AGRANULOOYTOSIS ,- Senior Thesis by GOrdon .M.. Gunn INTRODUCTION Fifteen years ago the medioal profession new nothing of the disease spoken of in this paper as agranulocytosis. Since Schultz, in 1922, gave an accurate description of a fulminat ing case, agranulocytosis has oomettoClOCo.'UPy more and more prominence in the medical field. Today, the literature is fairly teeming with accounts of isolated cases of all descriptions. Added to this a confus ing nomenclature, varied classifications, and heterogeneous forms of treatment; and the large question of whether it is a disease entity, a group of diseases, or only a symptom complex, and some idea may be garnered as to the progress made. Time is a most important factor in diagnosis of this disease, and the prognosis at best is grave. The treatment has gone through the maze of trials as that of any other new disease; there must be a cause and so there must be some specific treatment. -

Differential Blood Count (DBC) provides haematologist with valuable information about health of the patient. DBC determines the percentage of types of WBC this is important because it give exact count of five types of WBC such as neutrophil, lymphocyte, monocyte, eosinophil and basophile. Increase or decrease of DBC than the ideal count indicated that our body is not healthy. Precise counting of type of WBC is very important. Manual counting of White blood cells is time consuming and can lead to human error with increase in number of samples. Automatic cell counter sometimes misclassifies the cells having different morphology. Even they are very expensive and unaffordable by remote area health centres and hospitals. These problems are overcome by developing a system which is image based, cost effective, fast and accurate which has the capability to identify, classify the different type of white blood cell and perform DBC. MANSOUR, MD NONNEOPLASTIC DISEASES OF THE WHITE BLOOD CELLS • There are five major types of WBCs in the blood: neutrophils, lymphocytes, eosinophils, basophils and monocytes. • The normal function of the white blood cells depends on a tight regulation of their count and their function. Therefore, disease develops if there is a derangement of the cells count or function, it takes one of the following forms: o Cytosis: increase in the number of circulating cells above reference range. (Note: leukocytosis means an increase in the WBC count, neutrophilia means increase in the neutrophilic count, lymphocytosis means increase in the lymphocytic count, monocytosis means increase in the monocytic count, basophilia means increase in the basophilic count and eosinophilia means in crease in the eosinophilic count). o Cytopenia: decrease in the number of circulating cells below reference range. (Note: neutropenia means decreased neutrophils, lymphocytopenia, or simply lymphopenia, means decrease in lymphocytes, monocytopenia means decrease in monocytes, eosinopenia means decrease in eosinophils, and basopenia means decrease in basophils). o Abnormal or absent function • Cytosis: o Neutrophilia: defined as an increase in the neutrophilic count in the peripheral blood above reference range for age. o Causes: bacterial infection is the most common and most important etiology. Tissue necrosis in cases of burns or trauma and medications such as epinephrine and corticosteroids are also additional causes for neutrophilia. § Some physiologic conditions can lead to neutrophilia such as stress, smoking and pregnancy. o Pathophysiology: neutrophils are present in the blood in two populations: circulating and marginal (meaning neutrophils stuck to the vessel wall). -

ABSTRACT Chediak higashi Syndrome (CHS) is a rare autosomal recessive multisystem disorder with a defect in granule morphogenesis with giant lysosomes in leucocyte and other cells. CHS is a rare disease, approximately 200 cases have been reported so far. It was described in detail by Chediak in 1952 and Higashi in 1954. 1½ year old male child presented with multiple hypopigment patches on lower extremities, light colored hair, Hepatosplenomegaly and generalised Lymphadenopathy. PBS shows giant prominent liliac to purple granules in neutrophils, band forms, few lymphocytes and monocytes. Bone marrow is hypercellular showing giant prominent gray blue to purple heterogeneous granules often multiple seen in many myeloid precursors, Neutrophils, few lymphocytes and monocytes. -

During this time the understanding of the disease has developed in the curriculum of the medical profession. Since 1922, when first described as a clinical entity by Schultz, it has been reported more frequently as the years passed until at the present time agranulocytosis is recognized widely as a disease process. Just as with the development of any medical problem this has been laden with various opinions on its course, etiology, etc., all of which has served to confuse the searching medical mind as to its true standing. -

We report the case of a 15-year-old boy whose acute lymphoblastic leukemia was revealed by fatal cerebral hemorrhage associated with sepsis secondary to lung infection. Intracerebral hemorrhage remains a cause of death in hematologic malignancies. The patient presented with thrombocytopenia (24,000/mm3), leucostasis and hypofibrinogemia (1.10 g/L). Despite maximal medical and surgical treatment (platelets and fresh- frozen plasma transfusions, red blood cells transfusion, and craniotomy discharge), the patient died. The risk of death is high, and surgical treatment has not proven superior to medical therapy in terms of mortality rates and 6-month survival. -
