COMPLETED PASSPORT APPLICATION FORM (IM. 42-Pin.1/97) 1 Print the form in a single - To download the form from the High Commission’s website, sheet (double-sided) https://www.kln.gov.my/web/nzl_wellington OR retrieve the form in-person at the High Commission OR email the High Commission at [email protected] to request the form - Fill the form in CAPITAL LETTERS 2. CERTIFIED COPY OF CURRENT PASSPORT 2 You are NOT REQUIRED to - Certified copy of details in the passport (page 2-3), page furnish/mail us your containing valid NZ visa or NZ e-visa (NZ endorsement) & page original passport containing information of previous passport (page 47 or 49) - If your passport has expired for more than six (6) months, you MUST provide letter from NZ Department of Internal Affairs to confirm your citizenship status - Passport with expired NZ visa WILL NOT BE accepted Note: Documents can be certified at the High Commission/ Honorary Consulate in Christchurch/your local Justice of Peace 3. CERTIFIED COPY OF MALAYSIAN IDENTITY CARD (IC) 2 You are NOT REQUIRED to - Passport application CANNOT be processed if IC is lost or the IC furnish/mail us your is the old version (with the Bunga Raya symbol) original IC - In the above case, the High Commission may issue an Emergency Certificate (EC) for the applicant to return to Checklist to apply for EC Malaysia to replace IC with latest version (at Malaysian can be requested by Registration Department) and renew passport in-person at emailing us at Malaysian Immigration Department. LATEST PASSPORT PHOTOGRAPH (WITH WHITE BACKGROUND) 3 - Photo specifications and recommended Photo Studios in NZ can be referred in attachments - Photos produced by photo booth and polaroid will be rejected - Do not write on the back of the photos - Do not attach photos with paper-clips (will damage photos) 5. RIGHT THUMB PRINT (RTP) - TO BE AFFIXED AT "CAP IBU JARI To be affixed in Passport KANAN PEMBAWA" COLUMN Application Form - RTP must be in BLACK OR BLUE ink only - RTP can be done at the High Commission/Honorary Consulate in Christchurch/your local Justice of Peace - RTP must be witnessed and signed by Justice of Peace below/ outside the “Cap Ibu Jari Kanan Pembawa” column 6. FEE PAYABLE TO “HIGH COMMISSION OF MALAYSIA” Fee is to be paid in BANK NZ$80.00 (5 years validity) - for applicants who are 12 years CHEQUE ONLY old and above NZ$40.00 (5 years validity) - for applicants below 12 years old University student - for senior citizen applicants over a pplicants must include 60 years old copy of student - for university student applicants identification and below 21 years old confirmation of study updated as of 1 September 2019 Page 1 of 2 7. ADDITIONAL CERTIFIED COPY DOCUMENTS FOR APPLICANTS Applicant above 12 years BELOW 18 YEARS OLD old MUST provide copy of - IC except for children below 12 years old 2 IC - Birth certificate or Borang W/H 2 - Parents' Malaysian Marriage Certificate 2 - Parents' IC 2 - Parents’ details in passport (page 2-3), page containing valid NZ 2 visa or NZ e-visa (NZ endorsement) (for new-born baby only) Note : Father to sign Section E13(d) – “Tandatangan bapa / penjaga yang sah” 8. COLLECTION OPTION FOR NEW PASSPORT - Completed Letter of Undertaking 1 - Completed Registration Form for Malaysians residing in NZ 1 9. SELF-ADDRESSED PREPAID TRACKED COURIER BAG 1 - For the High Commission to return new passport to applicant Notes: A. The processing time will take between 3-4 months depending on your submission date. This duration takes into account that the application will be processed by the Malaysian Immigration Department HQ in Putrajaya, Malaysia. B. All photocopies of the Malaysian documents can be certified by one of the following three (3) offices only: (i) For application to be submitted in-person Consular Section High Commission of Malaysia 10 Washington Avenue, Brooklyn, Wellington Tel: (04) 385 2439 / (04) 801 5659 Visiting Hours: Monday to Friday (except public holidays) Submission between 9.45 am – 12.00 pm (please call for an appointment) (ii) Honorary Consulate of Malaysia 21 Leslie Hills Drive, Riccarton, Christchurch Tel: (03) 341 7599 (iii) Your local Notary Public/Justice of Peace C. For applications that require urgent attention, please refer to the following instructions: (i) Applicant may choose for the urgent application to be processed by the Consulate-General of Malaysia in Melbourne, Australia or the Malaysian Immigration Department HQ in Putrajaya, Malaysia: a. Application processed by the Consulate-General in Melbourne will take 1-2 months processing time and the applicant is required to include AU$15.00 (cash) for the purpose of returning the new passport b. Application processed by the Immigration Department HQ will take approximately 2 months processing time (ii) Follow the requirements as furnished in the checklist above (iii) Provide a justification letter explaining your intention to opt for the urgent process (iv) If the application is related to your NZ working or student visa, please provide a support letter from your employer (confirming that you will continue to be employed after issuance of working visa) or education institution (confirming the remaining study duration that you have to complete) (v) Provide an A4-sized self-addressed courier bag for the High Commission of Malaysia in Wellington to return the endorsed application to you (vi) The endorsed application will be returned to applicant in a sealed envelope with address to the Consulate- General in Melbourne or Immigration Department HQ (depending on applicant’s preference). The envelope is to be couriered to either office at applicant’s own expense updated as of 1 September 2019 Page 2 of 2 .
