Ÿ The words in the outer circle are from the great Indian epic "Mahabharat", saying that "nothing is above a Human". This is also the motto of the organization. Ÿ The triangle in the inner circle symbolizes the hands of three people and stands for community development through participation. Ÿ The light from the lamp in the small but in the centre symbolizes the development of the weakest and poorest person of the community. Founder trustees of the organization were inspired by Gandhian thinking and work of great men like Albert Schweitzer. They felt deeply the agony and hopelessness of poor villagers. They saw the plight of villagers and felt a need of medical services in these villages. Hence they brought like minded friends together and founded Gram Seva Trust, an organization dedicated to rural health and development. In 1994 the trust started a 30 bedded hospital with 5 staff members in an old dilapidated building, given by another trust. As the need arose the hospital was expanded to accommodate more patients and better services. Today the hospital can accommodate 100 patients and has all basic facilities required in a rural hospital providing health services at affordable rates and sometimes free of charge to the needy from nearly 300 surrounding villages of Navsari and Dang districts. The organization also wanted to improve health of the surrounding villages hence as and when need was identified different community projects were started in the surrounding villages with main focus on health and development of women and children. Immense efforts are put in to decrease malnutrition. Gram Seva Trust also reaches out to the tribals in deep interior villages of Vansda and Dang through satellite centers and general medical mega camps arranged every year. The organization has also started different projects for development of villages like Self Help Group project (a micro saving and micro lending project for women), trainings for income generation activities, adolescent girl project, tutorial classes to strengthen primary education and a Child Education and Development Centre to improve standard of education in children of landless labourers. Tribute The sad demise of Shri. Nalinbhai J. Desai on 07/03/2020, our founder trustee and Hon. Secretary is a great loss to the organization. Nalinbhai has been continuously working for the last 25 years, for the development of the organization. He was a good orator and had excellent leadership qualities. He had developed personal relations with the staff with his loving and caring nature. Gram Seva family pays homage to Nalinbhai, whose loss will be felt forever. Mr. Nalinbhai J. Desai Death: 07-03-2020 Annual Report-2019-20 Dear friends, We are happy to present to you the annual report of our activities for the year 2019-20. This year has brought happiness to us, for we celebrated silver jubilee of the organization with our staff and friends in December. But towards the end, with the Covid 19 pandemic globally, it brought calamity, stress, grief and deep sorrow. During lockdown, when most of the private hospitals were closed or partially functioning, we have tried to provide general and emergency services through our general OPD, casualty, ICU, NICU, delivery facilities etc. We have also tried to provide masks, soaps and detergent and food grains to needy villagers in surrounding villages. (A short report is given on page 13). Thus, we present the activities and new projects in our hospital, community health and rural development programmes, during our 26th year of service. With the continuing Covid 19 pandemic, the year 2020-21 will be a challenging year for all. We are trying to provide as many services as possible, but due to the pandemic and due to change in government policies we have to close some health services. The pandemic has also brought with it, economic crises. With the growing inflation it becomes difficult for us to provide health services at nominal rates but increase in unemployment and underemployment has decreased the paying capacity of the villagers and hence we have to give more relief and provide services at low rates. This is a financial burden to the organization and we look forward to support from our donors and well wishers. Then only the organization can reach to the poorest and remote interior areas like Dang and work for the health and development of the tribal and rural population. Our friends, well wishers and donors have supported the organization for the last 25 years. We hope to receive their continuous support and help us in our mission towards rural health and development in the future also. Thank you, 31st October, 2020 Dr. Ashwin Shah, Kharel (Managing Trustee) We request you all to visit our new website www.gramsevatrust.org 1 Annual Report-2019-20 NEW PROJECTS Childline 1098 services The Childline foundation, India, has been providing helpline services to children throughout the country. Gram Seva has been selected by Childline Foundation, to provide these services in Navsari district. Our Childline team will try to help any child aged 0 to 18 years, who is in a difficult situation and needs help. The team shall work with the support of district child protection department, police and local governing bodies. These are free services and any child or parent or guardian or any well- wisher can call on 1098 to avail these services. If any child needs counseling and guidance, it shall also be provided. These services are available 24 * 7. Women’s Health Project, Dist.Dang The organization is working in Dang district through its two satellite centres at Shivarimal & Kudkas village. Dang district is hilly and tribal area with minimal health facilities and hence lack of health care and screening for the people. Lack of awareness, poverty and transportation issues are prevalent due to which patients come to satellite centres or Kharel at advanced stages of diseases. Our women's health camps in Navsari district, last year, showed us that, women lacked awareness about gynaecological diseases. After the camps, awareness increased and more and more women have started coming to Kharel hospital for gynaecological and breast complaints. It was also our experience that women suffered from co-morbidities like high blood pressure, Diabetes, Anaemia etc. Dang being a tribal area there is high prevalence of sickle cell anaemia, a genetic disorder in the tribes. To address all the above stated health issues, the organization along with The Desai Foundation Trust, Navsari and Unified Healthcare for Rural Underserved, U.S.A. started “Women’s Health Screening Project, Dang” in the year 2019 – 2020. Under this project we covered 98 villages. Total 1781 women were screened for Breast and gynaecological disease. Pap test, CBC, Sickle test, Thyroid test, Blood Sugar, Blood pressure measurement, mammography and ultrasound tests were done. We had made arrangements for mammography and ultrasound tests at BA hospital and Hiral Clinic, Navsari since no facility was available in the vicinity in Waghai block. Transport, lodging and boarding arrangements were made for the patients and relatives for these tests. It is disheartening that out of 350 women identified, who required further investigation and management, only 30 women came for treatment and completed it. On follow up it was found that these women did not come for further management due to lack of awareness, burden of work, distance from Kharel and fear. We also found that many women were symptomless and did not find it necessary to leave their daily work and go for check-up. The organization has decided to work more intensively in these areas and hence increase awareness amongst the community regarding these diseases and the need for their early diagnosis and timely management. A paper on cervical cancer study from this project was published in the 'International Journal of Reproduction, Contraception, Obstetrics and Gynecology (IJRCOG)' October 2019 issue, under the name of 'A community based study to determine incidence of cervical cancer and willingness of women to participate in cervical cancer screening program in Navsari, Gujarat, India.' 2 Annual Report-2019-20 Table 1: Sr. No Indicator 2019-20 (Dist.
