Of the 93 passengers and 11 crew on board, the sole survivor was a nine-year- old Dutch boy. The aircraft was delivered on 8 September 2009, eight months old at the time of the incident. The aircraft had logged approximately 1600 hours on 420 flights. The weather at the time of the crash was cloudy, but with good visibility, and no fog or thunderstorms.

The investigation was led by the French Bureau d’Enquêtes et d’- Analyses (BEA) The US NTSB participated as the State of Manufacture of the General Electric engines.3 April 2011, Woods Hole Oceanographic Institute’s AUVs found the underwater debris field.

Advisers from Cessna Aircraft and Teledyne Continental Motors traveled to Ciudad Victoria to assist in the DGAC Investigation. The engines were field inspected. Engines and propellers were shipped to the manufacturers for exam. The fuel boost pumps were shipped to Cessna.

It was being ferried to a new owner after storage at Caracas with 3 crew on board. The aircraft struck Iliniza volcano, elevation 17,000ft. (Latacunga, Ecuador) There was no evidence of poor weather in the area around Lata - cunga Airport at the time. All 3 occupants were killed. The plane first flew on June 23, 1978 by Frontier Airlines. It later was operated by United Airlines and Atlantic Airlines de Honduras before changing hands to ConViasa.

The flight departed from the Queensway Bay Heliport, Long Beach, California, at 0907. The commercial pilot, a company employee, and one passenger were killed, and three pas - sengers were seriously injured. The helicopter was destroyed by impact damage and fire. The propeller was not under power when it impacted terrain. The wings, wreck - age, vegetation, and ground did not indicate signs of a fuel spill. It appeared that the airplane had run out of fuel and stalled on approach, causing it to enter a spin prior to impacting terrain in a near vertical attitude. Cited as cause was the pilot's inade - quate preflight planning, which resulted in the loss of engine power.

The pilot and the passenger were fatally injured. The flight was conducted under Russian flight regulations. According to preliminary information provided by the Russian government, the wreckage of the accident helicopter was not located until July 2008. Initial examination of the wreckage revealed that the helicop - ter impacted a swampy forest area in a nose down, right roll attitude. July 17, 2007 -TAM Lineas Aereas Flight 3054

It failed to stop and went off the runway. The runway is at a higher elevation than the surrounding street and residential area.The A320 cleared perimeter fence and a busy highway and collided with the Tam Express warehouse, bursting into flames. 187 aboard died and 12 on the ground.

The commercial pilot was fatally injured and the passenger was seriously injured. The 1952 helicopter, #1154, was on a ferry flight to Guatemala City, Guatemala. A visual flight rules flight plan was filed originating in Brownsville, TX, and was destined to an intermediate refueling stop at Ciudad Victoria, Mexico. Serial#1154, was a converted military OH-13 helicopter op - erated on a FAA experimental airworthiness certificate issued in March 2004. Guatemalan registration was pending. November 19, 2005-Cessna T210N Dana Point

No pre-accident anomalies were noted with any systems examined, and no distress calls or communications preceded or followed the upset. The pilot had moderate to severe coronary artery disease with evidence of at least one prior small heart attack. The combination of coronary artery disease and LVH would have substan - tially increased the risk for sudden cardiac death in this pilot. Four died in the crash.air - craft. Therefore, it seems likely that the pilot was unable to control the aircraft, an inability that can reasonably be explained by an incapacitating cardiac event. Two Hellenic Air Force F-16 fighter aircraft intercepted the aircraft and one of the pilots noticed that the captain's seat was empty and the first officer's seat was occupied by an incapacitated person after pressurization failed on the plane. Shortly thereafter, one person was seen to enter the cockpit wearing an oxygen mask.

The controller asked the pilot to "say again", but there was no response. She again asked, "say again" and the pilot transmitted, " I'm going down." There were no further communications from the pilot. Impacting the ground, the airplane cartwheeled. 50 feet wide debris field. Witness reports of the airplane's flight path along with the wreckage distribution path were consistent with the pilot's attempt to return to the airport. A post-impact fire consumed the fuselage.The reason for loss of engine power could not be determined. Oct 4, 2004-Hendrick Motorsports

Probable: Beechcraft Venue: United States Advocates: Girardi| Keese and partnering firms Summary: On October 24 2004, en route to theNextel Cup Series, a Beechcraft Super King Air 200 owned by Hendrick Motorsports crashed in fog into Bull Mountain, seven miles from Blue Ridge Airport, carrying two pilots and ten passengers. Fatalities in - cluded Jon Hendrick and his twin daughters,and Ricky Hendrick. The plane missed its first landing attempt before veering off course and crashing; and the plane had not climbed to its temporarily assigned altitude of 2,600 feet; it instead descended to 1,800 feet before crashing. The cause was ruled failure to use an instrument ap - proach and pilot error.

Radar showed a cluster of thunderstorms, level three to four were present in the vicinity of N9143B's position. N9143B started an uncontrolled descent from FL260 (about 27,500 feet msl). Witnesses reported hearing the engine make a winding noise. The airplane come out of the clouds about 300 feet above the ground, in a nose low spiral, and the right wing was missing. The ilot's inadequate weather evaluation resulted in in-flight separation of the right wing and right horizontal stabilizer and the subse - quent loss of control and crash.

Heavy rain and strong winds from typhoon "Xiang Sane" prevailed at the time of the accident. The airplane, during the take-off roll, was destroyed by its collision with the runway construction equipment and by post impact fire. There were a total of 179 people on board with 159 passengers, 3 flight crewmembers and 17 cabin atten - dants. There were a total of 83 fatalities (including 4 cabin crewmembers), and 44 people injured.

It was en route from New York to Cairo International Airport, Cairo, Egypt. The flight departed JFK with 4 flight crew, 10 flight attendants, and 203 passengers. All 217 people on board were killed. Visual meteorological conditions prevailed. NTSB determined probable cause to be the airplane's departure from normal cruise flight and subsequent impact with the At - lantic Ocean as a result of the relief first officer's flight control inputs. The reason for his actions was not determined. August 23, 1999-China Airlines Flight 642

The plane, which was landing during a typhoon, touched down hard, flipped over and caught fire. The right wing struck the ground first, and the landing gear and wing separated as the aircraft rolled inverted. 200 of the 317 passengers were injured and the aircraft was destroyed by impact.

Weather at Pohang was poor with degraded visibility and gusty 25 knot winds. The pilot failed at the 1st attempt to land. After the 2nd approach the plane touched down, but overran the runway. The aircraft skidded through 10 antennas, a reinforced barbed wire fence and came to rest against an embankment. The landing snapped the fuselage in half. Damage– Complete destruction of air frame, Injuries– multiple. 6 crew and 150 pas - sengers, no deaths. December 19, 1997-SilkAir Flight 185

Probable: Silk Air, Parker Hannifin Rudder Manufacturer Venue: United States and/ Korea Advocates: Girardi| Keese and partnering firms Summary: SilkAir Flight 185, a Boeing 737-36N, registration 9V-TRF, was en routet from Jakarta, Indonesia to Singapore, and crashed on 19 December 1997 after abruptly plunging into the Musi River from its 35,000 feet cruise altitude, killing all 97 passengers and 7 crew on board. For no apparent reason, minutes before the crash, the cockpit voice recorder and flight data recorder stopped recording, at different times, then the aircraft departed level flight and entered a steep vertical dive. All 104 people on board died in the crash. The pilot was deep in debt and had taken out an insurance policy on himself.

The airplane had been cleared to land on runway 6 Left at A.B. Won Guam International Airport, Agana, Guam, and crashed into high terrain about 3 miles southwest of the airport. Of the 254 persons on board, 228 were killed, and 23 passengers and 3 flight attendants survived the accident with se - rious injuries. The plane was destroyed on impact.

The aircraft was destroyed; 89 passengers,6 crew and 2 on the ground died. 3 passengers were Americans .Witnesses reported the aircraft failed to gain altitude after take-off, collided with a tall building and crashed into residential area 1 mile from the end of the departure runway. Pilot error, unfamiliarity with the craft was judged to be partially responsible. October 22, 1996-Millon Air Flight 406

Visual meteorological conditions prevailed and an IFR flight plan was filed. The airplane was destroyed. The airline transport pilot-in- command, first officer, flight engineer, 1 passenger, and about 30 persons on the ground were fatally injured. About 50 persons on the ground sustained serious in - juries. The flight was originating at the time of the accident. The flight was destined for Miami, Florida. October 2, 1996-Aeroperú Flight 603

Probable: Boeing, Aeroperú Venue: United States and/ Santiago Chile Advocates: Girardi| Keese and partnering firms Summary: On October 2, 1996, Aeroperú Flight 603 Boeing 757 en route from Jorge Chávez Int. in Lima, Peru (LIM), to Comodoro Arturo Merino Benítez Int. in Santiago, Chile, crashed on October 2, 1996. Shortly after takeoff the crew discovered that their basic flight instruments were behaving erratically and reported receiving contradictory serial emergency messages from the onboard computer, such as rudder ratio, mach speed trim, overspeed, underspeed and flying too low. The crew declared emergency and requested return to airport. Before a guide plane arrived, unable to judge their altitude, the 757's wingtip struck the water and crashed. Largest Chilean settlement at the time.

The flight departed JFK about 2019, with 2 pilots, 2 flight engineers, 14 flight attendants, and 212 passengers on board. All were killed, and the airplane was destroyed. Investigation revealed that the crash occurred as the result of a fuel/air explosion in the airplane's center wing fuel tank .Ignition energy for the CWT explosion most likely entered the CWT through the fuel quantity indication system wiring short circuit. There was no evidence of a missile or bomb. February 29, 1996-Faucett Flight 251

Probable: International Air Leases, Faucett, Boeing Venue: United States and/ or Peru Advocates: Girardi| Keese and partnering firms Summary: Faucett flight 251, Boeing 737-222 #OB-1451 crashed about 2015 east - ern standard time on VOR DME approach to runway 9 at Rodriguez Ballon Airport, Are - quipa Peru. The flight was en route from Lima International and had 117 passengers, 2 flight crew, 4 cabin crew, all fatally injured. The flight crew made no indication of problems to ATC and impacted near the top of a ridge 2.5 miles west of runway 9. The main fuselage carried over the initial ridge and impacted near the top of the sec - ond ridge 1/3 mile east. The empennage fell to an arroyo in between.
