We hope you find your placement at SHFT rewarding, enjoyable and you find that your time in your placement area is a valuable learning experience. How to find us: There are bus routes that run along Botley Road and there are bus stops close to the hospital entrance. Please ensure that you are prepared and have sourced the bus that will get you to the hospital in time to commence your shift. If you have your own car. Parking is available on site or on surrounding roads Hawthorn Lodge is a large Victorian building and we are located on the first floor. Please telephone the co-ordinator on arrival and you will be greeted and shown the facilities and will be introduced to the team. Nature of Service provided: Southern Parishes ICT provides a range of nursing and therapy interventions to patients who are predominantly housebound. We also provide support to a number of residential homes within our area. The surgeries we are commissioned to support are: Blackthorn & Bursledon, Hedge End, St Lukes, West End and we cover the areas of Hamble, Netley, Netley Abbey, Hedge End, West End, Botley and Curdridge. Description of the teams / professionals who work there: Our team consists of Nurses, Associate Practitioners, HCSW, Occupational Therapists and Physiotherapists. Our Band 3 and 4 staff cover both nursing and therapy visits. As described above we are an integrated team and as such we provide for patients for both nursing and therapy needs. We also work closely with The Tissue Viability Team and Palliative Care Team. The learning opportunities in our area: During your placement you will experience a wide range of opportunities to experience care in the community and will be able to organise visits with our allied health care professionals and within general practice. Senior students may be able to spend time with our palliative care colleagues, depending on their work load and availability during your placement. If the opportunity arises you may also be able to attend Multi Agency Risk Management (MARM) meetings and gain an understanding of how complex patients are managed in the community to provide the best care for their needs. We also work closely with our colleagues in OPMH and you may also be able to organise time with them during your placement. You will also have the opportunity to attend Multi disciplinary team meetings held by the surgeries, We are fortunate to be based on the Moorgreen Hospital site and our neighbouring building Tom Rudd Unit, has a small library which can be accessed by students. Shift patterns for our area: Early 8-4pm, Late 12-8pm. We work one weekend in four and you will be expected to work the weekend with your assessor. Our facilities: The team have a lunch break at 12.30pm. We have a water heater, microwave and fridge and a staff room for lunch. There are a varied number of small shops in the village of West End where you can buy lunch if you do not bring your own. Tea, coffee, sugar and milk are available. How to dress: All students are required to adhere to their Universities Uniform Policy as well as the Trust’s Uniform policy. Please ensure that you attend in uniform, our policies are available on the SHFT website. Guidance and policies to be aware of: Useful policies to access prior to attending would be: Lone working policy, IPC, Wound care and medications management. Preparatory reading prior to placement start: For all students attending placement with the team please ensure you revise IPC, A&P of wounds. Also long term conditions such as: Diabetes, MS.
