It is a resource for county agencies and health care providers who work with pregnant women and families. The Directory is intended to provide information on key points of entry or referral into the different types of services, and is not intended to be a stand-alone compendium of all services or providers. There are many other program descriptions, service directories, and hotlines that provide more detailed information on eligibility, covered services, and available providers. The Directory is not intended to advise about a specific person’s eligibility for any program or service, or to provide legal advice. Eligibility for many of the services and programs described in this Directory depends on a number of factors, such as level of income and assets and compliance with program requirements. An attorney should be contacted for legal advice. The Directory is provided by the Department of Health Services in response to a requirement in Wisconsin Stats. s.253.10(3)(d). Look at the Key to Types of Services Listed in the Directory (page 1) for descriptions of the types of services that are included in this Directory. Services are listed by county. In addition, adoption services and family planning services for the entire state are listed separately. 2. Use the State of Wisconsin map on the next page to find the county where you live. 3. Look for your county in the County Listing of Services (beginning on page 12) and find the type of service you need. 4. If the type of service you need is not available in your county, look in the pages for neighboring counties (see the state map). 5. Call one of the agencies listed and tell them what you need. 6. If the agency cannot help you with what you need, ask who you should call. 7. For assistance with adoption services, see the County Listing of Services (page 12) and the separate sections, Adoption Agencies Licensed by the State (page 215) and Adoption Agencies for Children with Special Needs (page 217). 8. COUNTY LISTING OF SERVICES Abbreviations are used in the County Listing of Services and in this section to indicate the different types of agencies and services. The services and abbreviations are divided into four groups: pregnancy and childhood, adoption, children with disabilities, and sexual assault and incest. This section describes the different services and indicates the abbreviation that is used in the County Listing of Services. A. Pregnancy and Childhood . HSS Human service departments or social service departments are operated by local government and are available in every county. They provide a wide range of support and referral services for women and families during pregnancy and throughout childhood, such as: • help with applications for many assistance programs, • help in finding needed services during pregnancy, • information on child development and parenting, • help in finding needed childcare and other services, and • help in screening families for stepparent adoptions. LHD Local health departments are operated by local government and are available in every county and in some of the larger municipalities. They provide a wide range of health and referral services for women and families during pregnancy and throughout childhood, such as: • information on good eating habits and health practices during pregnancy, • help in finding health and other services during pregnancy, • child health check-ups and immunizations, • information on child development and parenting, and • help in finding child health care, childcare, and other services. WIC Women, Infants, and Children Supplemental Nutrition Program agencies are available in every county and some of the larger cities. They provide a variety of services for pregnant and breastfeeding women, new mothers, infants, and children up to age 5 years. Services include: • nutrition screening and education, breastfeeding promotion and support, • an eWIC card which can be used to buy nutritious foods, and • help in finding needed health services and other services. CommunProg Community Programs include county departments of community programs and affiliated agencies, and are available in every county. County departments of community programs are responsible for making sure that county residents have mental health, substance abuse, and developmental disability services available to them, such as: • inpatient and outpatient care and treatment, • residential facilities, • emergency care, • supportive transitional services, • community support programs. NFP Natural family planning is a method of avoiding or achieving pregnancy, based on the knowledge of a woman's signs of fertility and infertility. NFP consultants are available in many counties to provide information about classes or individual training.
