Elderplan has been approved by the National Committee for Quality Assurance (NCQA) to operate as a Special Needs Plan (SNP) until 12/31/2016. NCQA’s approval is based on a review of Elderplan Model of Care. This plan is available to anyone who has both Medical Assistance from the State and Medicare. This plan is available to anyone with Medicare and Medicaid who is determined eligible for long term care using an eligibility assessment tool designated by New York State and needs community‐based long‐term care services for more than 120 days (e.g. in home nursing, therapies, home aide, personal care; Consumer Directed Personal Assistance Service; private duty nursing; or Adult Day health care). To get detailed information about your health care coverage, please see your Evidence of Coverage. This directory is sorted by specialty, in order for you to easily find a plan provider that is in your area and that fits your medical needs. The Table of Contents at the front of this directory will show you where each provider type is listed in this book. The Index pages at the back of the directory will show you where you can find a particular individual provider or facility. You will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician (PCP). The term “PCP” will be used throughout this directory. Generally, you must get your health care services from your PCP. The “network providers” listed in this directory have agreed to provide you with health care, vision and dental services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non‐network providers. If an out‐of‐network provider sends you a bill directly to you, you should not pay the bill, but should submit it to the plan to the address provided below, for processing and determination of enrollee liability, if any. However, as soon as possible, you or someone else should contact your PCP or Elderplan about your emergency. Your PCP or Elderplan will need to be involved in following up on your emergency care. When in the service area you must use plan providers for urgent care. You must use plan providers except in emergency or urgent care situations or for out‐of‐area renal dialysis. Introduction This booklet provides a list of Elderplan’s network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and Elderplan’s formulary. We call the pharmacies on this list our “network pharmacies” because we have made arrangements with them to provide prescription drugs to Plan members. In most cases, your prescriptions are covered under Elderplan only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will fill prescriptions at non‐network pharmacies under certain circumstances as described in your Evidence of Coverage. All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. This means the pharmacies listed here may no longer be in our network, or there may be newer pharmacies in our network that are not listed. This list is current as of October 2015. The selected remedial program was chosen in accordance with theNew York State Environmental Conservation Law and is not inconsistent with the National Oil and Hazardous Substances Pollution Contingency Plan of March 8, 1990 (40CFR300), as amended. This decision is based on the Administrative Record of the New York State Department of Environmental Conservation (the Department) for: Operable Unit 2 of the Brookfield Avenue Landfill Site inactive hazardous waste disposal site, and the public's input to the Proposed Remedial Action Plan (PRAP) presented by the Department. A listing of the documents included as a part of the Administrative Record is included in Appendix B of the ROD. Assessment of the Site Actual or threatened release of hazardous waste constituents from the Brookfield Avenue Landfill Site will be addressed by implementing the OU-1 Remedy identified in this ROD. The capping of the landfill and other engineering controls at the site will significantly reduce the threat to public health and the environment.     INTERNATIONAL DESIGN COMPETITION : 2001 ABOUT FRESH KILLS Fresh Kills Landfill is located on the western shore of Staten Island. Approximately half the 2,200-acre landfill is composed of four mounds, or sections, identified as 1/9, 2/8, 3/4 and 6/7 which range in height from 90 feet to approximately 225 feet. These mounds are the result of more than 50 years of landfilling, primarily household waste. Two of the four mounds are fully capped and closed; the other two are being prepared for final capping and closure. Fresh Kills is a highly engineered site, with numerous systems put in place to protect public health and environmental safety. However, roughly half the site has never been filled with garbage or was filled more than twenty years ago. These flatter areas and open waterways host everything from landfill infrastructure and roadways to intact wetlands and wildlife habitats. The potential exists for these areas, and eventually, the mounds themselves, to support broader and more active uses. With effective preparation now, the city can, over time, transform this controversial site into an important asset for Staten Island, the city and the region. Before dumping began, Fresh Kills Landfill was much like the rest of northwest Staten Island. That is, most of the landfill was a salt or intertidal marsh. The topography was low-lying, with a subsoil of clay and soils of sand and silt. The remainder of the area was originally farmland, either actively farmed, or abandoned and in stages of succession. Although Fresh Kills Landfill is not a wholly natural environment, the site has developed its own unique ecology.

(For the purpose of this study, the Project corridor refers to the route of the local S79 bus.) This corridor was one of five that the Client team-consisting of Metropolitan Transportation Authority (MTA) New York City Transit (NYCT) and the New York City Department of Transportation (NYCDOT)-selected for implementation of Select Bus Service (the branded name of Bus Rapid Transit in the City of New York). Based on an assessment of transportation conditions along the Project corridor and input provided at meetings with the public and with other stakeholders, the following conditions were defined along this corridor: • Bus travel time is slow, with one-way travel time on the S79 from the beginning to the end of the route generally exceeding one hour. • The frequency and location of local and express bus stops along Hylan Boulevard and the high volume of buses serving these stops adds to congestion along the corridor. • The absence of sidewalks, pedestrian ramps and pedestrian refuges at some locations along the corridor restricts mobility, access to bus stops and safety for pedestrians and bus passengers.

  • Fresh Kills Park East Park Roads SEIS
    Chapter 1: Project Description A. PROJECT IDENTIFICATION PROJECT BACKGROUND The City of New York, with the New York City Department of Parks & Recreation (DPR) as lead agency, is proposing park roads within the larger Fresh Kills Park, specifically within the proposed East Park. East Park is approximately 482 acres in size and is City-owned property under the jurisdiction of the New York City Department of Sanitation (DSNY). The boundaries of the East Park are defined to the east by Richmond Avenue; and to the west and south by Main and Richmond Creeks. Fresh Kills Park was the subject of a comprehensive environmental review that was completed with the Final Generic Environmental Impact Statement (FGEIS) released on March 13, 2009. This Supplemental EIS (SEIS) is being prepared to analyze in greater detail the potential impacts of the East Park road system, its alignment within the proposed East Park, and potential alternatives. The project site is located in the southwest portion of Staten Island, within Staten Island Community Board 2 (see Figures 1-1 and 1-2). During the latter half of the 20th century, Fresh Kills Landfill operated as the City’s principal municipal solid waste landfill, receiving household and municipal solid waste between 1948 and 2001. A state law enacted in 1996 mandated that solid waste landfill operations cease at Fresh Kills by December 31, 2001; landfilling subsequently ended on March 22, 2001. After that date, Fresh Kills Landfill was temporarily used only for the disposal of materials from the attacks of September 11, 2001. With the cessation of disposal operations at Fresh Kills Landfill, final closure and post-closure activities are underway.

Chokshi, MD, MSc Commissioner For Immediate Release Friday, June 25, 2021 HEALTH DEPARTMENT TO CONDUCT SECOND AERIAL LARVICIDING OF THE MOSQUITO SEASON TO MARSHES AND OTHER NONRESIDENTIAL AREAS OF THE BRONX, BROOKLYN, QUEENS AND STATEN ISLAND Aerial larviciding reduces the number of mosquitoes and minimizes the risk of mosquito-borne disease The Health Department applies environmentally friendly and EPA-approved pesticide to mosquito breeding grounds when appropriate throughout the season Friday, June 25, 2021 – To reduce mosquito activity and the risk of West Nile virus, the Health Department will conduct its second aerial larviciding treatment of the season to marshes and wetland areas of the Bronx, Brooklyn, Queens and Staten Island from Thursday, July 1 to Friday, July 2, and Tuesday, July 6 between the hours of 6 a.m. and 7 p.m., weather permitting. The helicopter will treat only nonresidential areas. While three days are allotted for the aerial larviciding, the application may be completed in less time. In case of bad weather, application will be delayed until from Wednesday, July 7 to Friday, July 9 during the same hours. The Health Department monitors mosquitoes that carry the West Nile virus. To date, no human cases of West Nile virus have been reported this season. During warm weather, mosquitoes can breed in any still water that has been standing for more than five days. In the targeted areas, existing hot and wet weather conditions can cause a rise in mosquito populations. Due to size and limited accessibility, these areas will be treated using a low-flying helicopter.
