Department Description The HOPE initiative has been developed to provide support to individuals discharged from the Emergency Department, medical ward or inpatient unit post a suicide attempt or displaying serious suicide ideation, intent or planning in the Sunshine, Brimbank and part of Hume (Bulla and Sunbury) catchment area. HOPE will support clients for a period of 12 weeks, providing assertive and coordinated care post- discharge. Engagement will take place via both outreach and clinic based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide. In keeping with the goals of the Victorian suicide prevention framework 2016–25, the Sunshine HOPE team aims to build resilience in individuals who have made a suicide attempt by providing intensive and individualised therapeutic care and psychosocial support. The Sunshine HOPE team will be a multidisciplinary team and each client will be allocated a Psychosocial Support Worker and Mental Health Clinician to deliver practical support and evidence based therapeutic interventions to address the unique social, economic, historical and environmental factors that have contributed to, or continue to contribute to, a person’s suicidality.

Expertise in identifying and engaging with key welfare, health and other support agencies and working collaboratively to improve consumer’s outcomes and recovery. High level skills in negotiation and advocacy. Skills in working with cultural and linguistic diversity Contribute to the clinical review and clinical governance in assess the clients’ progress and facilitate escalation of care if deterioration occurs. Serve as a role model for professional practice and exhibit effective communication of expected standards. Participate in internal and external meetings where appropriate and at the direction of the HOPE Team Leader and Senior Psychologist, EMH Manager, Area Senior Psychologist and the Area Senior Social Worker . Evidence of contributing to a cohesive multidisciplinary team. Interest in supporting social work students on placement during clinical rotations. Participant in regular clinical supervision with a Senior Social Worker and line management supervision and reflective practice. Ability to use information technology . Undertake evaluation using pre and post- intervention surveys. Participate in audits and review of data reports where required. Actively contribute to the evaluation of the HOPE program. Undertake administrative tasks according to the protocols established for the team and required for reporting on the minimum data set reporting framework. Contribute to the professional development and educational programs on suicide prevention topics for MWMAHS and other services as deemed appropriate. Contribute to research and evaluation activities within MWAMHS and across NWMH as appropriate. Demonstrate strong ongoing commitment to own professional development and the ability to support and develop knowledge and skills in other team members

Selection Criteria

Formal Qualification(s) & Required Registration(s):  Social Workers: o An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.

Essential:  Demonstrated experience in clinical mental health social work with minimum of two years post qualifying.  Demonstrated experience in providing family sensitive and inclusive practices such as Single Session Family Consultation and family / carer support and counselling.  Demonstrated experience in developing and maintaining working relationships with key welfare, health and other support agencies and working collaboratively to improve consumer’s outcomes and recovery. Demonstrated experience in risk assessment and safety planning for vulnerable and risk children and young persons and people experiencing family violence including Elder Abuse.  Demonstrated ability to consult, liaise and negotiate sensitively with clients, treating teams, their family or carers and health and welfare services.  Demonstrated ability to work in a culturally sensitive and inclusive way with a diverse consumer and family and carers  Hold a current driver’s licence and capacity to use in this role  Be familiar with and adhere to relevant professional codes of ethics, the values of Melbourne Health, and relevant legislation, in particular: the Mental Health Act (2014), the Guardianship & Administration Act (1986)), Victorian Charter of Human Rights and Responsibilities Act 2006 and clinicians’ responsibilities under s141 Health Services Act with regard to the sharing of health information.  Interest in providing educational programs on suicide prevention to mental health and community services.  Research skills and experience in undertaking formal program evaluations.  Comply with all legal requirements pertaining to the position including responsibility for maintaining current registration as required by your professional organisation or other applicable Acts.
