Our aims and objectives, whilst a Personal Medical Services (PMS) Practice offering primary care services for the diagnosis and prevention of disease, are the following: 1. Provision of good quality Primary Care Service 2. Delivery of the service in a confidential, clean and safe environment 3. Treat patients with respect and dignity at all times irrespective of their ethnic origin, religious beliefs, sexual orientation, age, mental and physical limitations. 4. To promote good health and wellbeing of patients through education and proactive management of disease/illness to optimise their health. 5. To involve patients in decision making with regards to their treatment and management of their condition. 6. Encourage patients to engage with patient participation group to help develop and maintain good quality service. 7. Efficient use of the NHS resources whilst providing clinically appropriate access to NHS services. 8. To encourage training and development for work experience students, medical students and registrars. 9. To be a learning organisation that continually improves what we are able to offer patients, with daily informal and monthly formal clinical meetings. 10. To involve and collaborate with multidisciplinary team, for example district nurses, social services, palliative care team, health visitors etc. to provide best care for the patients. 11. To ensure that the members of the team have the right training and skills to deliver the service confidently and competently, provide them with support and encourage personal development as appropriate. 12. To have zero tolerance policy for any form of abuse. 13. To provide our patients and staff with an environment which is safe, accessible, supportive and friendly. Lavender Hill Group Practice is one of 40 practices in Wandsworth that operates with Battersea Healthcare Community Interest Company (BHCIC) which was set up to provide quality healthcare for all the people in Wandsworth. Lavender Hill Group Practice Statement of purpose: June 2020 2 Emergency planning During an emergency, practices will support one another to continue to see patients if and where possible. Patients may be directed to dedicated hub sites. If the situation demands it, it may be necessary to monitor the appropriateness and need to use different practice locations. At any time, any of the practices listed may be used as an emergency site or for routine clinics to ensure patients continue to receive necessary care. This will be coordinated and agreed with the practices involved and communicated clearly to patients and will be time limited depending on the emergency. The Bridge Lane Annex was chosen as the location for Battersea Respiratory Clinic because it was found to fulfil all the major suitability criteria for a Hot Hub. These include having independent entrances, a car park, good ventilation and a sufficient number of clinical rooms. The layout has been adapted for working with suspected and confirmed Covid patients, with infection control as a priority. Advice on infection control and appropriate PPE was sought during set up and the building has been clearly zoned into clinical 'red and orange' and non- clinical 'green' areas. Support for the Hub set up was provided by NHS property services and included review of fire procedures, security and maintenance. The Hub will be managed by Bridge Lane Group Practice and the clinical service will be led by Dr Carolyn Bayer, Bridge Lane GP partner and Clinical Director of Battersea PCN, with support from the Clinical Directors of Wandsworth and Nightingale PCNs (Dr Matthew Beddoe and Dr Cath Ellis) and the PCN Clinical Leads. The Hub will be staffed by clinical staff from member practices of these PCNs (Battersea Fields Practice, Battersea Rise Group Practice, Begg Practice (St John's Hill), Bolingbroke Medical Centre, Bridge Lane Group Practice, Chatfield Medical Centre, Clapham Junction Medical Practice, Falcon Road Medical Centre, Lavender Hill Group Practice, Queens Town Road Medical Practice, The Junction Health Centre, Thurleigh Road). Bridge Lane staff will support safe working at the Hub by providing full daily inductions to all staff arriving at the Hub, including explanation of Hub protocols. These protocols have also been circulated to all PCN member practices. Bridge Lane staff will also provide day to day support with stock checks and be responsive to any issues arising in the Hub. Practices referring patients to the Hub are required to complete a referral form detailing reasons for making a referral. They are also required to book patient follow-up in their own practice, such that ongoing clinical responsibility for the patient will remain with the registered practice. All referrals will be audited, and feedback given to referring practices. The Clinical Leads will review the service provision regularly, including the volume of referrals and need for the service. Our patients benefit from a spacious waiting room, and a set of facilities to ensure accessibility to all patients including those with any special need, such as access ramps, lift, hearing loop and lower counters. There are 17 clinical rooms and a multi-purpose meeting room, large enough to host all practice meetings as well as meetings with other local Practices, and equipped to host remote meetings. Our practice clinical team comprises 5 partners, 4 salaried GPs and up to 3 GP registrars at any one time, two nurses and one HCA. Our reception and administration teams comprise 8 reception staff and 2 administrative staff as well as our practice manager. We offer a daily triage and emergency service with appointments in the morning and afternoons to accommodate patients’ preferences. We offer bookable face to face and telephone and video appointments. NHS 111 is able to book patients into specific clinics at the Practice. We offer daily longer children’s appointments and evening appointments for commuters and vulnerable patients who prefer to come in at quieter times. We offer email communication through the administrative staff. We have very strong relationships with local pharmacists and community staff including the district nurses, health visitors, community mental health team, drug and alcohol team and the local hospice. The Practice has direct access to a Social Prescriber and Pharmacist through the Battersea PCN as well as a Community Care Plus Mental Health Worker and Shared Care Drugs and Alcohol Worker. Following a successful initial pilot based at the Practice, we now have a Macmillan Primary Care Navigator as an extended pilot working with Macmillan Cancer Centre The Practice obtained in September 2019 the Gold Pride in Practice Award for delivering a fully inclusive healthcare service to our patients. We are one of the five practices of the Battersea Primary Care Network (PCN), which was formed in the Spring of 2019 bringing together the practices of North East Battersea (Lavender Hill Group Practice, Bridge Lane Group Practice, Battersea Rise Group Practice, Queenstown Road Medical Centre and Battersea Fields Practice) serving a diverse inner London population of approximately 55,000 patients. The formal PCN agreement was signed on 29th June 2019, following a key part of the NHS Long Term Plan published in January 2019. This brought together general practices to work at scale, targeting local population needs and sharing a joint workforce such as social prescribers and pharmacists.
