Introduction

The main purpose of this publication is to provide an introduction and overview of external investments in the Health Sector in Tajikistan. This document should promote further dialogue among stakeholders regarding current patterns of investment, including the alignment of those investments with the existing goals of poverty reduction and other development strategies.

The goal of the publication is to inform the decision making process of all those working in and supporting the Health Sector.

The objective of the publication is to facilitate the coordination process and to enhance collaboration and sharing of information within the Health Sector to improve decision making as stated in the Paris Declaration 1.

This project is led by WHO Country Office in Tajikistan as part of its mandate to advance the global agenda on health as well as directly contribute to national strategies and priorities. In addition, the project aims to bring the country’s realities and perspectives into a more global focus. The completion of the publication could not have been possible without the contribution of the donors and other international agencies working in Tajikistan and the commitment of the Government of Tajikistan in particular, as well as the Project Implementation Unit for the Community and Basic Health Project and the Department of Reforms and International Relations of the Ministry of Health.

Although this document is similar to other WHO publications that have successfully documented Health Sector investment in other countries, it is the first general overview describing externally funded projects in the Health Sector in Tajikistan. It is the present intention of the WHO Country Office in Tajikistan to provide updated publications to policy-makers on an annual basis.

The measure of success of this publication will be its appreciation and its use by the National and International Communities supporting the health reform process of Tajikistan.

Government and policy-makers are increasingly aware that access to knowledge enables better policy-making decisions.

Policy-makers at the national, regional, international level similarly acknowledge that they face common challenges and can learn and build on the experiences of each other. To improve the understanding and communication of these policy- makers, international technical assistance projects are detailed in this Health Sector inventory.

The challenge for any country experiencing health reform is to ensure that communication occurs among all its partners. This publication is there to meet and support that challenge by assisting the Government of Tajikistan in disseminating vital information about its Health Sector interventions to its partner organizations.

In Tajikistan health care funds are budgeted in a fragmented way, at the oblast , rayon and jamoat levels 2 . The problem is compounded by a lack of human resources in the Offices of the Ministry of Health. Complexities in the funding structure thus mirror complexities in the sector itself, often resulting in the fragmentation of externally funded projects.

Because of this fact, it may be more advantageous for donors to raise and spend aid “vertically” in order to show a direct link between monies and results. While this is an issue in all sectors, the consequences are particularly acute in health as this sector requires flexible resources that can be used to support recurrent costs and health systems. Although many donors may recognize the need to provide flexible funding to support country-owned health reform plans, the general concerns about public sector management and governance, particularly states in transition like Tajikistan, may make them reluctant to do so.

For the past 3 years, health issues in Tajikistan have been at the forefront of the debate on what aid effectiveness means from a sector perspective. Currently the idea of Sector Wide Approach (SWAp) is becoming increasingly popular. The SWAp system implies a unique strategy for the sector, jointly agreed upon by both donors and government. It is aimed to foster ownership, improve donor harmonization and aid predictability, and align policy behind a health-reform programme that is mutually agreed by each stakeholder.

The current publication aims to give an overview of the externally funded projects and the alignment of national strategies developed for the health sector in Tajikistan.

Methodology

The methodology used to compile information on all projects in the Tajik health sector funded by external donors included: • investigations on agencies committed to the Health Sector; • interviews and double checks; • questionnaires; • analysis of the data.

Interviews

WHO Country Office in Tajikistan conducted this research largely through donor input rather than through the implementing agencies. Three reasons supported this approach. First, it was our aim to provide a tool to facilitate the decision making process at donor level where a general overview of the Health Sector is essential. Second, donors tend to have longer planning periods and maintain a more consistent record of financial information. And finally, the varied nature of the implementing agencies would have produced high discrepancies in the way the information would have been provided (e.g. administrative cost and co- funding sources).

A list of donors working in the Tajik Health sector was compiled using three main sources: 1. The “ Foreign Aid Report ” and the “ Development Partner Profiles ” of the State Committee on Investment and State Property Management of the Republic of Tajikistan, supported by Asian Development Bank (Dushanbe, 2007); 2. Previous assessments done by the Ministry of Health of the Republic of Tajikistan in collaboration with the Health Policy Analysis Unit 3 (HPAU) and the World Bank’s PIU 4 in the Ministry of Health, Swiss Agency for Development and Cooperation (SDC) and Swedish International Development Cooperation Agency (SIDA). Both units were created in the frame of the Community and Basic Health Project ; 3. The “ Principals’ Group in Tajikistan ”5.

Agencies like the International Monetary Fund were also consulted. Furthermore, when information received from the original list was not coherent or complete, the implementing agency was also contacted.

A complete list of agencies contacted and/or interviewed can be found in Annex I. Meetings were held vis-à-vis between September and November 2007 where the aim of research as well as the questionnaire were presented.

Questionnaire

The WHO Country Office in Tajikistan prepared a questionnaire to collect necessary information on each of the projects selected in the health sector in Tajikistan. The collated information summarizes the needs of national and international implementing agencies.

The questionnaire can be divided in three parts .

The first part of the questionnaire is intended to delineate each implemented project, in part or full during 2006. It gives a quick overview of the donor, the implementing partner and/or counterparts, and the main area(s) of intervention. This type of information will prompt potential donors on the expertise already in the field and will thus encourage aid coordination.

The second part of the questionnaire provides a description of the ideas, approaches, purposes and activities of the project. These summaries were included to help donors better understand their field of concern and the techniques used to address those issues. The gathering of this information in this way should help support transparency among international and national agencies, including the storing and sharing of cumulative knowledge.

The third part of the questionnaire compiles information on the “components” and “areas of work” of each project. A “component” is the project input, an “area of work,” the major domain to which a project is committed. This information also documents, under commonly defined categories, the activities of agencies that collaborate with WHO Country Office Tajikistan. This data gathering system helps to avoid confusion as well as the duplication of information collected and reported from other sources. The data should provide a tool for decision-makers to mainstream future aid where need is still underestimated 6.

The detailed questionnaire, with explanation, is listed in Annex II. The information regarding each project reflects the data as it was provided by the agencies contacted, without addings or modifications.

Analysis and timeframe

An inventory regarding all projects in the Tajik Health Sector during 2006 was compiled and a database was created. The database provided information on the number of project and donors in the country, number of projects per donor, type of funds, nature and length of the projects, area of work and tools utilized by the agencies to carry out their objectives, and the outcomes on the respective districts and regions.

The duration of projects surveyed in 2006 varied considerably, with the oldest project beginning in 1999, and the most recent ending in 2010. Together, all projects span more than 10 years. There are other projects running concurrently in the Health Sector during this time period (1999-2010). However, the analysis is focused only on those that were either approved, started, or completed in 2006 or, if they overrun that year . The research was based on a calendar year rather than a fiscal year, due to the fact that there are significant differences between fiscal years among donors.

Data were analyzed through various interpretative keys : financial nature of the support, beneficiary areas of work, type of components chosen to realize the objectives, classification of the project, funds for development aid or funds for humanitarian aid, time range of the projects, and geographical coverage. Subgroups of projects were then compared among themselves to better determine project balance, displacement, and trends.

As the financial information was provided for the total duration of the project, an estimation of funds allocated yearly was calculated assuming a constant disbursement along the duration of the project. Similarly, it was assumed that there was an equal distribution of funds in all geographic areas. Finally, the same method was employed to calculate the disbursement for the project components in each area.

The source of our data on population is from the Centre of Medical Statistic and Information-Ministry of Health, in their publication titled “Health of Population and Health Care in Tajikistan” 7.

Geographic coverage and maps

The maps of the oblast level and of the district level (with relative borders) have been developed through a Geographic Information System.

Page -13- The area of the national territory is enforced at the official district level and consists of 58 districts which have a different administrative position, and are considered as a part of the surrounding districts either in terms of territory or in terms of population. The following adaptations had to be made: • Vaksh district includes Qurghan- Teppa city • district includes Kulob city • Shugnon district includes Khorog city • Gafurov district includes cities of Khudjand, Tavashar, Kairakum, and Chkalovsk .

The only exception is the capital, Dushanbe, which for reasons of clarity has been elevated from the city level to the district level and considered the 59th district. In the adapted cases, the borders between the city and the countryside are minimal, while the capital (from an internal political perspective and from the donors’ one) is considered an absolute entity. The alternative would be to consider Dushanbe as part of Rudaki or districts which would have given a distorted perception of the real aid provided to those districts.

The amount per district has therefore been calculated as the sum of the average quotas of the total amounts of each project involving the district in analysis. We considered both grants and loans.

Alignment of strategies

Finally, the alignments of strategies for the health sector in Tajikistan were analyzed for their respective years. The strategies considered were: the National Strategy for Health Sector for the year 2000-2010, the first Poverty Reduction Strategy for years 2002-2005, the National Development Strategy (NDS) for 2006 -2015, and the second Poverty Reduction Strategy for years 2007-2009. These strategies were also compared with the Millennium Development Goals.

The first Poverty Reduction Strategies (2002-2005) were included as it directly influenced many of the projects executed in 2006. Although the second Poverty Reduction Strategy (2007-2009) was not published until 2007, most of the agencies were contributing towards it since 2005. These agencies anticipated NDS and PRSP II in the new aligned approach, harmonizing their own work-plans to the MDGs.

Results

97 externally funded projects were identified for inclusion in this report. These projects in total (in the time rage shown in figure 3) bring a financial contribution to the Tajik Health Sector of 136 002 353 US dollars for the entire period of analysis.

According to the type of funds , 95 projects were grants, meaning 87% of the financial contributions. Two other projects were loans amounting to 13% of the contribution.

According to the project’s aim , 97% of the funds were allocated in 89 projects for development, while 8 projects with Humanitarian Aid accounted for 3% of the total funds.

The time range of the 97 projects analyzed is illustrated in Fig 3. The criteria for inclusion of a project was that at least some of the activities should have taken place during 2006. The projects could have started before or during 2006; and/or ended during or after 2006. This table summarizes all projects taking place in the Tajik Health Sector in the year 2006, including some projects implemented some years before or will continue for some years afterwards.

All projects were assigned to one or more of the five areas of work : Primary Health Care, Hospitals and Secondary Health Care, Community Health, Public Health and Institution Building.

Similarly, each project could have one or more of the following components : Technical Assistance, Training, Civil Work, Equipment, Drugs & Supplies, or Other.

Table 1 shows a summary of the number of projects and budget allocated for each component in each area or work.

Figure 4 shows the percentage spent in each area of work out of the total amount of 136 002 353 USD.

District level analysis

Out of the 97 projects, 49 projects were implemented at the national level, while 58 were focused at the district level. Ten projects were executed at both the national and district level.

Out of the 58 district projects, information on oblasts affected was obtained for 52 projects, and about rayon involve-ment on 41 projects.

Maps with allocation of funds for each oblast and rayon are presented below. A color scale reflects the amount of funds allocated to the geographical area. The color scale changes from one map to the other to better underline the differences between the districts.

Page -31- Analysis of strategies for the health sector

The National Development Strategy and the second Poverty Reduction Strategy are aligned with the Millennium Development Goals. The National Strategy for the Health Sector, as well as the first Poverty Reduction Strategy, were conceived before Tajikistan was chosen as a Millennium Project pilot country and do not refer explicitly to MDGs.

The base of the current strategy for the Health Sector was present seven years before the National Strategy on Health Care was developed. An improvement in the strategies along the years can also be noticed.

Millennium Development Goals

The crucial value of the health sector is underlined by the Millennium Development Goals (MDGs) approved in the UN Millennium Summit on September 2000. Although health was not considered a defined outcome in the MDGs, health itself was the target for three out of eight goals (MDG4, MDG5 and MDG6). Hence, health represents the biggest beneficiary of these efforts’ relapses, been seen as a prerequisite or a key factor for achieving other outputs. MDG1, MDG2, MDG7 are millennium goals based on indispensable health achievements/backings or having health related relapses.

National Strategy for Health Sector

The National Strategy for Health Sector 8 aimed to draw goals and priorities at national level, providing an action plan for the next 10 years, that is, until 2010. The paper was written by the Ministry of Health in collaboration with WHO in the year 2000. A previous version was published in 1997 but was not included for this analysis. This document was based on the renewed policy for WHO-Europe “Health for All in 21st Century”, accepted by World Health Assembly in May 1998.

This strategy has a horizontal approach with some vertical spotlights or objectives like infectious diseases, non-infectious diseases and mental health. This approach is revealed by the large attention devoted to the environment, the life style and the welfare expected outcomes.

As outlined, the fight against infectious and non-infectious disease is a worldwide concern and following NDS and PRS would also be addressed in Tajikistan. There is no matching currently for the mental health issue.

Poverty Reduction Strategy I (2002-2005)

The first Poverty Reduction Strategy Paper of the Government of the Republic of Tajikistan (Dushanbe, June 2002) was designed to cover the period 2002-2005. Health care is considered in this document as one of the national priorities to support the poverty reduction. Although the content is narrative and the priorities are not literally pointed out, the expected outputs are: 1. Development of a Primary Health Care system 2. Improvement of the quality of care as a means to provide greater access for the poor 3. Strengthening Public Health services 4. Improving medical statistics

What is evident from this chart is that a large portion is dedicated to the decentralization of the medical system and its financial sustainability. There are no matches on the maternal care as reported in the National Strategy for the Health Sector, and no vertical programmes are outlined.

National Development Strategy

The “ National Development Strategy for the Republic of Tajikistan for the Period to 2015 ” of the Government of Tajikistan (Dushanbe, August 2006) was recently edited with the purpose of balancing the national development plans to global

matters of concern as detailed in the MDG. The four long term priorities defined by NDS in the health sector are:

1. “Reform of the health care system, including development of the private sector and attraction of investment; 2. Improvement of maternal and child health 3. A significant slowdown in the spread of HIV/AIDS, a reduction in infectious diseases and the eradication of certain infections that can be controlled by vaccination; 4. Improved availability, quality and effectiveness of medical services.” 9

The second priority reflects MDG4 and MDG5 and the third priority covers MDG6. The first and the fourth priorities address structural and endemic problems tidily linked to internal problems of Tajikistan, thus having no correspondence with UN- MDG.

Poverty Reduction Strategy II (2007-2009)

Based of the long term priorities of the NDS, the Government of Tajikistan proposed a new medium term strategy, the Poverty Reduction Strategy Paper for 2007-2009 (Dushanbe, September 2006). This strategy defines four medium-term priorities for the health sector: 1. Implement health care system reforms emphasizing improvements in funding and administration mechanisms, increased performance of the primary health care system, and promoting private sector participation; 2. Improve medical services for mothers and children and reduce maternal and child mortality rates; 3. Combat the spread of HIV/AIDS, reduce the rates of infectious diseases and of certain vaccine-controlled diseases; 4. Improve the human capacity of medical staff, and provide better materials and equipment for medical institutions; ensure the availability of sufficient amounts of quality medicines.

These medium term priorities of PRS follow the long term ones of the National Development Strategy adding some monitoring indicators and some coherent objectives in the way of achieving those outputs.

Conclusions

The current inventory, in line with the purposes of Joint Country Support Strategy,10 in theory, aims to help donor agencies plan future programmes and to better coordinate their interventions. This information is the first attempt to have a general overview of the foreign financed projects in the Tajik Health Sector.

The SWAp system outlines a unique strategy for the sector, engaging both donors and government. This can help the international community to prioritize the goals and avoid overlapping and incoherence, and the government to receive important input from donor agencies. The SWAp also encourages a joint system (and procedures) for financing the sector, and a joint structure to monitor the flow of funds. This harmonization will provide more strength and authority to the national authorities committed to such duties, and finally, it will facilitate external support by creating, for the donor agencies, a more transparent method of aid contribution.

SWAp experiences throughout the world range from complete failure to absolute success. Therefore the WHO positions the SWAp as an “approach”, not a programme or an ultimate goal of all sectoral activities. The SWAp approach does, however, present itself as one of the logical ways to align national and international support in order to strengthen the healthcare system and develop a long- term strategy to maintain successful results.

Tajikistan would have to make a concerted effort to create appropriate conditions to introduce SWAp (such as develop a Comprehensive Health Sector Strategy), improve planning and budgeting processes, and strengthen institutional structures and systems. Common monitoring and review processes should be in place and the Health Management Information System strengthened. Government oversight on how the majority of these resources are distributed across the sector is also essential.

The current publication could be used as a starting point for future analysis of the externally funded aid in the health sector in Tajikistan.

All GAVI-eligible countries can receive ISS. ISS is no longer limited to countries with DTP3 coverage below 80%. GAVI support will last the duration of the updated comprehensive Multi-Year Plan for Immunisation (cMYP) which should be synchronised with the national planning cycle of the Ministry of Health. Countries can re-apply for ISS support in phase 2 and as often as a new cMYP is developed. The cMYP should be submitted with every application for ISS support.

Short description of project activities: The program supported with this Global Fund grant directly benefits migrants (and their families), prisoners and street children, as well as people living with HIV. The program provides these vulnerable populations with better access to voluntary counseling and testing services and scales up awareness-raising activities among them. It provides these target groups with improved access to health facilities providing services for the prevention and treatment of HIV and other sexually transmitted infections. In addition, the program provides prisoners with the means to prevent HIV, including education, condoms and a pilot needle exchange project in one prison. This grant also funds antiretroviral therapy, treatment for opportunistic infections, improved HIV diagnostics and monitoring, and strengthened national capacity in the provision of treatment, care and support for people living with HIV.

Short description of project activities: 1. AAR procured 2 Tractors (MTZ-82), 2 potato planting machines (L-201), 2 plows (PLN-3.35) for the Federations. But, it was delay due to train trouble and increase of transportation cost. 2. Although the Dushanbe headquarters of the Federation promised to provide seed potato for the projects, the headquarters broke the promise. So, AAR procured seed potato, 9.3 ton for the Federation in Jirgatal and 7.7 ton for the Federation in Tajikabad, providing chemical fertilizer (N 2.2 ton, P 1.2 ton, K 0.65 ton), pesticide 2 liters, and tractor fuel 690 liters for each Federation. Due to low quality of seed potato and delay of planting, there was little land to plant, so AAR bought “planted potato” in farmlands. 3. Each Federation harvested potato and provide 50 disabled people with potato, after reserving for workers.

Comments, if any: This unique project targets at providing disabled people with income-generating assets like tractors, based on the “Capability Approach” conceptual framework developed by Amartya Sen WHO was awarded the Nobel Prize for Economics. During the follow-up and evaluation process, in the meantime attention should be paid on distribution of income, agricultural management & practice skills and management style of the Federations. At the result of 1st year, the harvest was bad, so the Federations could not distribute potatoes, but keep potatoes for next year.

To organise and conduct a 2-day refresher training for the service providers of the project clinics in the two selected regions. Based on the results of the monitoring visits 2-day refresher training will be organised in each project clinic. Activities for objective 2: 1. To organize a meeting with community leaders in Shartuz region TFPA project coordinator and a representative (trainer) of the National Reproductive Health Centre will have an individual meeting with the community leaders in two target communities of the Shartuz region to identify the most active women in the communities. 2. To organise and conduct an extended meeting with community women in Shartuz and Kabadiyan to select outreach workers. Using the contacts provided by the community leaders the project coordinator will visit these women and request them to identify and bring other active women WHO might be interested to participate in the project to a joint meeting. 3. To prepare/review Information Education and Communication (IEC) materials, report cards, monitoring forms, manual on household visits, baseline. To prepare the necessary documents and materials required for a successful implementation of the community based activities. This will include amongst others the translation and reprinting of existing IEC materials into Tajik, development of report card and monitoring forms, to develop a questionnaire for the baseline study and to prepare copies of the existing guidelines for the household visits conducted by the outreach workers. 4. To organise and conduct initial five day training and a 3-day refresher raining for selected 25 identified outreach workers. Initial training The focus of the training will be on communication skills, SRHR issues, available SRHR services, guidelines to conduct the baseline survey, report/monitoring forms, work planning and to agree on the timeline for collection of the feedback 5. The 3-day refresher training will refresh the information disseminated during the initial training and will address any other issue raised by outreach workers following their experiences during the household visits or issues that were identified by TFPA staff during the monitoring visits. 6. Monitoring of the outreach workers by TFPA staff. Following the initial training for the outreach workers TFPA staff will monitor the activities (household visits) of the outreach workers. On the basis of outcomes of Knowledge, Attitude and Practice analysis provided by Peer education project in 2002 three round tables (one in each project region in rural areas of Khatlon Oblast) will be organized. In each meeting will be invited 25 participants from National Women and family affairs Committees, Youth Committee representatives, expert from Tajik Islamic Institute, TFPA staff and volunteers. The purpose of these meetings is to exchange and achieve a common idea on problems and barriers of providing SRHR information and youth education. 2. To conduct 2 focus groups (by force of active branch volunteers) on discussion attitude of parents and religious youth on SRHR of young people issues in three selected project regions. TFPA active volunteers will hold the given activity in target regions with the aim of assessing the level of knowledge and parents attitude to youth SRHR issues. 3. Information and educational activities for young people will be organized next project (2007) year, if parents and religious leaders will support project success. 4. To develop youth Website. Creation of website focus on building information and educational resource on SRHR issues of teenagers and youth including on line counseling. 5. Round table on creation youth website resource. The aim of conducting round table is establishing cooperation with Governmental and International Organizations on creation youth website resources. In 2006, the TFPA project coordinator and executive director will conduct 4 monitoring visits to each of the counseling centers. In addition to the monitoring activity; a short on-job refresher training will be provided to the service providers. The first refresher training will focus on counseling skills. The following topics of the trainings will be identified through a short questionnaire to assess the service providers' needs during the first visit in March 2006. 2. To organize and conduct 2 focus group discussions with labour migrants in the Kuylab area. TFPA staff will conduct 2 focus group discussion with labour migrants from the project region Kulyob to identify barriers to access STD/HIV/AIDS and SRHR information as well as their preferences on receiving the information (how, where,.). 3. To organize and conduct 2 focus group discussions with migrant workers on existing Information Education and Communication (IEC) materials on STD/HIV/AIDS. TFPA staff will organize and conduct two focus group discussions with labour migrants in the project region to test and discuss existing IEC materials on STD/HIV/AIDS. 4. To develop a plan of action to improve access to STD/HIV/AIDS information. Based on the outputs of the 2006 activities in this project, TFPA will develop a plan of action to address the barriers to access information on STD/HIV/AIDS and SRHR for labour migrants. 5. Condom distribution in the project counseling centers in Kulyab area. The national HIV/AIDS Center will provide the project with free condoms to be distributed at the project pilot counseling centers in the departure areas of the Kulyab airport and railway station. In order to receive the condoms TFPA will make an agreement with the national AIDS Center.

The purpose of the meeting is to present the results of the report on youth SRHR and discuss recommendations, to review national strategic plan on reproductive health and the national reproductive rights law in relation to youth SRHR and to discuss the reproductive law implementation in the light of youth reproductive rights. 2. Preparation of analytical report on youth SRHR in the country. The Association will prepare the report. The Association will collect information from following agencies: National Center on Medical Statistics, National Reproductive Health Center, Institute of Ob/Gyn and Pediatrics, Ministry of Health and Ministry of Education. The MA staff will make analysis. 3. Round table meeting on youth SRHR at the regional level. The round table meetings will be conducted in each of three selected regions by the Association staff. The purpose of the meeting is to improve understanding on youth SRHR among local juridicial, health, education, religion and security representatives and to establish a mechanism on ensuring reproductive rights for youth in the region. The report on youth SRHR, results of the National Advocacy Meeting and National SP on Reproductive Health and RR Law will be presented during the meeting.

This is highly needed in a context in which health can become a source of inter- govt tension. • Increase in Knowledge of rural populations on the symptoms, transmission and prophylaxis of the most relevant diseases, verified through monthly Knowledge, Attitude and Practice surveys • Increase link between authorities and health civil society structures in the target areas through exchange information and the creation of structures of dialogues in every jamoat of intervention. • As a result of one epidemiological (in 8 villages) and two parasitological (in 2 villages) studies recommendations have been submitted to the health authority and international organizations on optimum methods to fight against the mosquitoes in UZB cross border areas (Andijon and Farg'ona). • Three surveys on health awareness have been conducted; one before the CHWs begin health education activities and the others are yearly to assess the impact of ACTED health activities. • Monthly Knowledge, Attitudes and Practice surveys have been conducted to monitor the progress of those receiving health education. • 140000 students from 200 rural schools have been educated in health and hygiene issues including HIV/Aids, Reproductive and Child Helath, first aid and infection and waterborne diseases. • Enhance the health education of 80000 households and 200 schools people in at least 250 villages and mobilise them around health promotion that will lead to change practices and attitudes towards health and hygiene issues. • 20% reduction in incidence of major infections and viral disease. • Health promotion made a mandatory activity in schools and medical points of Sughd, Ferghona and Andijon regions. • At least 95% of the CHW are already employed in medical points or schools. • Prevention of malaria in Uzbek border areas through distribution of 6000 ITNs and establishment of Gambusia fish in rice field of 8 cross boarder villages of Uzbekistan.
