I have read the Moorestown Summer Enrichment Camp’s Health and Safety Procedures. I will impress upon my child the measures in place are for the safety of all students, and are to be followed.

I understand I am expected to take an active role in supporting the health of my child and others by reporting the health of my child each day. I will accurately complete the health reporting form each evening, and again each morning, prior to student drop off.

I understand the Moorestown Township Public School Summer Enrichment Camp is operating under the current guidelines provided by the New Jersey Department of Health for Youth Camps. I understand that, although all safety measures will be followed, there is always a risk of injury or illness.
