KNUDSON, PE REQUEST FOR QUALIFICATIONS WEST YARD SITE DEVELOPMENT DESIGN SERVICES PROJECT WS85450023-1 The City of Phoenix (City) is seeking a qualified consultant to provide professional services for the design and possible construction administration and inspection (CA&I) services for a new field service yard to accommodate various field service operations. The City has purchased a 34-acre parcel at Camelback Road and 45th Avenue, which is to be the site of this new service yard. It is anticipated that the new service yard will consist of an administration building, warehouse, locker rooms, maintenance shop facility, material storage, and various parking areas. The estimated construction cost is $20 million. SECTION I – PROJECT DESCRIPTION In 2003, plans for developing this site were completed, but the project was put on hold. The completed plans included an administration building of approximately 38,000 square feet, 12,000 square foot maintenance shop, 3,400 square feet of locker room facilities, 10,000 square feet of warehouse space, 8,000 square feet of garage space and a 2,000 square foot vactor dump area. Additional amenities at the site include both public and employee parking facilities, truck and equipment parking facilities, as well as landscape areas. It is anticipated that the previously completed plans will serve as a starting point, but ultimately, a completely new design will be necessary to meet current operational needs and to meet current City standards and needs. The field operations anticipated to occupy this field site include various Water Distribution field crews (valves, hydrants, meters, blue-stake, etc.), Wastewater Collections, and west area customer pay-station services. To accommodate the various field groups, it is anticipated that the administration building will include a break/lunch room, various conference and training rooms, offices for management personnel, and cubical areas for other City staff, and a public counter for customer service transactions. This project will be constructed using a Construction Manager at Risk (CMAR) delivery method. The CMAR contractor will be selected separately. SECTION II – SCOPE OF WORK The project scope should include, but is not limited, to the following: Develop conceptual design report and plan (30%); Develop 60%, 90% and 100% design plans, final design report and associated construction specifications; Provide Opinion of Cost at 30%, 60%, 90% and 100% stage; Review as-built drawings and identify all utility locations both vertically and horizontally within the proposed facility and what is needed to provide service to the site; Conduct project related design progress meetings; Coordinate with the Public Works Facility Review Committee if required; Design the facility with energy conservation measures incorporated; Conduct project related workshops with work groups relocating to new facility as needed; Design and coordination of power to the site; Actively coordinate and communicate with contractors and City staff; Review cost models provided by the CMAR at 60% and 90% design; Provide pre-construction services, as required by the City, including but not limited to developing bid schedule with the CMAR, attend the pre-bid and bid-opening meetings with the CMAR, issuing addendum and reviewing the Guaranteed Maximum Price (GMP) proposal; PROJECT TITLE PAGE 1 PROJECT NUMBER Design RFQ Rev. 7/17 Provide services, as needed, for land survey, construction easements, drainage improvements, public relations, geotechnical engineer, etc. if necessary; Coordinate with Planning and Development Department to obtain all applicable building/civil construction permits prior to construction; SECTION III - PRE-SUBMITTAL MEETING A pre-submittal meeting will be held on Tuesday, November 28, 2017, at 2:00 p.m., local time, at Phoenix City Hall, Water Services Department, 8 NW Conference Room, located at 200 West Washington Street, 8th Floor, Phoenix, Arizona. At this meeting, City staff will discuss the scope of work, general contract issues and respond to questions from the attendees. As City staff will not be available to respond to individual inquiries regarding the project scope outside of this pre-submittal meeting, it is strongly recommended that interested firms send a representative to the pre-submittal meeting. Questions may be submitted to the Contracts Specialist listed below until 5:00 pm, Wednesday, December 8, 2017. SECTION IV - STATEMENT OF QUALIFICATIONS EVALUATION CRITERIA A firm will be selected through a qualifications-based selection process based on the following criteria: A. General information (10 points) 1. Provide a general description of the firm or team that is proposing to provide services for the project. 2. Provide the following information: a. List the professional licenses held by the team and the key personnel who will be assigned to this project. b. Identify the location of the lead firm’s principal office and the home office location of key staff on this project. c. Provide an organization chart showing key personnel. B. Design Experience of the Prime Firm (20 points) Discuss the experience and qualifications of the prime firm in providing design services for similar projects. Identify projects the submitting firm has completed. For each project listed, please provide: 1. Description of the project 2. Project’s original contracted construction cost and final construction cost 3. Construction dates 4. Project owner 5. Reference information (two current names with telephone numbers per project) C. Construction Administration and Inspection Experience of the Prime Firm (15 points) Discuss the experience and qualifications of the prime firm in providing construction administration and inspection services for similar projects. Identify projects the submitting firm has completed. For each project listed, please provide: 1. Description of the project 2. Project’s original contracted construction cost and final construction cost 3. Construction dates 4. Project Owner 5. Reference information (two current names with telephone numbers per project) D. Experience of Key Personnel and Subconsultants (20 points) PROJECT TITLE: WEST YARD SITE DEVELOPMENT PAGE 2 PROJECT NUMBER: WS85450023-1 Design RFQ Rev. 9/17 Experience and qualifications of the specific project team expected to be assigned to this project. Include sub-consultant, including DBE’s experience. Identify each team member’s role in the projects identified. Specify if work was completed while employed under submitting firm or another firm. For each key person identified, list their length of time with the firm and at least two comparable projects in which they have played a primary role. If a project selected for a key person is the same as one selected for the firm, provide just the project name and the role of the key person. For other projects provide the following: 1. Description of project 2. Role of the person 3. Project’s original contracted construction cost and final construction cost 4. Construction dates 5. Project Owner 6. Reference information (two current names with telephone numbers per project) E. Project Understanding and Approach (30 points) Understanding and approach to the project, including a discussion of the important considerations of the project and the team’s approach to the project issues. F. Overall evaluation of the firm and its perceived ability to provide the required services (5 points) Overall evaluation of the firm’s capability to provide the required services as determined by the selection panel members. No additional submittal response is required. SECTION V - SUBMITTAL REQUIREMENTS Firms interested in this project should submit a Statement of Qualifications (SOQ). Submittal requirements are as follows: All firms should be registered in the City’s Vendor Management System prior to submitting an proposal. For new firms – the City will send an email to your firm with a vendor number within two days of submitting the request. The vendor number should be included on the cover of the SOQ. Information on how to register with the City is available at: https://www.phoenix.gov/finance/vendorsreg Information Sheet: Provide one page with the following information: full firm company name, address, phone number, vendor number, and the email address of your contact person for the project. Do not include additional information. Evaluation Criteria: Address the SOQ evaluation criteria and include a project organizational chart. Additional Content: Resumes and other information may be included (content shall be included within the permitted maximum page limit). Submittals: . Clearly display the firm name, vendor number, project title, and project number on the cover of the SOQ and submittal package. Submittals must be placed in the depository located in the reception area on the 6th Floor of Phoenix City Hall by the submittal due date/time.
