IBS is quite common in children as well as in adults. A well conducted, albeit relatively older study estimated that 14% of US high school students and 6% of6 middle school students suffer from symptoms consistent with8 IBS . More recent2, data4, 7 generally supports a very high prevalence of IBS in children and consequently a high economic burden . In clinical practice, an IBS diagnosis is not infrequently applied rather loosely since IBS is defined exclusively by a relatively complex symptom formula without a verifiable biological marker. Therefore, clinicians sometimes apply the IBS label to any patient without evidence of “definite” organic disease9 who complains of non-acute abdominal pain combined with altered bowel movement pattern. As pointed out by Quigley & Shanahan , a medical condition such as IBS, that is exclusively defined by symptom criteria, may create an illusion of understanding and equate complaints with “disease”. This aspect could be particularly relevant in children who may express symptoms vaguely and inconsistently. The real “intensity of complaints” may also be difficult to ascertain in this age group of patients.     TITLE: Trimebutine Maleate and Pinaverium Bromide for Irritable Bowel Syndrome: A Review of the Clinical Effectiveness, Safety and Guidelines DATE: 30 November 2015 CONTEXT AND POLICY ISSUES Irritable bowel syndrome (IBS) is a chronic gastrointestinal disorder characterized by abdominal pain or discomfort and altered bowel movement consistency and frequency resulting in constipation or diarrhea.1,2 It is a functional disorder and not an organic disease.2 According to the Rome III diagnostic criteria, IBS is defined as recurrent abdominal pain or discomfort for at least three days per month in the last three months and with two or more of the following: improvement with defecation, onset associated with a change in frequency of stool, or onset associated with a change in form (appearance) of stool.2,3 There are three main subtypes of IBS: IBS with constipation (IBS-C), IBS with diarrhea (IBS-D), and mixed IBS (IBS-M).2 Hard or lumpy stool with ≥25% of bowel movements and loose or watery stool with < 25% of bowel movements is indicative of IBS-C. Loose or watery stool with ≥25% of bowel movements and hard or lumpy stool with < 25% of bowel movements is indicative of IBS-D. Hard or lumpy stool with ≥25% of bowel movements and loose or watery stool with ≥ 25% of bowel movements is indicative IBS-M. 2 The reported prevalence estimates of IBS vary widely. Surveillance proposal for consultation We will not update the guideline at this time. Reason for the proposal New evidence We found 105 new studies in a search for randomised controlled trials (RCTs) and systematic reviews published between 01 September 2013 and 18 July 2016. We also considered studies identified by members of the guideline committee who originally worked on this guideline. Evidence identified in previous surveillance 3 years and 6 years after publication of the guideline was also considered. This included 52 studies identified by search. From all sources, 157 studies were considered to be relevant to the guideline. Surveillance proposal consultation document for Irritable bowel syndrome (2008) NICE guideline CG61 1 of 44 This included new evidence that is consistent with current recommendations on: diagnosis of irritable bowel syndrome (IBS) dietary interventions physical activity interventions drug treatments (antispasmodics, laxatives, anti-motility agents and antidepressants) psychotherapy hypnotherapy, biofeedback and relaxation therapy acupuncture and patient information. We also identified new evidence in the following areas that was inconsistent with, or not covered by, current recommendations, but the evidence was not considered to impact on the guideline: ondansetron vitamin D supplementation herbal medicines. We did not find any new evidence on reflexology, psychosocial interventions, or self-help and support groups.
