To Provide Procedures and Guidelines for Personnel Responding to and Operating at Working Structure Fire Incidents
I. INTRODUCTION A. Purpose: To provide procedures and guidelines for personnel responding to and operating at working structure fire incidents. B. Scope: This instruction applies to all personnel responsible for performing tasks in the operational area of a structure fire. C. Author: The chief deputy of Emergency Operations is responsible for the content, revision, and annual review of this instruction. D. Definitions: See Appendix I E. Underwriters Lab (UL) studies: See Appendix II F. Operational Modes: See Appendix III II. RESPONSIBILITY A. The first arriving company officer is responsible for: 1. Performing an initial size-up. 2. Developing a mental incident action plan (IAP) to determine the initial operational mode. 3. Transmitting the size-up radio report to the Los Angeles Communications Center (LACC). 4. Taking initial actions consistent with the incident priorities and tactical operations of the incident. 5. Considering the use of transitional attack when operating in the offensive mode and with fire showing. 6. Overall management of the incident. 2. Identifying incident objectives. 3. Communicating the current operational mode and providing status reports to LACC. C. The incident safety officer is responsible for: 1. Identifying and evaluating hazards, knowing the current operational mode, and advising the IC in the area of personnel safety. The safety officer has the authority to alter, suspend, or terminate any unsafe activity. The safety officer investigates accidents and near misses involving Department personnel. -

City of Scottsdale Fire Department Arizona Standards of Coverage and Deployment Plan 2015 Standards of Coverage and Deployment Plan Scottsdale Fire Department, Arizona Introduction The following report serves as the Scottsdale Fire Department Standards of Coverage and Deployment Plan. It follows closely the Center for Fire Public Safety Excellence (CPSE) Standards of Coverage model that develops written procedures to determine the distribution and concentration of a fire and emergency service agency’s fixed and mobile resources. The purpose for completing such a document is to assist the agency in ensuring a safe and effective response force for fire suppression, emergency medical services, and specialty response situations. Creating a Standards of Coverage and Deployment Plan document requires that a number of areas be researched, studied, and evaluated. This report will begin with an overview of both the community and the agency. Following this overview, the plan will discuss areas such as risk assessment, critical task analysis, agency service level objectives, and distribution and concentration measures. The report will provide analysis of historical performance and will conclude with policy and operational recommendations. Structural Fire Assignment
Cumberland County Standard Operating Guideline Order of Apparatus Arrival - Structural Fire Assignment Purpose: To establish a standard method for fire apparatus arrival and positioning at structure fire incidents. All personnel must use the procedures below during structure firefighting operations. It is important to note that unless otherwise directed by the Incident Commander you must follow this apparatus positioning guideline. Unit Officers must LIMIT radio transmissions to critical fire ground information. Single units should routinely not contact the Incident Commander for “instructions and assignments”. Procedure: While enroute to the incident scene Unit Officers must maintain situational awareness of their specific location and order of apparatus arrival. Personnel must not take action until their Unit Officer in charge directs them to do so. All drivers who are not specifically assigned to apparatus operations will assemble with their crew. Fire ground discipline is critical during all incident responses. In addition to the listed responsibilities Unit Officers must: A. Maintain crew integrity B. Ensure that personnel and apparatus take their assigned positions C. Follow this and other applicable policies, including the Incident Command System. The IC may modify these assignments as necessary. First Due Engine 1. Unit Responsibilities Initiate water supply by laying a supply line from the most suitable water supply, beginning a split lay or give instructions for reverse lay as necessary. Position the engine on Side Alpha, reserving adequate space for the aerial unit to position. Connect to the building standpipe and/or sprinkler system, if so equipped, on or closest to Side Alpha. Activate VTAC for optimal portable radio coverage. -

Flynn August 2009 National Fire Protection Association Fire Analysis and Research Division Abstract In 2003-2006, U.S. fire departments responded to an estimated annual average of 3,570 structure fires in dormitories, fraternities, sororities, and barracks. These fires caused an annual average of 7 civilian deaths, 54 civilian fire injuries, and $29.4 million in direct property damage. Fires in these properties accounted for 0.7% of all reported structure fires within the same time period. These estimates are based on data from the U.S. Fire Administration’s (USFA) National Fire Incident Reporting System (NFIRS) and the National Fire Protection Association’s (NFPA) annual fire department experience survey. Cooking equipment was involved in 75% of reported structure fires. Only 5% of fires in these properties began in the bedroom, but these fires accounted for 62% of the civilian deaths and 26% of civilian fire injuries. Fires in dormitories, fraternities, sororities, and barracks are more common during the evening hours, between 5 p.m. and 11 p.m., and on weekends. Garcia, Engineer Review Level: 2 Crystal Giannopulos, Administrative Support: Senior Administrative Clerk ADA PURPOSE The Fresno Fire Department (FFD or Department) utilizes 10 and 14-foot “Fresno” Attic Ladders, 10-foot collapsible Attic Ladders and the Little Giant Velocity 13 (carried on engine apparatus) and Defender 17 (carried on aerial apparatus). APPLICATION Specifically designed for use where space is limited, the “Fresno” Attic Ladder is particularly ideal for entering attics through ceiling scuttle holes and similar close- quarter situations. The little Giant Ladders are ideal where an articulating extendable ladder unit is helpful, such as smoke detector installs, negotiating over a fence or other situations where a step ladder is needed. OPERATIONAL POLICY The Little Giant Velocity 13 (carried on engine apparatus) shall not be used on the fire ground for firefighting inside or outside of a structure. It is not National Fire Protection Agency (NFPA) compliant. The Little Giant Defender 17 (carried on aerial apparatus) meets the NFPA standard for Manufacturer’s design of fire department ladders and has been tested to the requirements of NFPA standard on use, maintenance, and service testing of in- service fire department ground ladders. With such certifications and testing, the Little Giant Defender 17 can be used on all fire ground operations. 2016 18 November 9, 2018 Arson Fire Kills Three Fire Fighters and Injures Four Fire Fighters Following a Floor Collapse in a Row House—Delaware Executive Summary On September 24, 2016, a 41-year- old lieutenant and a 51-year-old senior fire fighter died due to a floor collapse in a row house at a structure fire. Two other fire fighters were critically injured. One of the injured fire fighters, a 48-year-old female died on December 1, 2016, due to injuries sustained from the collapse and exposure to fire in the basement. Another fire fighter spent 40 days in a metropolitan hospital before being released. Two other fire fighters received burns during fireground operations and one fire fighter sustained an ankle injury. All three were treated and released from the hospital on the same day. Ladder 2 arrived on-scene at 0301 hours and reported heavy fire showing from the rear of the structure. The Ladder 2 Officer requested a 4th engine be dispatched. Engine 1 and Engine 5 arrived and both laid supply lines from different directions. Battalion 2 arrived on-scene and assumed Command. Crews from Engine 1, Engine 5, and Ladder 2 went to the front door and entered the 1st floor at approximately 0307 hours. -
