Parents, Below is information sent to me through email about our camp. I know everyone already filled out the medical release and concussion forms. I sent those forms to OSU along with our payment and each athlete’s registration form. OSU told me to do this. Now they are telling me to bring another copy of these forms (for each kid) with us, along with the signed ringworm form. The only way around this is if you signed the forms electronically online. Please be at the school on Sunday, June 16th between 9:30am -9:45am. Bus leaves at 10am. The purpose of this email is to provide you with some important reminders before you come to campus. Please review the attached document prior to the start of the clinic. • Parent Consent and Concussion Form: The Parent Consent, Waiver & Release Form AND the Concussion Information Form are REQUIRED for all participants. These forms must be completed during online registrations. Upon receipt of mail-in registrations, we will email a link to complete these forms online. Athletes will not be permitted to participate without these forms. o If you completed an online registration and/or signed both medical forms online, you do not need to bring another set to check in. You also need to fill out a ringworm form that is included in the confirmation packet. OHIO STATE WRESTLING TAKEDOWN COMPETITION CAMP SESSION I: JUNE 16 - 19 SESSION II: JULY 10 - 13 GRADES 7 - 2019 HS GRADS Thank you for selecting The Ohio State University Wrestling Takedown Competition ICamp! ncluded in this packet are your required forms to participate, what to bring, maps, parking, check- in, meals and other important information for camp. PLEASE COMPLETE THE RINGWORM CONSENT TO TREAT FORM (ATTACHED) AND BRING WITH YOU TO EVENT’S CHECK IN. DO NOT MAIL PRIOR TO THE EVENT. Camp Schedule All campers will check-in from 12:30AM-1:30AM on the first day of camp at Blackburn House. Campers will receive a camp shirt at check in (sizes are based on first come first served). A parent should remain with the camper until the completion of check-in (see “Skin Check Section” on next page). Overnight campers will be checked out of the residence hall on the last day of camp at 11:00AM. A free parking voucher should be obtained from the camp staff for the garage during the camp’s check-in. This voucher is only valid for a one-time use. Day campers: If you are staying for any duration of the camp: The use of University parking lots requires a per- mit at all times. Spectators who will have a vehicle on campus for any duration of the camp, aside from check-in, pick-up or drop-off, are required to purchase a parking pass for the day(s) they attend.Y ou may also use the Pay-byPlate machines located in the parking lots or download the Parkmobile app @ http://us.parkmobile.com/ mobile-apps. What to Bring Day Campers: Campers should come dressed and ready to participate in their regular workout gear (we rec- ommend athletes bring and wear headgear at all sessions), running shoes, towels and wrestling shoes and a gear bag. Campers must bring 3 changes of clothes for the 3 different sessions each day. Overnight Campers: All campers should bring training gear (headgear, wrestling shoes, running shoes, etc.), a gear bag (backpack), towels, toiletries, bed linens, sheets, blanket, and pillow case. Campers must bring 3 changes of clothes for the 3 different sessions each day. Residence hall will have a mini-fridge and microwave in each room. Spending money to use for pizza or camp store (optional). Meals Day Campers: Lunch and dinner will be provided on day 1, 2, and 3. WRESTLING CAMPS CONFIRMATION PACKET IMPORTANT INFORMATION FOR ALL PARTICIPANTS Skin Check Each camper will be seen by a member of our medical staff who will also perform a skin check. Any athlete with a skin condition will not be allowed to practice or compete without proper paperwork from an Ohio State doctor. Campers should wear mesh shorts for this. In the event that we determine a camper is unable to participate in camp due to a skin condition, the camper’s registration fee will be fully refunded. The parent, guardian or other person who drives the camper to check-in should remain with the camper until the completion of the skin check. Campers who are driving to camp or arrive by airplane to camp without a parent or guardian are not required to have a parent or guardian present at check-in but will still be required to complete and pass the skin check before participation is allowed. Medications/Medical Medications are generally not permitted at camp. Participants may only carry medications to the event if such medications are intended to treat a potentially life-threatening condition. Inhalers are permitted at camp, and must be listed on the Par- ent Consent form. Participants must maintain possession of the inhaler and administer it according to his/her physician’s instructions. With prior written approval from the Camps Office, a participant may carry an EPI-pen. In order to bring an EPI-pen any other medication to the event, a parent or guardian must make a written request for approval to the Camps Office. An EPI-pen and any other approved medications must be in the original prescription container with the child’s name clearly marked. Children are required to self-administer medications whenever possible. Camp staff members, however, are able to administer these medications in an emergency. In order to adequately -as sess your information, please contact us at least 14 days prior to the start of the event. **Please note- Participants with a known previous injury, illness, and/or condition that requires medical clearance for physi- cal activity must provide documentation that he/she has been cleared by a physician. Participants that do not provide medical clearance documentation will not be permitted to participate. Accommodations If you have questions about accessibility or you need to request assistance to participate in any of the Ohio State Athletics Department’s sports camps, including accommodations for dietary restrictions, please contact Ashley Jones (Asst. Direc- tor – Buckeye Sports Camps Office) by email at [email protected]. In order to adequately assess your request, please contact us at least 14 days prior to the start of the camp. Spectators/Safety Parents and other spectators are welcome to observe the clinic. In order to create a safe environment for our participants and spectators, parents should follow the instructions of Ohio State personnel and only watch the camp from designated public areas at the facility. Please use caution at all times when walking around the facilities and/or watching the camp. Refunds A refund less a $50 administrative fee will be issued for any cancellation received at least 30 days prior to the start of camp. A refund less a $100 administrative fee will be issued for any cancellation received at least 14 days prior to the start of camp. The administrative fee and 3% online processing fee are not refundable for ANY reason. Because we have to guarantee our number of campers in order to prepare for camp, any cancellation received less than two (2) weeks prior to the start of each camp session will not be eligible for any refund except in the case of injury, illness, or mandatory school event, in which case supporting documentation from a doctor or school is required. A camp fee may not be transferred to any other Ohio State sports camp. A camp fee may not be transferred to any other camper. All cancellation notices must be received by the deadline above and submitted by mail, email, or fax - no phone calls or voicemails. All supporting docu- mentation required for a refund to be approved and processed must be submitted within ten (10) days after the conclusion of the camp session. Refund requests must be submitted on the Refund Request Form available at OhioStateBuckeyes. com/ camps. Campers who are injured while at camp but remain at camp will not be eligible for any refund.email, or fax - no phone calls or voicemails.
