Parents,

Below is information sent to me through email about our camp. I know everyone already filled out the medical release and concussion forms. I sent those forms to OSU along with our payment and each athlete’s registration form. OSU told me to do this.

Now they are telling me to bring another copy of these forms (for each kid) with us, along with the signed ringworm form. The only way around this is if you signed the forms electronically online.

Please be at the school on Sunday, June 16th between 9:30am -9:45am. Bus leaves at 10am.

Sorry for the inconvenience, Coach Barker

Greetings athletes, coaches and parents -

https://docs.google.com/document/d/1WHLnlXsqN0M-13GYNht2uCZvOnORqzso-kTC-KcIoPg/edit 2/2 CONSENT TO TREAT, RELEASE AND WAIVER RINGWORM TESTING AND TREATMENT

I, ______, am a camper at a wrestling camp at The Ohio State University (“Ohio State”). I am aware that ringworm is a skin disease which is common in the sport of wrestling. Since it is highly communicable to other wrestlers, due to the skin-to-skin contact which is part of wrestling, Ohio State closely monitors for skin diseases such as ringworm and immediately removes campers from camp participation if ringworm is suspected. Campers cannot return to wrestling camp until ringworm treatment has begun.

I understand that if this document is NOT signed and treatment is not begun by Ohio State as described below, that I will be removed from the camp and cannot return until I have seen a physician and appropriate ringworm treatment is confirmed by Ohio State’s certified athletic trainer. Since this typically causes campers to miss a large amount of camp time, Ohio State is offering the following alternative arrangement for your consideration:

• If Ohio State suspects that a camper has ringworm, the camper will be evaluated by a certified athletic trainer (AT) at Ohio State. • If the camper has ringworm, the camper may purchase anti-fungal crème (Lotrimin) from the Ohio State camp store (cost is $2). • The AT will assure that the camper uses the medication and will cover the affected area prior to allowing such camper to continue to wrestle. • The camper must check in daily with the AT to re-evaluate symptoms (camper will be withheld from daily activities until this occurs).

I agree that in consideration for being granted the opportunity to participate in Ohio State’s wrestling camp, I, for myself, my executors, administrators and assigns, do hereby release and forever discharge The Ohio State University and its Board of Trustees, its administrators, faculty members, employees, agents and students from any and all liability for losses, damages, injuries or costs associated with the diagnosis and treatment of ringworm, whether caused by my negligence or carelessness or the negligence of The Ohio State University or otherwise.

I represent and certify that I am at least 18 years old and that I have read, understand and agree to be legally bound by the foregoing consent, waiver and release.

I, ______[print name], do hereby consent to the medical care / treatment of ringworm as described in the bullet points above.
