1.1 Access to Information

This Health Board currently makes information routinely available to the public in relation to its functions, activities and schemes. Such information will continue to be available informally without the need to use the Freedom of Information Act. This manual highlights, in relation to each of the Health Boards activities, where information of this nature is available.

The Freedom of Information Act 1997 is designed to allow public access to information held by public bodies which is NQl' routinely available through other sources. Access to information under the Act is subject to certain exemptions and involves specific procedures and time limits. This manual provides a guide to the structure of the Midland Health Board so as to help you access information generally and under the Freedom of Information Act and should be read in conjunction with the following documents: e Midland Health Board Annual Report & Statistical Addendum Midland Health Board Annual Financial Statement Midland Health Board Annual Service Plan An Information Guide to how services are Administered (Prepared under Section 16 FOI Act 1997) Refer to section 10.0 for details on how to access information from the Board.

1.2 Freedom of Information Act 1997

The Freedom of Information (FOI) Act establishes three new statutory rights:- a legal right for each person to access information held by public bodies; a legal right for each person to have official information relating to himherself amended where it is incomplete, incorrect or misleading; a legal right to obtain reasons for decisions affecting oneself;

The Act asserts the right of members of the public to obtain access to official information to the greatest . extent possible consistent with the public interest and the right to privacy of individuals.

1.3 General Overview of Section 15

"Information Guide to Midland Health Board Services"

This manual has been prepared and published in accordance with the requirements under Section 15 of the Freedom of Information Act 1997. Its purpose is to facilitate access to official information held by the Midland Health Board. Specifically the manual provides information on:

Aims, structures and functions of the Board The services provided by the Board The classes of records held by the Board General access to information How to access information under the Freedom of Information Act

Detailed information on the rules procedures and guidelines on each service provided by the Board is available under a separate publication (see 1.4 below).

The Health Boqd is required to publish information in relation to the public's rights of appeal against decisions of the Board. In addition the Health Board must describe the classes of records that it holds and the Board's arrangements for enabling the public to access such records. Under Section 16 of the Freedom of Information Act 1997 the Midland Health Board is required to make its rules, guidelines and procedures available to the public in respect of the services provided.

Accordingly a reference manual has been prepared containing:

Details of schemes administered by the Board Legislation, regulations governing the services Policies, Procedures and guidelimes for the services Eligibility criteria applicable to each service Publics rights to appeal and complain

The Section 16 manual is available for inspection by the general public at the Board's Hospitals and main Health Centres. Strategic Objectives of the Midland Health Board I Mission Statement

In 1994 the Minister for Health published the Health Strategy - "Shaping a Healthier Future". The main theme of the Strategy is the reorientation of health services so that improving peoples' health and quality of life becomes the primary and unifying focus. These two concepts are described as follows:-

HEALTH GAIN: is concerned with health status both in terms of increased life expectancy and improvements in the quality of life.

SOCIAL GAIN: is concernedwith the broader aspects of the quality of life and the extent to which, for example, the provision of support services contributes to improvement in the quality of life of service users and their carers.

The Strategy reflects the commitment of Government to ensuring that health services should fust and foremost help those people whose needs are greatest and that this will be reflected in the way future resources are allocated. It also recognises the importance of the pursuit of quality and lays emphasis on constantly measuring and evaluating quality through clinical audit and consumer surveys. The Strategy places the consumer fust and sets out proposals for improving the participation of the public in the planning and evaluation of services. It sets out new arrangements for improved legal and fmancial accountability and includes a requirement on those providing services to take direct responsibility for the achievement of agreed objectives.

In response to "Shaping a Healthier Future" the Midland Health Board, in June 1995, published its Corporate Strategy which is intended to serve as a source of information, reference and guidance to all those involved in planning actions to achieve the objectives outlined in "Shaping a Healthier Future".

The Midland Health Board exists to seek to improve the health (health gain) and quality of life (social gain) of the people livingin Counties Laois, Offaly, Longford and Westmeath by:-

promoting healthy lifestyles

preventing, diagnosing and treating ill health

caring for those suffering from long term illness and disabilities

providing social services to individuals and families at risk 3.0 Aims, ~urposesand. A. Values The aim of the Board is that all of its services should be of a uniformly high quality. The Board believes that a quality service must be: EQUITABLE - persons with identical needs should receive the same standard of treatment and care regardless of where they live, where they are treated, what their income is or what their political or religious beliefs may be.

ACCESSIBLE - everyone should have ready access to the services they need, when they need them. In particular, our services should be equally accessible to both public and private patients. EFFECTIVE - each patient should get the best possible result from his or her treatment and care. EFFICIENT - services should be organised and delivered in a way that gives best value for money. APPROPRIATE - should meet local needs, avoid developing unnecessary dependency on services or institutions and be flexible enough to cope with the need to change. RESPONSIVE - reflect the needs and entitlements ofusers. DIGNIFIED - reflect the standards of courtesy, confidentiality and respect for the privacy and dignity of the individual that society expects of the caring services.

FARSIGHTED - by identifying and pursuing through prevention and promotion programmes, opportunities to contribute to improvements in the health of the community.

4.0 Area and Population

The Midland Health Board (MHB) region covers the counties Laois, Offaly, Westmeath and Longford. In 1996 the population of the region was 205,272 compared with 202,984 in 1991 census. 1.1% increase in that 5 year period.

Health Care needs are greatest at the extremes of age. A useful measure of this is the dependency ratio. This is a ratio of the number of persons aged 0-14 and 65 years and over to the number of persons aged 15- 64 years. The Midland Health Board comprises of 30 members -16 elected by the four County Councils, seven representatives of the medical profession, one representative from General Nursing, Registered Psychiatric Nursing, Dental and Pharmacy and three members as appointed by the Minister for Health & Children. The Board's main role is to oversee policy issues and to carry out the reserved function as defmed in the legislation. (See Sect. 6.1 for list of reserved functions).

The Managemen: of the Midland Health Board comprises of the Senior Executive Team and the Corporate Team who are responsible for the Executive Functions of the Board and ensure policy decisions of the Board are implemented within available resources.

In order to best serve the people of the area, their needs must be assessed and prioritised and available resourses then used to best effect to meet those needs. The Board's emphasis is shifting away fromprogrammes of services towards the particular needs of groups of people such as children, older people, mentally ill and people with disabilities. Episodic illnesses can affect people in all care groups and in all parts of the Board's area and need to be specifically addressed. Most episodic illnesses are catered for in general medical practice and in acute hospitals.

Reserved Functions of the Board as set out in the FIRST SCHEDULE of the Health Amendment (No. 3) Act 1996 and are performed by resolutions of members of the Board.

Executive Functions

Executive functions of the Chief Executive Officer as set out in the Health Amendment (No.3) Act 1996 Section 4 states that:

"Afunction ofa health board that is not a reserved function shall be a function of the Chief Executive Oflcer unless othenvise provided for, whether in this Act or in any other enactment, and afunction that is required to be so carried out shall be an executive function and "executivefunction" skall be construed and have effect accordingly". -Reserved Functions

1) Health Act 1947

a) Section 78 -Powers of health authority to acquire land. (Amended 18(a) 1996 Act)

b) Section 81 refers to Health Authorities Powers in relation to the making of compulsory acquisition orders.

c) Section 82 refers to procedures to he followed in relation to making of compulsory acquisition orders by a health authority. . d) Section 84 refers to procedures to be followed in relation to the coofiiation of compulsory acquisition orders by a health authority.

e) Section 88 refers to powers of health authorities to appropriate surplus land to other purposes (Amended 18(b) 1996 Act).

f) Section 89 refers to powers of health authorities to dispose of surplus land (Amended 18(c), 18(d), 18(e) 1996 Act).

2) Health Act 1970

a) Section 8 refers to the establishment of committees of health boards.

b) Section 9 refers to the payment of expenses to members of boards and committees.

c) Section 10 refers to the payment of allowances to the chairman and vice-chairmen of health boards.

I Acute Hospital Services I

Acute Hospital Services in the Midland Health Board area are provided at the General Hospitals at Mullingar, Portlaoise and Tullamore. This network of hospitals, working together from three sites and within the overall system for health care, aims to provide the range, quality and quantity of services, on a 24 hour basis, to meet the needs of the Board's population.

By providing teaching and professional training the Acute Hospital services maintain the Board's commitment to research, innovation and staff development.

General Manager:

Reporting to the Programme Manager the General Manager has operational responsibilities for the Acute Hospital Services Division i.e. to deliver a planned quantum of service, within the defmed level of resources made available and in line with pre-determined quality standards. The General Manager also has a role in relation to the strategic development of the Division's services.

Key tasks for the general Manager include: Provision of leadership for the division Planning and delivery of a quantum of service within budget and to a pre-determined quality standard Initiation, implementation and management of projects which will contribute to the:- * Operational efficiency and effectiveness of the Division Re-orientation of acute hospital services to increase health and social gain creation of an environment to secure the participation of medical and nufsing staff in the management of the Division development of links with other services and service providers e.g. G.P.'s

7.1.3 1 Sewices Provided I

Administration:

Administration in the Acute Hospital Services provides full clerical and administrative support in the following areas:

Accounts Department - Staff are responsible for the issuing of accounts and receipt of payments in respect of changes for hospital services and processing all payments.

Out Patient Department: Administrative staff provide support to the Out-Patient Department in the preparation, booking and reconciling of all Out-Patient Clinics Monday to Friday.

Admissions Department: Administrative Support is provided for the admission of patients to hospital.

Medical Records Department: Administrative support in this department provides patient's records for out-patient clinics, in-patient and the day ward on a five day week basis.

Ambulance Sewices: .

The Ambulance Service provides pre-hospital emergency care and the primary response in major emergencies. The Ambulance Service also caters for the routine movement of patients between hospitals both within the Boards area and to specialist hospitals outside the region. A reorganisation of Ambulance Services is currently in progress which will see the development of a Regional Ambulance Headquarters based at Tullamore. The overall responsibility for all Ambulance Services for the region is under Mr. G. Sweeney, Chief Ambulance Off~cee Acute Hospitals

Medical Consultant Services:

Medical Consultants employed in the Board's hospitals work in the following specialties:

1) Main Specialties:

ENT This specialty deals with the treatment of conditions relating to Ear, Nose and Throat.

General Medicine The treatment of diseases by non-surgical means

General Surgery The treatment of diseases, injuries and deformities by manual and operative methods..

Medicine for Older People This branch of medicine concentrates on the diagnosis (incorporating Clinical Age assessment) and treatment of age related problems.

This branch of medicine is concerned with pregnancy and childbirth including the study of the physiological and pathological function of the female reproductive tract and the care of the mother and foetus through pregnancy and childbirth

Orthopaedics This branch of medicine is devoted to the study and treatment of the skeletal system, joints, muscles and associated structures.

Paediatrics This branch of medicine is concerned with the development and care of children and the treatment and prevention of their diseases.

2) Each Hospital has the following specialties:

Accident & Emergency Dept. Providing emergency treatment.

Coronary Care: Treatment of Conditions associated with heart disease.

Intensive Care Unit: Responsible for the care and treatment of seriously ill patients.

Hospice Service: Provide facilities for the care of terminally ill Patients and is supported by the Hospice Care Team. Acute Hospitals

3) Supporting Specialties:

Radiology This branch of medicine is concerned with radioactive substances and uses techniques of visualisation with the diagnosis and treatment of disease using any of the various source's of x-rays.

Pathology Department: The Pathology Department (Laboratory Services) diagnoses, monitors and controls diseases in association with the medical profession. Departments within the Laboratory Service include:

Biochemistry:: The chemistry of living organisms and life processes

Blood Transfusion This Service includes the storing of blood to be used for transfusion and other purposes

Haematology: The study of blood and blood products

Histology: The science of dealing with the microscopic identification of cells and tissues.

Microbiology: The study of microorganisms including algae bacteria, viruses and fungi.

Bacteriology: This service is concerned with the microbiological analysis of food and drinking water supplies. Routine investigation of foods and water takes place.

Out-Patient Clinics SenicesNnits are also provided as follows:-

* Ante-NataVPre-Natal : This clinic specialises in the care of the expectant mother.

Audiometry: This clinic specialises in the measurement of bearing.

Breast Cancer Clinic: The Clinic provide care and treatment in relation to Breast Cancer.

Chemotherapy: This service provides treatment of cancer by use of chemical substances.

Diabetes Clinic: This clinic specialises in ongoing diabetic education.

Dietician: The Dietician is trained in the scientific use of diet in health and disease.

Endoscopy: This service specialises in inspections and investigations by means of scope.

ENT Clinic: This clinic provides services in respect of Ear, Nose and Throat.

General Medical Clinic This clinic specialises in the treatment of diseases by non-surgical means.

General Surgical Clinic This clinic treats diseases, injuries and deformities by manual and operative methods. Acute Hospitals

0 Medicine for Older People: This department specialises in the diagnosis and treatment of age related problems. This clinic deals with the management of pregnancy and labour.

Oncology: This clinic specialises in the study of tumors. . Ophthalmology: Clinic whicb specialises in dealing withihe eye, its anatomy, physiology and pathology. Orthopaedics: This clinic specialises in the correction of deformities of the muscoskeletal system. Orthoptics: This clinic specialises in correction of visual axes of the eye

Paediatrics: This clinic specialises in child development, the care and treatment of diseases in children.

Phlebotomy: This services deals with drawing of blood for analysis.

Radiology: ECHO Department: This department performs echocardiography whicb is the method of measuring the motion and internal structure of the heart.

E.C.G. Department: This department performs electrocardiographs which is the method of measuring the action of the heart muscle.

Urodynamics: Teatment of persons with urinary difficulties.

Child Psychiatry: Providing diagnosis and treatment of Psychiatric illness in children. Audiology: Diagnosis and treatment of hearing disorders.

Physiotherapy Department: The Physiotherapy Department provides a comprehensive physiotherapy programme of Out-Patient, In-Patient and Weekend Care including Education, Information, Assessment, Rehabilitation and Group Therapy. Patients are accepted for care regardless of age and condition for which they are being referred. A comprehensive physiotherapy programme of care is available to all referred patients.

Speech & Language This Department provides a service to clients presenting with speech, language and voice disorders,

Occupational Therapy . This Service is offered to those who are temporarily or permanently impaired or disabled by physical andlor mental illness or by social or developmental problems. Acute HOspital~

Pharmacy: The Pharmacy is involved in the provision of pharmaceutical care to patients. The Pharmacist is professionally, ethically and legally responsible to the patients for their quality of care. The Pharmacy is responsible ro the procurement, storage and dispensing of all pharmaceutical products. The main aim being to promote the safe, effective and economical use of drugs.

Nursing: 24 hour quality Nursing Care is provided within the Acute Services at Ward level and Accident & Emergency Departments. There is Nursing support at all nut-patient clinics. The Nursing function also incorporates the responsibility to provide research based policies and procedures to support quality initiatives and to function within budget provision. Specialist Nurse Services include - Oncology, Breast Care, Diabetics, Infection Control, Respiratory. 0 Nursing responsibilities also encompass health and safety at work including hepatitis screening and immunisations, compulsory training for manual handling and lifting, fue training, Cardio Pulmonary Resuscitation (C.P.R.).

The training and development of Student Nurses is the responsibility of the Regional School of Nursing. The Student Nurse Training Diploma Programme runs in conjunction with the Institute of Technology Athlone.

Non Nursing (Non Clinical Services): The non clinical services are provided by the following staff:

Attendant Staff: Duties of attendant staff include internal transfer of patients, catering, cafeteria duties, general cleaning duties on wards, serving and preparation of food at ward level.

Catering Staff: Catering staff provide a catering service to In-Patients, some Out-Patients, Hospital Staff. The Catering staff have two main areas of responsibility: Kitchen: Kitchen staff process food and supervise distribution of meals to wards. Kitchen staff are also responsible for provision of food to the cafeteria. Cafeteria: Cafeteria staff supply meals and snacks to employees.

Portering Staff: Portering staff provide services to all hospital wards, theaters, out-patients and other departments as the need arises. Ad hoc duties are provided by House Portering staff; collection of linen, waste, carrying of equipment.

Maintenance: The Maintenance Department of the Acute Hospital Services provides continuous daily maintenance and upgrading of Hospital Wards and Departments. 24 hour on call services are available.

I Services Provided I 1. SUPPLEMENTARY WELFARE ALLOWANCE SCHEME a) Basic SWA Paid to individuals who have no income or whose income is beiow the SWA rate. Usually paid to individuals who are awaiting or failed to qualify for a primary Social Welfare entitlement. b) SWA Supplement SWA Supplements are paid to certain individuals on low income and are designed to meet ongoing additional basic needs which cannot be met from their basic weekly income. These supplements include: Rent Allowance Mortgage Interest Subsidity Heating Supplement Diet Supplement 0 Travel Supplement 0 Other Supplements c) Exceptional Needs Payments These are once off payments in cases where there was an exceptional unforeseen situation and the individual could not be expected to meet the cost out of normal living expenses e.g. necessary items of furniture, electrical, transport, cots, prams, buggies, clothes and many other items. d) Urgent Needs Payments These payments are normally considered in the aftermath of an emergency such as flood or fue to meet immediate needs such as food, clothing, fuel, household goods and perhaps shelter. OTHER COMMUNITY WELFARE SERVICES a) Back to School Clothing & Footwear Scheme This scheme was designed to provide for necessary clothing and footwear requirements for child dependants of Social Welfare Recipients who are attending school. b) National Fuel Scheme This scheme provides a weekly payment to certain long-term Social Welfare recipients to meet heating needs during the winter months. c) Funeral Arrangements Where there is no next of kin to make funeral arrangements the Health Board may take responsibility for such expenses. d) Information Semce Providing information on service entitlement and access in respect of services provided by the Department of Social Community & Family Affairs and the Health Board. e) Referral Service The service provides an advice, referral, mediation, counselling and budgeting role so as to deliver a comprehensive package of support in recognition of the plight of those who are marginalised, disabled, alone, isolated and distressed.

The Midland Health Board Dental Services seeks to reduce the level of dental disease in children to improve the overall levelof oral health in the population and to provide adequate treatment services for children.

The Board also administers the Dental treatment Services Scheme providing dental services to eligible persons aged 16 to 34 years and persons aged 65 years and over.

Administration of the Adult Dental Treatment Services Scheme

The Dental Treatment Services Scheme came into operation in November 1994. Under the scheme all eligible persons i.e. medical card holders enjoy the following entitlements:

+ Emergency treatment for the control of pain (eligible persons 16 years +) + Routine treatment (eligible persons aged 16 - 34 and 65+). + Full denture treatment for eligible edentulous persons (aged 16 years +) . . General Practice Vocational Training Unit

The Department is entitled the Vocational Training Programme in General Practice and is situated in the G.P. Teaching Unit, General Hospital, Tullamore. Its aim is to provide post-graduate training to Trainee General Practitioners. It has teaching and administrative staff. The Scheme is administered by a Steering Committee which is representative of various sectional groups.

( Services Provided I The Department provides an educational service to G.P. eainees and it co-ordinates their appointment to Hospital posts. It also provides a support service to G.P. and Consultant teachers.

Access to the service is by appointment through open competition. The applicant must have full registration with the Medical Council of Ireland.

The Home Help Service is managed under the general direction of the Supt. Public Health Nurse. It is organised and supervised by the Home Help Organiser. Applications for Service are made by the Public Health Nurses. ~e&testin~is carried out on all clients by the~onununityWelfare Officer. ~achapplication is individually assessed. When the Home Help Service is granted, the needs of the patient are closely monitored by the Public Health Nurse and the service may be increased or decreased, depending on the circumstances.

ILarkm, Clerical Officer I Services Provided 1 Facilitate the development of General Practice as a whole in the Boards area with the framework set out in the blueprint "The future of General Practice in Ireland. Support to enable G.P.s develop their practices. Continue to improve co-operation between GPs and all other professionals in the Health Service The Unit also aids the provision of detailed prescribing patterns. The five unit pharmacists who work on apart- time basis are available to advise general practitioners on prescribing issues. The Unit monitors and regulates contracts between the Board and community pharmacists. This involves the processing of applications for additional pharmacy contractor agreements as well as applications for re-location of existing pharmacy contracts within the context of the health (Community Pharmacy Contractor Agreement) Regulations 1996. The Unit monitors budgets and regulates contracts between the Board and private dentists under the Dental Treatment Services Scheme. Primaty Immunisation contract with general practitioners are also processed and maintained by the unit. Provision of services for certain clients who contracted Hepatitis C and are covered by the Health Amendment Act 1996 is co-ordinated by the Unit. Co-ordinate the High Tech hgsScheme for the provision of High Tech Medical Products. In co-operation with I.C.G.P. support G.P's in meeting their continuous educationltraining needs.

Individuals who have an entitlement to certain Health Services under the Health Amendment Act 1996 can apply directly to the Primary Care Unit for access to health services. Applications for provision of High Tech medication are also processed through the unit. Other services provided by the unit are essentially of an administrative support nature for other Health Professional Services in the Region. Social Work and Family Support Services slrive to provide a broadly based service for children and their families and to ensure the establisbrnent of responsive and flexible work which met the changing needs of the regions population. The welfare of the child is the paramount consideration in any decision making, planning and service provision for children while having regard to the rights and duties of parents. The Midland Health Board derives its powers, functions and duties in respect of child welfare and protection from statute. Under these laws Health Boards are required to inter alia provide for the protection and welfare of children. Child & Family Services

The Child Care Act 1991 places a statutory responsibility on the Midland Health Board to perform a number of functions in realtion to the care and protection of children.

These functions are as follows:- * Take such steps as it considers requisite to identify children who are not receiving adequate care and protection Co-ordinate information form all relevant sources related to children in it's area. 0 Have regard to the rights and duties of parents whether under the constitution or otherwise Regard the welfare of the child as the first and paramount consideration. 0 In so far as is practicable, give due consideration, baving regard to hisher age and understanding to the wishes of the child. Have regard to the principle that it is generally in the best interests of a child to be brought up in hisher own family.

Social Workers are involved in providing services to children and their families to enable parents to nurture, support and care for their families. Such services include: The provision of the therapeutic counselling and practical support for children individually or to their parents and the family unit. The development and provision of group work with children involving drama, sports, social skills and summer camps. The placement and supervision of children in care The assessment of sus~ectedcases of child abuse. The provision of services to victims of domestic violence. The provision of an adoption and foster care service which includes assessment for foreign and Irish adoption and placement with relatives. The provision of services to adolescents including those who are homeless. The provision of an after care service. The provision of a tracing service for children and their birth parents.

Applications are available from the local Community Care Offices .:. Blind Welfare Allowance The Blind Welfare Allowance is an additional payment made to qualified persons who because of the nature of their disability require additional resources to meet their basic needs.

It is normally paid as a supplement to a blind person receiving Blind Pension or Old Age Pension. Applications are available from the local Community Care Offices.

GeneralcalServices Schem (Medical Cards) Under the General Medical Services Scheme persons are issued with medical cards for themselves and their .dependants and are entitled to the following services free of charge. A. General Practitioner Medical Services from doctors in private practice who are contracted to the Board. Individual choose the doctor they wish to attend. B. Prescribed drugs, medicines and medicaVsurgical appliances. C. Free out-patient and in-patient public hospital services.

Application forms are available form Community Care Offices and must be completed by relevant general . practitioner. Medical card applications are subject to a means test. .:. ~~~~~l~~~. .. care Allowance The Domiciliary Care Allowance is a monthly payment in respect of the care of children normally aged between 2 and 16 years with a severe physical, sensory or intellectual disability. The allowance is paid to the mothedfather or guardian in certain circumstances. Such children must normally reside in the community and require continuous care or supervision substantially greater than that, which would normally he required by a child of the same age.

Aural Services aim to improve the quality of bearing of persons who are experiencing a deterioration in their bearing.

Services are provided through ENT Departments in the General Hospitals and specialists services are provided by the National Rehabilitation Board which are arranged by the Health Board Community Care Offices.

1. SUPPLEMENTARY WELFARE ALLOWANCE SCAEME

Basic SWA Paid to individuals who have no income or whose income is below the SWA rate. Usually paid to individuals who are awaiting or failed to qualify for a primary Social Welfare entitlement. SWA Supplement SWA Supplements are paid to certain individuals on low income and are designed to meet ongoing additional basic needs which cannot be met from their basic weekly income. These supplements include: Rent Allowance 0 Mortgage Interest Subsidity Heating Supplement Diet Supplement Travel Supplement Other Supplements Exceptional Needs Payments These are once off payments in cases where there was an exceptional unforeseen situation and the individual could not be expected to meet the cost out of normal living expenses e.g. necessary items of furniture, electrical, trampart, cots, prams, buggies, clothes and many other items. Urgent Needs Payments These payments are normally considered in the aftermath of an emergency such as flood or fue to meet immediate needs such as food, clothing, fuel, household goods and perhaps shelter. Mullingar Resource Centre is a day Centre which caters for 56 Clients. The Clients come from Lough Sheever Centre, St. Peters Centre, from Community Residences and from their own homes.

The age group is 18 - 65. The range of disabilities is from low mild to low moderate.

[ Services Provided 1 Tops Programme: The programme has 11 places. Clients are placed on the Programme by the National Rehabiliatation Board. The course offers vocationaland social skills and work experience in the community. The course is monitored by the N.R.B and is of 2 years duration.

Modules include: Work experience placement Training skills Personal Development SociaYCultural activities Practical academies and new technology

At the moment the centre has applied for national accreditation and is developing documentation to achieve same.

The centre has attained approval status for Integrated Assessment System which facilitate the trainees concerned to access nationally recognised certification.

The Phoenix Resource Centre provides day services for people with physical and sensory disabilities and its objective is to combine work and leisure activities with rehabilitation and care needs, with the key aim of promoting independence, full participation in society and integration into the community.

P.P.0.L. - D isabled Peon- D.P.O.L. is a voluntary organisation founded in September 1993 for and by people with physical disabilities of County Longford in order to source vocational and recreational opporlunities in County Longford and to support members as a group with common needs. The organisation was founded by people with disabilities attending the Phoenix Centre and it is facilitated by the Midland Health Board.

Aims of Centre: To provide an environment where work skills are on offer. To provide an environment where recreational and social activities can be enjoyed in a positive way. 0 To improve the quality of life of young people with physical disabilities within County Longford.

The centre is staffed by an Occupational Therapist/Co:ordinator who co-ordinates activities and attendance at the centre. Other staff include driverlfacilitator, sessional community physiotherapists and sessional tutors. The Centre also avails O~FASCommunity Employment Workers on a scheme sponsored by D.P.O.L.

The Dental Treatment Services Scheme came into operation in November 1994. Under the scheme all eligible persons i.e. medical card holders enjoy the following entitlements:

Sperial Needs and General Anaesthetic Service Special needs patients are entitled to all ofthe above services. Persons with special needs are accorded a "priority status" with the Midland Health Board. In some cases treatment is not possible without recourse to general anaesthesia, for such cases a general anaesthetic service is provided at the General Hospital, Tullamore. Special Needs Centres are regularly screened and have backup Hygienist Service to maintain Dental Health as part of a preventative programme.

The Occupational Therapy Service is committed to assisting the Midland Health Board in achieving their objectives towards maximising the health (health gain) and quality of life (social gain) of the clients referred to Occupational Therapy. This is achieved through appropriate assessment and intervention by Occupational Therapists as part of the services offered by the Board to those who 9 temporarily or permanently impaired or disabled by physical and/or mental illness or by social or developmeq@lproblems.

Occupational .Therapy is the use of purposeful activity to restore or maintain a persons ability to function independently at hisher maximum level of function.

The objective of services for people with physical and sensory disabilities is to combine work and leisure activities with client-centred rehabilitation and care needs, with the key aim of promoting full participation in society and their local communities.

Day services and residential respite services are co-ordinated by Occupational Therapists who report to the Head Occupational Therapist in their locality who in turn report to the General Manager.

Services are available in the designated centres, through outreach programmes and in clients homes. The services provided at the designated centres also include - physiotherapy, tutoring in skills, recreational and sporting activities etc.. which are co-ordinated by the occupational therapist.

Those not attending designated centres and not within the 18-65 age group receive Occupational Therapy Services in their locality at departments in the acute hospitals, elderly care units, health centres and in their own homes.

16. Diversional Therapy

Access to services at designated centres is by application form which can be obtained from the Occupational Therapist, Co-Ordinator of the centre and is completed by the client. For services outside of the centres access is by written referral from General Practitioners, Public Health Nurses, Speech and Language Therapists, Physiotherapists and other Health Professionals. I Home Help Service I [ Description I The Home Help Service is managed under the general direction of the Supt. Public Health Nurse. It is organised and supervised by the Home Help Organiser. Applications for Service are made by the Public Health Nurses. Means testing is carried out on all clients by the Community Welfare Officer. Each application is individually assessed. When the Home Help Service is granted, the needs of the patient are closely monitored by the Public Health Nurse and the service may be increased or decreased, depending on the circumstances.

7.3.17 [ ClochanHouse ,] 1- 1- (Respite Services) Clochan House is the resource and respite unit for people with physical and sensory disabilities in Laois and Offaly between the ages of 18-65.

The centre was established in October 1993 and develops and changes its service supply with the demand and needs of this client group. Its overall aim is two fold providing respite to clients and carer and the development of the centre as an integral part of the community. This facility is annexed to the General Hospital, Tullamore but has its own separate entrance with wheelchair accessible sensory garden. The building consists of 5 bedrooms, 3 adjoining bathrooms, kitchen, lounge, storeroom, office, conservatory and function room.

The fifth bedroom is kept for emergency and short stay respite doubling as a treatment room when not in use. The whole building is wheelchair and visually impaired 'friendly' with the most up to date appliances and aids on hand for use and assessment purposes i.e. level deck showering facilities, Jacuzzi with tracking system, variable height beds and klectric bed. -1 -1 Stafiing is kept to a minimum to encourage the independence clients may have in their homes. It is co-ordimated by an Occupational 'Iberapist, with a full time 'house mother' and four hours of nursing daily which is co-ordinated to meet clients needs. Also the centre has its own bus with a driver attendant.

The clients are encouraged to integrate their personal assistant into their stay and other support resources include Irish Wheelchair Association, FAS workers and volunteers.

Lough Sheever Centre is a residential centre for adults with intellectual disabilities. It caters for a client group mainly in the severe, moderate and borderline mild range of handicap. Age range 18 to over 70. It provides general personal and nursing care to all its clients in its day and residential services as is appropriate to the individuals needs. It specialises in the care treatment and management of persons with severe challenging behaviour, many of whom are autistic.

All its services are based on each client having a personal plan in place with a key worker appointed to each person.

It follows a policy of developing and maintaining close contact with parents and relatives of clients

There is an activation programme in place which is available to all.

St. Peter's Centre is a residential Centre for adult and adolescent Intellectual Disability of both sexes ranging in age from 22 yean to 77 years. St. Peter's caters for moderate, severe and profound handicap.

In addition tothe Centre, there are also Community Houses in Longford, Athlone and Mullingar catering for 17 persons. Within the Centre itself, the total number of residents is 93. Between the Centre and it's Community Houses, there are 3 short-term-care beds which are used on a year round basis. The Adult Sight Testing Scheme provides free sight testing and a contribution towards the cost of glasses to eligible adults (medical cardholders). These services are provided by opticians in private practice and are administered by the Health Board. Authorisation must be obtained from the Health Board prior to service provision.

Applications are available from the local Community Care Offices.

Blind Welfare Allowance

The Blind Welfare Allowance is an additional payment made to qualified persons who because of the nature of their disability require additional resources to meet their basic needs.

It is normally paid as a supplement to a blind person receiving Blind Pension or Old Age Pension. 4- General M&l Services Scheme (Medical Cards) Under the General Medical Services Scheme persons are issued with medical cards for themselves and their dependants and are entitled to the following services free of charge.

A. General Practitioner Medical Services from doctors in private practice who are contracted to the Board. Individual choose the doctor they wish to attend. B. Prescribed drugs, medicines and medicaVsurgical appliances. C. Free out-patient and in-patient Public Hospital Services.

Application forms are available form Community Care Offices and must he completed by relevant general practitioner. Medical card applications are subject to a means test but cases ofhardship are also considered.

The Domiciliary Care Allowance is a monthly payment in respect of the care ofchildren normally aged between 2 and 16 years with a severe physical or sensory disability. The allowance is paid to the motherlfather or guardian in certain circumstances.

Such children must normally reside in the community and require continuous care or supervision substantially greater than that, which would normally he required by a child of the same age.

Applicants must undergo a medical assessment and only the means of the child is considered in determining eligibility.

Applications are availahle from the local Community Care Offices. .:. Drug Cost Subsidisation Scheme The Drug Cost Subsidisation Scheme assists individuals to meet a portion of the cost of prescribed drugs, medicines and surgical appliances, where constant medication is required.

Application for the Scheme are available in the Community Care Offices listed below and should he completed by the General Practitioner.

The Drug Refund Scheme assists individuals to meet a portion of the cost ofprescribed drugs, medicines and medicaVsurgical appliances. Receipts should he retained and forwarded to the local Community Care Offices on a three monthly basis stdgon the fust of January on an application form which is available from the local Community pharmacist. .:. EU Health Sewice Entitlement Under EU Regulation Nationals of memher states of the European Union and the European Economic Area, are entitled to health services in other EU memher states.

EU Nationals staying temporarily in Ireland are entitled to receive urgent necessary medical treatment free of charge. EU Nationals resident in Ireland are entitled in certain circumstances to receive a medical card without regard to their means and are entitled to the full range of services normally covered by a medical card. EU Migrant Workers come under the health care system of the country where helshe is employed and has the same entitlements as Nationals of that country. Services for people with a disability

D. Frontier works are those who live in one member state and work in another returning home on a daily or weekly basis. They are entitled to receive health services in the state in which they reside as if they were insured in that state. T%ey are also entitled to receive benefits in the state in which they are employed in accordance with the provisions of health services of that state.

-3 Home lmwovement Scheme for the Elderly

The aim of the Home Improvement Scheme is to improve the living conditions of older people. Improvements vary widely but may include provision of sanitary and water services and basic repairs to windows, doors and roofs. Applicants must be medical cardholders and satisfy certain conditions.

Applications forms may obtained from the local Community Care Offices or by contacting the local Public Health Nurse.

Under the Long Term Illness Scheme, persons suffering from a prescribed long-term illness are supplied free of charge with prescribed drugs, medicines and medicaYsurgical appliances for the treatment of that specific illness or disability.

Applications are made to the Community Care Offices listed below and must be approved on medical grounds There is no means test for this scheme. -:. Medical and Su- Medical and Surgical appliances are supplied to eligible persons, with a recognised medical needs, in order to maintain that person at home and to improve their quality to life.

Applicants for this scheme are usually made by a health professional e.g. Public Health Nurse, Physiotherapist, occupational therapist following assessment. Applicants must be either medical cardholders or eligible for the Long Term Illness and Disabilities Scheme and may have to pay a minimal contribution towards the cost of such equipment.

Mobility Allowance is a monthly allowance payable by the Health Board, subject to a means test, to individuals who have been medically assessed with a severe disability. Applicants must be in a condition to benefit form occasional hips away from home. Application forms are available from the local Community care Offices.

3 Motorised -orl Grant

The Health Board may in certain cirtumstances pay grants towards the purchase of cars to persons with severe physical disabilities Smices for people with a disability

The Board must be satisfied that the person is medically and physically capable of driving a vehicle and is qualified to hold a drivers licence.

Application and further particulars are available from the local Community Care Offices.

The Health Board in conjunction with the National Rehabilitation Board provides rehabilitation training for persons with a physical or sensory disability. The Health Board provide medical assessment while the National Rehabilitation Board provide assessment, training and employment placement for individuals with physical or sensory disability.

The Health Board provide a number of allowances to the National Rehabilitation Board in respect of training \ including:

Rehabilitation Maintenance Allowance which is an additional allowance paid to trainees who are in receipt of Disability Allowance Capitation Allowance Payment made on a capitation basis to bodies providing training services by arrangement with the Health Board Travel Allowance payable to training centres who provide transport for trainees

The Home Help Service is managed under the general direction of the Supt. Public Health Nurse. It is organised and supervised by the Home Help Organiser. Applications for Service are made by the Public Health Nurses. Means testing is carried out on all clients by the Community Welfare Off~ceeEach application is individually assessed. When the Home Help Service is granted, the needs of the patient are closely monitored by the Public Health Nurse and the service may be increased or decreased, depending on the circumstances.

Respite Care is a service which is available in Midland Health Board Hospitals and Community Nursing Units. It is a service which allows older persons to he maintained in their home environment with the appropriate level of care for as long as possible all year round. When a carer requires a break for social or holiday reasons the older person is admitted to hospital for Respite Care. Care is usually made available for a maximum of two weeks (particularly during the Summer month) in order to facilitate as many people as possible wishing to avail of the service.

Intermittent care is provided for patients who have been medically assessed as being in need of regular in-patient care for short periods of time, over a protracted period, on a planned basis.

-t Testing Scheme The Adult Sight Testing Scheme provides free sight testing and a conhibution towards the cost of glasses to eligible adults (medical cardholders). These services are provided by opticians in private practice and are administered by the Health Board. Authorisation must be obtained from the Health Board prior to service provision.

-2 Mind Welfare Alloww The Blind Welfare Allowance is an additional payment made to qualified persons who because of the nature of theu disability require additional resources to meet their basic needs.

Under the General Medical Services Scheme persons are issued with medical cards for themselves and theu dependants and are entitled to the following services free of charge.

A. General Practitioner Medical Services from doctors in private practice who are contracted to the Board. Individuals choose the doctor they wish to attend. Older People Services

B. Prescribed drugs, medicines and medica~s~gicalappliances. C. Free out-patient and in-patient public hospital services.

C. EU Migrant Worken come under the health care system of the counlq where helsbe is employed and bas the same entitlements as Nationals of that country. D. Frontier works are those who live in one member state and work in another returning home on a daily or weekly basis. They are entitled to receive health services in the state in which they reside as if they were insured in that state. They are also entitled to receive benefits in the state in which they are employed in accordance with the provisions of health services of that state.

Housine Aid Scheme for the Elderly The aim of the Housing Aid Scheme is to improve the living conditions of older people. Improvements vary widely but may include provision of sanitary and water services and basic repairs to windows, doors and roofs. Applicants must be medical cardholders and satisfy certain conditions.

Applicants for this scheme are usually made by a health professional e.g. Public Health Nurse, Physiotherapist, occupational therapist following assessment. Applicants must be either medical cardholders or eligible for the Long Term Illness and Disabilities Scheme and may have to pay a minimal contribution towards the cost of such equipment.

~ealdon Whe& Meals can he supplied to older people, in their own home, who are unable to provide for themselves. The objective is to help maintain older people at home for as long as possible.

Clients are provided with a hot meal daily or as required. The service can be accessed by contacting the local Public.:. Health Nurse or the Local Community Care Offices. Mobility Allowance is a monthly allowance payable by the Health Board, subject to a means test, to individuals who have been medically assessed with a severe disability. Applicants must be in a condition to benefit form occasional trips away from home. Application forms are available from thelocal Community care Offices.

.:* .:* Motorised Tra- The Health Board may in certain circumstances pay grants towards the purchase of cars to persons with severe physical disabilities

The Board must be satisfied that the person is medically and physically capable of driving a vehicle and is qualified to hold a drivers licence.

Q subvent~ontowards costs in Private NwmgHome Care A Nursing Home Subvention is a payment towards the cost of private nursing home care for older persons. Older persons making an application for a subvention towards the cost of such care will have to be sufficiently dependent to require maintenance in a nursing home and be unable to pay any or part of the cost of maintenance in a nursing home. Subvention payments will he made directly to the nursing home of the applicant's choice once the nursing home is registered in accordance with the Health Nursing Homes Act, 1990, the Nursing Homes (Care and Welfare) Regulations 1993 and the Nursing Homes (Fees) Regulations, 1993.

The Health Board provide medical assessment while the National Rehabilitation Board provide assessment, training and employment placement for individuals with physical or sensory disability.

The Health Board provide a number of allowances to the National Rehabilitation Board in respect of training including: Rehabilitation Maintenance Allowance which is an additional allowance paid to trainees who are in receipt of Disability Allowance Capitation Allowance Payment made on a capitation basis to bodies providing training services by arrangement with the Health Board Travel Allowance payable to training centres who provide transport for trainees

This section is responsible for the administration of the various Local Government Superannuation Schemes, Circulars, Regulations etc. for certain categories of staff employed with the Board. Superannuation contributions are deducted from the salaries of all officers from the date of their permanent appointment and from the wages of non-officer grades after an initial qualifying period.

8.9 I Bridge Project Management Unit I

The Bridge Project Management Unit was established in 1997 in order to facilitate the changes which will enable the Midland Health Board to meet the major challenges facing all health care organisations going into the 21" Century.

These are: to contain costs to increase population health outcomes to ensure equity in the provision of services to enhance the involvement of users of health services

A 'management by projects' approach has been adopted and the Unit is cur~entlyinvolved in facilitating various projects in support of implementation of the service plans in the 5 main care groups: Acute/Episodic Care, Children, Disability, Elderly and Mental Health and in developing the service plans for 1999.

Description: The function of the ambulance centre within the Midland Health Board region is primarily concerned with the pre-hospital emergency medical service and catering for the routine movement of patients between hospitals both within the boards area and to specialist hospitals outside the region.

The role of the Chief Ambulance Officer is very much concerned with developing smctures and procedures which facilitate the efficient and effective delivery of the service. The role is one of continual change in developing and implementing new standards in training, equipment and service delivery.

The future of the service will require a more patient orientated service targeted at the client group with episodic illness and injury. This refocusing will require some cultural change in the way we do things. This flexible attitude among both management and staff remains the most significant challenge.

The role of the Communications Officer is to act as editor of the Midland Health Board News, work with the Boards Health Promotion Staff in the development of Health Promotion Material and to advise on the presentation of such material.

The Communications Officer also provides professional advise to senior staff of the Midland Health Board in relation to Press Releases and Media

I Health Promotion Services I

The overall aim of the Health promotion service is to enable individuals and committees to take control of their health by making healthier choices easier choices. Such choices could include a health diet, regular exercise and avoidance of smoking. Healthier choices are easier to make when they are supported by health environments.

The service bies to achieve this aim by:

working in pmership with other policy makers and communities in order to create environments supportive of health e.g. no smoking policies, promotion of women's health.

Supporting and developing the health promotion role of other professionals and to encourage them to promote health in their own work.

0 Leading a series of health promotion programmes to encourage people to adopt healthier lifestyles. These include health information, healthy eating, substance issues, schools health, health promotion for older people, women's health promotion, market health promotion and exercise promotion.

7. Fluoridation of Water Supplies - this involves the monitoring of fluoride content of public water supplies to ensure compliance with relevant legislative standards.

Each customer of the Central Sterile Supplies Department (C.S.S.D.) at the Longfordlwesbneath General Hospital, Mullingar can expect a high quality and user friendly agreed service level, where requests outside the norm are always considered and processed as soon as possible, thereby ensuring an excellent service to all.

I Services Provided It provides operating theatres, wards, department, ambulance and other users wih whom supply arrangements have been agreed with equipment and materials for sterile procedures. The equipment and materials are:

1. Correctly prepared by suitably trained staff 2. Sterilised under properly conholled conditions 3. By machines which are regularly serviced and maintained

Green laundry is received from the laundry department at St. Lomans Hospital, it is sorted, checked, folded and autoclaved n provide sterile packs for the Boards Hospitals.

It provides a sterile service from Monday to Friday from 8.00a.m. to 5.00p.m. On-call is provided if required for weekend and Bank Holiday for emergency and major accidents.

It provides a full Laundry and Central Pool Linen and Theatre Service for all of the Midland Health Board, hospitals, health centres, community nursing units, nursing homes and theatres. The Central Laundry operates a shift working system with its staff on a 5 day cycle Monday to Friday from 8.00a.m. - 9.40p.m. (Monday -to- Thursday) and 8.00a.m. - 7.40p.m. (Friday).

The weekly production output is between 37,000 to 40,OO pieces categorised between bed linen, personal clothing and theatre linen. The number of staff employed is 24 plus 1 contract driver.

The Central Laundry provides a clothing repair service through the seamstress.

The main purpose "to return items entrusted to their care by the customer in as near to new condition as possible and fit for the purpose, to maintain a full laundry service and adequate supplies of clean lines to all hospitals, health centres, community nursing units, nursing homes and theatres for all of its patients".

The central Laundry operates all of its washing cycles in accordance with all Health & Safety and Infection Control Standards.

As already indicated the Health Board hold a wide range of Records and Information relating to its services, patients, clients and staff. Much of this information is provided in confidence by members of the public or is obtained in the course of carrying out diagnostic and treatment procedures to which the patientlclient has consented, i.e. Medical Records in Hospitals, Clinics, etc. Other information relates to the day to day management of the Board, e.g. Financial, Personnel, etc.

The Board owes a duty of confidence to those who provide the information; however due to the fact that the Board is a public body funded by the taxpayer, it recognises its responsibility to members of the public in relation to accessibility to the various records it holds, consistent with the right of privacy of the individual and the public interest.

Patient I Client Records

As stated about the Board hold a wide variety of records at various locations throughout the region, arising from contact which members of the public have with the Boards Services, Community Welfare Officer, etc. It is Board policy to provide access to the information held consistent with the right to privacy of the individual.

A. Discussion between Service Provider and patient I Client Because of the technical nature of many of the records maintained it would be beneficial that the initial request for information be dealt with on a one to one basis between the patientlclient themselves and the service provider, i.e. Nurse, Social Worker, Medical Practitioner etc.

This type of discussion allows for a greater explanation of the information sought and allows the requester ask questions andlor seek clarification on any issue which may be initially unclear.

It is Board policy to seek to improve the communications interface between staff and members of the public availing of the service.

B. Request for Access to a Record - Acknowledged - Response This may take the form of: 1. Seeking a copy of the record held i.e. file, tape (video or audio) etc 2. A reasonable opportunity to inspect the record

Such requests should be made in writing, to the Boards Officers listed or give details of the record required, and the form of access sought e.g. copy or inspect.

Where access to records of a third party is sought, written comment from the third party is required so that the privacy of the person to whom the record refers is protected, the Board may seek to obtain appropriate identification from the requester.

Staff Records

Purpose of FOI Act The long title sets out the purpose of the Freedom of Information Act. It asserts the right of members of the public to obtain access to official information to the greatest extent possible consistent with public interest and the right to privacy.

Acknowledcment of Reauea One of the Decision Makers (as listed in Appendix A) receiving the request shall acknowledge it in writing within two weeks of receiving the request.

The letter of acknowledgement shall state the period within which a decision must be made, the rights of review, the procedure governing the exercise of those rights and the time limits governing such exercise.

Decision of Reaugsts Within 4 weeks of receiving a request for access, the Decision Maker shall:

(1) decide whether to grant or refuse to grant the request or to grant it in part, (2) if he or she decides to grant the request, whether wholly or in part, determine the form and manner in which the right of access will he exercised, and (3) advise the requester in writing of the decision (4) Shall notify the requester of hisher right of appeal, should the decision be to refuse access. (5) Where there is third party information on the requested record, the Board is allowed a further 3 weeks to make a decision and the requester must be advised accordingly.

Review of Decision

A. Freedom of Information Appeal. If a member of the public feels aggrieved regarding a decision of the Board to limit access to records under the Freedom of Information Act, they have the Right of Appeal which should be made in writing to one of the Internal Reviewers (listed on Appendix B) within four weeks of the decision.

B. Information Commissioner If one is dissatisfied with the decision taken by the Internal Reviewers as outlined above then one may make a further appeal to the Information Commissioner not later than 2 weeks after the notification of the decision to the relevant person concerned.

The Information Commissioner may, on receipt of an application to him: 0 Review a decision 0 Following the review, may: D Affm or vary the decision D Annul the decision and if appropriate and make such decision as heishe considers appropriate.

B. Appeal to High Court A party to a review under (h) above or any other person affected by a decision of the Commissioner, following such a review, may appeal to the High Court on a point of law.

Fees may be charged as follows:

In respect of personal records, no fees will be charged for copying the records requested unless a large number of records are involved.

In respect of other (non-personal) information, fees may be charged for the time spent in efficiently locating an copying records, based on the standard rates in operation at the time of the request. No charges may apply in respect of the time spent by public bodies in considering requests. A deposit may be payable where 'the total fee is likely to exceed £40. In these circumstances the Health Board must, if requested, assist the member of the public to amend the request so as to reduce or eliminate the amount of the deposit.

Charges may be waived in the following circumstances:

Where the cost of collecting and accounting for the fee would exceed the amount of the fee; Where the information would be of particular assistance to the understandig of an issue of national importance; 0 In the case of personal information where such charges would not be reasonable having regard to the means of the requester
