HERSCHER HIGH SCHOOL OF NATIONAL HONOR SOCIETY

STUDENT APPLICATION

Please complete all sections. Do not be modest. All information can be used by the faculty council to assist with the selection process.

Completion of this form does not guarantee selection.

II. CO-CURRICULAR ACTIVITIES: List all activities in which you have participated during high school. Include clubs, teams, musical groups, etc., and major accomplishments in each. Check each year you participated.

III. COMMUNITY SERVICE: List all the community service activities you have participated in and note any major accomplishments of each. Service activities are those in which you participated for the betterment of your community. For Example: church groups, clubs, Boy Scouts or Girl Scouts, volunteer groups, or community endeavors. Anything in which you did not get paid or did not receive a grade for a class can be counted as Volunteer Hours or Service Hours. If you cannot get a signature before the deadline, please list the name of the adult supervisor who can verify your participation in each activity.

IV. POSITIONS: Leadership will be divided into two areas, leadership positions, and leadership qualities. List all elected or appointed positions held in which you have been an active member in high school. Include clubs, musical groups, committees etc., and major accomplishments in each.

SECTION A: ATTACH ELECTED AND APPOINTED POSTIONS: List all leadership positions held. Positions in which you were directly responsible for directing or motivating others should be included. For example, elected to student council, club officer, committee chairperson, newspaper editor, work manager, church youth group officer, 4‐H officer, etc.

SECTION B: STUDENT LEADER AND LEADERSHIP QUALITIES: Many students are leaders in various ways without ever being elected to a leadership position. In this section, describe how you are a leader in your classroom, school, and /or community. Student leaders are resourceful, good problem solvers, idea contributors, dependable, and have a positive attitude about life. Below make list of the leadership qualities that you possess and then give an example of a time when you demonstrated each leadership quality. Use as many of the spaces below as needed, you do not have to use each space, but the more information you can provide the better.

VI. WORK EXPERIENCE List below any job/work experiences you have had during high school that would support your selection for membership in National Honor Society. Work experience may be paid or volunteer. Work experience may include some of the following types of activities: babysitting, farm work, lawn work, employment at a local place of , etc.

IV. PERSONAL STATEMENT The last section of your application is a personal statement of no more than 200 words. Use your personal statement as an opportunity to express how you fulfill the four criteria of scholarship, leadership, service and character. You may TYPE a statement below, or attach a printed copy to your application.


