However, there is limited data on rectal douching behavior in populations practicing AI. We examined the content, types of products, rectal douching practices and risk behaviors among those reporting AI. Methods: From August 2011 to May 2012, 1,725 women and men reporting receptive AI in the past 3 months completed an internet-based survey on rectal douching practices. The survey was available in English, French, German, Mandarin, Portuguese, Russian, Spanish, and Thai and included questions on sexual behaviors associated with AI including rectal douching. Differences by rectal douching practices were evaluated using chi-square methods and associations between reported douching practices and other factors including age and reported STI history were evaluated using logistic regression analysis. Results: Respondents represented 112 countries, were mostly male (88%), and from North America (55%) or Europe (22%). Among the 1,339 respondents (66%) who reported rectal douching, most (83%) reported always/almost always douching before receptive AI. The majority of rectal douchers reported using non-commercial/homemade products (93%), with water being the most commonly used product (82%). Commercial products were used by 31%, with the most common product being saline-based (56%). Rectal douching varied by demographic and risk behaviors.

Weld, MD A dissertation submitted to Johns Hopkins University in conformity with the requirements for the degree of Doctor of Philosophy Baltimore, Maryland April 2019 Abstract The three parts of this thesis are as follows: 1) A comparative pre-phase I study exploring the impact of volume of microbicide gel vehicle on distribution through the distal colon, user experience, and acceptability (Section II); 2) An analysis of the colorectal distribution of lymphocytes and cell-free HIV surrogate in autologous seminal plasma following simulated anal intercourse (Section III); and 3) A survey study exploring the interest of U.S. youth living with HIV in long-acting injectable antiretrovirals (Section IV). Section II: Methods: Eight HIV negative men with a history of recent RAI were enrolled into a two- period, sequence randomized, dosing study comparing 3.5 mL and 10 mL of radiolabeled (1 mCi 99mTc-DTPA) universal placebo hydroxyethyl cellulose (HEC) gel. Each participant received two doses in the research unit, one of each volume, separated by a washout period of at least 2 weeks. Each research unit dose was followed by a self-administered take-home dose. Safety and gastrointestinal distribution were assessed after the research unit doses; safety, perceptibility, and acceptability were assessed after take-home doses. Results: There were no adverse effects of Grade 2 or higher and all resolved spontaneously. Both volumes were well tolerated and received high acceptability scores. The 3.5 mL and 10 mL gel volumes distributed similarly (p>0.2) within the rectosigmoid.

The word stopped her, but Scott kept moving. Her training in HIV taught her that "you can't bat an eye" when patients say things that surprise you. But she did need to know more. "I asked, 'Are you using like a Summer's Eve–type product or are you using a Fleet [enema]?" she said. "I just put it out there. If they're going to be bold enough to tell me, I'm going to be bold enough to ask." According to data presented at the Association of Nurses in AIDS Care 2019 Conference, half the gay and bisexual black men in the Deep South have used an enema — or "rectal douche" as it's called in the community — although what they are using to douche is unclear. For men who are receptive partners, it's even more common. This is despite the conventional wisdom that no one should regularly use an enema unless they have a neurologic condition, such as multiple sclerosis or spinal cord injuries, that necessitates it. Data also show that enemas, along with other sexual-hygiene practices, could increase the risk for HIV transmission and other sexually transmitted infections (STIs). But there are formulations that are safer, and research is ongoing into medicated formulations.
