Player Name:______________________________________________ Uniform Number:___________________________________________ (official use only) DALLAS HSAA BASKETBALL, UNIFORM AGREEMENT The HSAA player and his/her parent(s) hereby agree to accept a set of HSAA uniforms (1-2 jerseys and 1-2 pair of shorts) for use in the 2019-2020 basketball season with the understanding they are responsible for proper use, laundering and the return of the uniform set at the end of the season. Reasonable wear on a uniform is not considered damage. The HSAA player and his/her parent(s) also agree to pay a replacement fee to Dallas HSAA Basketball of $55 for each uniform piece (any of 1-4 pieces) damaged, or not returned at the end of the season (end of March 2020). The HSAA Player and his/her parent(s) also agree to pay for replacement of any other uniform pieces, or equipment issued for their use by HSAA Homeschool Basketball that becomes damaged, or is not returned. IMPORTANT NOTE: These uniforms are only to be worn during the official Dallas HSAA Basketball games by the player to whom they were issued. No uniform, or piece of a uniform, is to be worn for practices, shoot-arounds, home school activities, or at any other time. They are for game use only. Responsibility for the uniforms and any other equipment issued lies solely with the player and parent(s) receiving the items, If any uniform piece is loaned to someone else (including another HSAA player) and becomes damaged, the player and parent(s) are still responsible for the replacement fees. They will have to work out the conflict with the borrowing person or player who was in possession of the item when it was damaged, or lost. UNIFORMS AND EQUIPMENT NFHS Basketball Uniform Information: (http://nfhs.org/media/1018788/2018‐19_basketball_uniforms.pdf) Home uniforms shall be white and away uniforms shall be dark. Note that silver/light gray is not a legal uniform color. Headbands / NFHS Rule 3‐5‐4b: Headbands shall be a maximum of 2 inches wide. As directed by the OSAA Sports Medicine Advisory Committee, players wearing “Concussion Headbands” shall have a documented OSAA exception. Athletic Directors shall forward these requests to Kyle Stanfield along with a note from an appropriate medical professional supporting the student wearing the equipment and a photo of the headband. All matches will be played according to the rules as stated by FIBA, the world’s governing body of basketball, with the exception of the following: TIMING RULES Listed Separately, please CLICK HERE to review documents online TEAM REGITRATIONS Existing teams must register online through PlayHQ. New teams must Register Here Representatives will receive a unique PlayHQ individual team link after they successfully register their senior team online. This link must be shared with all teammates so that participants can register directly into that team. This unique team link must also be used during the season, if any players join or fill in. Additional players joining teams during the season must not only register using the unique PlayHQ team link, but full details must be included on the ‘LiveScore Courtside Monitors before their first game.’ Any player found playing under another person name will have the teams game forfeited, and risk being reported. Team Representatives will receive an automatic email regarding registrations whenever a name is added courtside. ‘Fill-In Players’ can play a maximum of two games before they must finalise registration. Failure to complete registration will have game results forfeited and further ladder points not awarded. WALKOVER FINES Teams who give forfeits receive a fine based on how much notice was given. Teams who provide the WBA office at least 24 hours’ notice of a walkover receives a $80 fine, while teams who give insufficient or no notice receive a $110 fine. Walkover fines must be paid at the teams following match. Failure to do so will forfeit any ladder points for games played while the fine is outstanding.

What words, logos and symbols are allowed on the jersey? Other than the uniform number, here are the only items allowed by rule on the jersey (Rule 1-22.7): Words • Player’s name • School name/nickname • Mascot name • Other names intended to celebrate or memorialize persons, events, or other worthy causes are permitted only in the back neutral zone Logos • NCAA • Conference • School • Mascot • Commemorative/memorial • Manufacturer (Men – not permitted on undershirt) • Flag 2. What size restrictions are there for these? When an institutional logo is used in place of the school name, nickname, or mascot name in the neutral zone of the game jersey, there is no restriction on the logo size. The logo may not be closer than one inch from the number. Other logos on the game jersey (institutional, conference, commemorative/memorial, manufacturer, and/or the institution’s national flag, must fit within a four-sided geometrical shape no larger than 2 ¼ square inches (Rule 1- 25.2). On the game shorts, there is no size restriction on institutional logos, names, nicknames or mascots. However, a single conference logo and/or single manufacturer’s logo is permitted as long as it meets the size requirement noted above. 3.
