Provider Directory PROVIDER DIRECTORY Your primary care provider (PCP) is your primary healthcare provider who: · Coaches and educates you with healthy lifestyle options · Provides you with preventive and wellness care · Identi®es and treats you for general medical conditions · Evaluates the critical nature of your medical condition; directing you to needed care · Makes referrals to medical specialists when necessary Your PCP will become your medical home. As a medical home, the PCP is your primary source for healthcare. They will refer you to a specialist if needed and help manage your chronic conditions. You will have an ongoing, trusting relationship with your PCP who knows your medical history. Choosing a PCP: All plan members must have a PCP. You must select a primary care provider who is in the plan network for you and your children from one of the following practices: · Family practice · General practice · Internal medicine · Pediatrician · Obstetrician/gynecologist (OB/GYN) Information in this directory is accurate as of the time of printing.
