Situation Summary Highlights of Current Situation Report − The current outbreaks continued to evolve with various clusters being brought under control while a number of provinces experience a surge with community cases and clusters of unknown sources of transmission. − City (HCMC) continued to report high daily cases with multiple clusters in many places including those with unknown or unclear epidemiological links. Ha Tinh, Nghe An and Tien Giang provinces started reporting cases from community with unknown sources of transmission especially in the past five day. Bac Giang, Bac Ninh continued to report new cases but majority were among the contacts of previously reported cases or those in the lockdown areas. − A total of 2,673 new cases were reported during this week (increased 49% compared to previous week) including seven deaths. These included 2,611 locally acquired cases and 62 imported cases. Of the 62 imported cases, there were three foreigners and 59 repatriated citizens. − In the current outbreaks, as of 20 June 2021, there have been 9,943 locally acquired COVID-19 cases including 163 health care workers (HCW) reported from 42 cities and provinces across the country. Twenty-four (31) deaths (PFC 0.3%) have been reported. − Five cities/provinces reported highest number of cases include Bac Giang (5,406); HCMC (1,618); Bac Ninh (1,525); Ha Noi (263); and (191). See Figures 3 and 4. − Twenty-one (21) provinces have passed at least 14 days since the last local cases reported: Yen Bai; Quang Ngai; Quang Ninh; Quang Tri; Thua Thien – Hue; Tuyen Quang; Son La; Ninh Binh; Thanh Hoa; Thai Nguyen; Hung Yen; Hai Phong; Tay Ninh; Gia Lai; Bac Lieu; Dien Bien; Dak Lak; Dong Thap; Tra ; Thai Binh; and Vinh Phuc. − Six (6) provinces have not reported secondary transmission: Hoa Binh, Nam Dinh, Phu Tho, Quang Nam, Bac Kan, An Giang. Highlights on the outbreak situation and response measures taken from major ongoing clusters: 1) Bac Giang: the situation continued to improve gradually with no new clusters being reported. As of 20 Jun, 5,406 cases have been reported. During the week, on average around 194 cases were reported per day (increased 43% compared to previous week daily average) and mostly among the Industrial Park (IP) workers who are in lockdown areas or have been in quarantine. New cases reported during the week still also included cases that were previously detected but have not been officially notified for national tally. Most of the cases have well-established epi links. At least 25,520 F1 contacts identified and tested. - Cases have been reported from all 10 city and districts in the province, 6 hubs of industrial parks (IP) (mostly concentrated in two IPs of Van Trung and Quang Chau in Viet Yen . Community cases are mainly from three communes of Quang Chau, Van Trung and Nenh town where big numbers of IP workers reside. See Figure 5 for Epi curve and Geographical distribution of cases in Bac Giang. - Six other provinces have reported cases which are related to Bac Giang included Lang Son, Tuyen Quang, Bac Ninh, Son La, Thanh Hoa and Thai Nguyen. - Given the stabilizing situation in the province, the special unit for outbreak control (headed by VM Nguyen Truong Son) has been dissolved and supported workforces (both from national level and other provinces) have gradually decreased since the second week of June. 2) (HCMC): During the week, the cities continued to report multiple clusters in many places and in various settings including residential areas, factories, office buildings, shops, marketplaces, health care facilities, etc. - As of 20 Jun, 1 618 cases have been reported from all the 24 districts. During the week, on average 118 new cases were reported per day. - Between 3-19 June, more than 900 new cases have been reported 35 hospitals and 4 health centres across the city which do not have clear epi links. Investigations are ongoing. Cumulatively, around 1050 cases without known sources of transmission are being investigated.

- About 29 clusters have been recorded in the city of those 23 clusters continue to report daily new case and 6 clusters (including Phuc Hung religious mission cluster) have been brought under control. Of note, there are 9 clusters related to enteprises and industrial companies which are under close monitoring. - The cluster at HTD/ HCMC is being under a high attention and closely followed up. From 11 Jun to 20 Jun, a total of 85 cases have been reported which are related to this clusters (27 additional cases reported during the week). All the cases were among the close contacts of the initial cases and already in quarantine. (See previous Sitrep for a background information of this cluster) - The City has strengthened NPI measures in the affected areas. The City People’s Committee has issued its own Directive No. 10 on 19 Jun to guide public health measures (even more strict than PM’s Directive No. 16 for some measures) to be applied in specific places with the effort and determination to take control of ongoing transmission within one week. - A Special Unit for outbreak commander and control has been established as per the MOH Decision No. 2910/QD-BYT headed by VM Nguyen Truong Son; three vice heads include Director of Cho Ray hospital, Director of PI HCM and Director of IPH in HCMC. There are 5 sub-units to support the city response which include i) Surveillance and case investigation; ii) Laboratory testing; iii) Medical environment management (quarantine operations, IPC in different settings); iv) Case management; and v) Communication. 3) Bac Ninh: A daily case number fluctuated during the week but overall has been on a down trend. As of 20 Jun, 1,525 cases have been reported from all the 8 districts/towns in the province; around 7,567 F1 contacts have been identified and tested. - Four main clusters include: 1) Bac Ninh city: 719 cases reported from 14 wards; mostly related to AAC company with additional cases detected among staff of a lockdown area (a ) in Khac Nien Dist.; 2) Thuan Thanh cluster: recorded the highest number of cases in the province (591 cases) but outbreak spots have been under control; 3) Que Vo cluster: recorded 127 cases in 12/23 communes and towns; source of transmission was identified mainly from Que Vo IP since 17 May, last cases were on 16 Jun; and 4) Others: 79 cases were scattered from other districts; mostly related to Mao Dien and Thuan Thanh communes; last cases reported on 3 Jun; lockdowns have gradually been lifted in various locations. - While the situation seemed brought under control in most areas in the province last week, 6 new cases were detected in AAC Technologies Viet Nam company, inside Que Vo 1 IP, mostly due to cross infection. The workers continued to go under quarantine while working inside the factory since the first case was reported. 4) Ha Noi – The situation continued stabilizing during the week. As of 20 Jun, 263 cases have been reported (5 new cases reported last week among those in quarantine) in 22 out of 30 districts and in 12 clusters. Approx. 4,072 F1 identified and tested. - The T&T – Times City cluster remains without unknown epi links - 5 Clusters being brought under control but still under close observation: K hospital (14 cases; last case reported on 31 May; lockdown lifted on 14 Jun; hospital was back to normal operation on 20 Jun); T&T group (unclear epi links, 63 cases reported since 22 May, so far only cases found among F1 contacts); NHTD#2 (20 cases in Phuc Tho (8); Dong Anh (7); Soc Son (2); Dan Phuong (2); Me Linh (1); Da Nang related (52 cases reported; last case was on 1 Jun); and Hung Yen related (8 cases reported; last case was on 16 May) clusters. - 7 Newly reported clusters: Centralized quarantine facility in Son Tay Dist. (56 cases reported since 23 May); HCMC related (1 case from 30 May on flight from HCMC-HAN, so far no additional cases detected among contacts); Bac Ninh related cluster (26 cases as of 1 Jun related to Mao Dien/ Thuan Thanh Dist. including 2 cases linked with Canon company in Que Vo IP); Bac Giang related cluster (3 cases reported starting from a case who returned from Viet Yen Dist. on 30 May); Hai Duong related (1 case detected through community screening at Duc Giang hospital on 30 May; all 19 F1 contacts tested negative so far); and Dong Anh cluster (starting from a vegetable market vendor who was detected on 7 Jun with unknown epi links, a total of 16 cases have been reported as of 9 Jun; 161 F1 and 283 F23 contacts identified); Duc Giang general hospital in Ha Noi (first case was detected on 14 Jun (a security staff); another staff (an accountant) was detected on 15 Jun. Both were without known sources of transmission. The hospital was put on temporary lockdown on 14 but lifted on 21 Jun after investigation and contact tracing were completed. - Even though Ha Noi situation continued to be stabilizing, having multiple clusters with different sources of infection including those with unknown epi links, urged the local health and authorities to maintain a close surveillance and monitoring. Targeted NPIs measures are expected to be relaxed from 22 Jun.

5) Ha Tinh – a total of 80 cases have been reported between 4-20 Jun. All the cases have clear established epi links. Cases are mostly linked with two cases previously reported. Two main clusters included Thach Bang public beach event and Vinhome Ha Tinh residential building. Three generations of transmission have been observed. Investigation and contact tracing are ongoing. The city has applied whole social distancing as per the PM’s directive No. 16 since 11 Jun. These measures are scheduled to be lifted on 22 Jun but non-essential services remain suspended. 6) Tien Giang – a total of 43 cases have been reported in the province between 5-20 Jun. Apart from cases linked with HCMC and a cluster at Trung Luong – My Thuan highway construction group, several cases continued to be during the week with unknown source of transmission. Investigation and response are ongoing. 7) Da Nang – after almost a month without new cases reported, from 19 – 20 Jun, the city recorded 33 cases with connections to HCMC cases. Investigation and contact tracing are ongoing. 8) Other provinces under monitoring: Lao Cai, Bac Kan, Ha Nam, Hai Duong, Nghe An, Long An, Binh Duong. In these provinces, apart from cases related to clusters in other provinces, there are also cases detected through community screening and with unknown sources of transmission. See Figure 6 for Epi curves of HCMC, Bac Ninh and Ha Noi in the current outbreak, to date. (See previous Sitreps for updates before 14 Jun 2021).

− During the week (from 14 – 20 Jun), Viet Nam reported 2,673 new cases including 2,611 locally acquired cases and 62 imported cases. ✓ The 2,611 locally acquired cases were reported from 21 cities/ provinces (See Figures 3 and 4). ✓ The 62 imported cases included 3 foreign nationals and 59 Vietnamese repatriated citizens. − As of 20 June, Viet Nam has reported a total cumulative number of 13,211 laboratory confirmed cases from 55/63 provinces including 66 deaths (PFC 0.5%) from January 2020 (see Figure 2); of those around 48.9% have recovered. Three are currently 5,922 patients being hospitalized, monitored and treated in 99 hospitals in 44 cities/ provinces. Majority of the cases are either asymptomatic (43.9%) or with mild symptoms (53.2%). (http://cdc.kcb.vn/covid). ✓ Of the 13,211 cases, 1,691 cases (12.8%) are imported. About 96% are Vietnamese (see Figure 1). ✓ The ages of cases range from 2 months to 100 years old. About 50.7% of all cases are in the 30-69 years old group, 1.9% above 70 years old, and the remaining 47.4% under 30 years old. The proportion of male vs female is around 43% vs 57%. (See Table 1). ✓ From 27 Apr to 20 June: more than 91,379 F1 contacts have been identified and have been quarantined and tested in 42 affected cities/ provinces. − The installation and use of Bluezone application continued being accelerated: Ministry of Information and Communication sent technical teams to Bac Giang and Bac Ninh to support its installation on smartphones of population and to provide App instruction. − Rolling out QR code registration posts − CovidMaps, a digitalized interactive epi mapping with real-time data and information, continued being developed in more localities to support local authorities in their decision-making process for outbreak response. So far, more than 10 provinces have developed CovidMaps.

Vaccine supply and deployment: - As of 20 Jun, a total of 2,422,643 doses of vaccines have been administered cumulatively. - Vaccination implementation: Viet Nam launched COVID-19 vaccination campaign Phase 1 on 8 Mar 2021 focusing on the first priority groups: healthcare workers at healthcare facilities, front-line workers working on outbreak prevention and response in 19 provinces. Phase 1 vaccination has completed on 12 May, 100,686 people have received the first shot. - Phase 2 vaccine allocation planned for 811,200 AZ COVID-19 vaccine doses through COVAX. Phase 2 vaccination has completed on 30 May, 63 cities/provinces have vaccinated to 941,262 people including first and second doses.

- Phase 3, 2nd COVAX shipment, with total of additional 1,682,400 doses. As of 20 Jun, 1,380,695 doses have been vaccinated. There are 93,142 mild AEFIs, 26 serious AEFIs and 2 fatal case reported. - 2nd bilateral shipment arrived on 26 May 2021, with 288,000 doses of Oxford/AstraZeneca COVID- 19 vaccines. Government has allocated the vaccines for 22 priority provinces. - 17 June- A Japanese donation of 966,320 doses of AZ arrived in on 16 June and transported to hotspot in HCMC. - 20 June- 500,000 doses of Vero Cell vaccine (Sinopharm) gifted by the Chinese government arrived at Hanoi's Noi Bai Airport. These doses will be administered to Chinese citizens residing in Vietnam, Vietnamese who will study, work or go to for business purposes, and those who require this vaccine, especially in areas near Vietnam's border with China. - 26 May 2021 Government approved the creation of a special fund to drive an effective vaccination programme against COVID-19 as per Resolution 53/ND-CP and Decision 779/ND-CP. The fund will be used for arranging financial resources and materials for the procurement, production and the use of vaccines. For update before 14 June 2021 – See previous Sitreps

Government direction and key public health response measures: (refer to previous Sitreps for updates before 7 Jun) Right after receiving a notification of cases in Ha Nam and Vinh Phuc, the MOH, NSC and local health authorities have been taking vigorous actions. Necessary public health measures have been deployed and adjusted regularly as the situation evolves. - 13 June, the Office of the Government issued the PM’s conclusions at a meeting with scientists, companies and institutes on COVID-19 research and vaccine production, and related ministries which included the following: (i) Implementing synchronously COVID-19 vaccine procurement, production and vaccination; (ii) vaccine production though is difficult and challenging but must move forward; (iii) mobilizing resources for vaccine research and production of vaccines; and (iv) establishing a national centre for vaccine research and production which meets national and international standards.

❖ Community lockdown and social distancing: - During the week 14-20 Jun- HCMC has strictly implemented public health measures. On 19 Jun, the Chairman of the City People’s Committee issued the Directive No.10 on strengthening public health measures for COVID-19 prevention and control for the City. Similarity Tien Giang, Binh Duong, Nghe An, and Lao Cai provinces also stared implementing the PM’s Directive No. 16 or PM’s Directive No.15 in their localities. In contrast, Ha Tinh, Bac Ninh and Bac Giang provinces started easing out their public health measures, i.e. shifted implementation of PM’s Directive No.16 to Directive No. 15 or Directive No. 19 in their localities. See Annex 1 for more detailed information. - As of 20 June, 10 hospitals and 7 clinics are under full or partial lockdown. These included hospitals in Ha Noi (2 hospitals), HCMC (7 hospitals and 7 clinics), and Binh Duong province (1 hospital). ❖ School closure: - As of 20 June- HCMC, Bac Giang and Bac Ninh provinces postponed 10th grade entrance exam until further notice. - From 25 May, summer holidays started, except students in grades 9 and 12 who follow online classes. ❖ Travel restriction - Filling out health declaration is required for those traveling to other provinces/cities by all means of transportation. - All provinces/cities apply travel restriction measures including health declaration, centralized/home quarantine for 14-21 days and/or sample collection for testing of all people arriving from localities with COVID-19 reported cases, and from those under social distancing. Detailed requirements and updates are available on local government official websites (i.e. DoH, PCDC) - The current 21-day centralized quarantine policy for incoming travellers was solely decided by the Government.

❖ Other updates on laboratory testing: - A broad testing strategy continues to be followed, with laboratory testing of all F1, F2 cases, members of outbreak communities/localities as well as ad hoc testing in high-risk location in certain provinces. - The number of RT-PCR conducted in the last week was 904,696 with 2,365 positive samples, giving a positivity rate of 0.26%. Currently there remain 175 laboratories permitted to test for COVID-19 by RT-PCR, with 144 designated as confirmatory laboratories. - Following Decision No. 2022 / QD-BYT Antigen RDTs may be used in outbreak settings to test suspect cases and their close contacts when RT-PCR is not available, the laboratories are overwhelmed and/or the turn- around-time of RT-PCR is too slow to support a rapid response, but all positives need to be confirmed with RT-PCR. Official Dispatch 3848/BYT-DP issued on the 10 May permits health case establishments and other units to conduct periodic testing of individuals at higher risk of COVID-19 infection by antigen RDT every 5-7 days or according to needs. At risk individuals include health care workers, inpatients, border and quarantine staff, essential workers, workers in the service industry and workers in industrial zone. All positives results must still be confirmed with RT-PCR. Antigen RDTs have been deployed to Bac Ninh and Bac Giang to relieve pressure on the testing laboratories. PI-HCM is arranging online training for antigen RDTs for DOH staff, with training materials based on the WHO training package, to prepare for roll-out of RDT testing to support outbreak response in HCMC. - Sequencing of two cases of community transmission in HCMC Thu Duc cluster was Delta variant whilst a case in District 3 was determined to be the Alpha variant. Virus associated with both the Go Vap and Hoan My Sai Gon hospital clusters identified as the Delta variant. - On 29 May NIHE reported that a Y144 deletion mutation in the Spike protein was found in 4 samples; 3 of the samples from Ha Noi and 1 from Hai Phong, following whole genome sequencing. This Y144 deletion mutation is similar to the Y144 deletion characteristic of the mutation of the Alpha variant, although the mutation has been reported in other the Delta variant it remains rare. An IHR notification was submitted by Viet Nam with this information on 29 May and the genome sequence of the 4 Delta variants with the Y144 deletion was submitted to GISAID on 01 June. As of 20 June virus from a total of 72 patient samples in Viet Nam have been identified as belonging to lineage the Delta variant through whole genome sequencing and reported on GISAID. There have been no additional detection of virus with the Y144 deletion, this mutation was not detected in the other 68 genome shared on GISAID. The WHO Virus Evolution Working Group is aware of this report and is investigating the current situation. WHO and MOH have been continuing work to clarify these facts following a large degree of media attention surrounding the Delta variant with the new mutation. - Meetings were held by MOH on 13 and 14 May with regional institutes and all 63 CDC. Emphasis was placed on ensuring the quality of laboratory testing. GDPM is currently working to organize three workshops with laboratories to emaphsise quality assurance of laboratory testing, including specimen pooling - Meeting was held between GDPM, RI’s, WHO and US-CDC on 27 of May to discuss currently laboratory issues including biosafety challenges, and to plan how to best use specimen pooling and RDTs to respond to the current outbreaks, whilst ensuring safety of staff. - Following images on social media of exhausted laboratory workers and sub-optimal use of PPE, GDPM has revised guidance related to PPE for specimen collectors and laboratory workers, removing the requirements for full body suit and guiding that a long-sleeved solid-front/wrap-around or back-fastening gown should be used instead. Revised guidance also specifically advises against the need for double masks and double gloves. Awaiting final issuance of the revised guidance. - MOH is exploring possibility of permitting saliva for COVID-19 testing with validation studies being planned.

❖ Case management: − As of 20 Jun, 66 cumulative deaths since Jan 2020. 5,922 cases are being isolated, monitored and treated in 99 hospitals in 44 provinces/cities. Majority of the cases are either asymptomatic (43.9%) or with mild symptoms (53.2%). Seven new death cases reported during the week (#8512, #4731, #8217, #12151, #3886, #4391, and #6043). − As of today, the number of cases in ICU is 64 cases, which is decreased 11 cases compared to last week. ICU occupancy rate is 9% (occupied ICU beds over current total ICU beds in designated for COVID-19 treatment HCFs). There are 12 critical cases with ECMO (3 in NHTD, 4 in HTD/HCMC, 1 in Cho Ray hospital, 2 in Bac Giang mental health hospital, 1 in Bac Ninh general hospital, 1 in Bac Ninh lung hospital), 31 critical cases with

mechanical ventilation, 21 severe cases with HFNC, 114 severe cases with nasal prong oxygen. For more details, pls visit: https://cdc.kcb.vn/covid ✓ Cumulative number of positive cases related to health care workers (HCW) is 163 in current wave, with 38 additional cases last week. ✓ Current capacity of Health care facilities: ✓ Total beds for COVID-19: estimated at 30,000 ✓ Total beds of ICU for COVID-19: estimated at 2,000 ✓ HCMC prepared 3,500 beds and 370 ICU beds in 9 HCFs for COVID-19 patients ✓ Bac Ninh established a new ICU centre with 100 beds ✓ Bac Giang established a new ICU centre with 100 beds ✓ About 1,400 HCWs mobilized to support Bac Ninh and Bac Giang ✓ Government response: ✓ The Government of Viet Nam requested to all hospitals at all levels to receive COVID-19 patients. ✓ Start implementing “Split hospital” model: hospital parallelly treats COVID-19 patients and treat other patients in a separate zone. ✓ Assigned a special team led by VM Nguyen Truong Son to directly support HCMC in response to current wave ✓ MOH continued urging all HCFs/hospitals to strengthen response activities to outbreak in HCFs: screening, early detection of COVID-19 among visitors, patients and HCWs, and to assess implementing criteria on safe hospitals from COVID-19 ✓ MOH adjusted the guidance of PPE use for HCWs

❖ Risk communication - Government continues to provide timely and transparent communication, advice and guidance to the public, in multiple channels, on various public health measures and actions by the Government. These are disseminated in local media as well as in social media platform, such as the MOH Facebook page: VNM MOH Sức khỏe Việt Nam. - WHO continues to provide technical and capacity development support to the MOH and counterparts in outbreak communications as well as on vaccine safety communications. - The OpenWHO portal is now available in Vietnamese version. This online training platform allows health workers and responders to access courses in local language to support their response to the ongoing COVID-19 emergency and other health threats. - Media agencies are given timely and regular updates on outbreak situation and Government action for content in media reports to the public. - Current topics of interest for the media are on vaccine supply, efficacy and safety, infection after vaccination, access to additional vaccines for Viet Nam, risk assessment of the ongoing outbreaks and updates on cases and public health measures. - Key communication products focusing on health workers were issued: o Thank You Health Workers and Responders from WHO o Advice to health workers to deal with stress o Appropriate Personal Protective Equipment for Health Workers - MOH and WHO continue with proactive messaging to support contact tracing, enhance vaccine confidence, and continuation of the 5K prevention practices (wearing face mask; hand washing and disinfection; keeping distance; no mass gathering; and filling out health declaration. Transmission Stage Assessment Overall assessment – with the current outbreaks ongoing in country, there are six provinces that remain in Stage 2; 28 provinces in Stage 1 and the remaining 29 provinces in Stage 0. During the week, two provinces have moved from Stage 1 to Stage 0; two provinces have moved from Stage 0 to Stage 1. There is a risk of some provinces moving toward Stage 1 and 2 depending on how the situation evolves and local response efforts.

Sub-national level - Six (6) provinces remain in Stage 2 – Bac Giang, Bac Ninh, HCMC, Ha Noi, Da Nang and Dien Bien. While situations in Bac Giang, Bac Ninh, Ha Noi and Dien Bien continued to improve, the situation in HCMC is still escalating with more clusters detected in more places and in different settings. Multiple generations of transmission have been observed in these provinces indicating localized transmission. - Twenty-eight (28) provinces are in Stage 1: Two provinces (Nam Dinh and Hoa Binh) have moved from Stage 0 to Stage 1 having imported cases from overseas reported during the week. Meanwhile two provinces (Son La and Ba Ria – Vung Tau) have moved from Stage 1 to Stage 0. This makes the number of provinces in this category unchanged. - Twenty-nine (29) provinces are in Stage 0: Two provinces (Son La and Ba Ria – Vung Tau) have moved from Stage 1 to Stage 0 having passed 28 days without cases reported, while two province (Nam Dinh and Hoa Binh) have moved from Stage 0 to Stage 1. This also makes the number of provinces in this category unchanged. The current outbreaks have been reported in 42/63 cities and provinces in the country which involve hospitals, industrial parks, communities and many other settings. During the week, additional cases and clusters have been identified including those with unknown sources of infections, especially in HCMC and other provinces. Both Alpha (B.1.1.7) and Delta (B.1.617.2) VOCs have been detected. As of the week ended on 20 June 2021, 29 provinces are in Stage 0; 28 provinces are in Stage 1, and 6 provinces remain in Stage 2. Given a complex nature of concurrently newly emerged clusters especially in HCMC and other hotspots, and with mass laboratory testing and contact tracing are ongoing, more cases are expected in those areas in coming days. Situation in Bac Giang, Bac Ninh and Ha Noi are expected to continue improving. Besides, imported cases among incoming travelers and repatriated citizens are also expected. *WHO/WPR Transmission Assessment criteria • Stage 0 – No transmission: No clear signals of transmission for at least 28 days. • Stage 1 – Imported transmission: Recent transmission is imported from another sub-national or international area or is linked to such importation within 3 generations, no clear signals of locally acquired transmission. • Stage 2 – Localized community transmission: recent locally acquired and localized to place(s), and there are no clear signals of large- scale community transmission. • Stage 3 – Large-scale community transmission: recent transmission is locally acquired and not specific to place(s) or population sub- group(s). The risk of infection for most people in this area is high Assessment done by WHO Viet Nam with concurrence from the Ministry of Health of Viet Nam.
