The District Agenda is comprised of five different sections:

The meeting will begin promptly at 7:00 p.m. with the first section which is called Audience Comments. The Audience Comments portion of the agenda is when individuals may comment on matters that concern the District. Each individual is limited to three (3) minutes for such comment. The Board of Supervisors or Staff is not obligated to provide a response until sufficient time for research or action is warranted. IF THE COMMENT CONCERNS A MAINTENANCE RELATED ITEM, THE ITEM WILL NEED TO BE ADDRESSED BY THE DISTRICT MANAGER OUTSIDE THE CONTEXT OF THIS MEETING. The second section is called Business Administration. The Business Administration section contains items that require the review and approval of the District Board of Supervisors as a normal course of business. The third section is called Business Items. The Business Items section contains items for approval by the District Board of Supervisors that may require discussion, motion and votes on an item-by-item basis. Occasionally, certain items for decision within this section are required by Florida Statute to be held as a Public Hearing. During the Public Hearing portion of the agenda item, each member of the public will be permitted to provide one comment on the issue, prior to the Board of Supervisors’ discussion, motion and vote. Agendas can be reviewed by contacting the Manager’s office at (813) 533-2950 at least seven days in advance of the scheduled meeting. Requests to place items on the agenda must be submitted in writing with an explanation to the District Manager at least fourteen (14) days prior to the date of the meeting. The fourth section is called Staff Reports. This section allows the District Manager, Engineer, and Attorney to update the Board of Supervisors on any pending issues that are being researched for Board action. The final section is called Supervisor Requests. This is the section in which the Supervisors may request Staff to prepare certain items in an effort to meet residential needs.

Public workshop sessions may be advertised and held in an effort to provide informational services. These sessions allow staff or consultants to discuss a policy or business matter in a more informal manner and allow for lengthy presentations prior to scheduling the item for approval. Typically no motions or votes are made during these sessions.

Overall, Pond #1 looked good. Minor shoreline weeds anticipated in between treatment periods were observed along the shorelines, which will be treated during our routine maintenance visits.

Pond #2 was identified with minimal Torpedograss extending from the shoreline, which will be targeted during our routine scheduled visits. Positive results may be expected within 14-21 days following treatment. 9/15/2017

Moderate Planktonic Algae was identified along the perimeter of Pond #4, which will be targeted during our upcoming routine visits. Algae is expected to clear within 10-14 days following treatment. 9/15/2017

Mitigation Site #6 continues to thrive with native species, which consisted of over 95% of the observed vegetation! The minor overgrowth identified along the perimeter (top right) will be treated during our upcoming routine visits. 9/15/2017

Management Summary

The Mira Lago West CDD continues to look great following our routine maintenance visits! Of the 8 sites, only Pond #4 was observed with Algae, likely a result of the large influx of storm water runoff produced by Hurricane Irma earlier this week. Algae is often anticipated following extended rain events due to the influx of nutrients transported from landscapes, which contain pesticide residues, fertilizer, and pet waste. The moderate Planktonic Algae within Pond #4 will be targeted during our upcoming routine visits, and is expected to clear within 10-14 days following Algaecide application.

The dissolved oxygen (D.O) level of each pond is graded on a scale of 0-10, with 0 being anoxic conditions and 10 being the highest dissolved oxygen. The D.O levels for the ponds are as follows: Pond #1: 9.35 mg/L Pond #2: 7.63 mg/L Pond #3: 8.87 mg/L Pond #4: 16.66 mg/L Pond #5: 6.2 mg/L Pond #7: 7.6 mg/L Pond #8: 11.96 mg/L The high D.O levels noted within Pond #4 may be a result of the Planktonic Algae identified along the perimeter, which may temporarily elevate Oxygen levels via photosynthesis.

Mitigation Site #6 looked good during today's inspection, with high volumes of native species observed throughout the entire site! Mitigation Sites play an important role in Florida communities, especially in their ability to control flooding in highly developed areas. As more impermeable surfaces become established on Florida soils (such as houses and roads), fewer areas become available for water to percolate through; this inadvertently increases the risk of flooding. These natural areas are designed to collect and redirect runoff to larger bodies of water, such as the Hillsborough River. In addition to flood control, these wetlands play a vital role in trapping and breaking down pollutants. As storm water runoff meanders through the thick vegetation of a Mitigation Site, pollutants such as fertilizers and heavy metals become sporadically deposited within the roots and soils. The native vegetation throughout the community will continue to be promoted in an effort to improve the functionality of storm water retention ponds and wetlands mitigation sites. Despite ideal conditions for invasive growth, minimal shoreline weeds were noted within Ponds #1, #2, #6, #7 and #8, all of which will be treated during our routine maintenance visits. Positive results may be expected within 14-21 days following treatment. Most ponds within Florida fall within a pH range of approximately 6 to 9 (higher values are generally found within brackish systems). Although there are no precise guidelines for pH within aquatic systems, a pH values above 9.5 or 10 are generally considered undesirable within lakes, streams and ponds. The pH levels for the ponds are as follows: Pond #1: 8.3 Pond #2: 8.5 Pond #3: 8.6 Pond #4: 8.1 Pond #5: 8.1 Pond #7: 8.1 Pond #8: 8.2 Lastly, the temperatures for the ponds were observed within desirable ranges and are as follows: Pond #1: 28.3°C, Pond #2: 28.6°C, Pond #3: 28.6°C, Pond #4: 29.2°C, Pond #5: 28.7°C, Pond #7: 28.8 °C, Pond #8: 28.8°C

Recommendations/Action Items

- Routine Maintenance.

- Continue to monitor all ponds for Algal growth.

- Treat the moderate Planktonic Algae within Pond #5.

- Continue to treat all sites for invasive vegetation during our routine visits. - Treat the minimal to minor shoreline weeds within Ponds #1, #2, #6, #7 and #8.

Bowman Consulting was selected as design consultant to develop infrastructure master plans for 7 Community Redevelopment Areas (CRAs) identifi ed by Martin County. Th e CRA’s were created in response to years of urban areas developing without planned infrastructure. To ensure the quality of the product, the fi rm part- nered with specialized consultants for the land use planning, water and wastewater systems, and funding/costs solutions. Th rough a charrette process, vision plans were adopted for each CRA. Th e plans were the result of a collaborative eff ort between the County, their planning consultant, Neighborhood Advisory Commit- tees (NACs), and the public (stakeholders of the community).

Opportunities were iden- tifi ed to address the specifi c aesthetic needs of each community through shared benefi ts such as streetscaping, open space, parking, and outdoor facilities. We then coordinated meetings with the Neighborhood Advisory Committees and the County to develop these multifaceted ideas and plans. Bowman provided an anal- ysis of the costs and possible funding mechanisms, (such as CDBG’s), in addition to a planning tool for the County to determine budget needs for the future Capital Improvement Plan, a 25-year maximum build-out plan, and identifi ed future im- provements to be included in future redevelopment projects.

Th is 1,110 +/- acre subdivision included an 18-hole golf course, clubhouse, more than 1,000 Single Family residential lots, as well as a commercial corridor. Bowman Consulting (formerly Creech Engineers) was responsible for engineering design, entitlement, and permitting for roads, master surface water management systems including +/- 50 acres of wetland enhancement and mitigation, master water distribution systems, and master wastewater collection/ transmission systems. Bowman Consulting worked hand-in-hand with a golf course designer to create a well-balanced great golf course experience while also managing costs; Bowman Consulting was able to virtually balance the site to avoid additional earthwork costs. Our fi rm’s responsibilities included boundary and topographic surveying, platting services, construction layout. Bowman Consulting master planned this community which had well sites that were abandoned and relocated. Also as part of the project, Bowman Consulting designed an irrigation system for the golf course, which utilized the stormwater from the existing golf course ponds.

Asset Management: JMT can assist the District by developing or updating land ownership and maintenance maps depicting District property and facilities and areas maintained by vendors such as landscapers or aquatic services. Wetlands and buffer areas maps can be developed to educate residents and vendors of the different vegetation restrictions that may exist. These maps are living documents that may be updated as your community grows or as vendor needs change. JMT can also provide asset reports cataloging facilities or property such as stormwater facilities or streets for condition, suggested maintenance, or replacement so that future capital fund expenditures may be scheduled in advance.

Capital Improvements: JMT can provide the District with planning assistance in projecting needed capital improvements and transition them into construction plans and permits. JMT can also assist in developing capital improvement budgets so that funding may be planned for long term improvements.

Drainage Improvements: JMT can identify nuisance drainage problems, identify potential solutions, and provide cost estimates for alleviating the nuisance. Sometimes simple regrading or modifications to curbs or landscaping can correct standing water on streets or in grassy areas.

Permitting: JMT will inspect existing ponds in accordance with Southwest Florida Water Management District (SWFWMD) maintenance schedules and all inspection reports will be filed and cataloged by our staff. Hank brings a rich and diverse background of 35 years of experience in engineering, planning, environmental compliance, project and program management, and crisis and emergency management planning and operations. Hank has lived in a CCD protected community in Riverview for the last 17 years. Kimberlee DeBosier, PE – Kim will serve as Principal‐in‐Charge, assuring that resources and staffing are available to ensure your satisfaction. She will also support Jordan in technical design when her expertise is needed. She has 39 years of professional civil engineering experience, the last 28 years being in the Tampa Bay area. Kim currently serves as District Engineer for two CDDs in Pasco County: Lexington Oaks and Heritage Springs. Robert E. Dvorak, PE – Robert is responsible for managing the water resource and drainage projects for JMT as well as the design, hydrologic and hydraulic modeling, and regulatory permitting for a wide range and variety of projects. Sergio Quevedo, PE, PTOE – Sergio has 15 years of experience. He is proficient in managing a staff of technical personnel performing traffic operational and safety studies, ADA assessments, signing and pavement marking design, signalization design, ITS design, and lighting design. He also has experience with signing and pavement marking, signalization, ITS and lighting design, as well as the development of plans using MicroStation CADD and GuidSign. Doug Skurski, PWS – Doug has 16 years of experience in wetland assessments, federal, state, and local permitting, protected species studies, GIS mapping and analyses, NEPA, habitat evaluation, and environmental impact mitigation. Various tasks completed for the Harbour Isles community include: • New Fitness Center – JMT completed the site design and permitting for a new fitness center adjacent to the existing clubhouse. Permits were obtained from Hillsborough County and the Southwest Water Management District (SWFWMD). We prepared the contract documents, including general conditions, and assisted in the bid phase. We provided construction phase services as well. • Night Swimming Assessment – JMT performed light readings and made lighting recommendations with respect to the availability of the community swimming pool during dark hours. • Traffic Analyses – JMT reviewed the Signal Warrant Study provided by the Florida Department of Transportation (FDOT) for the intersection of US 41/SR 45 at Spindle Shell Way. Discussions with FDOT and the neighboring development are underway. • SWFWMD Permit Review – JMT reviewed existing SWFWMD permits to determine when pond inspections are required for the development’s various permitted facilities. Engineering tasks are established by the CDD Board of Supervisors and assigned to the engineer to accomplish the specific objectives. The tasks vary in size and complexity and each has a specific scope and schedule which is coordinated with the Board and District Manager.

Various tasks completed for the Diamond Hill community include:  Pond Repairs ‐ JMT reviewed the stormwater detention pond conditions and performance. Specific concerns were recent repairs to drainage structures and inflow pipe foundations. Data collection, permit evaluation and research as well as hydraulic and hydrologic calculations were specific tasks. The final deliverable was a report addressing probable improvements and cost estimates for Phase I. Phase II consisted of developing construction documents for competitive bid. The design improvements consisted of an energy dissipating structure with sand cement armoring and regrading around the pond bottoms and side slopes.

Pond Inspections – JMT Engineering performed professional engineering services on an as‐needed basis in accordance with assignments provided by Heritage Pines Community Development District (HPCDD) in Pasco County. Each assignment addressed a particular need, goal or objective and included a specific scope, schedule and fee as determined through coordination and negotiation with the Board and District managers.

JMT was responsible for pond inspections and report writing. There are 62 water management facilities located in the HPCDD requiring inspection and certification of compliance for SWFWMD. JMT was tasked to research the files at SWFWMD to establish a list of facilities and prioritize the need for inspection and certification. JMT conducted several inspections per quarter to maintain a satisfactory schedule with SWFWMD.

Pond Improvements – JMT Engineering was tasked with addressing multiple pond erosion issues associated with steep pond side slopes at numerous locations throughout the development. The task involved selective surveying, site inspections, some geotechnical sampling and design of improvements. The design consisted of evaluating the soil types and developing a combined embankment and soil stabilizing fabric to maintain a 4:1 side slope. Construction drawings were developed and competitive bids were received to implement the design improvements. JMT also performed construction inspection services as well as final project certification for SWFWMD.

Reclaimed Water Plan Review – JMT was tasked with review of reclaimed water design plans performed by others. This review included recommendations for design options.

JMT is retained by the Lexington Oaks Community Development District (CDD) in Pasco County to provide engineering services on an as‐needed basis to assist the community in maintaining and improving their infrastructure assets and amentities.

Engineering tasks are established by the CDD Board of Supervisors and assigned to the engineer to accomplish the specific objectives. The tasks vary in size and complexity and each has a specific scope and schedule which is coordinated with the Board and District Manager.

Various tasks completed for the Lexington Oaks community include:  Permit Inventory and Inspection Scheduling – This task required research of SWFWMD permit documents archived at the central SWFWMD Brooksville Office. A map and spreadsheet were developed indicating the numerous permits, where the ponds were located within the community, when the pond inspections were to be completed, and the parcel number for Pasco County.  Pond Maintenance Plan – The objective of this task was to determine the extent of repair needed for each pond, establish an estimated cost, and prioritize each pond based on the functional performance requirements. Inspections were performed for each pond site noting any issue requiring maintenance or repair. Numerous ponds had erosion on the side slopes and several had degraded control structures due to skimmer settlement or foundation/embankment erosion. A report was prepared and presented to the CDD Board for additional action. The report is a planning tool to eventually address all the ponds.  Wall Inspection and Repair – There are numerous walls within the community that provide security and screening. These walls are concrete masonry and stucco with architectural features that mirror the community standards. Before the walls were painted, inspection was required to assure structure soundness and functional capability since cracks and settlement were noticed. The walls were inspected and proposals for

repairs were re viewed and approved by the engineer.  Aptitude Terrace – This task included the design of rear‐yard drainage systems to intercept runoff that was causing erosion along a retention pond bank. This pond had experienced chronic erosion problems over the years. This design will alleviate those problems.  ADA Facility Compliance – JMT developed a list of facility improvements to bring the CDD into compliance with ADA requirements. We have developed plans for the first set of new sidewalks to be constructed in the next few months.  Reserve study – JMT has prepared two updates to the CDD’s Reserve Study.  Court Resurfacing – JMT prepared plans for the resurfacing of two tennis courts and one basketball court.  Pool Heating Analysis – JMT prepared a comparative analysis of heating the District’s pool using natural gas or propane.

JMT Engineering has been retained by the Heritage Springs Community Development District (CDD) in Pasco County to provide engineering services on an as‐needed basis to assist the community in maintaining and improving their infrastructure assets and amenities.

Engineering tasks are established by the CDD Board of Supervisors and assigned to the engineer to accomplish the specific objectives. The tasks vary in size and complexity and each has a specific scope and schedule which is coordinated through the Board and District Manager.

Various tasks completed for the Heritage Springs community include: • Hole #17 Regrading – This project involved the design for the regrading of an area of the golf course which had been filled partially blocking the free flow of water over a 250 foot weir. This design was a compromise between the need to move the water during flood conditions and the need for a dry landing area for golfers. • Pond Inspections and Report Writing – The JMT team conducts inspections and writes reports as part of a professional engineering services as‐needed basis assignment. There are 39 water management facilities located in the HSCDD requiring inspection and certification of compliance for SWFWMD. JMT was tasked to research the files at SWFWMD to establish a list of facilities and to prioritize the need for inspection and certification. Currently, JMT is conducting several inspections per quarter year to maintain a satisfactory schedule with SWFWMD. • Warrington Way Drainage Project – This project was comprised of a bypass drainage system to capture and route water away from the Warrington Way storm sewer system to alleviate flooding problems. Warrington Way has historically flooded due to elevations in the roadway which are too low, allowing standing water when the flood stage in the adjacent wetlands reaches its peak. The drainage project allows for another outfall to the wetland, thus reducing flooding in Warrington Way. • Pond Repairs – This task included ongoing inspection of the ponds and developing plans for specific repairs. These repairs included numerous bank erosions, erosion along the edges of gabions, weir repairs, and flume repairs. JMT also assisted in a project to clear vegetation and obstructions from around wetland structures restoring the free flow of water out of the development. • Grass Carp Research – JMT completed a synthesis of research literature regarding the use of grass carp to control nuisance vegetation in stormwater detention ponds. A report with recommendations was presented to the Board.

JMT also developed a detailed engineering report addressing current and future construction costs for site facilities and provided support for legal issues.

The project services include: ERP application reviews, field inspections, as‐built processing, computer modeling, meetings with District staff and consultants, project evaluation reports including final regulatory determinations, and presentations for informal dispute resolution and formal expert witness testimony. Drew Park experiences frequent flooding that results in property damage, creates access problems to businesses and residences during storm events, and also creates public safety concerns. Flooding has persisted for years due to the lack of capacity in the primary and secondary drainage systems. The initial phase of the project involved hydraulic/hydrologic stormwater modeling to size new primary drainage conveyances to relieve the flooding problem throughout most of Drew Park and also included the development of conceptual roadway improvements for Lois Avenue and Grady Avenue. The recommended improvements included expansion of the existing stormwater retention pond and reconstruction of Grady and Lois Avenues to incorporate new primary drainage systems within the right‐of‐way of each street. Each of these phases is described below:

Drew Park Stormwater Retention Pond Expansion – To prepare for the new conveyance systems and provide adequate stormwater attenuation for the 3‐year storm event, the City’s existing stormwater retention pond was expanded and new inflow structures constructed. This work is complete.

Grady Avenue Right‐of‐Way Improvements – Martin Luther King Boulevard to Crest Avenue: These drainage improvements included concrete box culverts and necessitated the reconstruction of Grady Avenue as an urban typical section with new sidewalks and pedestrian features, limited on‐street parking, and streetscape treatment in accordance with the Drew Park CRA Streetscape and Beautification Master Plan. Significant utility coordination was required, including design of a piggyback sanitary sewer system. Construction of these improvements is complete.

Lois Avenue Right‐of‐Way Improvements – Tampa Bay Boulevard to Hillsborough Avenue: Like Grady Avenue, the construction of a new primary drainage conveyance system along Lois Avenue includes concrete box culverts and necessitates the reconstruction of Lois Avenue, which will be an urban typical section with sidewalks, three new traffic signals, and streetscape treatment in accordance with the Drew Park CRA Streetscape and Beautification Master Plan. Autumn Leaves Drive is located in the Northdale neighborhood of northwest Hillsborough County within Hillsborough County’s Rocky‐Brushy Creek Watershed. Stormwater runoff from Autumn Leaves Drive discharges into an existing retention pond that also receives drainage discharge from approximately 200 acres of drainage area. Runoff fills the pond quickly after storm events and backflows to Autumn Leaves Drive, causing street flooding.

JMT studied the Autumn Leaves neighborhood flooding and evaluated various design alternatives to the Autumn Leaves pond, the pond outfall control structure, or other drainage system elements to improve the roadway flooding conditions and potentially enhance the existing pond. The existing Rocky‐Brushy Creek Watershed Model (HCSWMM) was utilized for the study and updates were made to the model based on recent survey data in the immediate vicinity.

JMT developed stormwater system improvements that accommodate both the preferred higher water level in the pond and an improved level of service of the adjacent roadway systems. The proposed system included upsizing pipes, addition of a 180‐foot overbank weir on the existing pond, and incorporating a backflow prevention device. The Engineer’s Preliminary Estimate of Probable Construction Cost for these improvements was $390,000. JMT also provided post‐design construction inspection services.

LAKE LECLARE DRAINAGE IMPROVEMENTS Hillsborough County 601 East Kennedy Boulevard, Tampa, FL 33619 Project Manager: Robert Wisemen, (813) 307‐1747 Date: 2012

Lake LeClare Road is located in northwest Hillsborough County and has a history of flooding in the vicinity of the intersection with Tobacco Road. The purpose of this stormwater improvement project was to survey, study, and model the existing drainage conditions in order to recommend drainage improvements to minimize or eliminate flooding impacts in the project area and to prepare construction documents for the recommended alternative. The recommended alternative included modifications to the existing conveyance ditch on the south side of Lake LeClare Road as well as drainage structure upgrades at the intersection of Lake LeClare Road and Tobacco Road.

JMT developed drainage alternatives to improve flooding conditions at three south Dale Mabry Highway intersections (Henderson Boulevard, Watrous Avenue, and Neptune Street) and a 65‐acre portion of the Palma Ceia residential area that experience frequent flooding. The drainage improvements were developed to meet the City’s 5‐yr/4‐hr storm event Level of Service (LOS) for roads and the Southwest Florida Water Management District water quantity criteria.

The 1,825‐acre study area is located in the southwest portion of the City. The Henderson Boulevard and Watrous Avenue intersections are located in the Culbreath Bayou basin and the Neptune Street intersection is located in the John’s Branch basin. The Culbreath Bayou and John’s Branch basins both outfall into Old Tampa Bay. The Palma Ceia flood problem area is primarily located in the Bay to Bay basin and outfalls into Hillsborough Bay.

Gravity flow and pumping station (135,000 gpm to 166,000 gpm) alternatives were developed to meet the City’s LOS for the flood problem areas. The gravity flow alternatives developed included open channel improvements, detention ponds, and box culvert and circular pipe systems. The preferred alternative consists of a large pipe and box culvert system that drains the runoff from the three Dale Mabry project intersections and the Palma Ceia project area to Hillsborough Bay.

As District Engineer, Hank Schneider, PE will follow these guidelines to successfully accomplish each assignment:  Identify the right team to efficiently deliver each assignment.  Develop a well‐defined scope.  Keep the same people on the project team for the duration of the task.  Conduct team meetings, typically weekly or bi‐weekly.  Proactively anticipate potential schedule or budget challenges and resolve them in advance.

All JMT project managers are formally trained in JMT’s approach to project management, which is contained in a customized Project Management Manual complete with Standard Operating Procedures (SOPs) and tools for each PM to effectively manage any project through scheduling, budgeting, and quality control. For each project, the PM develops a detailed Project Work Plan that describes the scope and deliverables and details the schedule, milestones, work breakdown structure, task assignments, and Quality Control Plan, including specific responsibilities for each key team member. These SOPs and in‐house PM tools save significant time. Some of the tools used to maintain control of the schedule and budget are described in the next section.

Schedule & Budget Controls Schedule: With our depth of staff and vast array of multi‐disciplined experience, JMT has the capability to fast track any project. This expertise provides the client with a one‐stop shop for any project – whether we are serving as program manager, project manager, design support, or construction manager. We provide a complete array of engineering and management services integrated to our client’s needs.

Between the level of expertise and the personnel available that have previous experience working on similar contracts for other local agencies, we have the capacity and expertise to maintain accelerated task order schedules. Our depth of staff in the required disciplines ensures that each task assignment will be given the manpower necessary to provide the Mira Lago West CDD with a quality end product within the project budget and schedule.

Our schedules are developed in great detail, describing every activity involved in every phase. Moreover, activities are linked so that we are creating a critical path project schedule. This schedule is one of several that our task order manager reviews for compliance on a weekly basis. He will review the level of completeness of every activity with the individuals performing the work and confirm that the project is on schedule.

We have the capacity and expertise to complete all assignments for this project on time. We recognize that a major element used to evaluate the effectiveness of our services is the degree to which schedules are completed. Our project efforts are directed toward a high level of schedule control and our project management policies have been devised to support this objective. Engineering Services Mira Lago West Community Development District 49 Budget: Keeping in mind the CDD’s budget for each assignment, we will develop an appropriate scope to satisfy the budget constraints. Once underway, JMT focuses on cost control using a state‐of‐the‐art integrated accounting and resource planning software and a collaborative team approach. Each project phase is automated using our Microsoft 365 computer software which breaks down the project budget into discrete project phases (e.g., Master Planning, Design, Construction Management, etc.). During the performance of the project we track the hours and dollars expended on each project phase. All employee timesheets are input electronically at least weekly, so up‐to‐date project costs are available on each PM’s dashboard. Solomon generates monthly reports showing hours spent versus budgeted and job costs incurred versus budgeted as well as the monthly invoice. With these tools, consistent, precise budgets are maintained.

For each assignment, our project managers prepare a monthly progress report describing for each phase the percent of work complete, work performed during the report period, status with respect to schedule, unusual problems, delays, approval action, information required, and potential schedule slippage with recommendations.

Project team meetings are held weekly or bi‐weekly to discuss the progress of the project. Each phase is reviewed for progress, manpower allocations, budget, and schedule. Any issues are discussed and a strategy for their resolution is addressed.

Personnel Assignment & Project Efficiency The success of any project depends on utilizing the right personnel – those who possess the needed technical knowledge, successful project experience, specialized equipment, and collaboration skills.

Once notified of an assignment and based on the type and scope of work, Hank will assemble the right project team in terms of team size and expertise, with the objective of keeping the team as small as possible to meet the schedule and keeping the team intact throughout the entire project.

Clear expectations and close collaboration from the start of each assignment will enhance efficiency and minimize delays. JMT’s focus is always on the end result: accurate, concise, and clear deliverables.

We prepared the contract documents, including general conditions, and assisted in the bid phase. We provided construction phase services as well.

Traffic Analyses – JMT reviewed the Signal Warrant Study provided by the Florida Department of Transportation (FDOT) for the interesection of US 41/SR 45 at Spindle Shell Way. Discussions with FDOT and the neighboring development are underway.

SWFWMD Permit Review – JMT reviewed existing SWFWMD permits to determine when pond inspections are required for the development’s various permitted facilities. Inspections were completed and inspection reports were filed with SWFWMD.

Miscellaneous Assistance – JMT provides miscellaneous assistance regarding CDD assets, such as: • Pool paver subsidence • Nuisance flooding around landscaped areas and pool area • Utility valve subsidence • Monitoring progress of County’s plans to extend reclaimed water to the development within the next two years • Night time swimming lighting assessment • Review of improvement or repair proposals by vendors

Data collection, permit evaluation and research as well as hydraulic and hydrologic calculations were specific tasks. The final deliverable was a report addressing probable improvements and cost estimates for Phase I. Phase II consisted of developing construction documents for competitive bid. The design improvements consisted of an energy dissipating structure with sand cement armoring and regrading around the pond bottoms and side slopes.

Fairway Drainage Improvements – Construction plans to provide swale between the fairway and homes to collect and redirect stormwater modified ditch bottom inlets.

Planned Stormwater Improvements and Replacement Analysis – This report provided estimated costs of planned stormwater improvements or replacement of stormwater components for HPCDD. The analysis and evaluations encompassed the existing Villages, roads, and golf course stormwater systems and included each component integral to the stormwater system functionality.

Permit Inventory and Inspection Scheduling – This task required research of SWFWMD permit documents archived at the central SWFWMD Brooksville Office. A map and spreadsheet were developed indicating the numerous permits, where the ponds were located within the community, when the pond inspections were to be completed, and the parcel number for Pasco County.

Pond Maintenance Plan – The objective of this task was to determine the extent of repair needed for each pond, establish an estimated cost, and prioritize each pond based on the functional performance requirements. Inspections were performed for each pond site noting any issue requiring maintenance or repair. Numerous ponds had erosion on the side slopes and several had degraded control structures due to skimmer settlement or foundation/embankment erosion. A report was prepared and presented to the CDD Board for additional action. The report is a planning tool to eventually address all the ponds.

Wall Inspection and Repair – There are numerous walls within the community that provide security and screening. These walls are concrete masonry and stucco with architectural features that mirror the community standards. Before the walls were painted, inspection was required to assure structural soundness and functional capability since cracks and settlement were noticed. The walls were inspected and proposals for repairs were reviewed and approved by the engineer.

Aptitude Terrace – This task included the design of rear-yard drainage systems to intercept runoff that was causing erosion along a retention pond bank. This pond had experienced chronic erosion problems over the years. This design will alleviate those problems.

ADA Facility Compliance – JMT developed a list of facility improvements to bring the CDD into compliance with ADA requirements. We have developed plans for the first set of new sidewalks to be constructed in the next few months.

Reserve Study – JMT has prepared two updates to the CDD’s Reserve Study.

Court Resurfacing – JMT prepared plans for the resurfacing of two tennis courts and one basketball court.

Pool Heating Analysis – JMT prepared a comparative analysis of heating the District’s pool using natural gas or propane.

Hole #17 Regrading – This project involved the design of the regrading of an area of the golf course which had been filled partially blocking the free flow of water over a 250 ft. weir. This design was a compromise between the need to move the water during flood conditions and the need for a dry landing area for golfers.

Pond Inspections and Report Writing – The JMT team conducts inspections and writes reports as part of a professional engineering services as-needed basis assignment. There are 39 water management facilities located in the HSCDD requiring inspection and certification of compliance for SWFWMD. JMT was tasked to research the files at SWFWMD to establish a list of facilities and to prioritize the need for inspection and certification. Currently, JMT is conducting several inspections per quarter year to maintain a satisfactory schedule with SWFWMD.

Warrington Way Drainage Project – This project was comprised of a bypass drainage system to capture and route water away from the Warrington Way storm sewer system to alleviate flooding problems. Warrington Way has historically flooded due to elevations in the roadway which are too low, allowing standing water when the flood stage in the adjacent wetlands reaches its peak. The drainage project allows for another outfall to the wetland, thus reducing flooding in Warrington Way.

Pond Repairs – This task included ongoing inspection of the ponds and developing plans for specific repairs. These repairs included numerous bank erosions, erosion along the edges of gabions, weir repairs, and flume repairs. JMT also assisted in a project to clear vegetation and obstructions from around wetland structures restoring the free flow of water out of the development.

POINT OF CONTACT TELEPHONE NUMBER City of Tampa Yvette Pullara (813) 274-8092 Drew Park experiences frequent flooding that results in property damage, creates access problems to businesses and residences during storm events, and also creates public safety concerns. Flooding has persisted for years due to the lack of capacity in the primary and secondary drainage systems. The initial phase of the project involved hydraulic/hydrologic stormwater modeling to size new primary drainage conveyances to relieve the flooding problem throughout most of Drew Park and also included the development of conceptual roadway improvements for Lois Avenue and Grady Avenue. The recommended improvements included expansion of the existing stormwater retention pond, and reconstruction of Grady and Lois Avenues to incorporate new primary drainage systems within the right-of-way of each street. Each of these phases is described below:

Drew Park Stormwater Retention Pond Expansion – To prepare for the new conveyance systems and provide adequate stormwater attenuation for the 3-year storm event, the City’s existing stormwater retention pond was expanded and new inflow structures constructed. This work is complete.

Grady Avenue Right-of-Way Improvements – Martin Luther King Boulevard to Crest Avenue: These drainage improvements included concrete box culverts and necessitated the reconstruction of Grady Avenue as an urban typical section with new sidewalks and pedestrian features, limited on-street parking, and streetscape treatment in accordance with the Drew Park CRA Streetscape and Beautification Master Plan. Significant utility coordination was required, including design of a piggyback sanitary sewer system. Construction of these improvements is complete.

Lois Avenue Right-of-Way Improvements – Tampa Bay Boulevard to Hillsborough Avenue: Like Grady Avenue, the construction of a new primary drainage conveyance system along Lois Avenue included concrete box culverts and necessitated the reconstruction of Lois Avenue, as an urban typical section with sidewalks, three new traffic signals, and streetscape treatment in accordance with the Drew Park CRA Streetscape and Beautification Master Plan. Construction of these improvements is complete.

Autumn Leaves Drive is located in the Northdale neighborhood of northwest Hillsborough County within Hillsborough County’s Rocky-Brushy Creek Watershed. Stormwater runoff from Autumn Leaves Drive discharges into an existing retention pond that also receives drainage discharge from approximately 200 acres of drainage area. Runoff fills the pond quickly after storm events and backflows to Autumn Leaves Drive, causing street flooding.

JMT studied the Autumn Leaves neighborhood flooding and evaluated various design alternatives to the Autumn Leaves pond, the pond outfall control structure, or other drainage system elements to improve the roadway flooding conditions and potentially enhance the existing pond. The existing Rocky-Brushy Creek Watershed Model (HCSWMM) was utilized for the study and updates were made to the model based on recent survey data in the immediate vicinity.

JMT developed stormwater system improvements that accommodate both the preferred higher water level in the pond and an improved level of service of the adjacent roadway systems. The proposed system included upsizing pipes, addition of a 180 foot overbank weir on the existing pond, and incorporating a backflow prevention device. The Engineer’s Preliminary Estimate of Probable Construction Cost for these improvements was $390,000. JMT also provided post-design construction inspection services.

PARTICIPATION AGREEMENT Application for Membership in the Florida Insurance Alliance

The undersigned local governmental entity, certifying itself to be a public agency of the State of Florida as defined in Section 163.01, Florida Statutes, hereby formally makes application with the Florida Insurance Alliance (“FIA”) for continuing liability and/or casualty coverage through membership in FIA, to become effective 12:01 a.m., 10/01/2017, and if accepted by the FIA’s duly authorized representative, does hereby agree as follows:

(a) That, by this reference, the terms and provisions of the Interlocal Agreement creating the Florida Insurance Alliance are hereby adopted, approved and ratified by the undersigned local governmental entity. The undersigned local governmental entity certifies that it has received a copy of the aforementioned Interlocal Agreement and further agrees to be bound by the provisions and obligations of the Interlocal Agreement as provided therein;

(b) To pay all premiums on or before the date the same shall become due and, in the event Applicant fails to do so, to pay any reasonable late penalties and charges arising therefrom, and all costs of collection thereof, including reasonable attorneys’ fees;

(c) To abide by the rules and regulations adopted by the Board of Directors;

(d) That should either the Applicant or the Fund desire to cancel coverage; it will give not less than thirty (30) days prior written notice of cancellation;

(e) That all information contained in the underwriting application provided to FIA as a condition precedent to participation in FIA is true, correct and accurate in all respects.

The coverage is subject to 100% minimum earned premium as of the first day of the “Coverage Period”. 3. Total premium is late if not paid in full within 30 days of inception, unless otherwise stated. 4. Property designated as being within Flood Zone A or V (and any prefixes or suffixes thereof) by the Federal Emergency Management Agency (FEMA), or within a 100 Year Flood Plain as designated by the United States Army Corps of Engineers, will have a Special Flood Deductible equal to all flood insurance available for such property under the National Flood Insurance Program, whether purchased or not or 5% of the Total Insured Value at each affected location whichever the greater. 5. The Florida Insurance Alliance is a shared limit. The limits purchased are a per occurrence limit and in the event an occurrence exhaust the limit purchased by the Alliance on behalf of the members, payment to you for a covered loss will be reduced pro-rata based on the amounts of covered loss by all members affected by the occurrence. MIRA LAGO WEST COMMUNITY DEVELOPMENT DISTRICT

The regular meeting of the Board of Supervisors of the Mira Lago West Community Development District was held on Monday, August 7, 2017 at 7:01 p.m. at the SouthShore Regional Library, located at 15816 Beth Shields Way, Ruskin, Florida 33573.

Present and constituting a quorum:

Lennie Fine Board Supervisor, Chairman Hector Ortiz Board Supervisor, Vice Chairman Sam Schmieder Board Supervisor, Assistant Secretary Jennifer Parra Board Supervisor, Assistant Secretary Robert McCarthy Board Supervisor, Assistant Secretary

FIRST ORDER OF BUSINESS Call to Order

Mr. Phillips called the meeting to order and read the roll call.

SECOND ORDER OF BUSINESS Audience Comments

Mrs. Butts commented on the timer of the lights on the fountain behind her home. Mr. Fine addressed her concerns. The Board directed staff to work with the HOA to send out an email survey to residents to determine the best times to schedule fountain lighting.

THIRD ORDER OF BUSINESS Staff Reports

A. Aquatic Maintenance Report The Board directed District Staff to notify both the aquatic and landscape vendors to remove trash when on site.

B. Landscape Report The Board reiterated their desire for the trash issue to be addressed.

C. District Counsel Mr. Steady outlined his report and answered general questions from the Board.

Mr. Fine and Mr. McCarthy addressed the issues pertaining to the District Engineer and discussion ensued.

On a Motion by Mr. Fine, seconded by Mr. Ortiz, with 3 in favor and 1 opposed, the Board of Supervisors directed District Staff to file a claim and seek arbitration from the District Engineer for the Mira Lago West Community Development District.

D. District Engineer The Board chose to table the consideration of new District Engineer candidates to the next meeting to allow for further review.

E. District Manager Mr. Phillips stated that the next regularly scheduled meeting will be held on October 2, 2017 at 7:00 p.m. at the SouthShore Regional Library, located at 15816 Beth Shields Way, Ruskin, Florida 33573.

Mr. Phillips presented and reviewed the unaudited financial statement for July 2017 and general discussion ensued.

FOURTH ORDER OF BUSINESS Consideration of the Minutes of the Board of Supervisors’ Regular Meeting held on June 5, 2017

On a Motion by Mr. Schmieder, seconded by Ms. Parra, with all in favor, the Board of Supervisors approved the June 5, 2017 minutes as presented for the Mira Lago West Community Development District.

FIFTH ORDER OF BUSINESS Consideration of Operation & Maintenance Expenditures for May through June 2017

On a Motion by Mr. Ortiz, seconded by Mr. Fine, with all in favor, the Board of Supervisors approved the Operation & Maintenance Expenditures for May 2017 ($8,653.91) through June 2017 ($12,825.50) as presented for the Mira Lago West Community Development District.

SIXTH ORDER OF BUSINESS Public Hearing on Fiscal Year 2017/2018 Budget

On a Motion by Mr. Ortiz, seconded by Mr. Fine, with all in favor, the Board of Supervisors opened the Public Hearing on the Fiscal Year 2017/2018 Budget for the Mira Lago West Community Development District.

Mr. Phillips presented the Fiscal Year 2017/2018 Budget to the Board for consideration and general discussion ensued. The Board made no changes and accepted the budget as presented.

On a Motion by Mr. Ortiz, seconded by Mr. Fine, with all in favor, the Board of Supervisors closed the Public Hearing on the Fiscal Year 2017/2018 Budget for the Mira Lago West Community Development District.

SEVENTH ORDER OF BUSINESS Consideration of Resolution 2017-09, Adopting Fiscal Year 2017/2018 Budget

A member of the Audience inquired about a landscaping issue and the Board addressed their concern.

On a Motion by Mr. Fine, seconded by Mr. Ortiz, with all in favor, the Board of Supervisors adopted Resolution 2017-09, Adopting Fiscal Year 2017/2018 Budget, for the Mira Lago West Community Development District.

EIGHTH ORDER OF BUSINESS Consideration of Resolution 2017-10, Imposing Special Assessments & Certifying Assessment Roll

On a Motion by Mr. McCarthy, seconded by Mr. Schmieder, with all in favor, the Board of Supervisors adopted Resolution 2017-10, Imposing Special Assessments & Certifying Assessment Roll, for the Mira Lago West Community Development District.

NINTH ORDER OF BUSINESS Consideration of Resolution 2017-11, Adopting Fiscal Year 2017/2018 Meeting Schedule

On a Motion by Mr. Ortiz, seconded by Mr. Fine, with all in favor, the Board of Supervisors adopted Resolution 2017-11, Adopting Fiscal Year 2017/2018 Meeting Schedule, for the Mira Lago West Community Development District.

TENTH ORDER OF BUSINESS Consideration of Resolution 2017-12, Designating Primary Administrative Office & Headquarters of the District

On a Motion by Mr. Ortiz, seconded by Ms. Parra, with all in favor, the Board of Supervisors adopted Resolution 2017-12, Designating Primary Administrative Office & Headquarters of the District, for the Mira Lago West Community Development District.

ELEVENTH ORDER OF BUSINESS Consideration of Resolution 2017-13, Adopting Alternative Investment Guidelines Mr. Huber presented Resolution 2017-13 to the Board and outlined its contents and purpose.

On a Motion by Mr. Ortiz, seconded by Mr. McCarthy, with all in favor, the Board of Supervisors adopted Resolution 2017-13, Adopting Alternative Investment Guidelines, for the Mira Lago West Community Development District.

TWELFTH ORDER OF BUSINESS Discussion Regarding Pond 8 Restoration

Mr. Phillips presented a proposal from Site Masters for the work to be done to Pond 8 and discussion ensued.

On a Motion by Mr. McCarthy, seconded by Mr. Fine, with all in favor, the Board of Supervisors approved the proposal from Site Masters for the restoration of Pond 8 ($1,250.00) for the Mira Lago West Community Development District.

THIRTEENTH ORDER OF BUSINESS Supervisor Requests

Mr. Ortiz spoke about issues he was having with accessing his CDD email and requested that emails pertaining to Mira Lago West CDD be sent to his personal email account.

FOURTEENTH ORDER OF BUSINESS Adjournment

On a Motion by Mr. McCarthy, seconded by Ms. Parra, with all in favor, the Board of Supervisors adjourned the meeting at 8:27 p.m. for the Mira Lago West Community Development District.
