This form must be completed without any alterations to their format and no substitute form shall be accepted. All blank spaces shall be filled in with the correct and accurate information as required. 2. Offers/quotations submitted exceeding the Approved Budget for the Contract shall be rejected. 3. PAGCOR shall have the right to inspect and/or to test the real property to confirm their conformity to the technical specifications. Tie-Breaking In accordance with Government Procurement Policy Board (GPPB) Methods Circular 06-2005 - Tie-Breaking Method, the Bids and Awards Committee (BAC) 1 shall use a non-discretionary and non- discriminatory measure based on sheer luck or chance, which is “DRAW LOTS,” in the event that two or more Lessors have been post-qualified and determined as the Lessor having the Lowest Calculated Responsive Quotation (LCRQ) to determine the final LCRQ, based on the following procedures:

a. In alphabetical order, the Lessors shall pick one rolled paper. b. The lucky Lessor who would pick the paper with a “CONGRATULATIONS” remark shall be declared as the final Lessor having the LCRQ and recommended for award of the contract.

______A. SPACE SPECIFICATIONS I. Location and Site Condition  Location. The building should be within five (5) kilometer radius from Biñan Business Center, Laguna.  Accessibility. The property must be accessible directly to the main thoroughfare by public transport, such as PUV lines. (Road distance)

Should be more than 200 meters radial distance from school or places of worship at the time of publication of this RFQ.  Topography and Drainage. The property shall be in an area that is not prone to flooding.  Parking Space. The property must be located in a commercial/business district.  Land Classification, utilization and assessment. The building must be classified as mixed use (office, commercial, business). II. Neighborhood Data  Rental Rate. Property’s rental rate shall not be more than FOUR HUNDRED SIXTY TWO PESOS (Php462.00) per square meter per month (exclusive of VAT) including other common use rent.  Sanitation and health condition. The leased premises must have proper garbage facilities and complies with the health and sanitation standards required under the Sanitation Code of the .  Property utilization. The property’s highest and best use is for an office/ commercial building.  Police and Fire Station. The property must be located within 3.0 kilometer radius from police and fire stations.  Cafeterias. Shall have at least one food establishments/ eateries/ cafeterias/ restaurants adjacent to the property/building.  Banking/postal/telecom. The property must have an adequate telecommunication access. The building where the leased premises is located must have at least one (1) km radial distance from premises bank/financial institution III. Real Estate  Structural Condition The building is designed in compliance with the Building code of the Philippines with a structural stability certificate duly signed and sealed by a certified ASEP Structural Engineer.  Functionality  Room Arrangement The room must conform with PAGCOR’s lay-out plan  Circulation (Air) The leased area must have a height clearance of at least 3.0 meters from finished floor line to finished ceiling line for gaming area and a minimum of 2.50 meters for offices.  Light and Ventilation The building’s common areas must have adequate lighting and ventilation system.  Space Requirements: The building’s leasable space must be adequate for PAGCOR’s area requirement of minimum 196.0208 sq.m. of air conditioned space.  Facility Requirements  Water Supply and Toilet. Sufficient supply of water within the building.

Sufficient Electrical Fixtures, lighting fixtures and convenience outlets.  Fire Escapes. The building must comply with the Bureau of Fire Protection and Fire Code of the Philippines. IV. Free Services and Facilities  Back-up power supply. With offered FREE of charge use of generator set for PAGCOR.  Air-conditioning Units. Lessor to provide and maintain energy- efficient air-conditioning system in the leased premises.  Repair and Maintenance. With offered FREE of charge repair and maintenance of the leased area facilities, electrical lighting fixtures and LED bulbs.  Secured Parking Space (Complimentary). Lessor must provide at least fifteen (15) complimentary parking spaces, FREE of charge.  Water Consumption. With offered FREE of charge use of water consumption. B. Additional Requirements The lessor shall ensure that the leased premises is compliant to PAGCOR Detailed Construction Finishes Specifications (Annex A), to included but not limited to the following:  Air-conditioning. The lessor shall and assure that the ambient temperature will be twenty to twenty two (20 to 22) degrees Celsius. In case the room temperature goes higher than twenty four (24) degrees Celsius, Lessor should make the necessary correction on the air conditioning system within 72 hours from time of notification by PAGCOR. Penalty is as listed:  3/10 of 1% of monthly rated is fixed within 72 hours.  After 72 hours, 3% of monthly rate pro-rated according to the affected area but no less than 3/10 of 1% of monthly rate.

Penalties shall be collected by PAGCOR by way of deduction from the monthly rental.

Must likewise provide maintenance agreement/ warranty (whichever is applicable) of air conditioning system  Facility Requirements: Facilities/amenities:  FLOOR: Floor Finishes.

All floor finishes must be secured, safe, and free from any leaks / scratches, patches, water marks, hole and must be leveled.

Floor finishes must conform with the standard minimum requirements of PAGCOR (see Annex A: DETAILED CONSTRUCTION FINISHES SPECIFICATIONS )

(see Annex A: DETAILED CONSTRUCTION FINISHES SPECIFICATIONS)  CEILING: Ceiling finishes. All ceiling finishes must be secured, safe, and free from any leaks / scratches, patches, water marks and in any falling debris.

Ceiling finishes must conform with the standard minimum requirements of PAGCOR (see Annex A: DETAILED CONSTRUCTION FINISHES SPECIFICATIONS  WALL: Wall finishes. Perimeter wall of the leased space must be secured, safe and free from any leaks or any form of moisture, gas, or any harmful or toxic substances.

Wall finishes. All wall finishes must conform with the standard minimum requirements of PAGCOR (see Annex A: DETAILED CONSTRUCTION FINISHES SPECIFICATIONS )  RESTROOM: All restrooms must have complete set of fixtures as follows: a. Male restroom: water closet, urinal, lavatory, bidet, mirror, toilet paper roll holder, soap holder, floor drain and hand dryer b. Female restroom: water closet, lavatory, bidet, mirror, toilet paper roll holder, soap holder, floor drain and hand dryer

Plumbing fixtures / restrooms fixtures/ finishes and accessories must conform with the standard minimum requirements of PAGCOR (see Annex A: DETAILED CONSTRUCTION FINISHES SPECIFICATIONS)  The generator set. Lessor shall provide, maintain and shoulder generator set with a standby power of 24/7 FREE of charge.  Water supply and toilets. Lessor shall provide, maintain and shoulder water bills for water consumption FREE of charge.  Portable Emergency Lighting Layout: 1. Lessor shall provide portable emergency lighting for power failure.

2. All portable emergency lighting units must be Bureau of Product Standard (BPS).  General Electrical System: The entire electrical load (general lighting, air- conditioning, etc.) at full load operation must be fully connected and supplied by an emergency generator.

The building must comply with the Bureau of Fire Protection and Fire Code of the Philippines.  Fire alarm / detection system, fire-fighting equipment and fire / emergency exits. Lessor to provide fire suppression system such as heat/smoke detectors and automatic water sprinkler system.

Repair and preventive maintenance of the system/ equipment shall likewise be the responsibility of the Lessor. C. Other Terms and Conditions Relative to the Leased Premises  The Lessor shall provide the requirements under IV within one (1) month from the date of receipt of the Notice to Proceed prior to final turnover and acceptance.  PAGCOR has the following options: a. Operate and supply its own VIP snack and beverage bar within the casino premises. b. Designate concessionaire to operate and supply its own employees cafeteria in the space provided. D. Other Technical Documentary Requirements 1. On the deadline for the Submission and Receipt of Quotations a) Floor layout in accordance to the required layout of PAGCOR b) Vicinity/Location map showing compliance of the proposed location c) Fire Safety Inspection Certificate issued by Bureau of Fire Protection d) Complete sets of proposed Civil & Architectural , Electrical and Mechanical plans (size of plan is 20” x 30”, blue print) duly signed and sealed by respective professional Engineer/s 2. Winning Lessor shall submit to PAGCOR the following documents prior to turn over and acceptance:  Building Occupancy Permit;  Complete sets of as-built Civil & Architectural, Electrical & Mechanical plans (size of plan is 20” x 30”, blue print) duly signed and sealed by respective professional Engineer/s. Each plan should reflect the entire leased premises.  Certificate for the operation of generator set;  Mechanical permit  Permit to Operate (Generator Set) E. DURATION OF THE CONTRACT The lease shall be for a period of Three (3) years from the date of receipt by the winning lessor of the notice to proceed. F. ALTERATIONS PAGCOR, subject to Lessor’s approval, shall have the right to make alterations or minor decorations within the leased premises and employ its own contractors.
