It can mimic epididymitis and must be considered in all people presenting with sudden onset, severe testicular pain. Males less than 20 years are more likely to be diagnosed with testicular torsion, but it can occur at any age. Viability of the testis can be compromised as soon as 6-12 hours after the onset of sudden and severe testicular pain. SCOPE RNs must consult with or refer all suspect cases of epididymitis to a physician (MD) or nurse practitioner (NP) for clinical evaluation and a client-specific order for empiric treatment. A hydrocele is an abnormal fluid collection between the outer tissue layers of the testicle. These tissue layers naturally secrete fluid and when this fluid is not reabsorbed, as it usually would be, a fluid collection or hydrocele forms. The cause of most hydroceles is unknown, although some may be related to trauma, infection, or past surgery. A spermatocele is a cyst-like sac that is usually attached to the epididymis, the tube that sits behind the testicle and stores sperm. The sac of a spermatocele is filled with sperm. The exact cause of a spermatocele is unknown but it is thought that injury and obstruction may play a part in their formation. An epididymal cyst is much the same as a spermatocele. However, the sac attached to the epididymis is a true cyst and is filled with cystic fluid and not sperm. A hydrocelectomy is an operation to treat a hydrocele. An incision is made in the scrotum and the testicle containing the hydrocele is lifted out. The sac is then removed and the remaining tissue edges are stitched back. The tissue edges then heal onto themselves and the surrounding vessels naturally reabsorb any fluid produced. -

Possible long-term effects of torsion on testicular function are controversial. This review aims to address the impact of testicular torsion (TT) on the endo- crine- and exocrine-function of the testis, including possible negative effects of torsion on the function of the contralateral testis. Testis tissue survival after TT is dependent on the degree and duration of TT. TT has been demonstrated to cause long- term decrease in sperm motility and reduce overall sperm counts. Reduced semen quality might be caused by ischemic dam- age and reperfusion injury. In contrast, most studies find endocrine parameters to be unaffected after torsion, although few report minor alterations in levels of gonadotropins and testosterone. Contralateral damage after unilateral TT has been sug- gested by histological abnormalities in the contralateral testis after orchiectomy of the torsed testis. The evidence is, however, limited as most human studies are small case-series. Theories as to what causes contralateral damage mainly derive from animal studies making it difficult to interpret the results in a human context. -
Hemospermia: Long-Term Outcome in 165 Patients

We report our 15 years’ experience. From 1997 to 2012, 165 patients presented with hemospermia. Mean age was 38 years. Mean follow-up was 83 months. Laboratory evaluation and testis and transabdominal ultrasonography was done in all. Since 2008, all sonographies were done by the first author. One patient had urinary tuberculosis, one had bladder tumor and three had benign lesions at verumontanum. One patient had bilateral partial ejaculatory duct obstruction by stones. All six patients had persistent, frequently recurring or high-volume hemospermia. All pathologies were found in young patients. In the remaining 159 patients (96%), empiric treatment was given with a fluoroquinolone (Ciprofloxacin) plus an nonsteroidal anti-inflammatory drug (Celecoxib). In our 15 years of follow-up, no patient later developed life-threatening disease. Diagnostic evaluation of hemospermia is not worthwhile in the absolute majority of cases. Imaging studies should complement, but not replace, sound clinical judgment. When making a decision for conservative, non-surgical care, the provider must balance the potential morbidity of surgical exploration against the potential cost of missing a surgical diagnosis. -

Testicular volume Testicular volume is assessed using an orchidometer; a sequential series of beads ranging from 1 mL to 35 mL (see Image 1). Testicular volume is measured using the following steps: 1. Conduct the examination in a warm environment, with the patient lying on his back 2. Gently isolate the testis and distinguish it from the epididymis. Then stretch the scrotal skin, without compressing the testis 3. Use your orchidometer to make a manual side-by-side comparison between the testis and beads (see image 2) 4. QUALITATIVE AND QUANTITATIVE COMPOSITION Caverject Impulse dual chamber syringe is available in two strengths, 10 and 20 micrograms. Each 0.5 mL cartridge delivers a maximum dose of 10 micrograms or 20 micrograms of alprostadil. Alprostadil is the naturally occurring form of PGE1. Alprostadil is a white to off-white crystalline powder with a melting point between 115°C - 116°C and has a molecular weight of 354.49. Alprostadil is practically insoluble in water with a solubility of 8,000 micrograms in 100 mL double distilled water at 35°C.
