The Health and Community Services Department

The Jersey Health and Community Services Department is independent from the U.K. National Health Service and is the political responsibility of the Health and Community Services Ministry, consisting of an elected Minister and two Assistant Ministers who serve as part of the Island’s Government system. A Director General directs the department for Health and Community Services - Caroline Landon. The Group Managing Director for Health and Community Services is Robert Sainsbury. The position of Group Medical Director is Professor John McInerney.

Funded by the States of Jersey from taxation revenue, the Department of Health and Community Environmental Health for the residents of the island. The island has a resident population of approximately 99,000 which increases to approximately 120,000 in the summer tourist season.

The General Hospital

Jersey General Hospital is the sole location of acute hospital services in Jersey. The hospital provides approximately 250 acute beds, 2 day case theatres, 4 main theatres and 2 more being constructed. There is a dedicated endoscopy suite and Breast Clinic along with a full range of other services and facilities. A wide spectrum of care is available including the following specialties:-

Primary Care

Primary Medical Care is not the responsibility of Health and Community Services, but is provided as a fee- based service by independent General Practitioners. There are approximately 100 practising GP’s on the Island, grouped into Practices of various sizes. General Practitioner fees are reimbursed in part by the patient and in part by the Department of Social Security funded by Social Security contributions. A Health Exemption Scheme funded by the Department of Social Security provides full repayment of General Practice fees in case of hardship to patients on low incomes.

District Nursing and Health Visiting is provided by a charity; Family Nursing and Home Care, which receives some of its budget from the Health and Community Services Department. Patients are required to pay for some of these services.

The post holder will demonstrate awareness of the departmental budget and the factors which contribute to over/under spending.

Education Centre

The Education Centre is a shared facility for the Health and Community Services Department. The recently refurbished centre is furnished and equipped to a very high standard. The medical library is well stocked with books, journals and computer facilities, including internet access. The centre has numerous excellent teaching rooms and a 90 seat lecture theatre equipped with the highest standard presentation technology.

There is a full time administrator and one of the Consultants is elected as the Postgraduate Educational Tutor. The Post-Graduate Educational Committee meets on a regular basis.

Teaching

The Hospital has been a recognised ATLS centre since 1990 and ALS centre since 1995. Four courses of each are held annually, one of the ATLS courses is held in conjunction with an ATNC course. APLS courses commenced in 2002 and it is intended to run two courses each year.

The hospital has a Governance committee supported by a team of clinical audit officers. The post holder will be expected to contribute and participate in the Department’s ongoing clinical audit programme.

Health & Community Services work with Health Education Wessex to provide high quality medical education to FY1, FY2 and GP Trainee doctors on a rotational basis. Medical Students also rotate through Medicine and Surgery from Southampton.

The post holder will assist junior staff to identify and meet learning objectives by using the principles of teaching and learning.

Continued Professional Development

The post holder will seek opportunities to develop knowledge, skills and experience and be expected to maintain continued professional development to satisfy the requirement of yearly appraisals including attendance at relevant courses and conferences, involvement at locally organised CME events and comparative audit.

Health & Community Services supports the requirements for Continuing Medical Education and is committed to providing reasonable amounts of time and financial support for these activities. Study leave of 30 days in 3 years and an annual study leave allowance is provided, subject to the approval of the Clinical Director / Medical Director. The post holder will be expected to meet the Continuing Professional Development requirements of their relevant Royal College as required for revalidation.

Clinical Effectiveness

Active and regular participation in Clinical Audit will be expected. Increased emphasis is placed upon medical audit and the post holder will also play a crucial role in co-ordinating these activities

Corporate Governance

Health & Community Services place a high importance on excellence in clinical practice. Full participation in ensuring compliance with the organisation’s clinical governance process is required. Dr Peter Southall is the Lead for this area.

Professional Standards

All Medical Staff are expected to comply with management arrangements in place to follow the guidelines on practice laid down by the General Medical Council’s ‘Maintaining Good Medical Practice’ and to be accountable to the hospital for their actions and the quality of their work.

Appraisal & Review

The post holder is required to participate in the annual appraisal and review process which has been implemented for all consultants and is in line with the GMC guidelines for revalidation. Training is available for appraisers.

Allocate

The organisation uses electronic web-based software to support job planning, e-Leave, e-Appraisal and e- 360. Training in the use of these systems will be provided to the successful appointee.

Commencement of duties

The appointee will be required to take up the post no later than three months from the date of the offer of an appointment, unless a special agreement has been made between the appointee and Health & Community Services. 1. THE POST

This job description is intended as a guide to the general scope of duties and is not intended to be definitive or restrictive. It is expected that some of the duties will change over time and this description will be subject to review in consultation with the post holder.

This is a part time consultant post in oncology. The focus of this Consultant Oncologist post is to provide comprehensive specialist outpatients’ services in Oncology for the management of adults diagnosed with gynaecological malignancies, genitourinary cancer, head and neck cancer, brain tumours and Lymphoma. Oncology referrals are received from consultants within the hospital or a member of their team. These sites may be subject to negotiation.

The position is based in the purpose built Haematology-Oncology Unit at Jersey General Hospital which houses a 10 bedded treatment bay.

 This is a replacement post.  This is a part-time, 8PA position  There is no on call commitment.

The post holder will work alongside 1.8 FTE consultants in oncology.

1.1 Clinical Responsibilities

The post holder will be responsible for the assessment and management of patients diagnosed with gynaecological malignancies, genitourinary cancer, HPB, head and neck cancer, brain tumours and some acute oncology cover in collaboration with colleagues.. Additional support is provided by two specialist palliative care physician and a team of Specialist Chemotherapy Nurses. The major sites for this position are Genitourinary and Gynaecology. The number of minor site referrals comprises less than 25 new patients in total per year.

The work of the Department is largely out-patient based; however there are admissions to the general medical wards which require follow up for advice and on occasion’s chemotherapy administration.

The gynaecological patients are managed in conjunction with the Royal Marsden Hospital gynaecology MDT (Multidisciplinary Team). Other sites are managed in collaboration with Cambridge Hospital. The post holder is expected to attend all relevant local MDTs and the Gynaecological MDT which is tele- linked from the Royal Marsden on a Tuesday morning. Hepatobiliary patients are referred for review in the weekly hepatobiliary MDT in Southampton on Wednesdays and managed in conjunction with the Southampton team. There is a local GI MDT on a Tuesday morning which is optional to attend.

The post holder is responsible for the review of patients and prescription of chemotherapy in 5 clinical sessions per week. These comprise Pre-chemotherapy clinics in which patients are assessed prior to the chemotherapy that will be delivered in that week including patients receiving oral therapies and Review clinics, in which new patients and follow-up patients are seen.

Radiotherapy services by referral to Southampton General Hospital and are delivered in Southampton. There may be future opportunities to develop a radiotherapy service on island.

The Oncology Department is a very busy department covering a population of just over 100,000. The post holder will be expected to work with other departments including pathology, radiology, pharmacy and a broad spectrum of medical and surgical specialities, palliative care and community services.

The post holder has admitting rights to the Private Wing. Private Out patients may be seen and treated within the Haemo-Oncology Department. However, if any private practice is undertaken during normal working hours, then in order to fulfil their contractual obligations, the consultant should make up time spent on public work outside normal working hours to ensure that 8 PAs are dedicated to public work per week. Consultants will be billed for the use of Oncology clinic rooms for private patients at the rate of £15/hr.

There is no out of hours call in Oncology.

The post holder is expected to contribute to audit, to the development of protocols and the education of other staff within the department including nursing staff and F2s when attached to clinics.

The post holder is expected to provide emergency cross cover for other members of consultant staff during any absence during the week. Locum coverage will be obtained for annual leave, study leave or sickness absence in excess of 1 week.

The post holder is expected to update their skills by regular reading or on-line CPD and by attendance at 2-3 national or international conferences each year.

2. DUTIES

2.1 Duties of the post

The post holder will partake in the provision of non-surgical treatment for patients with Genitourinary, Gynaecology, HPB, Brain and Head & Neck cancers and flexible help with the Acute Oncology cover.

In providing a high quality oncology service to the residents of Jersey, the post holder will be expected to play a major role in the management and development of oncology services for Jersey residents including innovative ways of delivering integrated care across the primary and secondary care sectors.

The principle duties and responsibilities are:-  Out-patient clinics  Ward referrals  Cancer targets including attendance at MDT meetings

 Liaison with primary care  Audit, CPD and Clinical Governance  Development and promotion of evidence based practice and guidelines.  Team working and leadership.

The post holder will be expected to take part in management and audit activities and represent the service at Hospital meetings, as agreed with colleagues. The post holder will be expected to be fully involved in a ‘Risk Management” programme and support the Governance agenda.

The post holder is expected to provide cross cover for colleagues during absences not exceeding one week. For longer periods of absence a locum will be provided.

The post holder should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time.

2.2 Teaching and Research

The department is not currently recognised for junior doctor training and as such no formal teaching is attached to this role. Although the post holder may from time to time be expected to participate in the training of Medical staff and to ensure that the training and learning needs are identified and facilitated.

Opportunities for participating in research are limited but are supported when appropriate. The HSSD has a policy for professional leave and is supportive of external duties for the good of the wider profession.

3. ON CALL ROTA

There is no on-call commitment. The nursing team provide an out of hour’s on-call service for patients receiving chemotherapy.

4. THE SPECIALTY

4.1 Oncology Service

The Oncology Department provides a nurse delivered chemotherapy service within its 10 bedded treatment bay.

Outpatient clinics are held daily within the department.

Acute oncological problems and complications of chemotherapy are triaged by chemotherapy nursing staff and reviewed on the chemotherapy unit between 9 am and 5 pm Monday to Friday on a rotational basis by the post holder and the other two medical oncologists. After hours oncological problems are telephone triaged by the chemotherapy nursing staff and directed to the Emergency Department as appropriate.

Jersey General hospital has a tertiary link to Southampton, Cambridge and the Royal Marsden Hospital

Nursing staff and health care assistants support the medical team with outpatient clinics. There are nurse led clinics for Haematology and pre-treatment consultations. We have one specialist nurse who is a non-medical prescriber.

Patients are admitted to hospital under the on-call medical and surgical teams. The oncology service does not have designated oncology beds. However, there are 2 neutropenic side rooms on Sorel ward. Patients admitted with neutropenic sepsis are managed by the on-call medical team with

11 support from the Oncology Department. Additionally the post holder is expected to provide a consultatory service during office hours to patients admitted to the wards with a diagnosis of cancer as well as be responsible for the in-patient review of their own patients. On occasions the medical oncologist may be asked to review patients belonging to other oncology consultants who are not available to attend on the island.

MDT meetings for a variety of specialities are held weekly. The medical oncologist is expected to attend local MDTS appropriate to their cancer sites.

4.2 Unit Activity

Outpatient clinic templates are structured to comprise on average:

Pre-chemotherapy – 6-9 patients per session Review – 6-8 patients per session.

For the Haematology Oncology Unit.

Outpatient referral numbers average 35-45 new patients per month.

Outpatient attendances average 350 per month.

Chemotherapy treatments average 240-250 per year.

DNA rate – averages 3.8%.

Ward referrals fluctuate weekly, however, on average the numbers are 3-4 per week.

4.3 Administrative Duties

The post holder will be required to: Undertake the administrative duties associated with the care of patients and running of the service. The post holder will be expected to undertake their own prescribing in relation to chemotherapy. The post holder will be expected to attend the Oncology Executive Group meetings bi-monthly and wherever possible Medical Staff Committee meetings.

Secretarial support, office accommodation and a computer with access to email and internet will be provided within the Oncology Department.

Proposed Job Plan

All consultants have a 10 PA contract for public work this is not negotiable. There is no on-call associated with this post. The job plan will be subject to periodic review to ensure this is the case.  There is no requirement to provide an out of hours or on-call service.  The 3 consultants are expected to cover each other’s leave as described above.  More detailed job planning will take place depending upon the successful candidate’s special interests.  It is recognised that the consultants may also develop a private practice and with the agreement of the hospital management there may be some flexibility to support this. However, if any private practice is undertaken during normal working hours, then in order to fulfil their contractual obligations, the consultant should make up time spent on public work outside normal working hours to ensure that 8 PAs are dedicated to public work per week.
