GENERAL INFORMATION St John’s House Surgery is situated in the centre of Cleckheaton. It is easily accessible with a large car park in front of the surgery which allows four hours free parking.There is a pharmacy in the premises which is open seven days a week (except some bank holidays) and late most nights

Extended Hours The doctors offer an early morning clinic from 7.30 - 8.00 am on Tuesday, Wednesday and Thursday. There are appointments with the nursing staff between 7.30-8.00am on Tuesdays and Thursdays and until 7.00pm on Monday evenings.

Open Access Clinic Every morning we run an Open Access Clinic. This is a “sit and wait” clinic which starts at 8.30 am and is intended for cases that need to be seen on that day. If you arrive between 8.00 am and 10.00 am you will be seen by one of our GPs or Nurse Practitioner (who can prescribe)

When the Surgery is Closed If you need a Doctor urgently when the surgery is closed, you can telephone the Out of Hours Service on 0345 605 9999. PCT is responsible for arranging our Out of Hours cover.

Further Help and Advice Accident and Emergency Departments (Dewsbury and District Hospital, 0844 811 8110) or 999 ambulance services are for serious emergency cases. An emergency is a critical or life threatening situation which may include loss of consciousness, severe chest pain, head injury or loss of blood. People who use these services for minor problems may prevent others with life threatening conditions from getting the immediate care they need.

If the situation is less critical and you are unsure what to do, call NHS Direct, which is a 24- hour health advice line, staffed by nurses and health information advisors. The telephone number is 0845 46 47. You can also visit NHS Direct online at www.nhsdirect.nhs.uk - they offer a range of information; from what to do if you are feeling ill, to the opening of your late night pharmacy.

There is a NHS Walk-in Centre based at Dewsbury and District Hospital where you book in at Accident and Emergency reception. The centre is run by experienced nurses. Patients with minor illnesses and injuries will be able to get treatment at the centre without an appointment. The centre is open 9 am - 7 pm Monday to Friday and Bank Holidays 9 am – 4 pm. (Telephone 0124 542695 – but not for medical advice)

There is also a NHS Walk-in Centre based at School House Practice, Dewsbury Health Centre, Wellington Road, Dewsbury WF13 1HN, (Telephone 0845 121 1025). They see walk in patients between 12.00 am - 8 pm Monday to Friday and 8.00 am - 8.00 pm weekends and Bank Holidays.

Out of Hours Services are provided by West Urgent Care Services – (WYUCS). You can telephone 0345 6059999 when the surgery is closed.

Home Visits If you are unable to attend the surgery but need the doctor to call, please telephone the surgery before 10 am on 0844 576 9333

Repeat Prescriptions The doctor may agree for you to have repeat prescriptions for long-term medication. Requests can be made by calling in at the Surgery or posting your request (enclosing an SAE, if you would like the prescription to be returned by post).

Please note that repeat prescriptions will be ready for collection two working days after the request has been received. If you have difficulty obtaining your repeat prescription, for instance you may be housebound, please speak to our Prescription Clerk on 0844 576 9333

Please make sure you have enough medication to last you through weekends and extended holiday periods.

Practice Area - How to Register It covers Cleckheaton and Scholes and parts of Birkenshaw, Gomersal, Hartshead, Heckmondwike, Littletown, and Roberttown. Please come to the surgery to register as a new patient between 11 am and 3 pm. You may need to bring two forms of identity with you such as your passport, driving licence or utility bill. Our Practice area is displayed on the back of this leaflet.

Change of Personal Details, Name, Address, Telephone No etc. It is very important that we have correct and up to date details for yourself and your family. Please inform reception if you change your name, address or telephone number. We may need to check that you still live in our practice area.

Patient Participation Group We have formed a patient panel which will meet with three or four times a year to discuss areas for improvement, patient surveys and results and any other issues relevant to the practice. A GP, member of administration staff and practice manager are present at every meeting. The panel is made up of patients of a wide age range who have volunteered their time to improve our Practice standards. The minutes and agendas of the meeting can be found on the Practice website. If there is anything you would like to add to the agenda please put it in writing for the attention of our Practice Manager and we will cover it in the “any other business” section of the meeting.

Staff Employed by the Practice The Practice employs a Practice Manager, a Nurse Practitioner, a Phlebotomist, a team of Practice Nurses, a team of Medical Secretaries, Receptionists and Clerical Support Staff.

Services Available The GP’s offer appropriate advice and treatment for all patients with acute or chronic health-related conditions.

Patients aged between 16 to 74 who have not been seen by a health professional within a 3 year period and patients who are 75 and over, who have not been seen by a health professional within a 12 month period, may also request a consultation for review.

New Patient Checks We offer all new patients a New Patient Health Check with one of our practice nurses. Please ask reception about this when you register.

NHS Health Checks (“Heart MOT”) We offer all patients aged 40-74 years who do not have any pre-existing cardiovascular disease (Angina, Heart attack, High Blood Pressure, Diabetes), to come in for an NHS Health Check. This consists of some fasting blood tests (cholesterol, diabetes screen, liver, kidneys, anaemia and thyroid screen) and then an appointment with one of our practice nurses for a health review.

General Health Check We also offer a general health check and lifestyle advice to all other patients if they are concerned about the prevention of heart attack and strokes, for example if you have a family history of heart disease or stroke disease. This involves a short interview with the Practice Nurse, checking your blood pressure and urine and takes around 20 to 30 minutes.

Health Visitors The Health Visitor attached to the Practice can give advice on health care, particularly for expectant mothers, babies and small children. They are based at Cleckheaton Health Centre and can be contacted between 9am and 5pm, Monday to Friday, on 01924 351536.

Childhood Immunisation Clinics are held in the surgery on Thursdays between 9.30 am and 12 noon. Appointments are sent out by the Child Health Department.

District Nurse District Nurses see patients in their own home (eg. housebound patients) and hold clinics each day at Cleckheaton Health Centre. Please ring 01924 351529 to make arrangements to see a District Nurse. For advice, home visits or to leave a message call the messaging service on 01484 221600.

They will help you with any of the following:  Minor injuries, dressings, removal of stitches and ear syringing  Other referrals which your doctor may arrange  Advice on general health matters

Nurse Practitioner Joanne Collins, our Nurse Practitioner, is a more highly qualified and experienced nurse who has received additional training to enable her to see and recognise many conditions. Nurse Practitioners offer patients greater choice and access to a healthcare professional.

Practice Nurse Sisters Rosie Robertshaw, Clare Howarth and Margaret Middleton hold clinics by appointment for general health screening, ear syringing, routine cervical smears, blood pressure checks, travel immunisation, asthma, diabetic and health promotion clinics and contraception. They can also do dressings and offer dietary, weight management and smoking cessation advice. Please arrange your appointment through Reception.

Patients who are over 75 are invited for an annual health check (housebound patients are visited at home).

Maternity Care All the doctors provide full maternity services in conjunction with the midwife. Appointments with the midwife are available at the following Ante-Natal clinics:

Family Planning Confidential contraceptive services are available. Please make an appointment to discuss this matter with a nurse or a doctor. We fit the contraceptive implant and do contraceptive injections. The fitting of coils and caps is undertaken at the Cleckheaton Health Centre.

Cervical Smears This practice (in conjunction with the Central Services Agency) operates a 3/5 yearly recall system (depending on age), in line with current Department of Health guidelines. You will be advised of your test result by letter from WYCSA. If you have a query regarding the result, please contact the practice AFTER 11am. Test results are usually received at the Practice within a week.

This is a simple, painless method of investigating the health of the cervix and has proved to be effective in the early diagnosis of cancer in this area at a stage when it can be eradicated. We strongly recommend that you take advantage of this – please discuss any anxieties you may have with the Doctor or Practice Nurse.

Disabled Patients The premises are easily accessible to wheelchairs. A Hearing Loop System is in use at the Reception Desk.

Immunisations CHILDREN It is important that pre-school children are fully immunised. The immunisation schedule can be found in your child’s “Red Book”, our practice website or Department of Health Website. Only in special circumstances should any of the immunisations be omitted. Your Doctor, Practice Nurse or Health Visitor will be pleased to advise you.

ADULTS Influenza Flu vaccinations are offered to patients, particularly those most susceptible to the virus. Special “Open Days” are held, usually between September and November.

Travel Our Practice Nurse can advise recommended immunisation for foreign travel and give general travel health advice.

Access to Patient Information Access to patient information is strictly on a “need to know” basis, e.g. District Nursing, Health Visiting, Midwifery services, etc. Disclosure of information to anyone not associated with the NHS will only be given with the consent of the patient. If you would like to review your medical records, you will need to submit a request in writing - please contact our Practice Manager for more information.

Complaints Procedure We operate under the standard NHS Practice Complaints Procedure to deal with complaints about the service we provide. Our Practice Manager will give you further information and a leaflet detailing the Practice Based Complaints Procedure. Our aim is to give you the highest possible standard of service and we try to deal swiftly with any problems that occur.

The Patient Advice and Liaison Service (PALS) can provide confidential, on the spot advice and support, helping you to sort out any concerns you may have about the care we provide, guiding you through the different services available from the NHS. You can call them on 01484 464464.

We are pleased that you are registered with this Practice.

 We feel that it is important you know who is dealing with your enquiry on Reception – our staff wear name badges. Our receptionists are normally your first contact during surgery hours, whether calling by telephone or visiting the surgery and are skilled in helping you. We ask you to treat them with the same courtesy and respect that you would expect yourself.

 We would like to ensure that privacy, dignity and respect for cultural beliefs are respected. If you would like to speak confidentially to the Receptionist, please advise Reception on arrival. We provide wheelchair access and parking.

 A wide range of leaflets and a Practice Leaflet giving you information on the services we offer are available. Please ask the Doctor if you have any questions about your medical treatment.

 You have a right to access your health records (subject to certain limitations). We are committed to ensuring that your records remain confidential.

 We will try to meet your request for appointments at times to suit your convenience. We do not expect our patients to wait any longer than two days for a non-urgent appointment, unless you want to see a specific doctor. This service may sometimes be subject to delay due to holidays and sickness. Please let us know as soon as possible if you have to cancel an appointment. You will usually be seen within 30 minutes of your appointment time and will be informed if there is likely to be a delay. If we are running late, please be patient – on another occasion, it might by you that needs the extra time.

 All telephone calls will be answered as swiftly as possible. The first two hours of the morning is the busiest time – please keep telephone calls brief and avoid calling during the peak morning time for non-urgent matters.

 Repeat prescriptions will be made available after 48 hours. Urgent requests for repeat prescriptions will be dealt with and be ready for collection the same day.
