For first time UWM sponsored H-1B  Passport – biographical information page only (valid at least 6 months beyond start date of petition)  Copy of the Highest degree/diploma and Transcripts  Certified Translation of degree/diploma, if not in English o The certification must include the translator's name, signature, date, and the following statement: "I certify that I am competent to translate [name of language] into English and that this is an accurate translation of the original."  Certification of degree/diploma equivalence, if a non-U.S. degree o The certification must state the U.S. degree equivalent, and include the evaluator's name, title (position), signature, date and a statement regarding his/her competency to certify the accuracy of the evaluation.  C.V. or Résumé, the following information must be included: o Educational and professional history o List of professional memberships, awards, honors o List of publications (include copies of the cover pages of at least three publications) For ALL H-1B Applicants Currently in the U.S.  Copy of the I-94 (now obtained online at www.cbp.gov/i94)  Copy of current visa and passport (passport must include photo, passport number, and expiration date). *Translations If any documents are not in English, they must be translated. The beneficiary may not do the translating. The translation does NOT need to be done by a professional translator but it must contain the following statement. “I, (name), certify that I am competent to translate from the (name of language) language into English and that the attached is the accurate translation of the original documents.” The translation must also be signed and dated by the translator.
