Terms of Services

provided by the Visa Application Centre to Poland

All applicants willing to apply for a Schengen or National are requested to obtain a prior appointment to the Visa Application Centre in the respective Consular region. As per the established rules applicants may apply for a national visa to Poland based on the legal registration in a particular Consular region. In case applying for a Schenegn visa [not applicable to a visa for work] applicants may be legally registered in the other consular region but legally work / study and show a proof of the same while document submission.

To schedule an appointment to the VAC it is required to follow the below steps:

 Choose visa application center and visa category for which you will apply. As per the established rules applicants may apply for a national visa to Poland based on the legal registration in a particular Consular region. In case applying for a Schenegn visa [not applicable to a visa for work] applicants may be legally registered in the other consular region but legally work / study and show a proof of the same while document submission.  Visit any branch of Kredo of Idea bank (List of the bank branches can be found on our website: http://www.vfsglobal.com/Poland/Ukraine/ )  Read and accept Terms of Services provided by the Visa Application Centre to Poland  Give to the cashier original of your international passport (for entering your data and generation of a unique PTN code). Non-Ukrainian citizens should additionally submit: Ukrainian visa that allows returning to or permanent/temporary residence card  Pay the Service Fee by the applicant in person only according to the established price on the day of payment submission.

Payments are not being accepted on the base of copies of documents, including those which are notarized.

Acceptance of documents and payment of authorized person acting under a power of attorney issued by the applicant is not performed.

In case spouses submitting payment for each other original of marriage certificate should be given along with original international passport of applicant and original of international or internal passport of the spouse who is submitting the payment.

In case submitting payment for minor children (under 18 years old) by their parents/ legal guardians birth certificate of the child/document confirming his legal guardian status should be given along with original his international passport. Also original of international or internal passport of the parent/guardian who is submitting the payment is required.

In case submitting payment for an adult who is incapable - legal guardian of such person must provide the appropriate document proving him to be appointed as a guardian and authorized person legally entitled to act on behalf of an incapacitated person. Also original of international passport of applicant original of international passport or internal passport of the guardian who is submitting the payment is required. After submitting the payment cashier gives to the applicant 1 (one) receipt with unique PTN number mentioned which can be used for appointment booking. PTN is activated next working day after payment was made and is valid for 3 month after payment date in case possessing an international passport (further passport) valid 3 month prior the date mentioned below.

The PTN cannot be passed to third persons.

VISA APPLICATION PROCESS

In order to obtain the appointment to above VAC’s following steps should be followed:

Step 1: Know your visa type and check whether you are eligible to apply for a visa at the Visa Application Centre chosen by you. As a rule applicants legally registered may apply for a national visa to Poland, applicants from the other regions who work or study and are able to prove the same may apply for a Schengen visa [not applicable to a visa for work.

Step 2: You will have to visit one of the Kredo Bank or Idea Bank branches in person in order to pay the service fee. The payment is taken based on the origin of your international passport. In case of registration for spouses/minors, you have to provide and proof of relationship.

Applicants applying for a Schengen visa to Poland should submit visa applications in Visa application centers of the indicated directly without prior registration.

Applicants applying for National visa to Poland in all consular districts should make a payment in order to schedule an appointment date to VAC.

Applicants applying for LBAT should submit their applications in Visa application centers in Lviv, Uzgorod and Lutsk directly without prior registration.

At the time of payment, you will have to indicate your valid contact phone number and VAC Code:

Post payment you will receive unique banking code PTN.

IMPORTANT:

Please ensure that you give correct and valid contact number. If contact phone number provided is incorrect or belongs to the third person, registration will be impossible, and you will have to apply for a refund.

Please note that only 1 payment can be done for one applicant. In case there will be several payments done and the date of appointment for the first payment will be a future date – all other payments will be considered as invalid and will be passed for refund.

Step 3: PTN number received at the bank will be activated in the registration system next working day after payment. Appointment date will be offered automatically depending on the visa type you apply for. You can check your PTN status on the website.

Step 4: Once appointment date is settled, call center operator will contact you over the phone from 9:00 till 21:00 from Monday till Saturday, except of holidays. Step 5: Once you are contacted by call center operator, you will be informed on the date and time offered for an appointment at the VAC. If you confirm this date, appointment will be scheduled by operator. On the date of appointment you will have to submit your documents in the VAC (list of required documents is available on the website).

Please note that the next suggested date will be no earlier than 7 days after your first appointment date.

Step 6: If appointment date offered by the operator is not suitable for you, operator will cancel registration. Next appointment date will be selected by the system, and you will have to wait for the next call from operator.

Step 7: During next call from Call Center, operator will inform you about the second automatically appointed day and time offered by the system in order to submit the documents in the VAC. Once you accept this date, appointment will be scheduled by operator. On the date of appointment scheduled you will have to submit your documents in the VAC.

If you reject the second appointment date, your PTN number will be deactivated and you will have to apply for a refund of the payment in the VAC. Simultaneously you may prepay the Service Fee again in order to receive new appointment in case necessary.

Important information:

Maximum number of applicants prepaying the fee as a family is considered of 5 applicants. Each and every next applicant should provide his / hers own phone number. Parents and children only will be treated as a family. Siblings or any other relatives should make individual payment and provide their own phone number.

All the members of a family should apply at the same VAC and for the same Visa Categories. If you apply for a different Visa and at a different Visa Centre, please make a payment separately.

In case you do not receive a call in about 3 days from the date your payment had been successfully activated, please check status of your appointment on the website:

Status “PTN is in queue” will mean appointment had not yet been scheduled and you need to wait when appointment will be assigned to your PTN.

In case appointment date and time had been already reserved, operator will get in touch with you in a short period of time. Additional information can be taken form the Call Centre.

The applicant must personally come to VAC as specified in a confirmation date and time of the original receipts for payment of services for booking record to VAC and necessary for applying for a visa documents*

• Visa application questionnaire completed and signed personally by the applicant (form can be downloaded using the following link: www.vfsglobal.com/Poland/Ukraine or received form questionnaire directly in the VAC). • 2 (two) color photographs applicant passport size 3.5 cm h 4,5 cm. • Applicant's passport, which expires no earlier than three (3) months after the scheduled date of return of the applicant countries of the Schengen zone. • passport Ukraine (internal passport) - original copies of all pages containing any mark. • The original and copy of medical insurance with the original receipt for his payment. • Original receipt of payment services to VAC booking record.

* The list of documents required for filing visa applications may vary according to the requirements of the Consulate General of the Republic of Poland (hereinafter - the "Consulate"). Consulate reserves the right to request additional information or documentation and to refuse the visa applicant. In certain cases specified Consulate instead of transferring documents via VAC, provides Consulate / returns documents directly to the applicant.

When submitting the visa application and related documents to VAC for certain categories of applicants can be installed need for additional visa fee directly to the consulate (ranging from 35 € to 70 €, according to current tariffs Consulate), which can be found at the following link www.vfsglobal.com/Poland/Ukraine

Where the applicant for any reason not to appear in VAC specified in a confirmation date and time - a unique code for the record is automatically deactivated and the applicant loses the right to apply for a visa within previously paid.

Repeated appeals to the applicant VAC only possible when re payment of booking. Proper proof of timely arrival to the applicant VAC receipt is issued employee VAC after the filing of the data of the applicant and visa numbers are assigned to the case.

In exceptional cases (serious illness applicant, serious illness or death of close relatives applicant duly confirmed force majeure - supporting documents) can be made a positive decision on the second appeal of the Applicant's application for a visa within previously paid.

When submitting the visa application and related documents to VAC, applicant signs a written consent of the subject of personal data and two copies of the Act of works / services provided, one of which is in VAC, and another - issued to the applicant.

Payment VAC confirms that the applicant personally examined in detail and agrees to the Terms, legally binding treaty accession.

Not later than the following applicant to appeal VAC, working day package of documents transmitted to the applicant by the Consulate General of Poland, in compliance with safety regulations and requirements of the Consulate. Examination of documents Consulate lasts from six (6) days to thirty (30) calendar days, and then returned to VAC.

Applicant agrees that the decision to issue Schengen or national visa or denial of extradition, taken alone Consulate General of the Republic of Poland and VAC not responsible for any delay in deciding on obtaining a visa applicant.

VAC not responsible for possible violations and actions that are not within its competence, in particular: the shift timing visa at the General Consulate of the Republic of Poland, the refusal of a visa; vicarious consular, customs and immigration facilities; for loss or damage to the documents of applicants or other conditions that go out of control and do not depend on the will VAC.

The applicant has the opportunity to get your passport in person at VAC or use courier service (delivery of the passport by courier Applicant ordered and paid separately directly in the filing or after filing for VAC (details can be found at the following link: www.vfsglobal.com/Poland/Ukraine ) and is 150.00 UAH. In case of return of applicant documents via third parties - courier service VAC not responsible for their actions, including the loss or damage applicant documents.

For passport directly to VAC applicant must present Passport Ukraine (internal passports) and the original receipt of payment services.

If the applicant does not appear in its passport to VAC within ninety (90) days from the date of visa applications and related documents to VAC passport will be returned to store to the Consulate General of Poland and for its subsequent transfer to Foreign Ministry of Ukraine.
