Installation is simple and requires no tools or assembly. Once you’ve reviewed this user manual, place it in your automobile glove box for easy reference.

How It Works LeeWay operates using an electronic transponder, installed on the wind- shield inside your vehicle or front bumper (with a special bumper mount). As you pass through the toll plaza, an overhead antenna reads the tran- sponder, calculates the toll due, and automatically deducts the correct toll amount from your prepaid toll account.

A LeeWay account allows you to tie all your transponders to one pre- paid account., eliminating the need for employees or family members to wait in a lane for receipts.

Multiple Vehicle Discount For Discount Programs You may obtain a Multi-Vehicle Discount for your second vehicle at half price, the third vehicle at full price, the fourth vehicle at half price, etc., if you meet all the following criteria. • All vehicles must be registered or leased to the same natural person. This must be an individual, not a business entity. • A copy of ALL vehicle registrations are required. • You must purchase the same discount program for all vehicles. All vehicles must be listed in the same account. • The vehicles may not be commercial or business vehicles. • If adding or changing a vehicle on your account in the future, you must provide a copy of all vehicle registrations.

Standard Toll Option for Infrequent 2 Axle Users or 3+ Axle Users The Standard Toll Option is for infrequent bridge 2 axle users or 3 + axle bridge users that would like to have their tolls paid electronically. For 2 axle vehicles the full toll of $2.00 for Cape Coral and Midpoint Bridges and $6.00 for Sanibel Bridge will automatically be debited from your ac- count. A transponder must be purchased at the current selling price.

There are two types of toll lanes:

1) “Dedicated Lanes (all bridges) are for the exclusive use of LeeWay customers.

2) “Attended Lanes” (all bridges) are for customers who need change and/or receipts. Receipts are only available to customers without tran- sponders. You will need to stop for other customers. When it is your turn, please proceed through the lanes with caution.

Transponder Use and Warranty You must permanently affix the transponder in the vehicle it is registered to, as described in the installation instructions and License Agreement. You will need to maintain a positive account balance in your prepaid account. You must notify the LeeWay Service Center of changes to your LeeWay account if your LeeWay transponder is lost, stolen or damaged; if there are any address or vehicle changes (including license plate), or if your credit card information changes. Your transponder has been as- signed to your specific vehicle and must be attached to the windshield. ** Transponders do not work on tint. LeeWay is unable to install or remove any transponders on tint. If you have placed your transponder on tint you will be required to remove it and purchase a new one if needed.

Hard Case Transponders/Bumper Mount Transponders Warranty - Your transponder carries a 90 day warranty from the date of purchase. If your transponder fails to operate for reasons other than abuse or improper use and is returned to the LeeWay Service Center within 90 days of purchase date, we will replace the transponder at no extra charge.

LeeWay Mini Transponders If your Transponder fails to operate for reasons other than abuse or improper use within 90 days of purchase and it has not been removed from original vehicle that it was attached to, we will replace the Mini with a new one at no additional charge. If it has been removed or shows signs of prior removal, warranty shall be voided and you will be re- quired to purchase a new one.

Unpaid Tolls Failure to pay a toll is a violation of Florida Statute 316.1001, which may result in the issuance of a Uniform Traffic Citation. If your transpon- der does not work properly in the toll lanes, you must visit the LeeWay Service Center in a timely manner to have your transponder tested. You may be required to purchase a replacement transponder at the current selling price to continue the use of a discount program. Violations which occur as the result of an malfunctioning transponder can result in the issuance of a Uniform Traffic Citation. Page 8 I. INTRODUCTION TO LEEWAY If you have insufficient funds in your prepaid account, you must stop and pay the full cash toll in an attended lane until a payment is made to replenish your prepaid account.

Prepaid toll account A prepaid toll account is REQUIRED for all LeeWay accounts. Cus- tomers must maintain a positive balance in the prepaid account in order for ANY LeeWay discount program to be valid. An account mainte- nance fee of $0.50 will charged to your prepaid account every month.

The LeeWay system will automatically deduct every toll from your prepaid account when you cross any bridge, even for trips on bridges for which you have not purchased a discount or other facilities throughout the State of Florida, North Carolina, Georgia and more states as they are added. For trips across those bridges the full cash toll will be deducted.

The simplest method to establish and maintain a prepaid toll account is by credit card with automatic replenishment. See below under Au- tomated Account Options for details. Prepaid accounts may also be established with cash, checks, money orders or a called in credit card payment. Cash will not be accepted through the mail or in the 24-hour drop box. Using cash, check or money order, when your prepaid ac- count balance reaches $20 you will see a yellow traffic light (attended lanes only) as you pass through the toll plaza. This is your signal that the prepaid account needs replenishing. Customers using dedicated (transponder only) lanes will need to check their balances online or by contacting the LeeWay Service Center. You may mail us a check or money order, use our 24-hour drop box, submit a replenishment payment in person, or make a payment online. The minimum replenishment payment amount is $30.00 for all customers.

Automated Account Options Automatic Replenishment The simplest method for establishing and maintaining your prepaid account is by credit card payment with automatic replenishment. Your credit card is charged a minimum of $30.00 ($20.00 is recommended for each additional transponder) to establish your prepaid account. You may also use the following prepaid formula to calculated how much Page 9 I.I. INTRODUCTIONINTRODUCTION TOTO LEEWAYLEEWAY you’ll need to establish your prepaid account . Using the Tables on page 6; determine the toll based on the number of vehicle axles for the most likely vehicle(s) of your fleet to cross the bridges. Now calculate the toll based on the number of axles x the average number of trips per week x number of vehicles crossing the bridge x 6 weeks.

When your prepaid account balance reaches $10, your credit card will automatically be billed a minimum of $30.00 (or a higher amount autho- rized by you). Using this method, you will never have to worry about having to replenish your prepaid account again, as long as your credit card information remains current. Continuous Renewal of LeeWay 2 Axle Discount Program(s) (requires payment by credit card) You can sign up to automatically renew your discount program(s) con- tinuously year after year or until you elect to change programs. Annual discount program(s) can be automatically renewed every year in Octo- ber. Semi-annual discount program(s) can be renewed every year for either the first six months (renewed in October only), can be renewed every year for the second six months (renewed in April only), or can be renewed every six months (renewed both in October and April). With this option, you understand the discount programs will be renewed during every renewal period in the future unless you notify the LeeWay Service Center in writing to stop the automatic renewal of your LeeWay Discount programs. You agree to keep the Service Center updated with all vehicle and credit card changes. You understand you will not receive future renewal forms in the mail. Itemized Monthly Statements You can sign up to automatically have an itemized monthly statement of your toll usage mailed to you. Your prepaid account will be debited monthly in accordance with the terms in the LeeWay License Agreement. Customers that have online access may view and print previous months statements at no charge.

Vehicle Installation Use the following instructions to install a mini hard case transponder, mini (sticker) transponder, or external bumper mount transponder.

Metallic Windshields Some vehicles have metallic window tinting that interferes with the op- eration of the internal mount transponder. If you experience problems with your LeeWay mini or hard case tran- sponder, you may have to switch to an external bumper mount transpon- der. Additional costs may apply. Materials Required In addition to the LeeWay transponder you will need isopropyl alcohol or glass cleaner, and a clean, dry cloth.

Installation Steps for LeeWay Mini Transponders Please install your transponder immediately making sure it is below any window tinting or approximately four to six inches from the roof. ** Transponders do not work on tint. LeeWay is unable to install or remove any transponders on tint. If you have placed your transponder on tint you will be required to remove it and purchase a new one.

• Clean and dry the inside center windshield surface behind and around the rearview mirror using rubbing alcohol (Isopropyl) and a clean, dry cloth. • Remove the plastic backing on the back of the transponder to expose the adhesive surface. • Position the transponder in the upper part of the middle of your wind- shield. Generally this means behind or below the rearview mirror on the inside of your windshield (see Figure 2.1) . • Place the transponder on the windshield with the LeeWay logo upright, facing you. Make sure the transponder does not block your view while driving and press firmly. Avoid pressing directly on the computer chip in the device. • The Service Center has a test lane open during business hours to test your transponder and help with installation if you have a concern or need assistance.

Installation Steps for Bumper Mount Transponders Please install your transponder immediately making sure it is firmly affixed to the bumper. You can affix the bumper mount with the tape provided by the Service Center.

• Clean and dry the surface in the center of the front bumper using a clean, dry cloth. • Remove the plastic strips from the back of the mounting strips on the transponder to expose the adhesive surface. • Position the transponder in the middle of your front bumper with the transponder number upright, facing you, and press firmly. • The Service Center has a test lane open during business hours to test your transponder and help with installation if you have a concern or need assistance.

Transponders are assigned to specific vehicles (they are not transferable) and are designed to be permanently affixed. Affixing the transponder in any other location could result in incorrect readings and may lead to traffic citations.

To clean the LeeWay transponder, use a damp cloth to wipe the surface of the transponder. Do not submerge the transponder in water or use any cleaning agents. Do not paint or cover the transponder with any type of finish.

Note: To avoid being double charged, there should be only one Lee- Way, SunPass or E-Pass transponder in your vehicle at any time. Page 13 IV. FREQUENTLY ASKED QUESTIONS What are the benefits of LeeWay? LeeWay is the quickest and most convenient way to pay a toll. It is like having exact change all the time. And you will not have to roll down your window to pay the toll. LeeWay is commuter friendly, and will help ease congestion at the tollbooths, conserve fuel and reduce emissions. Additionally, LeeWay could save you money during the Variable Pricing Discount Hours. What are my responsibilities as a LeeWay user? You must permanently affix the transponder in the car it is registered to, as described in the installation instructions and License Agreement. You will need to maintain a positive balance in your prepaid account for your tolls and account maintenance fee. You must notify the LeeWay Service Center if your LeeWay transponder is lost, stolen or damaged; if there are any address or vehicle changes on your LeeWay account; or if your credit card information changes. The most convenient way is online. If you give us your email address you can set up online access and access your account at your conve- nience 7 days a week. If you do not have a computer or internet access you may call the LeeWay Service Center at (239) 533-9297 during office hours, to obtain your prepaid balance. Direct Online Access With direct online access to your account (set up by providing email ad- dress) You Can: Renew you discount program, view and/or print state- ments of toll usage, update and/or delete vehicle information, update mailing address or credit card informaton, sign up for or discontinue automatic replenishment, and make payments to your account. Direct online access changes and/or payments take effect immediately. Online without Direct Access If you have not set up online access to your account you can go to our website leewayinfo.com under forms to sign up for or discontinue au- tomatic replenishment, and/or continuous renewal, complete a renewal form,change credit card information, make a credit card replenishment payment, request an itemized monthly statement of a prepaid account, add a vehicle to an existing account, request a gift certificate,change vehicle information, change your address on file and close account. Note: It does take up to 2 business days to process your request this way. Can I move my transponder from one vehicle to another? No, according to Lee County Ordinances the transponder must be regis- Page 14 IV. FREQUENTLY ASKED QUESTIONS tered and permanently affixed to one vehicle. If a transponder is moved from vehicle to vehicle, a citation may be issued. What happens if I sell my car? Call the LeeWay Service Center at (239) 533-9297, and provide the up- dated information concerning your new vehicle. Be sure to remove your transponder from the sold vehicle and install it in your new vehicle (hard case only). Call or visit the LeeWay Service Center if you have trouble, or would like assistance in removing or reinstalling your transponder. Note: LeeWay Mini transponders cannot be reused. You will need to purchase a new LeeWay Mini when you purchase a new vehicle. What if I have more than one vehicle? You can have as many transponders on one prepaid account as you would like. What if I have both a SunPass and a LeeWay transponder? If a customer has two transponders in one car (example: one SunPass and one LeeWay), it will result in the customer being double charged. If you take advantage of LeeWay discounts, we suggest you keep your LeeWay transponder. What should I do if I receive a violation in the mail for a unpaid toll when I have a transponder? Do not disregard notice. Call the LeeWay Service Center and verify account information with a CSR. You may need to add funds to your account, change vehicle information etc.. How do I close my toll account? To close your toll account, contact the LeeWay Service Center via mail, in person or by email. You can return your transponder to the LeeWay Service Center for disposal. If sending your request by mail, please enclose a letter or other signed statement that you wish to close your ac- count. We will refund any unused balance from your prepaid account in approximately 15 days.

What are the office hours of the LeeWay Service Center? The LeeWay Service Center is open : Monday: 8:00 a.m.—6:00 p.m.(extended lobby hours of 8:00a.m. to 8:30 a..m. and 5:30 p.m. to 6:00 p.m.) Tuesday - Friday: 8:30 a.m.—5:30 p.m. Page 15 IV. LEEWAY LICENSE AGREEMENT

LeeWay License Agreement Please read this License Agreement carefully. By signing this License Agreement, you agree to the following terms: General: This LeeWay License Agreement (“Agreement”) with Lee County (or LeeWay) allows you to use the LeeWay Transponder to enter and exit toll lanes of the Midpoint Memorial, Cape Coral, Sanibel Bridges, designated SunPass, E-Pass, toll lanes or any other designated facility throughout the state of Florida. Your LeeWay transponder also works in North Carolina’s Quick Pass lanes and Georgia’s SRTA (State Road and Tollway Authority) lanes. Other states will be added in the near future. For the most current information check our website www.leewayinfo.com. This Agreement accompanies and is a part of your LeeWay Application. Your submittal of a LeeWay Application constitutes your acknowledgment and consent to this Agreement. This Agreement is a license only; LeeWay reserves the right to not issue a transponder to any applicant until any and all outstanding violations have been paid. You agree to: • Pay tolls charged to your LeeWay account. • Pay an account maintenance fee of $0.50 per month taken directly from your prepaid balance every month. • Pay $45 administrative fee for any account that is closed within 60 days or less of the origination day. • Mount and use the transponder in accordance with instructions provided to you in your transponder package. • Permanently affix the transponder to the vehicle to which it is issued. • Use the transponder on a vehicle with the number of axles identified on the application form. Use of the LeeWay transponder on a vehicle with a different number of axles may result in termination of thisAgreement by the Service Center and/or issuance of a traffic citation. • Obey all applicable laws and regulations. • Promptly review your monthly statement (if requested) and notify the LeeWay Service Center if you have any questions regarding any charges. Charges not questioned within 60 days of notice will be deemed valid. • Report any changes to your name, mailing address, telephone number, vehicles, license plates, and if applicable, credit card number and expiration date when this information is made available to you. • Failure to maintain accurate license plate information may result in additional toll fees or the issuance of a traffic citation. • Maintain a safe speed during the approach and travel through the toll facilities. • Pay the full toll for your trip in the lane, if your prepaid account balance is not maintained. (Attended lanes only) • In cases where your transponder is not read, but an image of the vehicle’s license plate is captured in the lane and the license plate information for the vehicle is assigned to your transponder, the appropriate toll amount may be deducted from your prepaid account by use of the image, hereinafter called a “Video Toll” or “V-Toll”. If within a 90 day period, your account exceeds more than 20 V-Toll transactions per license plate a service charge will be accessed per occurrence. Minimum Account Balances, Fees and Charges: • You agree to maintain a positive balance in your prepaid toll account for tolls and account maintenance fees. Failure to main- tain a positive balance shall constitute a breech of this agreement, a loss of your commuter discount program and administra- tive costs /issuance of a traffic citation as determined by Lee County. • If you selected the credit card option with automatic replenishment, you authorize Lee County to replenish your prepaid account by charging $30 (or a higher amount authorized by you) to your credit card each time your prepaid account balance reaches or falls below $10. See table below for amounts for more than one transponder per account. If you selected the credit card option without automatic replenishment, you agree to notify the Service Center to replenish the account with your credit card upon minimum balance notification in the attended lanes or upon viewing your balance online. • If you selected the check or money order option, you agree to provide to LeeWay a minimum check or money order payment of $30.00, each time your prepaid account balance falls below $20.00 or you replenish your prepaid account. DO NOT MAIL CASH. Payment must be received by the LeeWay Service Center prior to your prepaid account balance reaching a zero bal- ance. See table below for amounts for more than one transponder per account. • Your replenishment amount is subject to change based on your actual usage. You will be notified by LeeWay if we deem that your replenishment amount needs to be increased. • If your prepaid account balance is not maintained you will have to pay a full toll for your trip in the lane. (Attended lanes only) In an ORT (Open Road Tolling) lane if your account is out of funds an image of the vehicle’s license plate is captured in the lane and a Toll Due notice (toll plus fee) will be mailed to the 1st registered owner of the vehicle. • If your prepaid account does not have any toll trip history for a period of twenty four months or more, you agree to a dormant account fee of $2.00 (maximum) per month. The monthly dormant account fee will be deducted directly from the prepaid account until either toll trip history occurs or your prepaid account reaches a zero balance. • Failure to maintain the required balance or properly maintain your account will result in transactions being processed as viola- tions that are subject to fees, fines and penalties as provided by law. • A returned check fee in accordance with Florida Statutes 125.0105 of $25-$40 (if face value does not exceed $800) or 5% of the face amount, whichever is greater will be assessed. You will be required to pay this assessment in full with cash, a cashiers check or money order to maintain your account status. This assessment will be deducted from your prepaid account if you have one; if any check you give LeeWay is dishonored by your bank.

Additional Services: Lee County (or LeeWay) may enter into reciprocal agreements with other agencies and private entities. If your LeeWay transponder(s) is used at any toll facility, parking facility or other facility, including commercial applications accepting LeeWay as a payment mechanism. You agree that all costs incurred in connection with such use of your transponder (s) will be charged by LeeWay to your LeeWay prepaid account. You are responsible for all such charges. Confidentiality: Pursuant to state law (CH. 119, Florida Statutes), your credit card and financial information is confidential and can- not be released to the public. Transponders: Hard Case Transponders/Bumper Mounts • You agree to pay the current selling price for a transponder. • If your transponder fails to operate for reasons other than abuse or improper use and is returned to the LeeWay Service Center within 90 days of purchase date, we will replace the transponder at no extra charge. • If your transponder is lost or stolen you agree to notify the Service Center immediately, and will be liable for any unauthorized use of your transponder(s) occurring prior to such notification. You will be charged the current selling price for a replacement transponder. • If your transponder fails to operate in the toll lanes for any reason, you agree to promptly report to the Service Center to have the transponder tested. If the transponder needs to be replaced you agree to pay the current selling price for a transponder. LeeWay Mini Transponders • You agree to pay the current selling price for a transponder. • If your transponder fails to operate within 90 days of purchase and it has not been removed from original vehicle that it was attached to, we will replace the Mini with a new one at no additional charge. If it has been removed or shows signs of prior removal, warranty shall be voided and you will be required to purchase a new one. • If your transponder is lost or stolen you agree to notify the Service Center immediately, and will be liable for any unauthorized use of your transponder(s) occurring prior to such notification. You will be charged the current selling price for a replacement transponder. • If your transponder fails to operate in the toll lanes for any reason, you agree to promptly report to the Service Center to have the transponder tested. If the transponder needs to be replaced you agree to pay the current selling price for a transponder. Termination: LeeWay may terminate this Agreement at anytime. If you wish to terminate this Agreement you must notify the LeeWay Service Center. Upon termination your prepaid account balance will be refunded to you, less any amounts owed to LeeWay within fifteen (15) days. Following termination, you will remain responsible for any payment of amounts you own under this Agree- ment. If your prepaid account balance is insufficient to cover charges payable by you, you will remain liable for all such amounts. If such unpaid charges are not promptly remitted, you may become liable for additional service charges, fines, or penalties, in accordance with applicable law. You agree to pay all costs, including attorney’s fees, incurred by Lee County to enforce the terms of this agree- ment. Changes: LeeWay reserves the right to change the terms of this Agreement and its policies, and minimum prepaid account balances any time by providing written notice to you or by posting changes on the LeeWay website. You will be deemed to have received such notice ten (10) days after it is deposited in the U.S. Mail or the date posted to the website. You agree to all changes upon use of your transponder after that date. Release and Indemnification: You hereby release Lee County from all loss, damage, or injury whatsoever, known or unknown, arising out of or in any manner connected with this Agreement or the use or performance of the transponder. Neither Lee County nor its agents will have any obligation or liability with respect to your use or the performance of the transponder. Your sole and exclusive remedy from Lee County and its agents will be replacement of any defective transponder(s). You agree to indemnify, protect, and hold harmless Lee County and its agents from all liability for any loss, damage or injury to persons or property arising from or related to the transponder, or your use of the toll lanes of the Midpoint Memorial, Cape Coral or Sanibel Bridges. Failure to Comply with any portion of this Agreement may result in violation under Florida Statutes 338.155 and other applicable provisions by law. If a violation occurs, you will be subject to all fees, fines and penalties as provided by law. Failure to abide by any of the License Agreement terms and conditions may result in the termination of this agreement. LeeWay reserves the right to terminate your account and deactivate the transponder(s) issued under this license for failure to abide by any of the terms of the license. Any trips made through the Toll Plaza with a deactivated transponder or without paying a proper toll may result in a fine in accordance with ordinance 97-19, as amended. LeeWay reserves the right to report your delinquent account to an authorized credit bureau. Itemized Statements: Statements are available with your prepaid account. If you provide an email address you will have access to your account to view or print statements as needed at no charge.. However, should you desire a monthly itemized statement provided to you the following will be deducted from your prepaid account: 1-3 transponders per account - $1.50, 4 transponders per account - $2.00, 5 transponders per account - $2.50, 6-195 transponders per account -$2.50 plus $0.25 per each additional transponder 196 or more transponders will be charged the maximum statement fee of $50.00. Miscellaneous: This Agreement is governed by the laws of the State of Florida. Venue for any action from the use of the transponders shall be in a court with appropriate jurisdiction in Lee County, Florida. If for any reason your LeeWay account is insufficient to pay any amounts payable by you to us, you will remain liable for such amounts. This Agreement shall be binding upon and inure to the benefit of you and your successors in interest. You are not permitted to assign the obligations and benefits of this Agreement.
