Translator's note: Instructions for the interviewers are shown in parentheses, with italic capital letters. In the original questionnaire most of these instructions are given with arrows and other conventional Symbols generally used in the HPORI- questionnaires but not in this English translation. Content of cards showed to the respondents are shown here in boxes. If it is otherwise not indicated in the translation, respondents were not said that they should have felt free to refuse answering or to answer 'do not know'. However, interviewers were'instructed to code the 'don't know' answers with 0, and refused or inapplicable questions with 'x' (the latter is equivalent with the System missing value of the data file). Variable name(s) is (are) shown with bold letters at the end of the question. Some additional explanations about the meaning of the questions and the like tan be found in the footnotes.
