GENERAL CONDITION

1. The tender shall be submitted in the document purchased from the office or down loaded from the website 2. Last date of submission of tender is 30.06.2020 at 3.00 pm and tender will be opened on the same day 3.30 pm 3. No tender will be accepted after the stipulated time and date 4. The cost of tender document is Rs 500 + GST @18%. Those who wishes to down load tender document has to remit the cost of tender document along with the tender. Cost of tender document can be remitted in the form of DD drawn in favour of Managing Director Oushadhi, payable at Thrissur or by cash. In case tender cost is remitted by cash receipt shall be submitted along with the tender. 5. The cover containing the tender should be super scribed with words “Development And Implementation Of An Ayurvedic Hospital Management System[AHMS] In Oushadhi Panchakarma Hospital & Research Institute, Thrissur 6. The tender should be submitted along with E.M.D (2.5 % of quoted amount).EMD can be remitted in the form DD drawn in favour of Managing Director, Oushadhi payable at Thrissur or by cash 7. The entire work should be completed within 1 month from the date of receipt of firm order. 8. An amount equal to 5% of the quoted amount of work should be remitted by the successful tenderer towards the security deposit which will be released only after the satisfactory completion of the work . E.M.D remitted along with the tender can be adjusted against the security deposit. If the successful tenderer fails to remit the Security Deposit and execute an agreement in the prescribed format within 7 days from the date of receipt of work order, the tender is liable to be rejected and the E.M.D remitted should be confiscated . 9. The rate quoted should be inclusive of all taxes and it should be firm without any escalation till the work is completely executed. 10. 90% bill amount will be released after 15 days from the date of submission of final bill after successful completion of the work. Balance 10% will be released only after defect liability period of 12 months

3 11. Time being the essence of the contract, the time of completion stipulated should be strictly adhered to.

12. If the tenderer fails to satisfactorily complete the work within the period stipulated in the order, Corporation may at their option either (1) recover from the tenderer liquidated damages at the rate of 1% of the contract value, for every week or part there of delay subjected to maximum of 10% of the total contract value. (2) cancel the order wholly or partially and complete it themselves or reassign it to other agency at the risk and cost of the tenderer.

13. The tenderer should rectify the defects noticed during the guarantee period without any additional charges. 14. The tenderer can visit the work site prior to the submission of the tender if required. 15. Any clarification regarding the tender can be had from the office during the working hours on all working days. 16. The tender shall be kept open for a period of two month from the date of opening for acceptance. 17. The scope of the work eligibility criteria is attached as annexure I 18. Managing Director reserves unqualified right to accept or reject any or all tender or invite fresh tender or recheck any of the above conditions without assigning any reason whatsoever. The decision of the Managing Director will be final and binding on all.

1. Introduction

Oushadhi Panchakarma Hospital & Research Institute located in the heart of Thrissur, the cultural capital of Kerala, offers varieties of Ayurveda treatments at affordable price using quality medicines produced by ‘Oushadhi’. Oushadhi is committed to improve efficiency of hospital through Hospital Management System. Automation of Hospitals using Hospital Management System (HMS) not only enhances the efficiency of working of the Hospital but also optimizes the resources of the Hospital like equipment, manpower, medicines etc. Electronic Medical Records of the citizens/patients would help the department to provide quality healthcare facility to its patients because Hospital will have all the historical data about the medical history of all the patients Hospital Management System organizes the stable functioning of daily tasks and interactions and keeps a track of all the operations, performs its analysis and generates the reports. It includes the patient and doctors’ records as well as the data concerning financial affairs, supply management, etc.

2. Purpose

Purpose of this tender is intended to identify experienced firms that have the capacity to supply, install and support an Ayurveda Hospital Management System. This should cover all the operations of registration, admission, IP/OP, pharmacy, surveillance of disease, discharge, etc. This shall create a reliable, secure and dynamically updated database of medical information about patients in the form of Electronic Medical Record (EMR) of the patient’s health Database.

3. Scope of Work The scope of work shall be as per the specifications in the Tender

6 Document.The scope of work further includes variation or modification of design,quantity,quality of work,addition,omission or substitution of any work under the instruction of Oushadhi.It involves supply of cloud-based Ayurveda hospital management system.

4. Eligibility Criteria

The Bidders shall satisfy all of the criteria mentioned below: i. The participation is restricted to companies and similar concerns registered in ii. The Bidder should be an established IT services company, and should have been in operation for a period of at least 5 years as on 31.03.2020, as evidenced by the Certificate of Incorporation and Certificate of Commencement of Business issued by the Registrar of Companies.

iii. The Bidder must have prior experience of successfully completed in the process of executing a Hospital Management Information System in private or government hospital softwares.
