With reference to the subject we bring to your kind notice that we have submitted our project proposal vide no. 1 reference. For the same the essential details soughed (EDS) by State Level Environmental Appraisal Authority (SEIAA) vide no. 2 reference stating that the validity period of the demand draft was lapsed. And the form 2 which was submitted must be signed by the project proponent.
The referred points made clear and the documents submitted to office immediately. Now, we uploaded the same documents in the portal. Hence we request you to kindly accept our application and issue EC as early as possible.

Satyanarayana” over an extent of 0.5 Ha., Sy.No. 235, Dakamarri Village, Bheemunipatnam Mandal, Visakhapatnam District, Andhra Pradesh State.

With reference to the subject above we are herewith submitting Form-1, Form 2, EMP and further documents included with cluster letter and ADMG approved Mining Plan for the proposed building stone quarry located over an extent of 0.5 Ha. In Sy.No. 235 of Dakamarri Village, Bheemunipatnam Mandal, Visakhapatnam District, Andhra Pradesh State.
The EC processing fee of Rs. 10,000/- drawn in favour of SEIAA/SEIAC, AP, Vijayawada vide No. 182116 Dt. 18.07.2019 as a processing fee. The same is attached herewith.

DECLARATION LETTER a) I, Sri S.Satyanarayana, Applicant of the quarry named “Sri S.Satyanarayana” over an extent of 0.5 Ha, located in Sy.No. 235 of Dakamarri Village, Bheemunipatnam Mandal, Visakhapatnam District, Andhra Pradesh State, hereby declare that our organization does not have conflicts of interest with the chairman and members of SEAC, A.P and SEIAA A.P., b) We hereby declare that I (Sri S.Satyanarayana) myself authorized to submit the application and to do all necessary correspondence for obtaining environmental clearance from State Level Environment Impact assessment Authority (SEIAA, A.P) and to attend the meeting(s) of State Level Expert Appraisal Committee (SEAC) A.P., to present our case and to clarify the points raised by the Members of SEAC and SEIAA. c) We hereby declare that the information furnished above is true to the best of my knowledge and I shall hold personal liability for any deliberate misrepresentation and or concealment of factual information. If the information furnished is found to be false, the EC issued to our organization can be withdrawn and I am liable for the action as per rules applicable.
