DIRECTIONS FOR COMPLETING SELECTIVE ENROLLMENT HIGH SCHOOLS (SEHS) AND MILITARY ACADEMIES APPLICATION We encourage you to apply online at www.apply.cps.edu. Applying online provides an immediate response that your application has been received. If you prefer a paper application, complete the application on the reverse side. Do not submit both. The completed application MUST be submitted to the Office of Academic Enhancement, Attn: SEHS Application, 125 S. Clark St., 4th floor, Chicago, IL 60603. DO NOT SEND YOUR APPLICATION TO THE SCHOOL(S) TO WHICH YOU ARE APPLYING. Processing cannot be guaranteed for applications that are not submitted to the Office of Academic Enhancement. Applications must be postmarked by December 16, 2011. You will receive an email confirmation once your application has been processed. If you do not have an e-mail address, you may enclose a self-addressed, stamped postcard/envelope with your application. The postcard/envelope will be returned to you to indicate that your application has been received. SEHS TEST ELIGIBILITY REQUIREMENTS Testing eligibility for students will be based on the student’s 2010-2011 ISAT scores in reading comprehension and total math or, if the student did not take the ISAT, on a different nationally normed test (e.g., ITBS, Terra Nova, etc.). In order to be eligible for testing, students must have achieved a minimum stanine of five in reading comprehension and a minimum stanine of five in total math. No exceptions will be made for students who do not possess these minimum stanines. Students with an IEP or a 504 Plan and students in bilingual programs must have achieved reading comprehension and total math scores that equal at least 10 (e.g., 3 and 7; 4 and 6) in order to take the admission examination. MILITARY ACADEMIES INTERVIEW REQUIREMENTS Eligibility for students to participate in the admission interview and/or assessment process will be based on the student’s 2010-2011 ISAT scores in reading comprehension and total math or, if the student did not take the ISAT, on a different nationally normed test (e.g., ITBS, Terra Nova, etc.). In order to be eligible for testing, students must have achieved reading comprehension and total math stanines that equal at least 10 (e.g., 3 and 7; 4 and 6). DIRECTIONS 1. Use blue or black INK only, CAPITAL LETTERS ONLY, do not photocopy, do not staple, and do not allow letters or numbers to touch the lines of the squares. 2. If your child is currently enrolled in a Chicago public school, identify his/her Student ID number on the application. (The Student ID number can be found on your child’s report card or can be obtained by contacting your child’s school.) If your child is not currently enrolled in a Chicago public school, leave this section blank. 3. Identify whether or not your child is Hispanic/Latino AND your child’s race. This is for informational purposes only. 4. If your child attended a Chicago public school in the 2010-2011 school year, complete the application and submit to the Office of Academic Enhancement, Attn: SEHS Application, 125 S. Clark St., 4th floor, Chicago, IL 60603. You are not required to submit the application to your child’s counselor or principal. All necessary records (e.g., standardized test scores, classroom grades, IEP/504 Plan confirmation) can be obtained through the CPS Student Information System. 5. If your child: [a] attended a charter school in the 2010-2011 school year, or [b] or attended a non-CPS school in the 2010-2011 school year, complete the sections for student information and school selection and submit the application to the student's counselor or principal. The counselor or principal will complete the standardized test and performance data (dark gray section), attach a copy of the student's records (including a copy of the student's IEP/504 Plan, if applicable), and submit all materials to the Chicago Public Schools, Office of Academic Enhancement, Attn: SEHS Application, 125 S. Clark St., 4th floor, Chicago, IL 60603. You are strongly encouraged to request a receipt that indicates the time, date and to whom the application was submitted. 6. Submit one application per student. If more than one application is received, only the first application received will be processed. If you have already submitted an application and need to modify it, you must submit a Selective Enrollment/Military Academies Application Modification form, available at www.cpsoae.org (click “Apply.”) Checklist: Please check the following to ensure that the application has been completed correctly. o All address fields have been completed (address number, direction, street name, street type and apt #, if applicable). o If the applicant is a CPS student, the Student ID is listed. o The application is signed by the parent/guardian.
