in the viscid spu- tum of asthmatic subjects which triggers a cascade of inflammatory reactions that can result in Aspergillus-induced asthma, allergic bronchopulmo- nary aspergillosis (ABPA), and allergic Aspergillus sinusitis (AAS). An immunologically mediated disease, ABPA, occurs predominantly in patients with asthma and cystic fibrosis (CF). A set of criteria, which is still evolving, is required for diagnosis. Imaging plays a compelling role in the diagno- sis and monitoring of the disease. Demonstration of central bronchiectasis with normal tapering bronchi is still considered pathognomonic in pa- tients without CF. Elevated serum IgE levels and Aspergillus-specific IgE and/or IgG are also vital for the diagnosis. Mucoid impaction occurring in the paranasal sinuses results in AAS, which also requires a set of diagnostic criteria. Demonstration of fungal elements in sinus material is the hall- mark of AAS. -

Thorax, 33, 124-127. Acute pleurisy in sarcoidosis. A 47-year-old white man with sarcoidosis presented with a six-week history of acute painful pleurisy. On auscultation a loud pleural rub was heard at the left base together with bilateral basal crepitations. The chest radiograph showed hilar enlargement as well as diffuse lung shadowing. A lung biopsy showed the presence of numerous epithelioid and giant-cell granulomata, particularly subpleurally. A patchy interstitial pneumonia was also present. He was given a six-month course of prednisolone, and lung function returned to normal. However, the charac- teristics of pneumothorax in patients with IPF have not been elucidated. The purpose of this study was to clarify the clinical course, actual management, and treatment outcomes of pneumothorax in patients with IPF. Methods: Consecutive patients with IPF who were admitted for pneumothorax between January 2008 and Decem- ber 2018 were included. The success rates of treatment for pneumothorax, hospital mortality, and recurrence rate after discharge were examined. Results: During the study period, 36 patients with IPF were admitted with pneumothorax a total of 58 times. During the frst admission, 15 patients (41.7%) did not receive chest tube drainage, but 21 (58.3%) did. Of the 21 patients, 8 (38.1%) received additional therapy after chest drainage. The respective treatment success rates were 86.6% and 66.7% in patients who underwent observation only vs chest tube drainage. The respective hospital mortality rates were 13.3% and 38.0%. The total pneumothorax recurrence rate after hospital discharge was 34.6% (n 9). = Conclusions: Pneumothorax in patients with IPF was difcult to treat successfully, had a relatively poor prognosis, and showed a high recurrence rate. Pneumonia and pleurisy in sheep: Studies of prevalence, risk factors, vaccine efficacy and economic impact Kathryn Anne Goodwin-Ray 2006 ii Pneumonia and pleurisy in sheep: Studies of prevalence, risk factors, vaccine efficacy and economic impact A thesis presented in partial fulfilment of the requirements for the degree of Doctor of Philosophy at Massey University, Palmerston North New Zealand Kathryn Anne Goodwin-Ray 2006 iii iv Abstract The objectives of this thesis were to investigate patterns of lamb pneumonia prevalence of a large sample of New Zealand flocks including an investigation of spatial patterns, to evaluate farm-level risk factors for lamb pneumonia, to determine the efficacy of a commercially available vaccine for the disease and to estimate the likely cost of lamb pneumonia and pleurisy for New Zealand sheep farmers. Data were collected by ASURE NZ Ltd. meat inspectors at processing plants in Canterbury, Manawatu and Gisborne between December 2000 and September 2001. All lambs processed at these plants were scored for pneumonia (scores: 0, <10% or ≥10% lung surface area affected) involving 1,899,556 lambs from 1,719 farms. Pneumonia prevalence was evaluated for spatial patterns at farm level and for hierarchical patterns at lamb, mob and farm levels (Chapter 3). The average pneumonia prevalence in Canterbury, Feilding and Gisborne was 34.2%, 19.1% and 21.4% respectively. Odds ratios of lambs slaughtered between March and May were vastly higher than those slaughtered in other months indicating longer growth periods due to pneumonia. Since pneumonia scores were more variable between mobs within a flock than between flocks, it was concluded that pneumonia scores were poor indicators for the flock pneumonia level due to their lack of repeatability. -

Abstract Hippocrates and his followers, confronted with the infection of the lower respiratory tract, having understood that pulmonary diseases had a high rate of prevalence and mortality among the general population of the ancient Greek communities. He had used the "four humours theory" to explain its origin. Our study, reviewed Corpus Hippocraticum, in order to synthesize various fragments of different works, to compose the hallmarks in bronchiolitis, pleurisy, peripneumonia, pneumonia with their lethal complication empyema and to present the fatal lung infection, the pulmonary phthisis (tuberculosis). Vivid descriptions of the symptomatology were given, alongside with the efforts for treatment. Hippocrates was the first to use comparative hearing of both lungs, and the physician who have established thoracocentesis for the empyema's drainage, combined with parenteric nutrition and endotracheal intubation. -

ABSTRACT Allergic bronchopulmonary aspergillosis (ABPA) is a complex immunologic pulmonary disorder caused by hypersensitivity to fungus, Aspergillus fumigates. It clinically manifests with non-specific respiratory and systemic symptoms. ABPA is typically seen in patients with long-standing asthma or cystic fibrosis. Pleural involvement in ABPA is uncommon and secondary spontaneous pneumothorax is very rare. Herein, we report a case of 33 years old male patient presented with dyspnoea, low grade fever and productive cough. High Resolution Computed tomography (HRCT) scan of thorax was suggestive of ABPA with secondary pneumothorax.
