In general, only medicines classified as ‘NOT RECOMMENDED’ and ‘HOSPITAL ONLY’ are retained in this document long-term whilst all others are removed after 6 months as they are included in the CCG Formulary. Definitions of the recommendations made by the Cambridgeshire and Peterborough Joint Prescribing Group (CPJPG) can be found in the explanatory notes Any medication/drug licensed or becoming available after the revision date (below) for this document and not considered by CPJPG should be regarded as ‘Not Recommended’ or ‘Hospital only’. Omission of a drug from this table cannot be taken to indicate that it is recommended for prescribing in either primary or secondary care. (i.e. any medicine not listed in either the classification table or the formulary are classified as “Not recommended” ) Please contact the Medicines Management Team for further advice. Cambridgeshire and Peterborough CCG have undertaken a prioritisation process and agreed that some therapies will not be a priority for funding and will therefore not be funded other than in exceptional circumstances. This appendix to the formulary is a continually evolving document and will be updated after each CPJPG meeting (every 2 months). -

The South East London Red, Amber, Grey (RAG) medicines list for adults - updated January 2020 Definitions and notes to accompany the RAG list NON FORMULARY Drugs are included as a separate RAG list and shaded blue to guide prescribers regarding out of area requests. Requests for GPs to prescribe non-formulary drugs should be returned to the specialist and not prescribed in primary care. Inclusion of a drug on the RAG list does not confer that a drug is commissioned for use within SEL trusts, please refer to the SEL joint formulary. Definition of RED • A consultant or suitably trained specialist (e.g. a specialist non-medical prescribing nurse) within the secondary, tertiary, or primary care clinic should initiate, continue to prescribe and monitor red listed drugs. • RED list drugs are NOT recommended for GPs to prescribe and responsibility for prescribing, monitoring, dose adjustment and review should remain with the specialist. In very exceptional circumstances a specialist may discuss individual patient need for a RED drug to be prescribed by a GP and the GP should consider informing the Medicines Management team before a decision is made to prescribe for individual patients. • Supply of these medicines should be organised through the hospital pharmacy or where appropriate a home care company. If it is necessary to supply on a FP10HNC (previously FP10HP), liaison with a nominated community pharmacy is recommended. • Medicines for specified indications included in the red list should only be prescribed subject to inclusion in the SEL Joint Medicines Formulary and funding being arranged at the individual acute Trust. -
