There are appointments with the nursing staff between 7.30-8.00am on Tuesdays and Thursdays and until 7.00pm on Monday evenings. Open Access Clinic Every morning we run an Open Access Clinic. This is a “sit and wait” clinic which starts at 8.30 am and is intended for cases that need to be seen on that day. If you arrive between 8.00 am and 10.00 am you will be seen by one of our GPs or Nurse Practitioner (who can prescribe) 2 | P a g e Telephone Call Back Service If you telephone the Practice, you can request a call back – just ask the member of staff answering your call. [email protected] How to see a Doctor You can make an appointment by calling at Reception in person or by telephoning 0844 576 9333 between 8 am and 6 pm Monday to Friday. Appointments with a doctor can be booked at any time up to three months in advance. How to see a Practice Nurse You can make an appointment by calling at Reception in person, or by telephoning 0844 576 9333 between 8 am and 6 pm, Monday to Friday. Appointments with a nurse can be booked up to four weeks in advance. PLEASE LET US KNOW IF YOU ARE UNABLE TO KEEP YOUR APPOINTMENT When the Surgery is Closed If you need a Doctor urgently when the surgery is closed, you can telephone the Out of Hours Service on 0345 605 9999. Kirklees PCT is responsible for arranging our Out of Hours cover. Further Help and Advice Accident and Emergency Departments (Dewsbury and District Hospital, 0844 811 8110) or 999 ambulance services are for serious emergency cases. An emergency is a critical or life threatening situation which may include loss of consciousness, severe chest pain, head injury or loss of blood. People who use these services for minor problems may prevent others with life threatening conditions from getting the immediate care they need. If the situation is less critical and you are unsure what to do, call NHS Direct, which is a 24- hour health advice line, staffed by nurses and health information advisors. The telephone number is 0845 46 47. You can also visit NHS Direct online at www.nhsdirect.nhs.uk - they offer a range of information; from what to do if you are feeling ill, to the opening of your late night pharmacy. There is a NHS Walk-in Centre based at Dewsbury and District Hospital where you book in at Accident and Emergency reception. The centre is run by experienced nurses. Patients with minor illnesses and injuries will be able to get treatment at the centre without an appointment. The centre is open 9 am - 7 pm Monday to Friday and Bank Holidays 9 am – 4 pm. (Telephone 0124 542695 – but not for medical advice) There is also a NHS Walk-in Centre based at School House Practice, Dewsbury Health Centre, Wellington Road, Dewsbury WF13 1HN, (Telephone 0845 121 1025). They see walk in patients between 12.00 am - 8 pm Monday to Friday and 8.00 am - 8.00 pm weekends and Bank Holidays. 3 | P a g e Out of Hours Services are provided by West Yorkshire Urgent Care Services – (WYUCS). You can telephone 0345 6059999 when the surgery is closed. Home Visits If you are unable to attend the surgery but need the doctor to call, please telephone the surgery before 10 am on 0844 576 9333 Repeat Prescriptions The doctor may agree for you to have repeat prescriptions for long-term medication. Requests can be made by calling in at the Surgery or posting your request (enclosing an SAE, if you would like the prescription to be returned by post). Please note that repeat prescriptions will be ready for collection two working days after the request has been received. If you have difficulty obtaining your repeat prescription, for instance you may be housebound, please speak to our Prescription Clerk on 0844 576 9333 Please make sure you have enough medication to last you through weekends and extended holiday periods. Enquiries about Test Results For enquiries about your Test Results please telephone on 0844 576 9333 or call at Reception AFTER 11 am. Telephone Consultations If you need to speak to a doctor for advice please ring between 10.45 and 11.00 am. To speak to a nurse please ring the reception desk on 0844 576 9333 Practice Area - How to Register It covers Cleckheaton and Scholes and parts of Birkenshaw, Gomersal, Hartshead, Heckmondwike, Littletown, Liversedge and Roberttown. Please come to the surgery to register as a new patient between 11 am and 3 pm. You may need to bring two forms of identity with you such as your passport, driving licence or utility bill. Our Practice area is displayed on the back of this leaflet. Change of Personal Details, Name, Address, Telephone No etc. It is very important that we have correct and up to date details for yourself and your family. Please inform reception if you change your name, address or telephone number. We may need to check that you still live in our practice area. Practice Website Our practice website is under construction. It will be full of useful information regarding our services, opening times and general health advice. It is updated regularly. Any suggestions for any content you would like to see on the website would be gratefully received. You can view information about the Practice on the NHS Choices web site www.nhs.uk 4 | P a g e Patient Participation Group We have formed a patient panel which will meet with three or four times a year to discuss areas for improvement, patient surveys and results and any other issues relevant to the practice. A GP, member of administration staff and practice manager are present at every meeting. The panel is made up of patients of a wide age range who have volunteered their time to improve our Practice standards. The minutes and agendas of the meeting can be found on the Practice website. If there is anything you would like to add to the agenda please put it in writing for the attention of our Practice Manager and we will cover it in the “any other business” section of the meeting. Staff Employed by the Practice The Practice employs a Practice Manager, a Nurse Practitioner, a Phlebotomist, a team of Practice Nurses, a team of Medical Secretaries, Receptionists and Clerical Support Staff. Practice Manager Please contact the Practice Manager if you have any suggestions about how we may improve patient care, or any queries or complaints of an administrative or non-medical nature. Services Available The GP’s offer appropriate advice and treatment for all patients with acute or chronic health-related conditions. Patients aged between 16 to 74 who have not been seen by a health professional within a 3 year period and patients who are 75 and over, who have not been seen by a health professional within a 12 month period, may also request a consultation for review.
