BIGGEST LOSER: PET EDITION Contestant Release Form

All pets entering New Frontier Animal Medical Center’s 2nd Annual Biggest Loser: Pet Edition contest MUST be current on the following: DA2PP (canine participants)/FVRCP (feline participants), and Rabies vaccination. If any vaccinations are due, they must be administered by the start of the contest on August 20, 2016.

All participants must be approved by a New Frontier Animal Medical Center veterinarian prior to entering this contest. Unless your pet has been recommended for or authorized by your New Frontier Animal Medical Center veterinarian, it is required that a wellness exam is completed prior to the final registration date of August 20, 2016. The contest will be limited to the first 40 qualified applicants.

The Biggest Loser: Pet Edition contest will run from August 20 to October 15, 2016. On Saturday, August 20, the contest begins with an initial weigh-in, body measurement, and photograph, performed by a New Frontier Animal Medical Center Nutrition Team Advocate. Contestants are then required to fulfill a bi-monthly weigh-in with a New Frontier Animal Medical Center Nutrition Team Advocate. At each weigh-in, your pet will be weighed, measured, and photographed to record your pet’s progress and ensure healthy weight loss.

After each weigh-in, one winner (one canine and one feline) will be announced based on their healthy weight loss progress and their photo will be posted on the New Frontier Animal Medical Center website and Facebook page. Weigh-in winners are determined by the percentage of total body weight lost during the interval between weigh-ins, not the cumulative weight lost to date.

The Contest will conclude Saturday, October 15, 2016 with a final weigh-in, measurement, and photograph. The winner of the Contest will be determined based on total percent body weight lost over the course of the entire contest (August 20 through October 15, 2015). The winner will be spot-lighted on our website and Facebook. They will also be announced at the Grand Finale Celebration on October 22, 2016.

The New Frontier Animal Medical Center veterinarians and Nutrition Team Advocates reserve the right to remove any contestant from the Contest at any point in time as the result of unhealthy weight loss or other health concerns.

The New Frontier Animal Medical Center veterinarians and Nutrition Team Advocates will use all reasonable and necessary precautions against risk or injury related to weight loss, but will not be held liable or responsible for any other risk or injury not related to weight loss and it is understood that you assume all other risk.

The New Frontier Animal Medical Center veterinarians and Nutrition Team Advocates make no guarantee of a pet’s weight loss as a result of participation in the Contest.
